1.3 PROJECT INFORMATION

A. Project Description: USDA Consolidation Project - Bid Package 2

Solicitation Number: GS-08-P-17-JF-C-0001

1. The General Services Administration (GSA) intends to award a firm-fixed-price General

Construction Contract for the Alteration of space within the Wallace F. Bennett Federal

Building (FB). The impetus for this project is to support GSA’s mission to reduce private lease space and back-fill vacant space in federal properties. This project will construct approximately 82,000 square feet of office, storage, conference, and specialized space for

Bid Package 2 two USDA Departments: Forest Service Geospatial Technologies Application Center, and the Farm Service Agency Aerial Photography Field Office. In addition, this project will modernize portions of the building’s existing common space including elevator lobbies, corridors, public restrooms, and joint use conference space. The work in the existing FB will be concentrated within portions of the basement, 2nd, 3rd, 5th, 6th, 7th floors, penthouse, and roof. This construction project includes demolition, asbestos abatement, interior tenant improvements, as well as mechanical, electrical, fire notification/suppression, and structural construction to support the modernized spaces.

2. The Government will be using a brand name product to fulfill a portion of the construction requirements through a subcontract for the brand name product under the

General Contractor. The requirements call for a subcontract to DIRTT Environmental

Solutions, Ltd. through one of its approved affiliated companies. Contractor shall involve and coordinate with DIRTT Environmental Solutions, Ltd. within two weeks of Notice to

Proceed.

3. The FB is located at 125 South State Street, Salt Lake City, UT. It is an existing multi-tenant professional office building with 8 stories above grade and a partial below grade basement and parking garage. Work will be performed in the Basement, 2nd Floor, 3rd

Floor, 5th Floor, 6th Floor and 7th Floor. The building is currently occupied by the following federal agencies:

1.4 WORK COVERED BY CONTRACT DOCUMENTS

1. ARCHITECTURAL:

a. Selective demolition

b. New offices, conference rooms, breakrooms, support spaces

2. STRUCTURAL: A new opening will be cut through an existing shear wall. New non-bearing masonry partition walls will be installed. Existing masonry openings in the basement will be infilled, and new door openings will be cut in the existing wall. New folding partition support will be installed, and new housekeeping pads will be placed.

3. MECHANICAL: The existing building mechanical systems will remain unchanged and be reused. The tenant and joint use spaces will have new diffusers and grilles to accommodate new room layout. Tenant and joint use spaces will be rebalanced for appropriate air flow. Digital scanning and tape storage will have standalone HVAC systems connected to emergency power. Server room will have a completely new cooling system provided and installed by others. Restrooms will have new plumbing fixtures.

Exhaust system serving restrooms to remain unchanged. New exhaust grilles will be installed and connected to existing ductwork. New sinks and point of use water heaters will be provided for new break rooms and coffee bars. New plumbing piping will be connected to existing utilities.

4. ELECTRICAL :

a. POWER: The existing building electrical system will be reused. The building electrical service is 480Y/277V.

b. LIGHTING: All new light fixtures will have LED lamps. The light levels will meet or exceed the recommendations of Illuminating Engineers Society. Lighting controls will use occupancy and vacancy sensors.

c. TELECOMMUNICATIONS: The existing building telecommunication system will be used. For renovated areas, new pathways for future communication cables will be provided on both the 6th and 7th floors. For the 7th floor only communication cables and terminations will be provided.

5. FIRE ALARM: Relocation/remounting of existing fire alarm initiating devices and notification appliances as required to accommodate the proposed tenant improvement.

6. FIRE SPRINKLERS: Add and/or relocate fire sprinklers as required to accommodate building remodel. Type, number and location of fire sprinklers shall conform to all applicable requirements of NFPA 13. Install sidewall type fire sprinkler to protect computer and scanning rooms with water filled piping located outside of the room.

7. HAZARDOUS MATERIALS ABATEMENT: Removal of ACM mastic on existing floor surfaces where disturbed. Asbestos is also found in the cores of existing fire-rated doors. NOTE: THE INTENT IS TO NOT DISTURB ASBESTOS-CONTAINING

MATERIALS TO THE EXTENT POSSIBLE – LEAVE UNDISTURBED IN-PLACE

WHEREVER POSSIBLE.

B. Type of Contract:

1. Project will be constructed under a single prime contract.

2. This is an Alteration project, as further defined in the Facility Standards for the Public

Buildings Service.

1.5 SIGNIFICANT ASPECTS

A. This project is a substantial investment by both GSA and USDA to provide healthy, modern, and efficient workspace for USDA’s employees moving into the federal building from their expiring private lease. It is highly unlikely that there will be any further, significant investment in USDA’s spaces for the term of the lease, 20 years, which is why it is of utmost importance that the Contractor deliver workmanship of the highest quality, to minimize the need for repair or replacement of failing building elements within their suite.

1. Schedule is very important. USDA’s current private market lease is ending in January

2019. An extension to that lease would be costly and must be avoided. USDA and their contractors also require time to set-up the scanning and computing environment prior to occupancy. Phasing and on-time delivery of Early Delivery spaces will be critical to maintain by the Contractor.

2. Effective communication and efficient coordination will be essential. Work in an occupied building is challenging, however most challenges can be successfully overcome with proper communication and coordination between all project team members.

3. This project shall be executed adjacent to occupied space. The safety and security of building employees is critical. Noise, dust, and disruption must be mitigated.

4. This project will involve a thoughtful approach to work sequence in order to maintain egress, safety, security, and continuity of work in adjacent spaces.

5. This project will require the abatement of hazardous materials. The building will be fully occupied during construction and special consideration of how and when abatement work occurs will be required, such as night, weekend, or holiday work.

6. Removal of demolished materials will require coordination of the freight elevator with

GSA. Protection of people, finishes, maintenance of egress, dust control, etc. will be required.

7. As a result of this project's delivery time-frame, timely and complete submittals will be necessary.

8. The FB is LEED EB Silver. Activities, materials, and waste management shall not negatively impact this achievement.

9. USDA, under separate contract, will be utilizing the services of Cherokee Nation

Technologies for a portion of the Computing Environment, Tape Library, and Generator

Room scope of work. The Contractor shall refer to the Responsibility Matrix in the

Project Specifications for a detailed understanding of scope responsibility. Coordination and communication with Cherokee Nation Technologies will be necessary to ensure no one contractor’s work is disrupted or impeded.

10. GSA, under separate contract, will be procuring furniture for USDA. Schedule coordination will be required as part of this Contract. The furniture installer will be responsible for delivery and installation, however all parties must communicate and coordinate to ensure no one Contractor’s work is disrupted or impeded. The electrician as part of this Contract will be required to assist in providing power to the insinstsinstalled furniture.

1.6 PHASED CONSTRUCTION – EARLY DELIVERY SPACES

A. The Work shall be conducted in phases, with each phase substantially complete as indicated.

1. A suggested Phasing Plan is attached at the end of this section. Contractor shall develop and submit a Schedule and Phasing Plan for approval prior to commencing construction.

B. Before commencing Work of each phase, submit an updated copy of Contractor's construction schedule showing the sequence, commencement and completion dates, and move-out and -in dates of Owner's personnel for all phases of the Work.

1.7 ACCESS TO SITE

A. General: Contractor shall have limited use of Project site for construction operations during construction period. Contractor's use of Project site is limited only by Owner's right to perform work or to retain other contractors on portions of Project.

1. Coordinate schedule, hours of operation, use of site, staging, delivery of materials and equipment and disposal with GSA building representative.

2. The equivalent of 10 parking spaces will be provided for Contractor’s use in the GSA parking lot, south of the building. This area can be used for parking, delivery of materials, and staging of materials. Coordinate exact locations with GSA Building

Manager.

3. Contractor may use (4) designated existing non-occupied storage cages in the parking garage. Areas “Q” and “R” are approximately 10’x10’ and areas “”T” and “U” are approximately 6’x23’. Coordinate exact locations with GSA Building Manager.

4. Contractor may use some existing non-occupied portions of the building as needed.

5. All non-cleared personnel must be escorted.

6. Driveways, Walkways and Entrances: Keep driveways parking garage, loading areas, and entrances serving premises clear and available to Owner, Owner's employees, and emergency vehicles at all times. Do not use these areas for parking or for storage of materials.

a. Schedule deliveries to minimize use of driveways and entrances by construction operations.

b. Schedule deliveries to minimize space and time requirements for storage of materials and equipment on-site.

B. Condition of Existing Building: Maintain portions of existing building affected by construction operations in a weathertight condition throughout construction period. Repair damage caused by construction operations.

C. Condition of Existing Grounds: Maintain portions of existing grounds, landscaping, and hardscaping affected by construction operations throughout construction period. Repair damage caused by construction operations.

1.8 COORDINATION WITH OCCUPANTS

A. Full Owner Occupancy: Owner will occupy site and existing building during entire construction period. The 5th, 6th and 7th Floor work areas will be vacant during construction. Cooperate with

Owner during construction operations to minimize conflicts and facilitate Owner usage.

Perform the Work so as not to interfere with Owner's day-to-day operations. Maintain existing exits unless otherwise indicated.

1. Maintain access to existing walkways, corridors, and other adjacent occupied or used facilities. Do not close or obstruct walkways, corridors, or other occupied or used facilities without written permission from Owner and approval of authorities having jurisdiction.

2. Notify Owner not less than 72 hours in advance of activities that will affect Owner's operations.

3. Special Consideration must be given to occupants of the 3rd, 5th, 6th and 7th Floor occupants.

a. The SSA will be in full operation during the course of this project. Close coordination with the Building Manager must be made for any and all disruptions to the third floor tenants, including, but not limited to, construction operations that create excessive noise (drilling, hammering, etc.), planned power or water outages, impacts to HVAC systems, and access to other floors to perform the third floor work. Contractor shall give GSA a minimum of 72 hours’ notice of such interruptions.

b. Contractor may use the existing Freight Elevator within the SSA Suite. Refer to

Section 015000 TEMPORARY FACILITIES AND CONTROLS.

1.9 WORK RESTRICTIONS

A. Work Restrictions, General: Comply with restrictions on construction operations.

1. Comply with limitations on use of public streets and with other requirements of authorities having jurisdiction.

B. On-Site Work Hours: Coordinate working hours during week and on weekends with GSA

Building representative.

C. Existing Utility Interruptions: Do not interrupt utilities serving facilities occupied by Owner or others unless permitted under the following conditions and then only after providing temporary utility services according to requirements indicated:

1. Notify GSA Building Manager not less than two days in advance of proposed utility interruptions.

2. Obtain GSA Building Manager’s written permission before proceeding with utility interruptions.

D. Noise, Vibration, and Odors: Coordinate operations that may result in high levels of noise and vibration, odors, or other disruption to Owner occupancy with Owner.

1. Notify Owner not less than two days in advance of proposed disruptive operations.

2. Obtain Owner's written permission before proceeding with disruptive operations.

E. Controlled Substances: Use of tobacco products and other controlled substances within the existing building and on Project site is not permitted.

F. Employee Identification: Provide identification tags for Contractor personnel working on

Project site. Require personnel to use identification tags at all times.

Building representatives

G. Employee Screening: Comply with Owner's requirements for drug and background screening of

Contractor personnel working on Project site.

1. Maintain list of approved screened personnel with Owner's representative.

1.10 SECURITY CLEARANCES

A. Contractor shall comply with the following requirements pertaining to security clearances:

1. All personnel performing work under the Contract on the Project site must obtain an

Enter on Duty (EOD) determination before they will be granted access to the site.

2. To obtain an EOD determination, Contractor shall submit for all such personnel fingerprints on Form FD258 and a completed Contractor Information Worksheet (CIW).

3. In addition, all such personnel who will be on site 6 months or longer must apply for and receive clearance in accordance with Homeland Security Presidential Directive 12

(HSPD-12).

1.11 SENSITIVE BUT UNCLASSIFIED (SBU) BUILDING INFORMATION

A. The following requirements apply to information contained in this Contract that has been designated as Sensitive But Unclassified (SBU) building information:

1. The Contractor shall submit to GSA all Contractor-generated documents that may contain

SBU building information before the original or any copies are disseminated to any other parties. If GSA identifies SBU content in such documents, the Contracting Officer may direct the Contractor to imprint or affix SBU document markings to the original documents and all copies prior to dissemination.

2. The Contractor shall protect SBU building information by strictly limiting access to those individuals having a need to know such information acting on behalf of Federal, State, and local government entities, nongovernment entities engaged in the conduct of business on behalf of or with GSA, and firms currently registered as “active” in the System for

Award Management (SAM) database via https://www.sam.gov. If a subcontractor is not registered in the CCR and has a need to possess SBU building information, the subcontractor shall provide to the Contractor its DUNS number or its tax ID number and a copy of its business license. This clause shall not be construed to encumber the dissemination of SBU building information to public safety entities.

3. The Contractor shall use session or file encryption when transmitting SBU building information electronically outside of the GSA firewall and network. The Contractor shall encrypt sessions or files with an approved NIST algorithm, such as Advanced Encryption

Standard (AES) or Triple Data Encryption Standard (3DES), in accordance with Federal

Information Processing Standards Publication (FIPS PUB) 140-2, Security Requirements http://www.gsa.gov/portal/category/26757 https://www.sam.gov/

Bid Package 2 for Cryptographic Modules. Encryption tools that meet FIPS 140-2 are referenced on the

B. All encryption products used to satisfy the FIPS 140-2 requirement should have a validation certificate that can be verified at the following URL:

http://csrc.nist.gov/groups/STM/cmvp/validation.html#02 (Note that not all vendors of security products that claim conformance with FIPS 140-2 have validation certificates.)

1. When transmitting SBU building information contained in any physical media, including hardcopy and portable electronic data storage devices, the Contractor shall only use methods of shipping that provide services for monitoring receipt such as track and confirm, proof of delivery, signature confirmation, or return receipt. Portable electronic data storage devices include but are not limited to CDs, DVDs, and USB drives.

2. The Contractor shall maintain a list of the entities and the firms to which it has disseminated SBU building information. This list must include at a minimum (1) the name of the entity or firm; (2) the name and contact information of the individual at the entity or firm who is responsible for protecting the SBU building information; and (3) a description of the SBU building information provided. Upon completion of the Work, the

Contractor shall collect all lists maintained in accordance with this clause, including those maintained by any subcontractors and/or suppliers, and submit them to the

Contracting Officer. The Contracting Officer may withhold final payment until the lists are received.

3. This Contract requires that the Contractor retain beyond the period of performance certain records that may contain SBU building information. With the exception of such records, upon completion of the Work, the Contractor shall either destroy all records containing SBU building information or return them to the Contracting Officer, as directed by the Contracting Officer. The Contractor shall destroy electronically stored

SBU building information by deleting it or otherwise rendering it unreadable and incapable of being restored, in accordance with guidelines provided for media sanitization within Appendix A of NIST Special Publication 800-88, Guidelines for

Media Sanitization, available at http://csrc.nist.gov/publications/nistpubs/800-

88/NISTSP800-88_rev1.pdf. Examples of acceptable destruction methods for SBU building information are: burning or shredding hardcopies; physically destroying portable electronic storage devices; deleting and removing files from electronic recycling bins;

and removing material from computer and hard drives using a permanent-erase utility such as bit-wiping software or disk crushers.

4. The Contractor shall verify to the Contracting Officer in writing that SBU building information has been disposed of as required in Paragraph (7)(f) of this clause. The

Contracting Officer may withhold final payment until written verification is received.

5. The Contractor shall immediately report to the Contracting Officer all improper disclosures of SBU building information. If the Contractor fails to comply with any provision of this clause, the Contracting Officer may withhold approval of progress payments until the Contractor provides a corrective action plan explaining how it will rectify any noncompliance and prevent future improper disclosures of SBU building information.

6. The Contractor shall insert the substance of this clause in all subcontracts.

1.12 SPECIFICATION AND DRAWING CONVENTIONS

A. Specification Content: The Specifications use certain conventions for the style of language and the intended meaning of certain terms, words, and phrases when used in particular situations.

These conventions are as follows:

1. Imperative mood and streamlined language are generally used in the Specifications. The words "shall," "shall be," or "shall comply with," depending on the context, are implied where a colon (:) is used within a sentence or phrase.

2. Specification requirements are to be performed by Contractor unless specifically stated otherwise.

B. Division 01 General Requirements: Requirements of Sections in Division 01 apply to the Work of all Sections in the Specifications.

3.1 CONSTRUCTION PHASING AND EARLY DELIVERY SPACES

A. The work in the Data Center on the 6th Floor must be phased and closely coordinated with the

Data Center Contractor, Cherokee Nation. The purpose of the phasing is to allow the BOR Data

Center to be in continuous use throughout the project. Construction of the Distribution Room

(6200) will be completed prior to any work in the Shared Server Room 6407. The BOR Servers will be relocated, by Cherokee Nation, to a temporary location in the Distribution Room, once that room is complete. Once the temporary BOR Servers are in operation, Cherokee Nation will remove any remaining BOR Servers equipment. Then construction can begin in the Shared

Server Room. The logical steps follow (but may be adjusted by the Contractor to meet the requirement):

3. Cherokee Nation relocates and installs temporary BOR Server in Distribution Room 6200 and removes equipment from Shared Server Room.

a. Temporary swing space includes a temporary cooling system, by Cherokee Nation.

1. Substitutions for Cause: Changes proposed by Contractor that are required due to changed Project conditions, such as unavailability of product, regulatory changes, or unavailability of required warranty terms.

2. Substitutions for Convenience: Changes proposed by Contractor or Owner that are not required in order to meet other Project requirements but may offer advantage to

Contractor or Owner.

1.4 ACTION SUBMITTALS

A. Substitution Requests: Submit three copies of each request for consideration. Identify product or fabrication or installation method to be replaced. Include Specification Section number and title and Drawing numbers and titles.

1. Substitution Request Form: Use CSI Form 13.1A.

2. Documentation: Show compliance with requirements for substitutions and the following, as applicable:

a. Statement indicating why specified product or fabrication or installation cannot be provided, if applicable.

c. Detailed comparison of significant qualities of proposed substitution with those of the Work specified. Include annotated copy of applicable Specification Section.

Significant qualities may include attributes such as performance, weight, size, durability, visual effect, sustainable design characteristics, warranties, and specific features and requirements indicated. Indicate deviations, if any, from the Work specified.

d. Product Data, including drawings and descriptions of products and fabrication and installation procedures.

e. Samples, where applicable or requested.

f. Certificates and qualification data, where applicable or requested.

g. List of similar installations for completed projects with project names and addresses and names and addresses of architects and owners.

h. Material test reports from a qualified testing agency indicating and interpreting test results for compliance with requirements indicated.

i. Research reports evidencing compliance with building code in effect for Project, from ICC-ES.

j. Detailed comparison of Contractor's construction schedule using proposed substitution with products specified for the Work, including effect on the overall

Contract Time. If specified product or method of construction cannot be provided within the Contract Time, include letter from manufacturer, on manufacturer's letterhead, stating date of receipt of purchase order, lack of availability, or delays in delivery.

k. Cost information, including a proposal of change, if any, in the Contract Sum.

l. Contractor's certification that proposed substitution complies with requirements in the Contract Documents except as indicated in substitution request, is compatible with related materials, and is appropriate for applications indicated.

m. Contractor's waiver of rights to additional payment or time that may subsequently become necessary because of failure of proposed substitution to produce indicated results.

3. Architect's Action: If necessary, Architect will request additional information or documentation for evaluation within seven days of receipt of a request for substitution.

Architect will notify Contractor of acceptance or rejection of proposed substitution within

15 days of receipt of request, or seven days of receipt of additional information or documentation, whichever is later.

a. Forms of Acceptance: Change Order, Construction Change Directive, or

Architect's Supplemental Instructions for minor changes in the Work.

b. Use product specified if Architect does not issue a decision on use of a proposed substitution within time allocated.

1.5 QUALITY ASSURANCE

A. Compatibility of Substitutions: Investigate and document compatibility of proposed substitution with related products and materials. Engage a qualified testing agency to perform compatibility tests recommended by manufacturers.

1.6 PROCEDURES

A. Coordination: Revise or adjust affected work as necessary to integrate work of the approved substitutions.

PART 2 - PRODUCTS

2.1 SUBSTITUTIONS

A. Substitutions for Cause: Submit requests for substitution immediately on discovery of need for change, but not later than 15 days prior to time required for preparation and review of related submittals.

1. Conditions: Architect will consider Contractor's request for substitution when the following conditions are satisfied. If the following conditions are not satisfied, Architect will return requests without action, except to record noncompliance with these requirements:

a. Requested substitution is consistent with the Contract Documents and will produce indicated results.

b. Substitution request is fully documented and properly submitted.

c. Requested substitution will not adversely affect Contractor's construction schedule.

d. Requested substitution has received necessary approvals of authorities having jurisdiction.

e. Requested substitution is compatible with other portions of the Work.

f. Requested substitution has been coordinated with other portions of the Work.

g. Requested substitution provides specified warranty.

h. If requested substitution involves more than one contractor, requested substitution has been coordinated with other portions of the Work, is uniform and consistent, is compatible with other products, and is acceptable to all contractors involved.

B. Substitutions for Convenience: Architect will consider requests for substitution if received within 60 days after commencement of the Work. Requests received after that time may be considered or rejected at discretion of Architect.

1. Conditions: Architect will consider Contractor's request for substitution when the following conditions are satisfied. If the following conditions are not satisfied, Architect will return requests without action, except to record noncompliance with these requirements:

a. Requested substitution offers Owner a substantial advantage in cost, time, energy conservation, or other considerations, after deducting additional responsibilities

b. Requested substitution does not require extensive revisions to the Contract

Documents.

c. Requested substitution is consistent with the Contract Documents and will produce indicated results.

d. Substitution request is fully documented and properly submitted.

e. Requested substitution will not adversely affect Contractor's construction schedule.

f. Requested substitution has received necessary approvals of authorities having jurisdiction.

g. Requested substitution is compatible with other portions of the Work.

Architect’s Supplemental Instructions form.

1.4 PROPOSAL REQUESTS

A. Owner-Initiated Proposal Requests: Architect will issue a detailed description of proposed changes in the Work that may require adjustment to the Contract Sum or the Contract Time. If necessary, the description will include supplemental or revised Drawings and Specifications.

1. Work Change Proposal Requests issued by Architect are not instructions either to stop work in progress or to execute the proposed change.

2. Within time specified in Proposal Request after receipt of Proposal Request, submit a quotation estimating cost adjustments to the Contract Sum and the Contract Time necessary to execute the change.

a. Include a list of quantities of products required or eliminated and unit costs, with total amount of purchases and credits to be made. If requested, furnish survey data to substantiate quantities.

e. Quotation Form: Use forms acceptable to Architect.

B. Contractor-Initiated Proposals: If latent or changed conditions require modifications to the

Contract, Contractor may initiate a claim by submitting a request for a change to Architect.

1. Include a statement outlining reasons for the change and the effect of the change on the

Work. Provide a complete description of the proposed change. Indicate the effect of the proposed change on the Contract Sum and the Contract Time.

2. Include a list of quantities of products required or eliminated and unit costs, with total amount of purchases and credits to be made. If requested, furnish survey data to substantiate quantities.

3. Indicate applicable taxes, delivery charges, equipment rental, and amounts of trade discounts.

4. Include costs of labor and supervision directly attributable to the change.

5. Include an updated Contractor's construction schedule that indicates the effect of the change, including, but not limited to, changes in activity duration, start and finish times, and activity relationship. Use available total float before requesting an extension of the

6. Comply with requirements in Section 012500 "Substitution Procedures" if the proposed change requires substitution of one product or system for product or system specified.

1.5 CHANGE ORDER PROCEDURES

A. On Owner's approval of a Work Changes Proposal Request, GSA’s standard modification will be issued for signatures of Owner and Contractor.

1.6 CONSTRUCTION CHANGE DIRECTIVE

A. Construction Change Directive: Architect may issue a Construction Change Directive on

Architect’s standard Construction Change Directive form. Construction Change Directive instructs Contractor to proceed with a change in the Work, for subsequent inclusion in a Change

Order.

1. Construction Change Directive contains a complete description of change in the Work. It also designates method to be followed to determine change in the Contract Sum or the

Contract Time.

B. Format and Content: Use Project Manual table of contents as a guide to establish line items for the schedule of values. Provide at least one line item for each Specification Section.

1. Identification: Include the following Project identification on the schedule of values:

a. Project name and location.

b. Name of Architect.

c. Architect's project number.

d. Contractor's name and address.

e. Date of submittal.

2. Arrange schedule of values consistent with format of AIA Document G703.

3. Arrange the schedule of values in tabular form with separate columns to indicate the following for each item listed:

a. Related Specification Section or Division.

b. Description of the Work.

c. Name of subcontractor.

d. Name of manufacturer or fabricator.

e. Name of supplier.

f. Change Orders (numbers) that affect value.

g. Dollar value of the following, as a percentage of the Contract Sum to nearest one-hundredth percent, adjusted to total 100 percent.

1) Labor.

2) Materials.

3) Equipment.

4. Provide a breakdown of the Contract Sum in enough detail to facilitate continued evaluation of Applications for Payment and progress reports. Coordinate with Project

Manual table of contents. Provide multiple line items for principal subcontract amounts in excess of five percent of the Contract Sum.

a. Include separate line items under Contractor and principal subcontracts for Project closeout requirements in an amount totaling five percent of the Contract Sum and subcontract amount.

5. Round amounts to nearest whole dollar; total shall equal the Contract Sum.

6. Provide a separate line item in the schedule of values for each part of the Work where

Applications for Payment may include materials or equipment purchased or fabricated and stored, but not yet installed.

a. Differentiate between items stored on-site and items stored off-site. If required, include evidence of insurance.

7. Provide separate line items in the schedule of values for initial cost of materials, for each subsequent stage of completion, and for total installed value of that part of the Work.

8. Each item in the schedule of values and Applications for Payment shall be complete.

Include total cost and proportionate share of general overhead and profit for each item.

a. Temporary facilities and other major cost items that are not direct cost of actual work-in-place may be shown either as separate line items in the schedule of values or distributed as general overhead expense, at Contractor's option.

9. Schedule Updating: Update and resubmit the schedule of values before the next

Applications for Payment when Change Orders or Construction Change Directives result in a change in the Contract Sum.

1.5 APPLICATIONS FOR PAYMENT

A. Each Application for Payment following the initial Application for Payment shall be consistent with previous applications and payments as certified by Architect and paid for by Owner.

1. Initial Application for Payment, Application for Payment at time of Substantial

Completion, and final Application for Payment involve additional requirements.

B. Payment Application Times: The date for each progress payment is indicated in the Agreement between Owner and Contractor. The period of construction work covered by each Application for Payment is the period indicated in the Agreement.
