SECTION SF 30 BLOCK 14 CONTINUATION PAGE

SUMMARY OF CHANGES

SECTION SF 30 - BLOCK 14 CONTINUATION PAGE

The following have been modified:

ANSWERS TO QUESTIONS

1. In section L.2.4.5.1 it states: “Pricing and supplemental pricing for this solicitation represents minimum quantities plus four option periods (lots). Lots for this requirement are outlined in the electronic pricing spreadsheet and Enclosures 3-11….”

It appears that there are only a maximum of 3 option periods for any one lot, not 4. Please explain.

· Section L.2.4.5.1 has been revised.

2. In The Pricing Sheet: in the Instruction Page Tab it states:

“Additional Information: Provide any additional information deemed necessary to explain HCW compensation.” But the only box into which one can enter text is the Blended Rate Explanation box. Should additional information be placed there also?

· The electronic Pricing Sheet has been revised to allow for Additional Information to be place in the box now labeled Blended Rate Explanation / Additional Information (if applicable).

3. In the Pricing Sheet the 3rd Option Periods for Lots A, B, F and I are short-approximately 8 months, yet the number of hours is the same as the preceding 2nd option periods which are full years. Is this intentional?

· The electronic Pricing Sheet has been revised to correct the hours in option period 3 for lots A, B, F and I.
