PERFORMANCE WORK STATEMENT

FOR

SNOW REMOVAL

AT

ROME RESEARCH SITE

ROME, NY

ULDF#: 15-0512 DATED: 30 April 2015

PERFORMANCE WORK STATEMENT

1.0. OBJECTIVE: The objective of this requirement is to maintain accessible roads, walkways and adjacent areas for two government agencies located at Griffiss Business and Technology Park in Rome NY - the Air Force Research Laboratory Information Directorate (AFRL/RRS) and Defense Finance and Accounting Service (DFAS).

1.1. DESCRIPTION OF SERVICES:

1.1.1. Furnish all labor, material and equipment necessary to remove snow, haul snow, and/or mitigate ice on road network, walks, parking areas, and around buildings as required, eliminating hazardous driving and walking conditions during the winter season. In addition, furnish all labor and equipment necessary to remove ice mitigation materials from all roads, walks, parking areas, and all adjacent lawn areas at the conclusion of the snow removal season. The site shall be accessible 24 hours a day providing access for the security police, fire department, civil engineering, and AFRL Information Directorate and DFAS personnel. Area of work is as designated on as Appendix A and B.

1.1.1.1. Priority 1 routes from parking areas to building entrances shall be addressed by the contractor at the beginning of snowplowing operations and most frequently to emphasize accessibility 0600 – 1600 hours, Monday through Friday, the peak hours of operations. For Priority 1 routes the contractor must use Morgo “Snow-Plow” ice melt material or an approved equal.

1.1.1.2. For all other areas the contractor shall apply straight “salt” only to mitigate ice and to eliminate hazardous driving and walking conditions during the winter season.

1.1.2. Contractor shall respond when designated facility personnel require an unscheduled special clearing of snow or ice mitigation with a one-hour advanced notice.

Each contract year should be inclusive of 40 hours of emergency service. Any service above and beyond 40 hours shall be written as an Addendum to the contract.

1.1.3. If contractor fails to keep the site open, necessitating assistance and use of available Government snow removal equipment, contractor will be required to reimburse the Government. The amount of reimbursement will be based on standard rental fees (per current R.S. Means cost data) for the specific piece of equipment used with operator.

1.1.4. Contractor shall perform spring clean up by power brooming, street sweepers, hand brooming, etc.

1.1.4.1. Remove all ice mitigation materials and other debris shall be removed from Rome Research Site properties identified in Appendix A & B, including grass areas adjacent to pavements.

1.1.4.2. Remove all ice mitigation materials and other debris shall be removed from DFAS properties identified in Appendix A & B, including grass areas adjacent to pavements.

1.1.5. Contractor shall replace any damage caused by snow removal or ice mitigation operations, as directed by the Contracting Officer.

1.2 ASCERTAINING CONDITIONS:

1.2.1 The contractor shall ascertain at all times the road and weather conditions at the site. When snow accumulation reaches a depth of 2 inches, plowing operations shall begin within one hour to provide site access during the hours the site is manned.

When icy/slippery conditions exist, Ice mitigation operations shall begin within one hour to provide safe driving and walking conditions at the site during the hours the site is manned. The site shall be accessible 24 hours a day providing access for the security police, fire department, civil engineering, and AFRL Information Directorate and DFAS personnel. The contractor shall place special emphasis on accessibility 0600 – 1600 hours, Monday through Friday, the peak hours of operations.

1.3 PERIOD OF PERFORMANCE:

1.3.1 Snow removal operations shall last from 01 November through 30 April of each contract year. Spring clean-up operations shall be completed by 31 May of each contract year.

1.4. EMERGENCIES: Contractor shall provide and ensure a means of answering emergency calls on a 24-hour a day basis.

1.5. GENERAL INFORMATION:

1.5.1 QUALITY CONTROL. Contractor shall develop and maintain a quality program to ensure services are performed in accordance with commonly accepted commercial practices for snow removal and ice mitigation. The contractor shall develop and implement procedures to identify and prevent defective services from recurring.

1.5.2 QUALITY ASSURANCE. The Government will periodically evaluate the contractor's performance by appointing a representative(s) to monitor performance to ensure services are received. The Government representative will evaluate the contractor's performance through intermittent on-site inspections and receipt of complaints from RIOCO. The Government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated RIOCO complaints. Likewise, the Government may decrease the number of quality control inspections if merited by performance.

1.5.3 GOVERNMENT REMEDIES. The contracting officer shall follow the requirements of FAR 52.212-4(a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, for contractor’s failure to correct nonconforming services.

1.5.4 PERIODIC PROGRESS MEETINGS. The CO, Director of Safety, COR, other government personnel as appropriate, and the Contractor shall periodically meet to discuss the Contractor’s performance. The following issues shall be discussed, opportunities to improve the contract, any modifications required of the contract, unsatisfactory inspections and trends against each performance objective observed, positive performance and steps taken by the Contractor to prevent unsatisfactory occurrences in the future. The Contractor shall provide a summation of unsatisfactory inspections and customer complaints and provide insight into any identified trends. The minutes of these meetings will be reduced to writing, signed by the CO and any other signatures as deemed appropriate, distributed to the functional area and the contractor.

Should the contractor not concur with the minutes, the contractor shall provide a written notification to the CO identifying areas of non-concurrence for resolution.

1.6 STAGING: The contractor will be allowed to stage equipment on site in the locations designated by the COTR.

1.6.1 SALT CONTAINMENT. Salt piles may be stored on-site in locations designated by the COR. Storage shelters must be provided by the contractor, such shelters must have an impermeable base and be water tight.

1.6.2 UTILITIES. The government will furnish a reasonable amount of utilities necessary to accomplish the work in accordance with this contract. The Contractor shall not change or modify any utility system or component; or connect any Contractor property, equipment or system without prior CO review and approval. The contractor shall practice energy conservation in the Government provided facilities.

1.7 SAFETY: Comply with all safety and health requirements necessary for the protection of personnel, facilities and equipment including, but not limited to all applicable Occupational Safety and Health Administration (OSHA) standards, while performing this contract at the Air Force Research Laboratory (AFRL), Rome Research Site (RRS), and Rome NY.

1.7.1 Maintain a written Contractor Safety Program, as required by OSHA and make said plan available for review by the government upon request by the contracting officer.

1.7.2 In the event of a mishap during the performance of this contract on AFRL RRS premise, notify the contracting officer in an expeditious manner. Notification shall include the following information:

a. Contract, Contract number, Name and Title of Person(s) Reporting

b. Date, Time and exact location of accident/incident

c. Brief Narrative of accident/incident (events leading to accident/incident)

d. Cause of accident/incident (if known)

e. Estimated cost of accident/incident (material and labor to replace/repair)

f. Nomenclature of equipment and personnel involved in accident/incident

g. Corrective Actions (taken or proposed)

h. Other pertinent information

1.8 ENVIRONMENTAL: No hazardous materials or substances, petroleum products, or materials of environmental concern shall be intentionally discharged or spilled onto the ground, asphalt, or concrete-covered surfaces or improperly disposed at job sites.

A spill is an uncontrolled release to the environment. Spills of any type material (excluding clean water) shall be reported to the Hazardous Waste Program Manager for evaluation to determine cleanup requirements. The contractor shall clean and remediate any spills or releases. If corrective action is not undertaken in a reasonably prudent time, the government reserves the right to execute cleanup and obtain reimbursement from the contractor. For spill occurring during normal duty hours, contact the RIOCV Hazardous Waste Program Manager at 315-330-2098. After duty hours the contractor shall notify the Rome Research Site Security Office at 315-330- 2961.

1.9 SECURITY: Security and base access requirements are contained in clause 5352.242-9000 “Contractor Access to Air Force Installations.” Contractor employees will be required to obtain and display identification badges. Badges are good for one year and the contractor must provide an employee list to the security office at the beginning of every snow plowing season if contract options are exercised.

1.10 SPECIAL QUALIFICATIONS: Equipment operators must possess current and appropriate operator license or certificate for the equipment utilized.
