Attachment J-5 Quality Assurance Surveillance Plan (QASP)

QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

1. Contract or Task Order Title (Add contract/task order number at award): DISA Computing Services Facilities Team Technical Support

2. Work Requirements:

· Task 1 – Facility Programming and Technical Support

· Task 2 – Facility Maintenance Management and Support

· Task 3 – Engineering Project Management and Support

· Task 4 – Date Center Raised Floor Technical Support

3. Primary Method of Surveillance:

Random Sampling: Tasks 1-4 services are sampled to determine if the level of performance is acceptable.

4. Scope of Performance:

These tasks focus on facilities and infrastructure Base Operating Support (BOS) maintenance, repair, restorization and modernization project management, facilities management support, developing standards and raised floor configuration management for DISA-CSD, CD521. The contractor will provide appropriately skilled personnel in the specific labor categories requested by the Government to support CD521 activities. This work falls under the areas of: engineering and architecture, engineering technician, raised floor configuration management.

Subtask 6.1.1 – Facility surveys and trip reports are technically correct, completed IAW standard document format.

Subtask 6.1.3 – Project programming documents shall be technically correct.

Subtask 6.2.1 – Support Agreement reviews shall be reviewed for cost and scope.

Subtask 6.2.2 – BOS maintenance contract acquisition and evaluation documentation shall be technically correct.

Subtask 6.2.3 – Facility documentation shall be technically correct and maintained in a logical manner to easily locate as needed.

Subtask 6.2.4 – Support to the facility manager shall be identified by and at the discretion of the facility manager.

Subtask 6.3.1 – Engineering project scoping, design reviews and contract acquisition documents shall be technically correct.

Routine inspection of deliverable materials.
Subtask 6.3.2 – Engineering project construction contract acquisition, project management, and evaluation documentation shall be technically correct.

Subtask 6.4.1 – Raised floor master plans shall be technically correct.
Performance occurs with no required re-performance or re-work at least 90% of the time. Problems that occur are minor and are resolved in a satisfactory manner.
Routine inspection of deliverable materials.
Subtask 6.4.2 – Customer Letter Estimate reviews shall be technically correct.

6. Acceptable Quality Level (AQL):

The AQL for this project is 100%, as the mission of Computing Services is to deliver world-class computing facilities, infrastructure and operations in support of the unique mission needs of the DoD and the Warfighter.

7. Evaluation Method:

The Task Monitor (TM) shall review monthly status reports to ensure facility surveys, maintenance, project management, and raised floor configuration management is in line with CSD requirements and the PWS. Review comments regarding the contractor’s performance will be forwarded to the COR. The status report shall include:

1. Description of the activities completed during the previous month and the activities scheduled for the upcoming month. The report shall reflect the status of maintenance and project schedules along with an explanation for any deviations from the defined schedule.

1. Information on hours expended by individual, labor category, and Work Breakdown Structure (WBS).

1. Report on trips completed and planned, including activities/personnel visited, accomplishments, and follow-up activities that are required as a result of the trip. Reports shall include the name of the person(s), cost of the trip, and duration.

8. Incentives (Positive and/or Negative):

There are no incentives coupled to this performance work statement.
