Acquisition Plan Action ID: 36C248‐21‐AP‐2801

1. Contracting Activity: Department of Veterans Affairs, VISN 08, Orlando Veterans Affairs Healthcare System, Network Contacting Office 08, Commodities.

2237 Ref Number: 675‐21‐2‐2440‐0202

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Orlando VA Healthcare System, Cardiology Service requests a brand name Carto 3 Software and Hardware Upgrade for the Carto 3 System. Estimated Amount; $143,000.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

A previous market research on 06/25/2020 and 05/25/2020 in the Strategic Acquisition Center, General Supply Administration, Solutions for Enterprise‐Wide Procurement (SEWP) resulted in zero findings. No CARTO 3 System or software available on the National Acquisition Center (NAC). The original equipment manufacturer Biosense Webster belongs to Johnson &Johnson Medical Company. Biosense Webster has two SDVOSB vendors that are capable to provide the items.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106‐1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
