1.4 WORK COVERED BY CONTRACT DOCUMENTS
A. The Work of Project is defined by the Contract Documents and consists of the following:
1. This project serves to replace the existing standby diesel generator with a new standby diesel generator at a Federal Bureau of Prisons Federal Prison Camp. The project consists of removing the existing generator at the Powerhouse and installing the new generator next to the new switchgear installed under a separate contract and incorporating the generator into the existing control scheme. A load bank that is rated for 100% of the generator rating shall be provided. Site grading and locating of existing utilities in the area of work are also required for the installation of the generator. An access road for service of the generator shall be created. The generator shall be connected to an existing fuel oil tank with all new above ground fuel oil piping, pumps, filtration system. Status of generator and fuel oil system shall be tied into the main campus DDC system including integration into existing front-end with new graphics.
2. The project also serves to replace an existing 40 ton chiller and chilled water distribution pumps and piping in a housing unit with a larger 125 ton chiller sized to serve the entire building with all new primary / secondary pumping and insulated hydronic piping. Hydronic piping complete with all new hydronic specialties shall be connected to the existing AHU mounted chilled water coils.
The project consists of the removal of existing pumps, piping, electrical disconnects, wiring and conduit. The new chiller and chilled water pumping system shall be tied into the existing campus automation system including integration into the existing front-end with new / updated graphics.
B. Type of Contract:
1. Project will be under a single prime contract.
1.5 INTENTION OF DOCUMENTS
A. The Contract Documents require that the Contractor provide all design, details, materials, installation, and performance of related site, utility, and other required work for the project. The Contractor shall, without additional compensation, coordinate and join together all the various subdivisions of the work and provide a complete, operable installation, ready for its designed use.
1. BRAND NAME OR EQUAL: An “equal” product may be submitted. Should it be desired to submit an equal product, it is required to submit the equal product’s technical specifications with
011000 SUMMARY Page 3 of 6 the bid to be verified as an equal product. Failure to do so may result in determining this bid as non-responsive. The awardee will be required to document their product via the submittal process upon contract award.
B. Minor material and work not specified or shown, but necessary for proper completion of the work, shall be provided as specified or shown without additional compensation.
1.6 WORK UNDER SEPARATE CONTRACTS
A. General: Cooperate fully with separate contractors so work on those contracts may be carried out smoothly, without interfering with or delaying work under this Contract or other contracts. Coordinate the
Work of this Contract with work performed under separate contracts.
B. Preceding Work: Owner has awarded a separate contract for the following construction operations at
Project site. Those operations are scheduled to be substantially complete before work under this Contract begins.
1. Replace Switchgear: To Master Service to provide new medium voltage switchgear, generator controls modifications, and site pad prep.
1.7 INSPECTION BY REGULATORY AUTHORITIES
A. This project is subject to inspection by the following regulatory authorities:
1. U.S. Department of Justice, Federal Bureau of Prisons. The Federal Bureau of Prisons is the
Authority Having Jurisdiction for the work of this project.
1.8 ACCESS TO SITE
A. To ensure the work creates minimal disruption at the institution, the contractor shall submit a work sequence/schedule that demonstrates minimal interruption of normal operations. The normal work schedule for the Contractor is 6:00 AM - 4:00 PM, Monday - Friday, excluding Federal holidays.
Contractor shall secure work site and all tools by 3:30 each afternoon. Work hours and work days may be altered only with a 48 hour advance approval of the Contracting Officer.
Note: This is a correctional institution and delays could be encountered due to the following reasons:
1. Fog or any other weather related events.
2. Security concerns within the institution.
B. To comply with the Federal Information Processing Standard Publication 201 (FIPS 201) and Federal
Bureau of Prison security requirements, all contract workers must comply with PIV-1 (Personal
Identification Verification) which includes but is not limited to: a National Crime Information Center (NCIC) check and participating in institution orientation training prior to entering the secured areas of the institution.
C. Contractors will require staff escorts while on the job site at FPC Alderson. Therefore, close coordination will be required for minimal interruption of work.
D. All contractors will meet their escort and enter thru the construction entrance gate. An area outside the institution gate will be made available to the Contractor for a temporary field office, material staging, and parking. Electric is available for a job trailer.

E. All tools and materials brought inside the gates of the institution, but not permanently affixed to the building at the end of the work shift, must be removed to outside the gates of the institution overnight, unless specifically authorized by the Owner. Tools and materials allowed to remain shall be secured in approved gang boxes or steel shipping containers, and will be inventoried daily. Furnish one key to
Owner.
F. To minimize loss of productivity, Contractor’s personnel are encouraged to bring their lunch, and eat at the work site, rather than leaving the institutional grounds. However, if leaving for lunch, they will be escorted from the job site and checked out. The Contractor shall then coordinate with their escorts to be allowed re-entry into the institution.
G. Use of Site: Limit use of Project site to areas within the Contract limits indicated. Do not disturb portions of Project site beyond areas in which the Work is indicated.
1.9 COORDINATION WITH OCCUPANTS
A. Full Occupancy: The Government will occupy site and existing building(s) during entire construction period. Cooperate with the Government during construction operations to minimize conflicts and facilitate Government usage. Perform the Work so as not to interfere with Government's day-to-day operations. Maintain existing exits unless otherwise indicated.
1. Maintain access to existing walkways, corridors, and other adjacent occupied or used facilities. Do not close or obstruct walkways, corridors, or other occupied or used facilities without written permission from the Government and approval of authorities having jurisdiction.
2. Notify Government not less than seven days in advance of activities that will affect Government's operations.
1.10 WORK RESTRICTIONS
A. Work Restrictions, General: Comply with restrictions on construction operations.
1. Comply with limitations on use of public streets and with other requirements of authorities having jurisdiction.
B. Existing Utility Interruptions: Do not interrupt utilities serving facilities occupied by Government or others unless permitted under the following conditions and then only after providing temporary utility services according to requirements indicated:
1. Notify the Institution not less than seven days in advance of proposed utility interruptions. A temporary generator shall be provided by the Contractor for all shutdowns that are expected to last more than 3 hours.
2. Obtain Owner’s written permission before proceeding with utility interruptions.
C. Noise, Vibration, and Odors: Coordinate operations that may result in high levels of noise and vibration, odors, or other disruption to occupancy with the Government.
1. Notify the Institution not less than two days in advance of proposed disruptive operations.
D. Nonsmoking Facility: Smoking is not permitted within the Institution.

E. Controlled Substances: Use of tobacco products and other controlled substances on Project site is not permitted.
F. Contraband: The Contractor shall inform his employees that intoxicating beverages, drugs, weapons, etc., will not be allowed on the site. Introduction of contraband shall be subject to the penalties prescribed by Title 18 USC 1791.
G. Photography of the Work of the Contract by persons other than representatives of the Federal
Government is prohibited.
H. Cellular telephones are prohibited on the project construction site. .
I. Employee Screening: Comply with the Government's requirements for drug and background screening of
Contractor personnel working on Project site.
1.11 SPECIFICATION AND DRAWING CONVENTIONS
A. Specification Content: The Specifications use certain conventions for the style of language and the intended meaning of certain terms, words, and phrases when used in particular situations. These conventions are as follows:
1. Imperative mood and streamlined language are generally used in the Specifications. The words
"shall," "shall be," or "shall comply with," depending on the context, are implied where a colon (:)
is used within a sentence or phrase.
2. Specification requirements are to be performed by Contractor unless specifically stated otherwise.
B. Division 01 General Requirements: Requirements of Sections in Division 01 apply to the Work of all
Sections in the Specifications.
C. Drawing Coordination: Requirements for materials and products identified on Drawings are described in detail in the Specifications. One or more of the following are used on Drawings to identify materials and products:
1. Terminology: Materials and products are identified by the typical generic terms used in the individual Specifications Sections.
2. Abbreviations: Materials and products are identified by abbreviations scheduled on Drawings.
1.12 MISCELLANEOUS PROVISION
A. Chemicals:
1. Submit the Material Data Safety Sheet (MSDS) for all chemicals to be used at the Institution for approval by the Institution Safety Department.
B. Safety:
1. The Institution may conduct construction site inspections at any time for safety violations.

SECTION 011002 – PROJECT SECURITY
PART 1 - GENERAL
1.1 RELATED DOCUMENTS
A. Drawings and general provisions of the Contract, including General and Supplementary Conditions and other Division 01 Specification Sections, apply to this Section.
1.2 SUMMARY
A. Section includes:
1. Site access and security features of the project.
2. Protection of the project documents and other potentially sensitive information.
B. In order to reduce the potential for compromising the security of the correctional facility, or portions thereof, the Contractor shall:
1. Limit the availability of potentially sensitive information concerning design layout, security features and electronic devices and systems, and utility systems for the Work of the Contract.
2. Limit Sensitive But Unclassified - SBU project information to persons having a legitimate need-to-know.
3. Adhere to procedures developed by the FBOP and the Contractor to limit access to the Project Site to persons having official business.
4. Comply with security procedures required for contract employees to enter operating FBOP correctional facilities to perform project related work.
5. Utilize commercial encrypted messaging systems (SSL) when electronically transmitting Sensitive but Unclassified - SBU design data to other design/build team members who have a need-to-know.
1.3 DEFINITIONS
A. Sensitive But Unclassified - SBU: Building designs, floor plans, construction plans, specifications and details, equipment locations and types, information concerning utility service designs and layouts, and any other information considered a security risk.
B. Non-Sensitive Information: All project related information, including written and electronic media, not classified as Sensitive But Unclassified - SBU. The Contractor shall request and receive approval from the Contracting Officer prior to making project related information available to the public.
C. Reasonable Care: Limiting dissemination to authorized users. Dissemination of information shall only be made upon determining the recipient is authorized to receive it.
D. Need-To-Know: Architects, engineers, consultants, contractors, subcontractors, suppliers, and other government entities the Contractor deems necessary to perform the Work of the Contract.
E. Secure Areas: The area inside the security perimeter fence of an operational FBOP correctional facility and within 100 meters of the perimeter patrol road and any ancillary building.

F. Operational Institution: FBC Alderson is an operational institution.

PART 3 - EXECUTION
3.1 REQUIREMENTS FOR CONTRACT WORKERS ENTERING INTO SECURE AREAS AT
OPERATIONAL FBOP FACILITIES
A. The following procedures shall be implemented for entry of Contractor worker into secure areas at operational FBOP facilities.
1. A list of personnel shall be provided to the Contracting Officer for all Contractor workers who may have a need to perform work inside of secure areas.
2. Refer to the Solicitation Documents for specific information regarding entry requirements.
Background checks for all contract workers are required prior to receiving clearance to enter activated federal prison facilities. The background investigation is made in order to determine the appropriateness of the individual listed on the form to enter secure areas of an operational correctional facility and is a mandatory requirement for all work and site visits inside the secure facility perimeter.
3. The security check-out process will require the completion of an Authority to Release Information form, and a National Crime Information Check (NCIC).
4. The completed background check forms shall be hand delivered by the Contractor to the FBOP
Contracting Officer to initiate the check. Once the check has been completed and a decision made, the Contractor shall be informed of the personnel approved for entry. At this time employees shall be required to take an orientation class which will be longer than 4 hours but will not exceed 8 hours. In cases of background check disapproval, the FBOP shall only inform the
Contractor that entry approval has been denied, in compliance with the Privacy Act requirements.
5. The Contractor shall coordinate scheduled work inside the secure areas of the existing institution with the Construction Representative.
6. Contractor personnel shall wear clothes that are suitable for the work they are performing. The inmate population wears khaki and olive green clothes. For this reason, no Bureau of Prisons employees or contract personnel can wear these colors. No sleeveless shirts (tank tops), or camouflage clothing is allowed.
7. The contractor shall be constantly aware of security and maintain a professional relationship with the inmates at all times if they come in contact with the inmate population.
8. Contractors will not have unsupervised contact with inmates.
9. Contractors will not supervise inmates and will not be responsible for the administrative management of inmates.
10. If a contract worker recognizes an inmate, report it to your escorting staff immediately.
Recognition of an inmate must be documented.
11. Any violations of the institution rules and regulations are to be brought immediately to the attention of the appointed Government employee for prompt and proper action.
12. Contractors will be escorted by Federal Bureau of Prisons staff members, at all times.
3.2 KEY CONTROL PROCEDURES
A. The FBOP shall maintain control of all security keys. All keys must be accounted for and should be kept under good control management. Report any missing keys (including personal keys) to the staff escorting officer immediately.

3.3 TOOL INVENTORY/CONTROL PROCEDURES
A. Tool accountability is the responsibility of the contractor. All tools must be inventoried entering and exiting the work site and must be accounted for at the end of each day. Any broken tools shall be inventoried and all parts must be accounted for on each broken tool. Report any lost, missing or broken tools to the escorting staff immediately. Do not leave equipment and/or tools lying around the work site.
Don’t spread yourself or your work crew too thin that you lose good management of your equipment and tools. Bring only the items being utilized for that day’s work. Tools and equipment can remain on-site with the permission of the Security Officer and after inventory is complete in front of the Security
Officer. However, all materials and tools shall be stored in tool shed or shipping container with prison lock at the end of the day as directed by Owner.
3.4 WEAPONS
A. Weapons are strictly prohibited. No weapons are allowed on institution property, including stored weapons in contract workers’ personal vehicles. All vehicles entering the property are subject to search procedures. All contract workers shall be instructed to leave all weapons at their residence.
3.5 VEHICLES
A. All vehicles entering the grounds must be removed at the end of each work day. When vehicles are not being utilized or operated, the vehicle shall be turned off and the keys secured.
3.6 SECURITY REQUIREMENTS FOR SENSITIVE BUT UNCLASSIFIED - SBU INFORMATION
A. The Contractor shall maintain a secure location at the contract site where Sensitive But Unclassified -
SBU documents are stored. A secure location is an area under supervision of the Contractor’s employees that is not accessible to unauthorized persons. Secure storage facilities are required for Sensitive But
Unclassified - SBU documents when not attended.
B. The Contractor, subcontractors, and vendors shall provide secure storage for all Sensitive But
Unclassified - SBU construction documents for this project at their off-site places of business.
3.7 PHOTOGRAPHY
A. Photography of the Work of the Contract by persons other than representatives of the Federal
Government is prohibited, with the following exceptions.
1. Photographs required of the Contractor to comply with contract requirements, for digital records of construction progress necessary for monthly reports, or for issue resolution will be taken by
FBOP staff at the direction of the Contractor. Electronic media of the photographs will be made available to the Contractor.
2. Photographic records made of the construction or equipment installations showing Sensitive But Unclassified - SBU information shall not be made available to the public.
3. Use of other photographic records for dissemination in advertising, publishing and printing is prohibited unless prior approval is granted by the FBOP.
4. Cameras and cellular telephones equipped with photographic capabilities are prohibited on the project construction site.

3.8 RETAINING AND DESTROYING DOCUMENTS
A. Necessary record copies for legal purposes must be safeguarded against unauthorized use for the term of retention. Documents no longer needed shall be destroyed.
B. Destruction shall be accomplished by burning or shredding of hard copy, and/or physically destroying
CDs, deleting and removing Sensitive But Unclassified - SBU files from the electronic recycling bins, and removing material from computer hard drives using a permanent erase utility or similar software.
C. At Project Closeout for all contracts using Sensitive But Unclassified - SBU information, the Contractor shall submit a compliance certification letter to the FBOP certifying that all known copies of non-record plans and specifications containing Sensitive But Unclassified SBU building information have been located and disposed of in a manner that ensures information security. This certification extends to
Sensitive But Unclassified - SBU documents and electronic media issued to subcontractors.

PART 1 - GENERAL
1.1 RELATED DOCUMENTS

1.2 SUMMARY
A. Section includes administrative and procedural requirements for substitutions.
B. Related Requirements:
1. Section 016000 "Product Requirements" for requirements for submitting comparable product submittals for products by listed manufacturers.
1.3 DEFINITIONS
A. Substitutions: Changes in products, materials, equipment, and methods of construction from those required by the Contract Documents and proposed by Contractor.
1. Substitutions for Cause: Changes proposed by Contractor that are required due to changed Project conditions, such as unavailability of product, regulatory changes, or unavailability of required warranty terms.
2. Substitutions for Convenience: Changes proposed by Contractor or Government that are not required in order to meet other Project requirements but may offer advantage to Contractor or
Government.
1.4 ACTION SUBMITTALS
A. Substitution Requests: Submit three copies of each request for consideration. Identify product or fabrication or installation method to be replaced. Include Specification Section number and title and
Drawing numbers and titles.
1. Documentation: Show compliance with requirements for substitutions and the following, as applicable:
a. Statement indicating why specified product or fabrication or installation cannot be provided, if applicable.
b. Coordination information, including a list of changes or revisions needed to other parts of the Work and to construction performed by Government and separate contractors, that will be necessary to accommodate proposed substitution.
c. Detailed comparison of significant qualities of proposed substitution with those of the
Work specified. Include annotated copy of applicable Specification Section. Significant qualities may include attributes such as performance, weight, size, durability, visual effect, sustainable design characteristics, warranties, and specific features and requirements indicated. Indicate deviations, if any, from the Work specified.

d. Product Data, including drawings and descriptions of products and fabrication and installation procedures.
e. Samples, where applicable or requested.
f. Certificates and qualification data, where applicable or requested.
g. List of similar installations for completed projects with project names and addresses and names and addresses of Engineers and owners.
h. Material test reports from a qualified testing agency indicating and interpreting test results for compliance with requirements indicated.
i. Detailed comparison of Contractor's construction schedule using proposed substitution with products specified for the Work, including effect on the overall Contract Time. If specified product or method of construction cannot be provided within the Contract Time, include letter from manufacturer, on manufacturer's letterhead, stating date of receipt of purchase order, lack of availability, or delays in delivery.
j. Cost information, including a proposal of change, if any, in the Contract Sum.
k. Contractor's certification that proposed substitution complies with requirements in the
Contract Documents except as indicated in substitution request, is compatible with related materials, and is appropriate for applications indicated.
l. Contractor's waiver of rights to additional payment or time that may subsequently become necessary because of failure of proposed substitution to produce indicated results.
B. Engineer's Action: If necessary, Engineer will request additional information or documentation for evaluation within seven working days of receipt of a request for substitution. Engineer will notify Contractor of acceptance or rejection of proposed substitution within 10 working days of receipt of request, or five working days of receipt of additional information or documentation, whichever is later.
a. Forms of Acceptance: Change Order, Construction Change Directive, or Engineer's
Supplemental Instructions for minor changes in the Work.
b. Use product specified if Engineer does not issue a decision on use of a proposed substitution within time allocated.
1.5 QUALITY ASSURANCE
A. Compatibility of Substitutions: Investigate and document compatibility of proposed substitution with related products and materials. Engage a qualified testing agency to perform compatibility tests recommended by manufacturers.

1. Conditions: Engineer will consider Contractor's request for substitution when the following conditions are satisfied. If the following conditions are not satisfied, Engineer will return requests without action, except to record noncompliance with these requirements:
a. Requested substitution offers Government a substantial advantage in cost, time, energy conservation, or other considerations, after deducting additional responsibilities
Government must assume. Government's additional responsibilities may include compensation to Engineer for redesign and evaluation services, increased cost of other construction by Government, and similar considerations.
b. Requested substitution does not require extensive revisions to the Contract Documents.
c. Requested substitution is consistent with the Contract Documents and will produce indicated results.
d. Substitution request is fully documented and properly submitted.
e. Requested substitution will not adversely affect Contractor's construction schedule.
f. Requested substitution has received necessary approvals of authorities having jurisdiction.

PART 1 - GENERAL
1.1 SUMMARY

Requirements.
1.2 ENGINEER’S SUPPLEMENTAL INSTRUCTIONS
A. The Engineer of Record may issue supplemental instructions or interpretations to the Work not involving adjustment to the Contract Sum or the Contract Time.
B. A copy of all Engineer’s Supplemental Instructions (ESI) issued by the Engineer of Record shall be forwarded immediately to the FBOP by the Contractor.
C. ESI’s are intended to be utilized by the Engineer to provide clarifications and additional information concerning the accepted design drawings and documents. ESI’s are not to be used to make changes to the accepted design documents. Any proposed changes to the accepted design shall be reviewed and accepted by the FBOP prior to implementation. Design changes shall be incorporated into the Contract by use of the Contract Modification process listed in this section.
D. If the Contractor believes that a change in the Contract Sum, Contract Time, or accepted design is involved, the Contractor shall issue a Contractor Proposed Change (CPC) modification request.
1.3 PROPOSAL REQUESTS
A. Owner-Initiated Proposal Requests: Owner will issue a detailed description of proposed changes in the
Work that may require adjustment to the Contract Sum or the Contract Time. If necessary, the description will include supplemental or revised Drawings and Specifications.
1. Work Change Proposal Requests issued by Owner are not instructions either to stop work in progress or to execute the proposed change.
2. Within time specified in Proposal Request or 7 days, when not otherwise specified, after receipt of Proposal Request, submit a quotation estimating cost adjustments to the Contract Sum and the
Contract Time necessary to execute the change.
a. Include a list of quantities of products required or eliminated and unit costs, with total amount of purchases and credits to be made. If requested, furnish survey data to substantiate quantities.
b. Indicate applicable taxes, delivery charges, equipment rental, and amounts of trade discounts.

c. Include costs of labor and supervision directly attributable to the change.

B. Contractor-Initiated Proposals: If latent or changed conditions require modifications to the Contract, Contractor may initiate a claim by submitting a request for a change to Owner.
1. Include a statement outlining reasons for the change and the effect of the change on the Work.
Provide a complete description of the proposed change. Indicate the effect of the proposed change on the Contract Sum and the Contract Time.
2. Include a list of quantities of products required or eliminated and unit costs, with total amount of purchases and credits to be made. If requested, furnish survey data to substantiate quantities.
3. Indicate applicable taxes, delivery charges, equipment rental, and amounts of trade discounts.
4. Include costs of labor and supervision directly attributable to the change.

6. Comply with requirements in Section 012500 "Substitution Procedures" if the proposed change requires substitution of one product or system for product or system specified.

1.4 CHANGE ORDER PROCEDURES
A. On Owner's approval of a Work Changes Proposal Request, Owner will issue a Change Order for signatures of Owner and Contractor on Standard Form 30 (Amendment of Solicitation/Modification of
Contract).
1.5 CONSTRUCTION CHANGE DIRECTIVE
A. Construction Change Directive: Owner may issue a Construction Change Directive on Standard Form 30
(Amendment of Solicitation/Modification of Contract). Construction Change Directive instructs
Contractor to proceed with a change in the Work, for subsequent inclusion in a Change Order.
1. Construction Change Directive contains a complete description of change in the Work. It also designates method to be followed to determine change in the Contract Sum or the Contract Time.
B. Documentation: Maintain detailed records on a time and material basis of work required by the
Construction Change Directive.
1. After completion of change, submit an itemized account and supporting data necessary to substantiate cost and time adjustments to the Contract.

1.4 SCHEDULE OF VALUES
A. Coordination: Coordinate preparation of the schedule of values with preparation of Contractor's construction schedule.
1. Coordinate line items in the schedule of values with other required administrative forms and schedules, including the following:
a. Application for Payment forms with continuation sheets.
b. Submittal schedule.
c. Items required to be indicated as separate activities in Contractor's construction schedule.
2. Submit the schedule of values to Contracting Officer at FBOP at earliest possible date, but no later than seven days before the date scheduled for submittal of initial Applications for Payment.
B. Format and Content: Use Project Manual table of contents as a guide to establish line items for the schedule of values. Provide at least one line item for each Specification Section.
1. Identification: Include the following Project identification on the schedule of values:
a. Project name and location.
b. Name of Engineer.
c. FBOP's project number.

Provide multiple line items for principal subcontract amounts in excess of five percent of the
Contract Sum.
a. Include separate line items under Contractor and principal subcontracts for Project closeout requirements in an amount totaling five percent of the Contract Sum and subcontract amount.
4. Round amounts to nearest whole dollar; total shall equal the Contract Sum.
5. Provide a separate line item in the schedule of values for each part of the Work where
Applications for Payment may include materials or equipment purchased or fabricated and stored, but not yet installed.
a. Differentiate between items stored on-site and items stored off-site. If required, include evidence of insurance.
6. Provide separate line items in the schedule of values for initial cost of materials, for each subsequent stage of completion, and for total installed value of that part of the Work.
7. Each item in the schedule of values and Applications for Payment shall be complete. Include total cost and proportionate share of general overhead and profit for each item.
a. Temporary facilities and other major cost items that are not direct cost of actual work-in-place may be shown either as separate line items in the schedule of values or distributed as general overhead expense, at Contractor's option.
8. Schedule Updating: Update and resubmit the schedule of values before the next Applications for
Payment when Change Orders or Construction Change Directives result in a change in the
Contract Sum.
1.5 APPLICATIONS FOR PAYMENT
A. Each Application for Payment following the initial Application for Payment shall be consistent with previous applications and payments as certified by Engineer and paid for by Owner.
1. Initial Application for Payment, Application for Payment at time of Substantial Completion, and final Application for Payment involve additional requirements.
B. Payment Application Times: The date for each progress payment is indicated in the Agreement between
Owner and Contractor. The period of construction work covered by each Application for Payment is the period indicated in the Agreement.
C. Application for Payment Forms: Use AIA Document G702 and AIA Document G703 as form for
Applications for Payment.
D. Application Preparation: Complete every entry on form. Notarize and execute by a person authorized to sign legal documents on behalf of Contractor. Contracting Officer will return incomplete applications without action.

1. Entries shall match data on the schedule of values and Contractor's construction schedule. Use updated schedules if revisions were made.
2. Include amounts for work completed following previous Application for Payment, whether or not payment has been received. Include only amounts for work completed at time of Application for
Payment.
3. Include amounts of Change Orders and Construction Change Directives issued before last day of construction period covered by application.
4. Indicate separate amounts for work being carried out under Owner-requested project acceleration.
E. Stored Materials: Include in Application for Payment amounts applied for materials or equipment purchased or fabricated and stored, but not yet installed. Differentiate between items stored on-site and items stored off-site.
1. Provide certificate of insurance, evidence of transfer of title to Owner, and consent of surety to payment, for stored materials.
2. Provide supporting documentation that verifies amount requested, such as paid invoices. Match amount requested with amounts indicated on documentation; do not include overhead and profit on stored materials.
3. Provide summary documentation for stored materials indicating the following:
a. Value of materials previously stored and remaining stored as of date of previous
Applications for Payment.
b. Value of previously stored materials put in place after date of previous Application for
Payment and on or before date of current Application for Payment.
c. Value of materials stored since date of previous Application for Payment and remaining stored as of date of current Application for Payment.
F. Transmittal: Submit 5 signed and notarized original copies of each Application for Payment to
Contracting Officer by a method ensuring receipt within 24 hours. One copy shall include waivers of lien and similar attachments if required.
1. Transmit each copy with a transmittal form listing attachments and recording appropriate information about application.
G. Initial Application for Payment: Administrative actions and submittals that must precede or coincide with submittal of first Application for Payment include the following:
1. List of subcontractors.
2. Schedule of values.
3. Contractor's construction schedule (preliminary if not final).
4. Products list (preliminary if not final).
5. Submittal schedule (preliminary if not final).
6. List of Contractor's staff assignments.
7. Initial progress report.
8. Report of preconstruction conference.
9. Certificates of insurance and insurance policies.
10. Performance and payment bonds.
H. Application for Payment at Substantial Completion: After Engineer issues the Certificate of Substantial
Completion, submit an Application for Payment showing 100 percent completion for portion of the Work claimed as substantially complete.
1. Include documentation supporting claim that the Work is substantially complete and a statement showing an accounting of changes to the Contract Sum.
