About this file

The document is an Indian Affairs Indian Economic Enterprise Representation Form used for contracting opportunities under the Buy Indian Act. The form requires offerors to self-certify as an "Indian Economic Enterprise" (IEE) by providing details including the name of a Federally Recognized Tribal Entity, Unique Entity ID (UEI), legal business name, and the name of the 51% (or greater) owner of the IEE firm.

The form emphasizes that enterprises must meet IEE requirements at three key points: (1) when submitting an offer, (2) at contract award, and (3) throughout the contract term. Contracting Officers retain the right to request additional eligibility documentation at any stage. The document also warns that submitting false or misleading information is a violation punishable under federal law, with potential penalties under 18 U.S.C. 1001, 31 U.S.C. 3729-3731, and 18 U.S.C. 287.
