About this file

This is a memorandum from the Arizona Department of Veterans' Services (AZDVS), located in Phoenix, Arizona, specifically from the Office of the Director. The document is authored by Monica DeLaRosa, Senior Procurement Officer, and addresses contract CTR067907 for the State of Arizona Veterans' Home - Phoenix, dated October 3, 2024.

The memorandum clarifies a contract extension for an unspecified service or goods that was inadvertently allowed to expire on September 30, 2024. Due to inadequate department staffing, the contract tracking and execution were not properly managed. A formal contract addendum was submitted to the vendor on October 3, 2024, to remedy the expiration. The department recommends moving forward with Contract Amendment One (1) to extend the contract through September 29, 2025, ensuring continuity of day-to-day business operations. During the expired period, invoices continued to be submitted and the contractor was paid for work performed.
