SECTION SF 1449 - CONTINUATION SHEET

The following have been modified:

PROPOSAL SUBMITTAL REQUIREMENTS (BETA)

Offers shall be submitted in electronic form, via email, to the contract specialist prior to the closing time outlined in the solicitation. Offerors shall submit the following information with their offer:

Compliance/Exceptions - On a separate sheet of paper, identify any “exceptions” to the specifications and state precisely how the offered supplies/services differ from the applicable specification paragraph(s). Any exception which is not expressly approved by the contracting officer in writing is hereby rejected and void. Any offerors taking exceptions determined to be unacceptable will be ineligible for award.

Offerors shall closely examine the features of their offered products/services and compare them to the listed technical requirements in the Performance Work Statement. On a separate sheet(s) of paper, address each paragraph/sub-paragraph number listed in the Performance Work Statement and type either "will comply" or "take exception to" next to it.

Failure to comply with the submittal requirements of this paragraph may result in rejection of the offer.

1) TECHNICAL

a) Personnel Resumes - (Not to Exceed 2 Pages Per Resume) Offerors shall submit one (1) resume for each individual that may be sent to perform the required services with their offer. This includes individuals of firms subcontracted to as well. Each resume must list the subject’s job title and demonstrate that individuals meet all the requirements contained in the Performance Work Statement (PWS) and have taken all training requisite to be certified in all standards required by the PWS.

The Offeror shall describe their work experience, education, and level of security clearance, and shall state whether the person is presently employed with the Offeror, or whether they are proposed under a letter of intent. The Offeror shall certify that proposed personnel shall be available within one (1) week of award. If the employee is not a current employee of the offeror or a proposed subcontractor, a copy of the accepted offer letter shall be provided.

b) Technical Approach Summary - The offeror shall demonstrate within a total of five (5) pages maximum, an understanding of the requirements contained in the Performance Work Statement (PWS), demonstrating knowledge and the capability to perform the requirements. This shall include the offerors plan to certify their worker in Nortel, Avaya and/or GenBand as required by the PWS; if proposed employees are not already certified.

c) Management Approach & Staffing Plan - The offeror shall provide a Management Approach and Staffing Plan which demonstrates how the proposed personnel shall support task areas described in the PWS with their labor allocation. This shall include how the offeror intends to cover both employees in the event of absences (vacation, etc.). This shall include any subcontracting/teaming/consulting agreements to be utilized in contract performance.

The offeror shall address the extent of small business participation in performance of any resultant contract. The solicitation, however, does not require participation by such entities. The offeror shall provide targets, expressed as percentages of total contract value, for small businesses, small disadvantaged businesses, women-owned small businesses, veteran-owned small businesses, service-disabled veteran-owned small businesses, and HUBZone small businesses in any of the North American Industry Classification System (NAICS) Major Groups as determined by the Department of Commerce. The targets may provide for participation by a prime contractor, joint venture partner, teaming arrangement member, or subcontractor. Targets will be incorporated into and become part of any resultant contract.

Provide goals, expressed as dollars and percentages of total contract value and percentages of total subcontract value for small businesses and each sub category of Small business listed below.

Small Disadvantaged Business (including ANC’s and Indian Tribes) (SDB)
Women-Owned Small Businesses (WOSB)
HUBZone Small Businesses (HUBZone)
Veteran-Owned Small Businesses (VOSB)
Service-Disabled Veteran-Owned Small Businesses (SDVOSB)

The Government will evaluate the degree to which the proposal demonstrates specific knowledge, capability and relevant experience to perform the tasks outlined in the PWS. In addition, the socio-economic plan will be evaluated as part of the Technical Approach Summary subfactor. The socio-economic plan evaluation will include the extent to which firms are specifically identified, the extent of the commitment to use such firms, the complexity of the work small firms are to perform, the realism of the proposal and the extent of participation of such firms in terms of the value of the total acquisition.

If the offeror fails to provide targets for the contract and/or any of the seven socio-economic factors, the offeror's proposal will be evaluated at zero percent for the contract and/or any socio-economic factor for which a target is not provided. However, an offeror that provides no socio-economic plan in response may be considered ineligible for award.

Note: The Socio-economic Plan factor and the separate requirement for submission of a Small Business Subcontracting Plan are included in the solicitation in an effort to promote small business participation under the solicitation and the resultant contract. The Socio-Economic Plan is a part of Management Approach and Staffing Plan subfactor and is separate and distinct from the requirement for an offeror to submit a Small Business Subcontracting Plan. The requirements for a Small Business Subcontracting Plan are set forth in the Section I of the solicitation under FAR clause 52.219-9.

2) PAST PERFORMANCE

a) Contract Data Sheets - Offerors shall complete Contract Data Sheets (Attachment I) for themselves, as well as any major subcontractors, for up to three (3) of their most recently completed Federal Government contracts (not to exceed three years since completion) for supplies/services that are similar in scope, size and complexity to this procurement. The experience should include a list of subcontractors utilized, dates of performance and how the experience is directly related and/or similar to this requirement. If you do not have any Federal Government contracts, then list state, local, or commercial contracts, in that order. Offerors are encouraged to submit brief and concise responses. Data submitted other than that requested on the Contract Data Sheets will not be considered. Failure to submit Contract Data Sheets shall be considered certification that the contractor has no past performance for the Government to evaluate.

b) Past Performance Questionnaires - For each contract submitted for consideration on a Contract Data Sheet (Attachment I), a corresponding "Past Performance Questionnaire" (Attachment II) shall be forwarded to and completed by the customers for whom the work was performed. The Government is under no obligation to consider incomplete data or data submitted in any other format. It shall remain the offeror's sole responsibility to request and track each past performance questionnaire to ensure receipt by the Government from the customers prior to the closing date and time established for receipt of offers in the RFQ.

Relevant past performance is defined as experience (for projects completed within the past three (3) years) that is of the same or similar scope (e.g. PWS requirements and tasking, etc.), of the same or similar magnitude (e.g. performing services of a dollar value comparable to the dollar value of the offeror’s proposal), and of the same or similar complexity (e.g. telephone switch maintenance) to that which is described in the solicitation.

The offeror shall either demonstrate relevant past performance or affirmatively state that it possesses no relevant past performance. Failure to submit the completed "Contract Data Sheets" and "Past Performance Questionnaires" shall be considered certification (by signature on the proposal) that the offeror has no past performance of like or similar items and/or work for the Government to evaluate. More relevant past performance is typically a stronger predictor of future success than past performance of lesser relevance; thus, greater weight will be given to those proposals with more relevant past performance than those of less relevance.

Past performance also speaks directly to the quality of the contractor’s performance under the corresponding contract/order identified on the Contract Data Sheets. Past performance quality will include, but not be limited to the assessment of the firm’s quality of supplies/services provided, adherence to schedules, cost control, business relations, management of key personnel, and utilization of small businesses for relevant projects.

c) Past Utilization of Small Business Concerns - Large Business Concerns that submit an offer in response to this solicitation shall also provide their Small Business Goals and actual accomplishment against said goals for the one (1) Past Performance Questionnaires provided. Goals shall be expressed as dollars and percentages of the total subcontract value for each of the following socioeconomic subcategories: Small Businesses (SB), Small Disadvantaged Businesses (SDB), Women-Owned Small Businesses (WOSB), Veteran-Owned Small Businesses (VOSB), Service-Disabled Veteran-Owned Small Businesses and Historically-Underutilized Small Businesses (HUBZone SB). In addition, other than small businesses are required to submit one copy of the two most recently approved semi-annual Individual Subcontracting Reports (ISR) submitted in the Electronic Subcontracting Reporting System (eSRS) for each of the contracts referenced in their Past Performance Survey (Attachment II).

Small businesses, or those other than small businesses with limited or no Federal Government experience, are required to describe efforts made to carry out the policy of FAR Clause 52.219-8 Utilization of Small Business Concerns in the award of subcontracts for the contracts provided on Attachment II.
