Fire Protection System Maintenance Services HSCG84-16-R-LAS020 Air Station Sacramento
ATTACHMENT 2
PERFORMANCE REQUIREMENTS SUMMARY (PRS)
1.The Government expects the Contractor to perform all services required in the Performance Work Statement (PWS) at an acceptable level for the Operation, Inspection Testing, Repair, and Maintenance of Contractor-Owned Fire Extinguishing and Alarm Systems Whenever the Government learns of substandard performance, either through incidental inspections, direct observation, customer complaints, or any other mechanism, the preferred course of action will be discussions with the Contractor to inform the Contractor of a problem. Discussions should effectively prevent recurrences of substandard performance. However, this Performance Requirement Summary (PRS) provides an alternative mechanism by which the Government’s payment to the Contractor may be reduced in the event of substandard performance.
2. Due to the life-safety nature of these services, a Maximum Allowable Defect Rate (MADR) of zero (0) defects has been established for some of the work requirements. If the number of defects observed during a month-long period is equal to or less than the MADR, then there will be no deduction in payments to the Contractor for that task.
However, if the number of defects observed during a month-long period is greater than the MADR, then there will be a deduction in payments to the Contractor for that task.

EXAMPLE OF PAYMENT COMPUTATION FOR SERVICES NOT
PERFORMED
Assume a MADR of 0 defects per month for Item 2 - Preventive Maintenance (Fire Extinguishing), a lot size of 20 maintenance items and that 4 defects were found by the
COR.
The payment would be as follows:
(1) Maximum contract payment per month (Total CLIN amount) $5,000
(2) Maximum payment percentage for the service (Column 4, PRS Table) (Weight) X 40%
(3) Maximum payment for acceptable service $2,000
(4) 4 defects exceeds MADR
(5) Percentage of lot found unacceptable = 20% (4 defects divided by 20 units = 20%)
(6) Percentage of lot found acceptable (100% minus Line 5) 80%
(7) Credit for sample defective corrected = (1 defect corrected) (defects in sample corrected divided by lot size, then multiplied by 100) or (1 divided by 20 X 100) 5%
(8) Payment for percentage of lot found acceptable service (Line 3 times (line 6 + Line 7)) $1,700
Surveillance Procedures: The COR may inspect all work required by the contract to ensure contractor compliance IAW the PWS. Evaluator will record results of inspections, noting the dates and times of inspections. The Contractor will be notified. The Contractor shall be given two (2) hours after notification to correct the deficiencies for daily work and 24 hours for all other work following the notification. If the deficiencies cannot be corrected within these times frames, or cannot be corrected at all, deductions will be assessed.
