About this file

The document is a Living Wage Certification Form issued by the Town of Manchester, Connecticut, related to a tank painting inspection services contract. The form requires bidders to certify whether they are a "Covered Employer" under the Manchester Living Wage Ordinance (Chapter 212), and if so, commit to paying the required living wage to eligible employees throughout the contract term. Bidders must indicate their status by checking either that they are a covered employer or that they are exempt due to specific conditions such as being a nonprofit, employing fewer than 25 eligible employees, or having an annual contract value less than $25,000.

The certification form is designed to ensure compliance with local wage standards and must be returned with the bid or RFP submission. Failure to properly complete and indicate the employer's status may result in bid rejection. The form provides multiple exemption pathways, including charitable organizations with 501(c) tax-exempt status, small employers, and contracts below a certain monetary threshold. The document requires the bidder's authorized representative to sign and date the form, certifying the accuracy of the representations made regarding their employment and wage practices.
