About this file

This document is an Amendment (Amendment No. 4) to a Mandatory Use Contract issued by the State of Ohio's Department of Administrative Services (DAS), specifically for the State of Ohio's Department of Rehabilitation and Correction (DRC) and the Department of Youth Services (DYS). The contract is with Mid America Health of Ohio, Inc., and pertains to a Statewide Comprehensive Dental Services contract with the original contract number CSP906718, initially effective from 07/01/18 to 06/30/21.

The amendment extends the contract for an additional one month, from 07/01/25 through 07/31/25, with all other prices, terms, and conditions remaining unchanged. The contract is a requirements contract that allows the Ohio DRC and DYS to purchase dental services as needed, with no guaranteed volume commitment. The contract has undergone multiple previous amendments, including renewals and price list updates, with this fourth amendment being a short-term one-month extension. The full contract and amendments are available on the DAS website at http://www.ohio.gov/procure.
