Site Visit Report

Visit Date: 27 March 2019

Location: Site VI Floating Docks A, B, C, D, Subase and Spud Barge

Attendees: Derrick Yarbrough, SPK14 Randy Duncan, SPK14 GD-MS North Florida Waste Management Hayward Construction Five Guys Farms, LLC

Q & A:

1. Will this effort require a continuous presence?

No, this will be an initial clean-up effort followed by up to three (3) response calls as necessary.

2. This is four (4) events with three (3) additional option events?

No, this is four (4) total events – one initial cleaning event followed by three (3) additional events as needed.

3. How do we bring a boat aboard? Can we drive it down the river or does it need to be hauled on to the base? If so, where can it be put in at?

Security has a vetting process that will have to be adhered to whether the boat is hauled onto the base or driven down the river. If the boat is hauled onto the base, there is a boat ramp available.

4. What about the area under the hard fixed pier?

Under the hard fixed pier is not part of the area to be cleaned unless that debris is attached to debris which is in, around, or under the floating docks.

5. Is the basin bottom included?

No, this is only for floating debris that can get sucked into the boat intakes.

6. The SOW specifies dive services, but it appears that some of this clean-up can be done without diving?

The goal is to remove all of the debris that can foul the boat engines. This includes floating debris in, around, and under the floating docks as well as debris (primarily reeds) sitting on the docks. A good portion of the debris probably can be reached from the dock or from a boat, however, there needs to be a dive element to clear debris out from under the floating docks.

7. The drawing in the SOW does not seem to agree with the actual area needing to be cleaned.

That is correct, we will update the drawing to correctly depict the areas to be cleaned.

8. With regard to a 24 hour response time, EM385 dive requirements call for a 10 day lead time for dive plan approval. Is there a way to get a one-time standing dive plan approved for all potential dives?

Yes.
