USNS SAFEGUARD – ALLISION 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 20 Feb 2013, a set of test weights failed and dropped approximately 60 tons of weight onto the ship’s deck below. SUBIC DRYDOCK agreed to pay for tow deck repair costs in the amount of $51,200.00.

USNS LAWRENCE H. GIANELLA – MAGELLAN DOCK WALL 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 24 Apr 2013, during approach to berth, negotiating a turn into berth, and trying to avoid the tug and tow, USNS GIANELLA made contact with the dock wall, creating a small crack to the cement wall.

USNS CHAVEZ – CONTAMINATED FUEL TRANSFER 1 man 2 day onsite audit/2 man 2 day document review/3 man 1 week report On 16 Aug 2013, during an underway replenishment with a US warship, CHAVEZ delivered DFM into the warship’s JP5 tanks. Investigation revealed a portion of the DFM piping was improperly labeled by manufacturer.

USNS 1ST LT HARRY L. MARTIN – ALLISION 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 26 Sep 2013, USNS MARTIN was being towed from Blount Island Command to North Florida Shipyard by tugs owned by Moran Towing (GOCO) and allided with Matthew Bridge in Jacksonville, FL. Received a claim on 28 Apr 2014 in the amount of $4,105,195.91. Case still open/pending further action.

USNS MERCY – FIRE DAMPNER MAINTENANCE 1 man 2 day onsite audit/1 man 2 day document review/3 man 1 week report On 16 Oct 2013, Global Fire and Safety Inspection report identified 134 of 569 fire dampers were found faulty. Scope of work was beyond capability of S/F and required contractor support.

T/B SEA EAGLE/MOBRO – COLLISION 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 20 Dec 2013, while departing AUTEC, the tow hauser parted with the barge. While adrift, the barge MOBRO struck AUTEC vessel RANGER.

USNS ARCTIC – ALLISION – 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 31 Dec 2013, ARCTIC passed in close proximity to breakwater while departing Fujairah. Follow on inspection by Navy divers found cosmetic damage and minor damage to STBD bilge keel. There were no indications of soft grounding or allusion and instruments showed good water on charted route.

USNS DREW and USNS CHAMBERS – ORDNANCE DROPPED 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 15 Jan 2014, during at sea replenishment between DREW and CHAMBERS, pallet containing ordnance was lost due to mechanical failure.

USNS RAINIER, USNS PERRY, and USNS CHAMBERS – ORDNANCE DROPPED 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 22 Apr 2014, during replenishment at sea operations between PERRY and RAINIER, one load of ordnance were dropped into the sea by PERRY’s embarked helicopter. Later that day, following maintenance and operational checks, the same helicopter dropped another load of ordnance into the sea during VERTREP operations between RAINIER and CHAMBERS.

USNS MILLINOCKET – PANAMA CANAL1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 11 Jun 2014, during MILLINOCKET’s transit of the Panama Canal, ship’s stern was pulled in a see-saw motion by two AFT mules. The vessel allided with the canal wall. Case still open/pending further action.

USNS FISHER – FUEL SPILL 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 09 Jul 2014, FISHER spilled approximately 11,000 gallons of fuel into Boston Shipyard drydock.

USNS WATERS – LAUNDRY ROOM FIRE 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 16 Sep 2014, WATERS experienced a fire in the outboard dryer of ship’s laundry room. No definitive cause identified, but the cause was thought to be a faulty thermal sensing bulb, in combination with residue petroleum products (e.g. grease on the towels).

USNS SACAGAWEA – PIER ALLISION – 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 05 Nov 2014, while moored Tengan Pier in Okinawa, SACAGAWEA parted several lines and caused damage to pier bollards due to a large swell from a nearby Typhoon.

USNS EARHART and USNS DIEHL – COLLISION – 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 20 Nov 2014, during an at sea replenishment, EARHART and DIEHL collided due to a loss of steering casualty onboard EARHART. No personnel injuries reported.

USNS KOCAK – GROUNDING –W/PM3 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 22 Jan 2015, after loading cargo and ammunition for exercise Cobra Gold in Okinawa, and while exiting the harbor, the ship ran aground. There were no personnel injuries. No oil leaks from the hull, but several holes in the hull on the starboard side.

USNS KOCAK – BOILER EXPLOSION – 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 08 Mar 2015, while anchored White Beach, Okinawa, ship’s main propulsion boiler exploded. No personnel injuries. No fire or fuel leaks and no hull or tank breaches aside from those experienced in the grounding.

USNS ARCTIC – ORDNANCE DROP – 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 18 MAR 2015, during an at sea replenishment with a US warship, an H60 helicopter lifted two containers of ordnance off ARCTIC’s deck. Almost immediately, the helicopter became unstable and jettisoned the containers. The weight of the ordnance was incorrectly listed on the containers.

M/T ALPINE ETERNITY – ALLISION – 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 22 Mar 2015, MSC foreign flag time-chartered tanker Alpine Eternity allided with an apparently uncharted oil wellhead. Possible oil pollution and/or environmental damage.

USNS FALL RIVER – ALLISION 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 27 Mar 2015, during a berth shift in JEBLC, FALL RIVER’s port bow allided with Pier 14S after confusion regarding the intentions of both Captains and the transfer of steering control.

USNS LAWRENCE H. GIANELLA – ALLISION 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 11 Apr 2015, GIANELLA’s starboard side stern area allided with the pier fender and made contact with the curbing on the pier. Damage amount is well below P&I deductible, recourse would be limited to the OPCO. OPCO are agents of the US government, so a suit against them is barred by the exclusivity provision of the suits in Admiralty Act incorporated into the public vessels act. MSC – No intent to pursue matter in contract. No basis to assert tort claim.

USNS FISHER – BREAK FROM MOORING – 1 man 1 day onsite audit/1 man 2 day document review/3 man 2 week report On 14 Apr 2015, a weather front moved through Corpus Christi, TX, producing winds in excess of 65 kts. FISHER parted all mooring lines and drifted approximately 300 ft up river. Pilot boarded the vessel and two tugs stabilized the vessel, with two additional tugs called to shift the vessel back to berth.

USNS MERCY – ALLISION – IN PROGRESS - 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 27 May 2015, while getting underway from Pearl Harbor, MERCY ordered a backing bell with the assistance of two tugs. Master assumed the conn as the stern approached ARIZONA memorial and aft tug was reported 25ft from landing dock and closing. Aft tug reported that its rudder made contact with ARIZONA boat landing fender and prop wash from MERCY’s full ahead engine order caused motion and subsequent damage to landing dock.

USNS 1ST LT JACK LUMMUS – ALLISION – 1 man 2 day onsite audit/2 man 4 day document review/3 man 2 week report On 29 May 2015, while pulling into Agadir, Morocco, LUMMUS (GOCO ship operated by MAERSK) was pushed onto the pier by the tugs resulting in a hole in the aft port corner of the stern above the waterline. Vessel was making her arrival into Agadir for AFRICA LION Exercise backload.
