About this file

This Request for Statement of Qualifications (RFSQ) is issued by the County of Los Angeles Treasurer and Tax Collector, Public Administrator Division, for the provision of mortuary services. The County seeks to establish a Master List of qualified mortuary service providers to deliver services as described in the Statement of Work. Qualified establishments and vendors meeting the minimum requirements specified in Section 1.4 of the RFSQ are invited to submit a Statement of Qualifications (SOQ). The Master List will have an initial term of five years, subject to earlier termination or extension at the County's option. Two optional Establishments/Vendors' Conferences are scheduled to address questions regarding the RFSQ specifications: the first on Tuesday, February 7, 2017, at 10:00 a.m. Pacific Time at the Quartz Hill Public Library in Quartz Hill, California, and the second on Thursday, February 9, 2017, at 2:00 p.m. Pacific Time at Kenneth Hahn Hall in Los Angeles, California. Prospective vendors should review the entire RFSQ, including service requirements, SOQ content and format requirements, the selection process, and a sample Master Agreement available through the County's website at http://camisvr.co.la.ca.us/lacobids or the TTC's website at http://ttc.lacounty.gov.

The RFSQ does not specify pricing terms, set-asides, or award amounts in the available documentation. Vendors meeting the minimum qualifications will be added to the Master List for potential future contract awards. The specific funding source, contract value, and additional terms will be determined through subsequent procurement processes with individual vendors selected from the Master List.
