Section C — Performance Work Statement

Section C-1 General Information

C-1.1 Introduction

Fleet Readiness Center (FRC), is an industrial facility located on the Marine Corps Air Station

(MCAS) Cherry Point, NC, that reworks, repairs, and overhauls aircraft, engines, and components for customers, both U. S. and foreign military. Its mission is “Generating Combat Air Power for

America’s Marines and Naval Forces”.

The FRC East comprises:

Production buildings

Production support buildings

Administrative buildings

Various support buildings along “A” Street

Outlying buildings on “C” Street, Cryogenics Road, a leased warehouse on NC highway

24 adjacent to Carteret Craven EMC and Bldgs. 4841 and 4930 on the opposite side of the runway from FRC East.

C-1.2 Organization of This PWS

The specific requirements and relevant information contained in this PWS, constituting Section C, are organized as follows:

Section C-1 provides general information necessary to understand the requirements.

Section C-2 provides definitions and acronyms used throughout this document.

Sections C-3 and C-4 provide information as to Government and Contractor provided items.

Section C-5 presents the detailed work requirements of the PWS.

Section C-6 identifies the standards and regulations the contractor shall follow while performing work at the FRC East.

Section C-7 identifies projected workload for the area the contractor will perform.

All sections and Technical Exhibits of this PWS are parts of a single, interrelated document.

C-1.3 Summary Scope of Work

The Contractor shall perform the functions listed below in supporting operation of the FRC East’s transportation requirements. In doing so, the contractor shall provide all the services described in this PWS to FRC East activities, detachments, tenants, and customers located at the FRC East and adjacent and outlying areas. Adjacent areas are those in the Town of Havelock, NC. Outlying areas are any locations within a 200-mile driving radius of the FRC East, with the exception of the

FRC East Detachment at MCAS Beaufort, SC. Outlying areas include, but are not limited to, locations such as Camp Lejeune, Raleigh Durham Airport, Marine Corps Air Station New River, NC, MCAS Beaufort SC, and Norfolk VA.

This includes providing all personnel, equipment, materials, supervision, management, and any other resources necessary to fulfill the requirements except those specified in Section C-3, as

Government Furnished Property (GFP) and Services. These functions include operation of material handling equipment (MHE), weight handling equipment (WHE), ground support equipment (GSE), and garrison mobile equipment (GME) and provision of administrative and maintenance services performed in the following functional areas:

Most of this work will be at the Marine Corps Air Station (MCAS), Cherry Point, NC. Some portion requires the contractor to travel and work outside the MCAS complex, particularly to

MCAS New River, NC, and MCAS Beaufort, SC. Trips to and from MCAS New River, NC occur approximately 7 times per week during 1 st shift; some trips may extend into the first few hours of 2 nd shift. The work area outside the MCAS New River, NC complex includes its adjacent and outlying areas. Trips to and from MCAS Beaufort, SC occur approximately once or twice a month during 1 st shift; some trips may extend into the first few hours of 2 nd shift.

C-1.4 Background Information

For more than six decades, the FRC East (formerly the NAVAIR Depot Cherry Point, NC, the

Naval Air Depot) has provided maintenance, engineering, and logistics support for a variety of aircraft, engines, and components for all branches of the U.S. Armed Forces and foreign militaries.

The FRC East performs depot-level repair, maintenance, and support services for the H-1, H-46, H-53, AV-8, F-35 and V-22 aircraft, engines (T58, T64, T400 and F402) and components. In-service repair is also provided for the C-130, H-60, and EA-6B aircraft.

The FRC East’s Integrated Management System (IMS) is registered to the following standards:

ISO 9001, ISO 14001, AS9100, AS9110 and OHSAS 18001.

C-1.5 General Requirements

The contractor shall comply with the requirements set forth in this PWS. In addition, the contractor shall also be aware of and comply with applicable Federal, State, and local laws and

MCAS and FRC regulations. All the Contractor’s Standard Operating Procedures (SOPs) shall comply with applicable laws and regulations. The Contractor shall be ISO 9001 or AS9100 certified and must have obtained certification from an accredited Registrar approved by the

International Accreditation Forum (IAF) or the ANSI-ASQ National Accreditation Board. The on-site manager shall ensure that contractor personnel comply with the FRC East's ISO registered

IMS in performance of their duties under this contract.

C-1.5.1 Plans & Reports

C-1.5.1.1 Award Performance Plan

Within 30 calendar days after award, the contractor shall provide the Government a written Award

Performance Plan (APP) for approval. The purpose of the APP is to show how the contractor will plan, organize staff, manage, and control work under the contract. The APP consists of sections, each a sub-plan, for an essential area of contractor performance. The paragraphs below list basic requirements for APP sections and the type sections required. Details of individual sections are included throughout Section C-1. (See TE 4, CDRL A001.)

C-1.5.1.1.1 APP Section Requirements

C-1.5.1.1.2 Required APP Sections

The APP shall include the following sections:

Personnel and Staffing, including an organization chart (C-1.5.2.7)

Safety and Health (C-1.5.7.3)

Property Control (C-3.1.2)

C-1.5.1.2 Operations Reports

The contractor shall prepare and submit to the Government written reports associated with operations under this PWS. The sections below list basic requirements for operations reports and the types of reports required. Sections throughout C-1 describe details of individual reports, such as conditions for use, content, and submission requirements. (See TE 4, CDRL A002.)

C-1.5.1.2.1 Reports Requirements

Each operations report will use a form if so specified in TE 1. Otherwise, operations reports will include the minimum essential information that the Government would need to act on the

Contractor’s report. In almost all cases not requiring a form, a clear, concise letter report from the contractor is sufficient.

C-1.5.1.2.2 Types of Reports

TE 1 lists required Contractor-prepared operations reports. See also TE 4, Contract Data

Requirements Lists for CDRLs that cover specific operations reports.

C-1.5.2 Personnel

C-1.5.2.1 Supervision

The contractor shall provide a project manager who shall be responsible for performance of all requirements prescribed herein. The project manager shall ensure that contractor personnel use procedures that comply with the Contractor’s ISO procedures AND conform to the FRC East’s

IMS in performance of their duties under this contract.

The contractor shall provide a designated on-site manager. The on-site manager shall act for the contractor and shall be responsible for the day-to-day performance of the work, serve as a point of contact for all customer relations, and be responsible for resolving all quality, timeliness, and accuracy issues. The contractor shall also designate sufficient alternates to the on-site manager to serve in his or her capacity when the on-site manager is absent.

The contractor shall provide a means of immediately contacting the on-site manager and alternates

7 days a week, 24 hours per day, 365 days a year.

The Contractor’s on-site manager shall attend any meetings determined necessary by the COR or the Contracting Officer to discuss performance under the award or resolution of any performance problems. The contractor shall ensure that the on-site manager is available for such meetings with not less than 1-hour notice in advance. The COR will notify the on-site manager by telephone of such meetings.

Additionally, the on-site manager shall be familiar with the U.S. Navy (USN) policies and procedures governing fraud, waste, and abuse (see Section C-6). The on-site manager shall monitor contractor employee activities for such actions or behaviors and take necessary actions to prevent or correct such occurrences. Additionally, the on-site manager shall notify the COR immediately upon observance of behavior constituting fraud, waste, or abuse by contractor or

Government employees.

C-1.5.2.2 Personnel Experience

The contractor shall furnish adequate numbers of qualified personnel to ensure satisfactory performance of the services required by this PWS. Contractor personnel shall be able to speak, write, read, and understand English, including any unique terminology used in their functional specialties.

C-1.5.2.3 Minimum Operator Qualifications

Contractor personnel operating transportation equipment in the performance of services under this contract shall be capable of:

- Lifting up to 40 lbs. without assistance

- Walking, stooping, and bending for a considerable portion of their work shift

All contractor operators must comply with the licensing requirements of the State of North

Carolina and any local, state and federal laws. Operators of 10/10 preservation oil trucks

(“10/10 trucks”), low boys and vehicles able to carry 15 or more passengers shall hold a valid NC

Commercial Driver’s License (CDL) with appropriate endorsements (a HAZMAT endorsement for 10/10 truck operators). Contractors operating government provided equipment in performance of this contract shall be certified, by the contractor and at the contractor’s expense, as being fully qualified to operate the vehicles/equipment to which they are assigned. 10/10 truck operators specifically shall be trained by the contractor at the contractor expense in accordance with Part III of NAVAIR 00-80T-109 chapter 8, section 8.2.1 and 8.2.2. The contractor does not perform nozzle operations; therefore the training requirements of 8.2.3 do not apply. Operators of all equipment shall be able to operate a handheld VHF radio. Note: The Government will license contractor operators for equipment for which Government-specific operating requirements or training is required in accordance with FRCEASTINST 4790.17_ (i.e. all Ground Support

Equipment). The contractor shall be responsible for submitting required documentation and request for training for individuals requiring certification or licensing in accordance with

FRCEASTINST 4790.17_ in sufficient time to ensure re-certification occurs prior to expiration and that adequate certified/licensed personnel are available to perform the work at all times.

The prime contractor shall document all operator qualifications.

C-1.5.2.4 Standards of Conduct

The contractor shall not employ any person whose employment under this contract could in any way result in a conflict of interest with the mission of the FRC East. All personnel employed by the contractor in the performance of this contract, or any agent of the contractor entering the

Government installation shall obey all regulations of the MCAS and FRC East.

The contractor shall be responsible for employee competency and conduct and for taking disciplinary actions with respect to their employees. The removal from the job site of contractor personnel shall not relieve the contractor of the requirement to provide personnel to perform the specified tasks outlined in this PWS.

The Government reserves the right to deny access to any contractor employee to enter the FRC if his or her presence would be detrimental to the safety and security of the FRC East and its workforce, or if the contractor employee does not meet the minimum qualifications outlined herein.

The Government reserves the right to remove any contractor employee from the job site who knowingly and willfully endangers persons or property. The Government also reserves the right to deny access to FRC East to former government employees who were terminated from employment for disciplinary reasons. In such cases, the COR will advise the contractor of the reason for the removal or withdrawing his or her authorization to enter the installation.

C-1.5.2.5 Dress Code & Grooming Standards

The contractor shall ensure that all personnel maintain a standard of grooming and personal appearance that is in keeping with their positions in a Government facility and shall be appropriate to traditional Government industrial complex. FRCEAST NOTICE 12000_ outlines standards of dress, grooming, and character. Eccentricities or extremes in dress or appearance are unacceptable.

C-1.5.2.6 Use of Drugs & Alcohol

The contractor shall establish a Drug Fee Workplace Program IAW FRCEASTINST 5355.1_.

While on Government property or performing elsewhere under this PWS, contractor personnel shall not:

Possess or use alcoholic beverages

Possess, use (unless specifically prescribed by a licensed physician for medical treatment), or distribute controlled substances

The contractor shall (and the FRC East reserves the right to do the same) remove from duty any contractor employee who is:

Incapacitated

Under the influence of alcohol, drugs, or other incapacitating agents

Distributes controlled substances

The contractor shall not return that employee to duty unless and until he or she is fully able to perform normally assigned duties. Illicit use or distribution of controlled substances may lead to prosecution. Removal of employees for such causes does not relieve the contractor of the requirement to provide sufficient personnel to adequately perform services specified under this contract.

All FRC East personnel, including contractor personnel, are subject to urinalysis testing by the

Government.

C-1.5.2.7 APP: Personnel & Staffing

The Personnel and Staffing Section of the APP will include the information described below (See

Section C-1.5.1. and TE 4, CDRL A001.)

C-1.5.2.7.1 Organization Chart

The contractor shall furnish to the COR a copy of the contractor’s organizational chart as currently planned to staff the requirements of this contract. The chart shall include the names, addresses, and on-site and emergency (after hours) telephone numbers of the on-site manager and alternates.

The contractor organizational chart shall also identify all other significant personnel and their roles within the Contractor’s organizational structure. The contractor shall provide the COR an updated chart within 7 calendar days after changes are made. (See TE 4, CDRL A003.)

C-1.5.2.7.2 List of Employees The contractor shall provide the COR a complete list of all employees who will perform under this contract. The list shall contain the Contractor’s full name, job/labor category, work assignment, applicable licenses and endorsements, and work site. The contractor shall notify the COR, in writing, of any addition, deletion or changes to the list of employees within 7 calendar days of date of change. (See TE 4, CDRL A004.)

C-1.5.2.7.3 Proof of U.S. Citizenship

All of the Contractor’s personnel employed in performance of this award shall be U.S. citizens.

Proof of U.S. Citizenship may be required by MCAS or FRC East Security.

C-1.5.2.8 Prerequisite Training

Employees proposed by the contractor shall have all requisite training, experience, and state licenses and endorsements to perform successfully the work by the time commencement of performance is required. In addition, the contractor shall ensure that employees are trained sufficiently in environmental protection and safety to ensure compliance with all Federal, State, and local laws and Department of the Navy regulations required in performing this PWS.

C-1.5.2.9 Security Clearances

Unless otherwise stated, contractor personnel do not need access to classified material. However, within 30 calendar days of award, the contractor shall review and comply with requirements in

FRCEASTINST 5500.1_, Security Manual concerning contractor employees who may need access to restricted areas, sensitive information, and sensitive unclassified system operations.

C-1.5.3 Operating Hours

C-1.5.3.1 Normal FRC East Operating Hours

Normal FRC East operating hours is Monday through Friday from 0630-1530 (i.e., regular day shift hours or 1st shift) and from 1530-2345 (i.e. night shift regular hours or 2nd shift). The contractor shall provide the services defined in this PWS during normal FRC East operating hours.

Please note that the Contractor may find it necessary to provide a limited number of personnel prior to the normal 1 st Shift starting time to prepare equipment for the services to start at the beginning of the shift, and after 2 nd shift to prepare equipment for the following day’s operations.

For informational purposes, the majority of the services will be required on the 1 st shift (~75%).

The remainder of the services will be required on 2 nd shift (~17%) and After Hours/3 rd shift (~8%)

(Refer to C-1.5.3.3 for a description of After Hours Work).

C-1.5.3.2 Federal Holidays and FRC East Shutdown

The FRC East is closed on the following Federal holidays: New Year’s Day, Martin Luther

King’s Birthday, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving Day, and Christmas Day. In the event that any of the above holidays occur on a Saturday or Sunday, then such holiday shall be observed on the same day as

FRC East employees. Contractor employees will not work on Federal holidays.

In addition, closures due to an Executive Order or an administrative leave determination will also prevent the contractor from working.

Closures of the FRC East resulting from the above will not impact the firm fixed price per month to be paid to the Contractor.

FRCEASTINST 3140.1_, Destructive Weather Manual, governs MCAS and FRC EAST closure for bad weather. Personnel can call 252-464-8333 or 252-466-2911 for information on closures.

Local radio and TV stations also may broadcast bad weather notification. If the FRC is shut down, the Government will not normally require the contractor to work that day or for the next shift. However, under circumstances covered by the Destructive Weather Manual, the

Government may require the contractor to continue working to report for work, regardless of shut down. If the FRC has been shut down, the contractor shall contact the COR to determine if the contractor is required to report to work that day or for the next shift. The COR will address the expected duration of work and work requirements the contractor will need to accomplish.

In the event of early dismissal, the contractor shall comply with general requirements for occupants, activities, and Departments on board MCAS/FRC EAST under FRCEASTINST

3140.1_, Destructive Weather Manual, (e.g. securing spaces, material, and equipment; unplugging electrical equipment; etc.) and provide transportation services to support Activities and

Departments on-board in complying with this instruction. This may include any of the work required by the PWS and this instruction.

Unless circumstances dictate otherwise, the contractor shall follow the normal work procedures in this PWS while performing under the Destructive Weather Manual. The Government will release contractor personnel as soon as practicable and as appropriate to ensure the safety and security of their personnel.

C-1.5.3.3 FRC East Operating Hours, After Hours Work

In performance of this award, the COR may specifically require the contractor to perform work after hours to support FRC East operations. After hours are other than normal FRC East operating hours. After hours include Third Shift, Saturdays, Sundays, and Holidays. Such services may include any or all of the requirements in this PWS. For informational purposes, After Hours Work represents roughly 8% of the total effort. COR approval is required for any after hours work with the contractor in advance.

The pricing for After Hours work is based on a firm fixed price per “day” (man day)(refer to contract CLIN for After Hours support). The contractor may only invoice for a half day or full day(s) or work. A half day is defined as at least 4 hours of work performed on any given day.* A full day is 8 hours of work performed on any given day.**

*One 15 minute break is included. NOT included are the following: half hour for lunch, or time spent by contractor employee in a leave status.

**Two 15 minute breaks are included, plus half hour for lunch.

C-1.5.4 Interaction with Other Organizations

C-1.5.4.1 Customer Liaison

The COR is the designated liaison among internal and external FRC customers and the contractor for oversight of project requirements, schedules, and for delivery of products to customers.

C-1.5.5 Quality Assurance & Quality Control Plans

C-1.5.5.1 Quality Assurance and Quality Surveillance

This PWS identifies acceptable performance levels, including quality and schedule standards. The

Contractor will be responsible for the quality of the services provided to the Government in accordance with the Quality Control Plan (QCP). The QCP must include a detailed process for monitoring and documenting the performance requirements and standards identified in this PWS.

The contractor will be required to submit a Quality Control Plan within ten 10 days of the contract effective date, in accordance with CDRL A006.

The COR or Contracting Officer will notify the contractor of any required changes, or acceptance of the QCP within 10 calendar days of receipt. If modifications are required, the Contractor shall make them at no additional cost to the Government, and submit the revised QCP to the COR within 7 calendar days of receiving the notification. The contractor must obtain approval of the

QCP by the Government before submission of the first payment request. Failure to do so in no way excuses the contractor from the obligation to perform. During the period of performance, the contractor must submit QCP changes to the COR for concurrence within 10 calendar days of change implementation. The contractor shall perform all inspections and verifications necessary to substantiate conformance and shall perform all inspections otherwise required.

In addition to the QCP, the contractor must also submit Monthly Quality Reports, in accordance with CDRL A005 and PWS Section C-5.1.3. The Contractor’s QCP shall list the contents of the

Quality Report. The Quality Report shall include, at a minimum, a listing of each of the Tasks and

Sub-Tasks defined in PWS Section C-5, broken down to the CLIN Level. The Quality Report shall list the Quantity of each Task and sub-task performed, the Acceptable Quality Level (AQL) for each Task and Sub-Task, and the Quantity and percentage of times that the AQL was met.

C-1.5.5.2 Quality Control (QC)

The contractor shall plan and implement a QC program. The QC program shall include the following:

An inspection system (for services rendered)

Provisions to avoid organizational and personnel-related Conflicts of Interest between

PWS requirements and other contractor activities or its personnel

Description of records and availability for review

Material and equipment accountability

Methods for identifying, correcting, and preventing defects in services performed

Monthly Quality Report

C-1.5.5.3 Performance Evaluation Meetings

The COR will schedule the periodic performance evaluation meetings with the Contractor’s site manager. For these periodic meetings, the COR will notify the contractor no less than 2 working days in advance.

C-1.5.6 Security & Preparedness

C-1.5.6.1 Base Regulations

All contractor personnel employed in performance of this contract, or any representative of the

Contractor, entering the MCAS, FRC East, or Government spaces off-site shall abide by the

Security/Access Requirements identified in the “Special Contract Requirements” section of the solicitation and resulting contract.

C-1.5.6.2 Security and Safety Indoctrination

The Government will provide security and safety indoctrination training for new contractor personnel within 30 calendar days after their arrival. This training will include general security and safety education, operations security (OPSEC) awareness, and information systems security.

The Government does not envision that the contractor will need access to classified information in the performance of its work. The security indoctrination is to ensure that if contractor employees inadvertently gain access to classified or sensitive information, they will know how to safeguard it.

C-1.5.6.3 Badges & Passes

The Government shall exercise full and complete control over granting or denying employee badges and passes. The contractor shall account for all Government passes and security identification badges issued to contractor personnel. The contractor shall ensure that all departing contractor personnel turn in passes and employee badges to FRC East Security by close of business on the employee’s last day of work.

Refer to “Special Contract Requirements” section of the solicitation and resulting contract for more information on badges and Common Access Cards (CAC).

C-1.5.6.4 Physical Security

The Government currently provides for the safety of its employees and for security at the FRC

East facilities. However, the Government is not responsible for any loss or damage to the

Contractor’s and contractor staff’s equipment, belongings, or persons, nor is the Government responsible for any costs that result from such loss or damage.

The contractor shall safeguard all Government property in work areas and secure all Government material in the Contractor’s possession when not under direct physical control of contractor employees.

The contractor shall insure that its employees do not remove Government property from the FRC

East or adjacent and outlying areas without the written permission of the COR. The Government considers the Contractor’s failure to comply with this requirement to be cause for employee removal in addition to any other actions the Government may take.

The contractor shall establish and implement methods of ensuring that all keys and keycards issued to the contractor by the Government are not lost, misplaced, or used by unauthorized persons. The contractor will not duplicate keys issued to the contractor by the Government. The contractor shall report the occurrence of a lost key or key card immediately to the COR and FRC

East Security.

The contractor shall prohibit the use of keys issued by the Government by any persons other than the Contractor’s employees. Also, the contractor shall prohibit the opening of locked areas by the

Contractor’s employees to permit entrance of persons other than Contractor’s employees engaged in the performance of assigned work in those areas.

The contractor shall establish and implement methods to ensure that its employees do not reveal any lock combination to unauthorized persons.

Certain restricted areas, identified as such, within the confines of the FRC-East are “Off Limits” to all personnel not assigned to work in those spaces.

Contractor employees shall not loiter in any work area of the FRC East. Contractor personnel will depart the FRC East within 30 minutes of completion of their assigned shift or duties. If work outside of normal hours is required by the COR, the contractor shall notify FRC East security of their need to remain in the workspace.

C-1.5.6.5 Fire Prevention

The contractor shall become familiar and comply with FRC East fire prevention and reporting in

FRCEASTINST 11320.1_, Fire Protection Plan. The contractor shall be cognizant of and observe all requirements for handling and storage of combustible supplies and material in accordance with this instruction.

C-1.5.6.6 Disaster Preparedness

The contractor shall take all actions as required by FRCEASTINST 3140.1_, Destructive Weather

Manual. This includes, but is not limited to:

Making all preparations for scheduling affected contractor personnel

Making contractor personnel available for work and aware of their responsibilities in case of a declared disaster

C-1.5.7 Safety and Health

C-1.5.7.1 Safety

Safety, Environmental, Quality, and Security Requirements: All personnel who enter or work in assigned spaces of this facility are required to comply with all federal, state, local, and site specific safety, environmental, quality and security requirements. FRC East has adopted and implemented additional safety programs to improve safety, quality, environmental, and security measures. Any individual found to not comply with this effort will be asked to immediately halt work.

Supervisors, managers, and employees are to report issues of non-conformance through the proper chain of command. Any individual or entity not willing to comply will state such and shall not be authorized to enter the facility. FRC East may take actions that result in removal of individuals or entities from this facility due to issues of non-compliance with stated instructions.

Documented proof of all required certification training/licensing listed in the contract shall be available upon request by their Contracting Officer Representative or any auditor.

All personnel working on site at the FRC East under this contract shall document their receipt and review of the information listed in reference (a) annually.

Foreign Object (FO) is defined as any article or substance alien to the aircraft or assembly which is allowed to invade the product. Foreign object damage (FOD) is the damage that occurs due to these FOs. All FRC East work sites shall be maintained in such a manner as to prevent FOD to aircraft and/or aircraft components. Work sites shall be kept clean at all times. All debris, scrap material, tools, and equipment will be cleared from the work site as work progresses. At no time shall hoses, power cords, materials, etc. be permitted to create tripping hazards in areas of the work site.

In those cases where a contractor supervisor determines that solving a safety or health problem is beyond their control, but within the control of the FRC East, the contractor shall notify the appropriate point of contact identified in the contract.

Contractor supervisors shall take the FRC East Environmental Management System Annual

Refresher and convey the information to their personnel annually.

Work being performed is in critical areas, therefore constant control of tools and materials are required at all times. Tools and hardware will be controlled at all times to prevent migration out of the work site. Lost tools shall be reported to the tool control manager at 252-464-9741. Tools found unattended will be confiscated and reported to the contracting officer.

The contractor shall report completion of required training as well as area/task specific training by nested contractors. Information on completion of this training will be forwarded to the FRC East shop supervisor of the area where the contractor is nested within. A nested contractor is defined as a contractor that has a payroll number and performs/clocks work in the same manner dictated of permanent shop employees.

Gasoline is prohibited for any purpose other than fueling motor vehicles. All gasoline-powered vehicles/equipment are prohibited inside the FRC East buildings. In addition/ diesel or propane-powered equipment or vehicles used inside must have an approved Exposure Control Plan in place before work can begin. Approval of this Exposure Control Plan is accomplished through the contractors Accident Prevention Plan, reviewed by FRC East Industrial Hygiene, 252-464-7037.

The work on this document is to take place in a Class I Division 2 location up to a level of 18 inches above the floor. Items powered by electric motors or internal combustion engines that are not marked as approved by the manufacturer (DS, EE, DY, DX, EX, LPS, GS, CNC, GS/CNC or

GS/LPS) will not be permitted for use.

The work is to take place in a Class I Division 1 hazardous area. Items powered by electric motors or internal combustion engines that are not marked and approved by the manufacturer as DX or

EX will not be permitted for use.

The contractor shall maintain a safe work environment in FRC East work areas in accordance with

FRCEASTINST 5100.2_, Occupational Safety, and Health Manual and other requirements as set forth in this contract. If the contractor fails or refuses to promptly comply with safety requirements, the Government may issue an order stopping all or part of the work until the contractor takes satisfactory corrective action. The contractor shall not make time lost due to any such stop work order the subject of claim for extension of time or for excess costs or damages.

The Contractor’s equipment operators may be required to enter shop areas where hazards exist which require the use of safety glasses, safety shoes, and hearing protection. Operators will have regular and recurring exposure to moderate discomforts from non-toxic fumes, high noise levels, machinery with moving parts, etc. associated with an industrial environment.

Occupational Safety and Health Administration (OSHA) Voluntary Protection Program (VPP)

The FRC East is participating in the OSHA VPP.

All contractors performing work who have 10 employees at any time during the past calendar year, whose employees worked 1,000 or more hours in any quarter shall furnish FRC East their Total Case Incidence Rate for recordable nonfatal injuries and illnesses and their Days

Away from Restricted work activity, and/or Job Transfer Rate for the past three years. The

COR may be contacted to determine where in FRC East this data is to be submitted.

C-1.5.7.2 Mishap Reporting

The contractor shall report all mishaps (safety, security or environmental violation) related to the performance of this Contract to the FRC Safety/Security/Environmental Office, their contractor supervisor, and the COR and QAE/TPOC immediately after dialing 911 (if required) to report the mishap. The initial FRC-East notification can be made via phone or email and should include as many applicable details as possible (date, time, identification numbers, tags, company, etc.).

Within one (1) working day of the mishap, the contractor shall prepare and submit a Mishap

Report Form, FRCEAST CP 5102/1 to the Occupational Safety and Health Office and the COR.

The contractor shall submit this report for any incident that involves employee injury, no matter how minor or any “near miss”, where there could have been injury. Within 24 hours of a mishap involving property or material damage, the contractor shall prepare and submit a Property Damage report Form, FRCEAST CP 5100/2A to the Occupational Safety and Health Office and the COR.

Chapter 6 of FRCEASTINST 5100.2_ further explains the procedures the contractor shall follow in case of a mishap. (See TE 4, CDRL A007.)

Control of Mishap Scene

In case of accident or mishap, the contractor shall take control of the scene to prevent further injury to persons or damage to property. The contractor shall also preserve evidence until released by the accident or mishap investigative authority or commander on the scene. The contractor shall, if the Government elects to conduct an investigation of the accident or mishap, cooperate fully and assist Government personnel in the conduct of such investigation until the investigation is complete.

C-1.5.7.3 APP: Safety & Health

The contractor shall plan and implement a safety and health program. The contractor shall submit its Safety and Health Plan to the COR. The contractor shall maintain and update the plan throughout the performance of this award (See Section C-1.5.1. and TE 4, CDRL A008.)

The Contractor’s Safety and Health Plan shall include:

Standards and Codes — Identification of and provisions for compliance with applicable safety standards and codes.

Occupational Safety and Health (OSH) Inspections — Provisions for granting access immediately and at reasonable times to OSH officials for inspections. The contractor is subject to enforcement authority by Federal, State, and local safety and health officials.

Environmental Requirements — Provisions for compliance with all applicable OSHA regulations, Federal, State and local laws or permits that govern the Contractor’s work and DoD, DoN, MCAS, and FRC environmental requirements and instructions. The contractor shall conduct all tasks in accordance with applicable requirements.

Notification of Environmental Spills — Provisions for notifying the proper authority if the contractor spills, releases, or observes anyone who spills or releases any hazardous substance into the environment.

Safety and Fire Hazard Requirements — Compliance with all FRC East fire health and safety regulations. Base fire prevention personnel may make periodic routine inspections for regulation compliance. The contractor shall not burn any material on base.

Safety Equipment — Provisions for appropriate toe, head, eye, and ear safety equipment.

C-1.5.7.4 Emergency Medical Care

The contractor is responsible for the protection and treatment of contractor employees suffering on-the-job illness or injury. Only emergency medical care is available in Government facilities to contractor employees who suffer on-the-job illness or injury. The Government may, at its option, require reimbursement from the contractor employee or the Contractor, for any emergency medical care.

C-1.5.8 Vehicle & Equipment Operations

The contractor shall operate motor vehicles and equipment in any area serviced by the contractor under the requirements of this PWS in accordance with applicable laws and regulations. After calling “911” if necessary, contractor personnel shall report all vehicle and equipment accidents associated with performance of work under this PWS to FRC Security and Safety personnel and the COR as soon as safety allows.

Before any contractor personnel operate any assigned equipment he or she shall conduct an operational check of the equipment in accordance with FRCEASTCP Form 11200/2 (Rev 1/91).

For 10/10 trucks specifically the contractor shall perform daily checks prior to use in accordance with the NAVAIR 00-80T-109 Chapter 13 and complete the daily checklist of figure 13-1 in the

NAVAIR 00-80T-109. Samples shall be taken and inspected in accordance with NAVAIR 15-01-

500 Preservation of Naval Aircraft. The 10/10 truck daily checklist shall be signed off by the person performing the inspection and their supervisor or relief supervisor. The Government will train and annually certify personnel assigned to operate vehicles and equipment on the airfield in operation and runway protocol of hand-held radios. Typically, the one-time training takes 1 to 2 hours.

C-1.5.9 Preventive & Corrective Maintenance

The Government will furnish preventive maintenance (PM) and Corrective Maintenance (CM) of

Government-furnished equipment (GFE). The government will be responsible for scheduling PM checks, PM and CM as required on assigned GFP.

Using Government furnished materials, the contractor shall perform equipment servicing and minor maintenance/repair on Government-provided equipment as required in Section C-5 and normal operator maintenance/servicing on office-type equipment (such as toner, imaging unit and transfer roller replacement) provided for use. The COR or QAE/TPOC may grant exceptions to this requirement in instances where the equipment is beyond economical repair or the repair is beyond that specified as required by the Contractor. Upon notification from the COR or

QAE/TPOC, the contractor shall prepare, crate or palletize, load, deliver, and off-load such equipment to a location designated by the COR or QAE/TPOC for disposal or repair.

C-1.5.10 Relationship & Liabilities of Parties

It is expressly agreed and understood that the contractor renders its services in its capacity as an independent Contractor. The contractor shall be solely responsible for any liability producing act or omission by it or its employees or agents.

C-1.5.11 Chain of Custody Requirements

In the performance of this PWS, contractor employees shall NOT sign the DD Form 1348 or any other document indicating the condition of nor the acceptance of goods or services transported and delivered. Contractor employees may be required (for example, deliveries to building 147) to sign a log book upon completion of delivery. The sole purpose and use of the logbook by the

Government is to indicate the date/time goods were delivered,…
