The Uniformed Services University of the Health Sciences (USUHS) is the Nation's federal health sciences university and is committed to excellence in military medicine and public health during peace and war. We provide the Nation with health professionals dedicated to career service in the Department of Defense and the United States Public Health Service and with scientists who serve the common good. We serve the uniformed services and the Nation as an outstanding academic health sciences center with a worldwide perspective for education, research, service, and consultation; we are unique in relating these activities to military medicine, disaster medicine, and military medical readiness. USUHS and the Armed Forces Radiobiology Research Institute (AFRRI), both located in Bethesda, MD, have a requirement for laboratory animal diagnostic services. These services were done in-house at USU in the past; however are now being contracted out. These services are also done in-house at AFRRI and will continue to be done there, but, on occasion, there may be a need for off-site coverage of some laboratory diagnostic work.

This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes (RFQ) for commercial items in accordance with the procedures of FAR Part 12 – Acquisition of Commercial Items and FAR Part 13 – Simplified Acquisition Procedures, to include FAR Subpart 13.5, Simplified Procedures for Certain Commercial Items. This RFQ incorporates all provisions and clauses in effect through Federal Acquisition Circular (FAC) 05-83 (August 5, 2015) and Defense Federal Acquisition Regulation Supplement (DFARS) (June 26, 2015).

In order to be eligible for award, offerors must be registered in www.SAM.gov, with completed representations and certifications.

The government intends to award, on a competitive basis, a single award Blanket Purchase Agreement (BPA) for the performance of various laboratory animal diagnostics services over a period of 5 years.

· The contractor will arrange and assume costs (Courier services) for sample delivery to laboratory for analysis

· Lab will be required to provide report no later than 72 hour after receipt of sample for all assays excluding bacteriology and environmental testing as a minimum 4 days is required for proper culture of most organisms and further identification of isolates can only be performed after the initial testing is complete

· Expedited analysis (STAT results, 24hour turn around) may be requested on a case-by-case basis, excluding bacteriology and environmental testing

· Email and/or Online access to results is required and should be made available to the point of contact on the order

· Contractor must provide technical feedback services, if there is a dispute with the report

· The test will be run on: ferrets, rodents (mouse, rat, hamster, gerbil, and guinea pig), rabbits, swine (minipig), and nonhuman primates

The government intends to award a five year, FFP, Blanket Purchase Agreement to the offeror representing the best value to the government based on the evaluation factors listed below. Evaluation factors are listed in order of importance, with most important listed first. The government will conduct a trade-off between price and non-price factors. The Government is not required to award to the lowest priced offeror or the highest technically rated offeror.

Offerors responding to this RFQ, must submit quotes in accordance with the page limitations listed for each evaluation factor. Offerors who submit quotes in excess of the page limitations listed for each evaluation factor, may not be considered for award.

Evaluation Factors (listed in order of importance):

Evaluation factors are listed in order of importance, with most important listed first. The government will conduct a trade-off between price and non-price factors. The government reserves the right to award without discussions.

Your price quote should include shipping to and from Bethesda, MD. The government will award a single firm-fixed-price (FFP) Blanket Purchase Agreement (BPA) resulting from this solicitation to the responsible offeror whose offer, conforming to this announcement, will be most advantageous and represent the best value to the government. Evaluation factors will be technical, price, corporate experience, and past performance:

1. Technical:

a. Submit test prices for at minimum the tests listed above and answers to questions Q1 – Q7 above, along with your price quote using the format provided above. The tests listed above will be used for price comparison purposes because they are the most commonly used tests by USUHS and AFRRI. However this does not limit USUHS and AFRRI to those tests. USUHS and AFRRI can add additional tests offered at any time.

b. The ability to accept GPC (Government Purchase Card) for payment is necessary. Purchase card holders located at USUHS will be authorized callers against the BPA. Please confirm your acceptance of VISA for payment terms.

c. Courier service – offerors must provide a description of the flexibility of their courier service. This contract serves two different entities (USU and AFRRI) with possibly different needs for pick up.

d. STAT results – offerors must provide information (price and speed) on the turn-around time for emergency reporting of results.

e. Availability of online access to results and/or emailed results

2. Price - Offerors must provide a unit price for each item listed on the attached spreadsheet but can provide unit prices for all offered services. Provide years two through five pricing if possible. The expected duration of this BPA is five years from August 2015 to August 2020. Updated pricing will become effective on the anniversary date of the initial award. Price adjustments will not be made at any other time during the performance period of the BPA. Any offeror who does not provide a unit cost for all items listed in the attached spreadsheet will not be considered for award. USU will evaluate price for the lab tests listed on the attached spreadsheet in a “shopping cart” manner.

3. Corporate Experience – Offerors must provide a narrative of their corporate experience in providing the requested service. Offerors must also provide a description of their technical feedback services, if there is a dispute with the reports submitted.

4. Past Performance - The Government shall evaluate the offeror’s present and past performance on similar tasks within the past three years. Offerors must provide two (2) customer references, for similar work performed. The Government reserves the right to select which reference(s) to contact. The following must be provided for each listed contract:

· Name of client and address

· Name of current point of contact including telephone number , e-mail address and title

· Contract number

· Contract value

· Contract period of performance

· A clear description of the supplies/services provided

DoD-wide Contractor Manpower Reporting Requirements:

The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for USU via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/.

Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October for the period of performance of the preceding fiscal year. Deadlines:

Questions/clarifications regarding this solicitation must be submitted via email to stephen.enokida@usuhs.edu by September 3rd. Any question received after September 3rd will not be answered. Questions will not be answered over the phone or submitted by any other means. All questions will be answered in an amendment to the solicitation posted on or around September 4th.

Quote packages are due by 3:00 PM Eastern Local Time on September 9th. Quotes must be submitted via email to stephen.enokida@usuhs.edu. Quotes will not be accepted by any other means. Late quotes will not be considered. Quote packages should include a cover sheet that provides the following information:

· Company Name

· Point of Contact, to include email and phone number

· DUNS Number

· Statement that includes acknowledgement of latest amendment number
