The following have been added by full text:
GOVERNMENT RESPONSE
Question: Can we bid on an individual service such as occupational therapist or do we have to fill the needs for all services requested?
Answer: You must bid on the individual service requested which in this case is a Physical Therapist.
Question: Is a renewal period available for further contract periods?
Answer: Not at this time. The period of performance is through the end of the Extended School Year 2 July 2010.
Question: Will we be required to submit the resume and licensure of the proposed therapist?
Answer: Yes, to be considered you must submit a resume, a copy of their license, and your quote along with other information requested in the check list (attached in FBO)
Question: What is the current contract rate for PT services?
Answer: There is no current contract rate for PT services. The Government will award a contract to the lowest, technically acceptable offeror.
Question: Who are the current vendors providing these services?
Answer: There is no one currently providing these services.
Question: Does the PT have to use their own vehicle or will a vehicle be provided to drive to homebound students’ locations? Approximately how far would a PT have to drive to each home round trip?
Answer: The PT must use their private vehicle no more than a few miles round trip to see a homebound student.
Question: Where can we find a copy of the Fort Campbell Vehicle Access Control Procedures? Can you please give specific examples of what you mean by “time and expenses” associated with compliance to the ACPs?
Answer: Vehicle Access Control Procedures means vehicle registration and current proof of insurance.
Information on this can be found at Gate 4 of Ft. Campbell. An example of “time and expenses” would be the time it takes to fill out proper paper work for a daily pass and the government would not pay you for this time. At some installations there is a fee for a pass card but not at Ft. Campbell.
Question: Is there a current contractor(s) that provides similar services? If so, can you please tell us who the contractor(s) are, when the contract(s) expire, and what the current rate(s) are? Are you satisfied with the current contractor(s) services? If not, please explain.
Answer: There is currently no contractor providing these services.
Question: It says there is a Size Standard of $7.0 M. Is this correct? Is there a set aside on this RFQ?
Answer: According to the SBA this is correct. There is no set aside for this RFQ.
Question: You list 8 Facility Locations in the Fort Campbell School District. Will you need Physical Therapists at all 8 locations or one that will float to all 8 schools for the 744 hours?
Answer: The Government requires only one PT to float to the listed schools if there is a child in need of services at a particular school.
Question: How many copies of the quote and the proposal do you need? Do you want specific profiles for possible Physical Therapists that you can interview, or just certification that we can provide the contractors on January 4th?

Answer: One signed copy is required. You will need to submit the PT certification of the individual(s) you are proposing that can start on January 4 or soon thereafter.
