About this file

This document is a Statement of Work (SOW) template for the OSD Statewide Contract FAC110, which covers Hazardous/Universal, Medical, and Electronic Waste Disposal and Emergency Response services. The template is designed to be used by an eligible entity (potentially a state or local government organization) when engaging with a vendor under the statewide contract. The specific parties and exact jurisdictions are left blank in this template, but it appears to be a standardized contract vehicle for waste disposal services.

The contract is structured to provide waste disposal services with a defined term that cannot extend beyond June 30, 2026. The SOW allows for specific project scoping, pricing, and service details to be filled in by the participating entity. Key components include a single point of contact requirement for both the vendor and the eligible entity, provisions for service delivery, and a clear hierarchy of contractual documents. The template anticipates potential task-specific requirements that can be customized, with pricing and service specifics to be determined at the time of individual contract execution under the broader statewide contract framework.
