About this file
This document is Amendment No. 1 to a Mandatory Use Contract for Medical and Surgical Supplies issued by the State of Ohio Department of Administrative Services (DAS), in collaboration with MMCAP Infuse, a consortium contract lead. The amendment involves Medline Industries Inc. as the primary contractor and is specifically applicable to the Ohio Department of Mental Health and Addiction Services and the Ohio Department of Veterans Services. The contract is administered through MMCAP Infuse's Master Price Agreement number MMS2200733, which resulted from a bid process opened on July 11, 2022.
The amendment renews the contract for an additional 22 months, extending the effective period from 03/02/25 through 12/31/26. Key modifications include adding language regarding the prohibition of expenditure of public funds for offshore services, removing the Ohio Department of Health from the contract participants, and updating the Table of Contents. The contract allows Ohio state agencies to purchase medical and surgical supplies in any amount and at any time, with no guaranteed purchase volume. The contract is available through the DAS web site and the Ohio|Buys electronic procurement system, enabling ordering agencies to access and place orders for medical and surgical supplies under the established terms.
