About this file
This document is a Certificate of Insurance Requirements form from the City of West Sacramento for standard city contracts. The form outlines comprehensive insurance coverage requirements for vendors and contractors, specifying mandatory insurance types including Commercial General Liability ($1,000,000 per occurrence), Commercial Automobile Liability ($1,000,000 per accident), Workers' Compensation (per statutory requirements), and Professional Liability ($1,000,000 per claim) if applicable.
The insurance requirements mandate that all policies except Workers' Compensation and Professional Liability must name the City of West Sacramento as an additional insured, with a required Additional Insured Endorsement. Vendors must provide a copy of the endorsement with their Certificate of Liability Insurance, and all certificates must include a 30-day written notice provision for any cancellation or material changes. The document also requires specific endorsements such as Primary and Non-Contributory coverage, Separation of Insureds, and a Waiver of Subrogation for Workers' Compensation, ensuring comprehensive protection for the city and its employees across various potential liability scenarios.
