STATEMENT OF WORK

FCC17Q0010

Multi-Facility Cleaning Support Services

1) General:

The work to be accomplished shall consist of furnishing materials, labor, supplies, equipment and services of trained personnel to clean and maintain cleanliness of building. The FCC will provide to the contractor paper towels, toilet paper, toilet seat covers, and hand soap and plastic trash bags. Contractor shall be insured to cover injury of its employees, accidental damage to FCC property, or personal property of FCC employees. A certificate of insurance shall be provided to the Contacting Officer’s Representative (COR) within 5 calendar days of contract award.

Services are not to begin before 6:00 pm and must be completed before 6:00 am Monday thru Friday (except all federal holidays and closings) unless other arrangements are made with the COR.

2) Period of Performance:

Base Period:

September 13, 2017 – September 12, 2018

Option Year 1:

September 13, 2018 – September 12, 2019

Option Year 2:

September 13, 2019 – September 12, 2020

Option Year 3:

September 13, 2020 – September 12, 2021

3) Type of Contract:

Firm Fixed Price (FFP)

4) Location: All work shall be performed in the locations below:

7) Technical Direction A. Performance of the work under this contract is subject to the Technical Direction of the COR. The term “Technical Direction” is defined to include, without limitation, the following:

B. Directions to the contractor which redirects the effort, shifts work emphasis between work areas, requires pursuit of certain lines of inquiry, fills in details or otherwise serves to accomplish the statement of work;

C. Provision of information to the contractor that assists in the interpretation of drawings, specifications or technical portions of the work description; and, D. Review and approval/rejection of technical reports, drawings, specifications, and technical information to be delivered by the contractor.

E. All Technical Direction must be within the general scope of work stated in the contract. The COR do not have the authority to, and may not issue any Technical Direction that: 1. Constitutes an assignment of additional work outside the general scope of the base contract; 2. Constitutes a change as defined in the base contract clause entitled “Changes”; 3. In any manner causes an increase or decrease in the prices set forth in Section B; or 4. Changes any of the expressed terms, conditions or specifications of the contract.

F. All Technical Direction must be issued in writing, or shall be confirmed in writing by the COR within 3 working days after issuance. The contractor shall proceed promptly with the performance of duly issued Technical Directions.

However, if in the opinion of the contractor, the direction issued by the COR is within one of the proscribed categories in 1 through 4 above, the contractor shall not proceed, but shall notify the Contracting Officer in writing, within 3 working days after receipt of any such direction. The Contracting Officer will determine if the Technical Direction is proper, or if a contract modification would be required to properly implement the direction.

Failure of the parties to agree upon the nature of the direction or upon the contract action to be taken with respect thereto shall be subject to the base contract clause entitled “Disputes.”

8) Contractor Supervision The Contractor shall provide total supervision of its staff. Government personnel are not authorized or permitted to supervise any Contractor personnel. The Contractor's Program Manager for this contract TBD, who shall communicate with the COR to determine the Government's work requirements as set forth in the Statement of Work and shall assure that these requirements are fulfilled. In the event the designated Program Manager is incapacitated due to illness or injury or otherwise is to be removed by the Contractor during the contract performance, replacement of the incumbent Program Manager shall be subject to the approval of the Contracting Officer.

Although the COR will coordinate directions and assign task within the scope of work of the contract, supervision of individual workers shall remain the responsibility of the contractor.

9) Scope:

EB Facility Scope of Work:

Services shall be accomplished Monday through Friday, between the hours of 8:00 a.m. and 4:00 p.m. Any weekend access to perform monthly or annual services shall be scheduled in advance with the COR. Building is approximately 5,000 square feet.

A. Daily:

i) General:

(a) Empty all wastebaskets; replace plastic liner in basket.

(b) Fill paper towel and soap dispensers as needed.

(c) Empty paper shredder as needed.

ii) Bathrooms

(a) Clean all mirrors and bright work.

(b) Clean and sanitize sinks.

(c) Clean and sanitize commodes and urinals.

(d) Empty trash receptacles.

(e) Empty sanitary receptacles.

(f) Fill paper towel and toilet paper dispensers.

(g) Fill soap dispensers.

(h) Check and fill toilet seat cover dispenser if needed.

(i) Mop and sanitize floors.

(j) Sanitize and clean partitions between urinals and commodes.

(k) Clean light fixtures.

(l) All trash will be removed from building and placed in proper dumpster behind building.

iii) Kitchen

(a) Clean all kitchen counter tops.

(b) Clean sink and wall behind sink.

(c) Clean tables and chairs.

(d) Wipe off kitchen appliances.

(e) Sweep kitchen floor.

(f) Empty trash baskets.

iv) Public Area

(a) Clean lobby door glass in front and rear entrance.

(b) Vacuum mat in front and rear entrances.

(c) Sweep back and side tile hallways.

(d) Empty trash cans in front and back of building.

(e) Empty cigarette butt receptacles located at the door entrance ways.

B. Weekly:

(a) Vacuum all carpet areas to include offices, lobby, hallways and conference rooms.

(b) Sweep porches, wooden deck and stairs.

(c) Wipe off water fountains and water coolers.

(d) Empty the two cigarette butt receptacles located at the smoker outposts.

(e) Wipe clean trash container in kitchen.

C. Bi-Weekly:

(a) Clean toaster and microwave oven internally.

(b) Wipe down and clean conference room tables and place chairs around conference tables.

(c) Empty recycle containers.

D. Monthly:

(a) Mop, sweep, wax and buff tile floor in kitchen, back hallway and side hallway.

(b) Dust office furniture (desks, chairs, cabinets, etc.), window ledges, ceiling vents, registers, window blinds, and wall fixtures.

(c) Wipe down all interior doors and door frames, walls, and light switches.

(d) Organize janitorial supply closet and provide list to Director of needed supplies.

E. Quarterly:

(a) Power scrub 4 bathroom floors and clean tiles around base of walls.

F. Yearly:

(a) Clean windows inside and out.

(b) Shampoo carpet, approximately 2,000 square feet in September.

10) OET Lab Scope of work:

Services shall be accomplished Monday through Friday, between the hours of 8:00 a.m. and 4:00 p.m. Any weekend access to perform monthly or annual services shall be scheduled in advance with the COR. Building is approximately 20,000sf.

A. Daily:

(a) Empty all wastebaskets; replace plastic liner in basket; and empty paper shredder.

(b) Bathrooms

(c) Clean all mirrors and bright work.

(d) Clean and sanitize sinks.

(e) Clean and sanitize commodes and urinals.

(f) Empty trash receptacles.

(g) Empty sanitary receptacles.

(h) Fill paper towel and toilet paper dispensers.

(i) Fill soap dispensers.

(j) Check and fill toilet seat cover dispenser if needed.

(k) Mop and sanitize floors.

(l) Sanitize and clean partitions between urinals and commodes.

(m) Clean light fixtures.

(n) All trash will be removed from building and placed in proper dumpster behind building.

v) Kitchen

(a) Clean all kitchen counter tops.

(b) Clean sink and wall behind sink.

(c) Clean tables and chairs.

(d) Wipe off kitchen appliances.

(e) Sweep kitchen floor.

(f) Empty trash baskets.

vi) Public Area

(a) Clean lobby door glass in front and rear entrance.

(b) Vacuum mat in front and rear entrances.

(c) Sweep back and side tile hallways.

(d) Empty trash cans in front and back of building.

B. Bi-Weekly:

(a) Vacuum all carpet areas to include offices, lobby, hallways and conference rooms.

(b) Empty trash compactor in kitchen (Wednesday and Friday).

(c) Wipe down and clean conference room tables and place chairs around conference tables.

(d) Sweep and mop front entrance foyer.

(e) Empty recycle containers.

(f) Change all linen on the bed in health room as needed.

C. Monthly:

(a) Mop, sweep, wax and buff tile floor in kitchen, back hallway and side hallway.

(b) Dust window ledges, wipe down all interior doors, dust window

D. Quarterly:

(a) Mop, sweep, wax and buff tile floor in the following rooms: 110, 112, 113, 114, 115, 116, 117, 122, 124, 125, 131 and 134.

(b) Shampoo carpet. 5,000 square feet.

(c) Power scrub 4 bathroom floors and clean tiles around base of walls.

E. Yearly:

(a) Clean windows inside and out.

11) PSHSB Facility Scope of work:

Services are only required three (3) times per week (Monday, Wednesday & Friday), between the hours of 8:00 a.m. and 4:00 p.m. Any weekend access to perform monthly or annual services shall be scheduled in advance with the COR. Building is approximately 5,000 square feet.

A. Daily (3 times a week):

(a) Empty all wastebaskets; replace plastic liner in basket.

(b) Bathrooms

(c) Clean all mirrors and bright work.

(d) Clean and sanitize sinks.

(e) Clean and sanitize commodes.

(f) Empty trash receptacles.

(g) Empty sanitary receptacles.

(h) Fill paper towel and toilet paper dispensers.

(i) Fill soap dispensers.

(j) Check and fill toilet seat cover dispenser if needed.

(k) Mop and sanitize floors.

(l) Sanitize and clean partitions between urinals and commodes.

(m) Clean light fixtures.

(n) All trash will be removed from building and placed in proper dumpster at OET Laboratory.

i) Kitchen

(a) Clean all kitchen counter tops.

(b) Clean sink and wall behind sink.

(c) Clean tables and chairs.

(d) Wipe off kitchen appliances.

(e) Sweep kitchen floor.

(f) Empty trash basket.

ii) Public Area

(a) Clean lobby door glass in front and rear entrance.

(b) Vacuum mat in front entrance.

(c) Sweep tile hallway.

B. Weekly:

(a) Vacuum all carpet areas to include offices, lobby, hallways, work room and conference rooms.

(b) Wipe down and clean conference room tables and place chairs around conference tables.

(c) Sweep and mop front entrance, hallway, bathrooms and kitchen area.

C. Monthly:

(a) Mop, sweep, wax and buff tile floor in kitchen, and work room.

(b) Dust window ledges, wipe down all interior doors, dust window blinds and dust work consoles/desks.

D. Quarterly:

(a) Mop, sweep, wax and buff tile floor in the kitchen area.

(b) Power scrub 3 bathroom floors and clean tiles around base of walls.

(c) Vacuum air intake vent grate in Operations Room.

E. Yearly:

(a) Clean windows inside and out.

(b) Shampoo carpet. 5,000 square feet.
