About this file

This document is an Affirmation and Disclosure Form from a state contract for tree and brush services, specifically involving the Ohio Department of Transportation (ODOT). The form is completed by A Woman's Touch Tree Care, a contractor based in Columbus, Ohio, and addresses compliance with Executive Order 2022-02D regarding services performed outside the United States, particularly emphasizing prohibitions on services from Russian companies or institutions.

The contract covers tree and brush power shearing, trimming, mulching, and removal services across multiple ODOT districts in Ohio, including Districts 1-12 and Central Garage in Columbus. The vendor will be required to provide services in all counties within the districts they bid on, with operating times based on an eight-hour day, five-day (40-hour) work week. ODOT will contact vendors to review proposed projects and request quotes, with vendors expected to begin services within five calendar days of notification. The contract includes environmental requirements, such as ensuring mulched debris does not create hazards and adhering to site-specific environmental conditions provided by ODOT.
