About this file

This is a Request for Proposal (RFP) issued by the State of Missouri's Office of Administration, Division of Purchasing, on behalf of the Department of Commerce and Insurance, for Medical Care Utilization Review Services. The solicitation seeks qualified contractors to provide independent review organization (IRO) services for external reviews of health insurance coverage determinations under Missouri law (sections 376.1350–376.1399, RSMo). The contractor must conduct medical necessity reviews, appropriateness assessments, and health care setting evaluations for cases referred by the Department of Commerce and Insurance's Division of Consumer Affairs. Historical volume data shows case referrals ranging from 13 cases in 2010 to 130 cases in 2019, with 71 cases referred in 2025; expedited reviews requiring 72-hour turnaround averaged 10–13 cases annually in recent years. The contract period is one year from the effective date, with renewal options for up to four additional one-year periods at the state's discretion. Proposals are due by September 16, 2026, at 2:00 PM Central Time, submitted electronically through MissouriBUYS. The state anticipates awarding contracts to three responsive, responsible, and reliable vendors to ensure adequate coverage and meet potential service demand.

Pricing shall be firm and fixed on an hourly basis per medical personnel classification (physicians, nurse reviewers, and other health care professionals), with standard external reviews due within 20 calendar days and expedited reviews within 72 hours; the state will not guarantee minimum or maximum service levels but notes average review times of 2.8 hours for physicians and 3.2 hours for nurses based on 2024–2025 data. Contractors must be accredited or certified as an IRO by a nationally recognized organization and registered as a utilization review agent under Missouri law; minimum qualifications include one year of experience in medical care utilization review, electronic case transmission platforms, expedited reviews, conflict-of-interest policies, quality assurance procedures, and health-related medical expertise. Key personnel requirements include a Medical Director with an unrestricted medical license, minimum two full-time registered nurse reviewers, and clinical peers in multiple specialty areas with recent clinical experience; personnel must be from different geographic and work settings and preferably licensed in Missouri. No incumbent contractor information is provided, though current contracts reference numbers RFPS30034902201029, CS220129001, CS220129002, and CS220129003 are available for review. The contractor must comply with HIPAA requirements via a Business Associate Agreement, maintain written policies on conflicts of interest and quality assurance, and submit annual reporting in Excel format detailing case volumes, resolution rates, timelines, and case types.

1.2 Background and Historical Usage Information:

1.2.1 Missouri law (sections 376.1350 – 376.1399, RSMo) outlines health care utilization review procedures for health plans subject to regulation by the state. Specifically, section 376.1387, RSMo and 20 Code of State Regulation (CSR) 100-5.020 outlines a process by which consumers who receive an adverse determination under their fully insured health plan can seek an external review of the health carrier’s determination from an independent review organization (IRO). The IRO provides the Department of Commerce and Insurance (DCI) with an independent resource to evaluate medically necessary and covered services. DCI’s Division of Consumer Affairs oversees and monitors this process. Additional information about the external review process in Missouri can be found at the following website: https://insurance.mo.gov/consumers/health/externalreviewprocess.php.

1.2.2 DCI is aware of no more than one lawsuit arising from external review activities. For historical purposes only, the case was Hundley v. Wenzel, 59 S.W.3d 1 (Mo. App. 2001).

1.2.3 The quantities below represent the total number of cases referred by DCI for external review during the calendar years 2010 to 2025.
