About this file
This is a memorandum from the Arizona Department of Transportation (ADOT) dated 12/18/2024, specifically from Gary Henry, Senior Procurement Specialist, to the Procurement File. The document involves Geveko Markings Inc. as the contractor and is copied to Elizabeth Hubbard, Procurement Manager, and Risk Management.
The memorandum concerns a waiver of Workers' Compensation endorsement requirements for Contract CTR069748, which appears to be related to Bulk Thermoplastic and Thermoplastic Preformed Pavement Marking. The waiver is granted because the contractor currently does not have a presence or employees in the State of Arizona. However, the document stipulates that if Geveko Markings Inc. establishes a presence in Arizona in the future, the Workers' Comp endorsement requirement will be reinstated.
