About this file
This is a Certificate of Insurance Requirements checklist issued by the City of West Sacramento applicable to standard city contracts. The document outlines mandatory insurance coverage specifications that all contractors and vendors must maintain throughout the contract term. Required coverage includes Commercial General Liability ($1,000,000 per occurrence/$2,000,000 aggregate), Commercial Automobile Liability ($1,000,000 per accident), Workers' Compensation and Employers' Liability per statutory requirements, and Professional Liability/Errors & Omissions ($1,000,000 per claim) if applicable. All policies except Workers' Compensation must name the City of West Sacramento as an additional insured, with a copy of the Additional Insured Endorsement attached to the Certificate of Insurance to verify proper endorsement; failure to provide this documentation will result in an incomplete certificate.
The certificate must include specific endorsements for both General Liability and Automobile Liability coverage, specifically Additional Insured Endorsements equivalent to CG 2010 0714 naming the City, its officers, officials, employees, agents, and volunteers; Primary and Non-Contributory Endorsements stating the insurance is primary and non-contributory; and Separation of Insureds provisions ensuring the Consultant's insurance applies separately to each insured. Workers' Compensation policies must include a Waiver of Subrogation Endorsement in favor of the City, its officers, agents, and employees. All certificates must provide thirty days' written notice to the Certificate Holder prior to cancellation or any material change in coverage. Contractors should forward this checklist to their insurance agent or broker to ensure compliance with these requirements.
