FLEET SERVICES
PERFORMANCE WORK STATEMENT
SECTION I DESCRIPTION OF SERVICES
1. SCOPE.
The Contractor shall provide all supervision and personnel. The Contractor shall supply all equipment, tools, material, and supplies to properly perform the Aircraft Fleet Service Operations as defined in this Performance Work Statement (PWS), except as specified in Section III, Government - Furnished Property and Services, at Wright-Patterson AFB, Ohio.
The Contractor shall perform fleet services in a manner that conforms to all federal, state, and local laws and regulations. The Contractor shall provide, operate, and maintain vehicles (cleared for the flight-line operations through the Airfield Manager). The estimated quantities of work are listed in Appendix 1, ESTIMATED WORKLOAD DATA. All regulations and instructions listed in Appendix 2 APPLICABLE REGULATIONS, MANUALS, TECHNICAL ORDERS SPECIFICATIONS, and FORMS are to be followed and used for references. All definitions and terms unique to this PWS are listed in Appendix 3, DEFINITIONS. Appendix 4 lists procedure for the VOLUNTARY PROTECTION PROGRAM.
1.1 SPECIFIC TASKS.
1.1.1 Receipt of Tasking/Availability: The Contractor shall be capable of receiving tasking(s) at all times and available Monday through Sunday, 24 hours a day. Each tasking shall be completed within the allowed performance time regardless of the time of issuance. Performance time for each tasking will run sequentially for multiple tasks issued on a single aircraft.
Performance time will run sequentially for multiple aircraft. In the event Contractor is not able to perform services concurrently on several aircrafts, the Government shall prioritize the tasking by aircraft tail number. Performance time for each aircraft is based on the total tasking for that aircraft and is independent of any tasking on other aircraft. Contractor shall complete each service within the required performance time.
1.1.2 Initial Fleet: The Contractor shall provide the following services and items for scheduled and unscheduled missions as required not later than three (3) clock hours prior to scheduled departure, except as noted below:
1.1.2.1 Deliver Government provided expendable items listed in attachment 2 of AMCI 24-101, Volume 10.
1.1.2.2 Deliver ice water treated IAW recommended procedures by the Base Water Surveillance Program and potable water to cargo compartment on all aircraft. Remove trash from aircraft and dispose of trash in approved, on-base containers (unless aircraft coming from overseas location or Customs and Agriculture is needed). Service aircraft-installed water systems to comply with standards stated in AMCI 24-101, V10.
1.1.2.3 Deliver Regulated Items: Upon notification of scheduled aircraft configuration the Contractor shall review and perform services in accordance with AMCI 24-101, V10 for the list of required military regulated items. This document can be found at:

The Contractor shall deliver items to the aircraft, stowing them IAW AMCI 24-101, V10. The Contractor shall perform document delivery in accordance with AMC 4128. If an Air Transportable Galley/Lavatory (ATGL) is on board, Contractor shall inventory the ATGL using the provided AFTO Form 244. Upon completion of mission, the Contractor shall complete the AF Form 4128 and AFTO Form 244 and shall report any missing items to the Contracting Officer Representative (COR). The Contractor shall restock and keep up to date all Passenger Service Kits.
1.1.3 Final Fleet: The Contractor shall accomplish final fleet service no later than one (1) clock hour prior to scheduled take-off time. Final fleet items are ensuring all lavatories have been serviced, all expendable kits requested have been supplied and all meals have been delivered.
1.1.3.1 The Contractor shall pick up and deliver lunches from the in-flight kitchen to the aircraft.
89th Airlift Squadron (AS) Superintendent of Flying (SOF) will order meals and email or call the Contractor time of pickup for meals at the in-flight kitchen for the next days flights. All orders shall be delivered to the aircraft not later than one (1) hour prior to scheduled departure. Contractor shall procure all items from in-flight kitchen. The flight crew shall pay for coffee and lunches at the same time the order is placed. The Contractor shall pick up monies for lunch orders at designated locations (89th AS, SOF Building 4006). The Contractor shall log all in-flight lunch/coffee orders showing the name, date, and time orders were placed.
NOTE: The Government shall provide required military regulated items. Items provided are listed at attachment 4 of AMCI 24-101, Volume 10. Government shall provide expendable supplies listed at attachment 2 of AMCI 24-101, Volume 10.
1.1.3.2 Every effort shall be made by aircrews to call in any last minute fleet service requests before one (1) hour prior to takeoff.
1.1.4 De-Fleet service: The Contractor shall provide a completed Service (de-fleet) Checklist to the COR in accordance with the Appendix 4 checklist to comply with standards stated in AMCI 24-101, V10, Section 1 and the FDA Food Code, Chapter 4. Contractor shall complete performance of de-fleet checklist within three (3) clock hours after receipt of tasking.
1.1.5 Lavatory Service: The Contractor shall provide a complete lavatory service to comply with standards stated in AMCI 24-101, V10. Contractor shall complete performance within two (2) clock hours after receipt of tasking, or one (1) hour prior to aircraft takeoff. The Contractor shall comply with all Air Force, federal, state, and local environmental requirements. Contractor shall provide sanitation service to all aircraft lavatories and Air Transportable Galley Units (ATGL). Operational lavatory servicing vehicle on WPAFB shall meet all safety and environmental standards. All vehicles shall be clean and marked with Contractor identification as provided in this Performance Work Statement (PWS) and its attachments.
Contractor shall deodorize and clean urinals and surrounding areas, and replenish lavatory supplies.
Contractor shall supply alcohol wipes on all aircraft that have lavatory facilities, but do not have the potable water systems serviced. Lavatory service includes discharge of lavatory residue and replacement with water/deodorant solution. Contractor shall dispose residue from lavatories into a Lavatory Service Truck (LST). Contractor shall empty and flush the LST at a minimum of twice a week, and more as required, at the designated Wright-Patterson AFB sewage disposal station.
Note: Government personnel (Aerial Port and Maintenance) are responsible for the maintenance, transport, and delivery of all portable lavatory equipment, i.e., Air Transportable Galley (ATGL), portable urinals, and portable lavatory units.
1.1.6 International Waste Service: The Contractor shall not have responsibility for international trash. The Government shall dispose of international waste in accordance with United States Department of Agriculture Regulation Title CFR 330.400, and Title 9 CFR 94.5. Waste includes meals and other foods that were available for consumption by passengers and crew on international flights. The Government shall transport refuse (any refuse on or removed from a means of conveyance that has been in any port outside the United States and Canada within the previous two-year period or a United States territory, possession or the State of Hawaii in the previous one year period, unless specifically inspected by USDA- APHIS-PPQ) directly from the aircraft to the secured foreign garbage (environmental trash) cage in building 4021, Area C, without diversion. The Government shall keep regulated refuse separate from non-regulated refuse and not commingle which is prohibited.
Note: Government personnel shall collect and dispose of all bio-hazard medical waste, aero-medical waste, and infectious medical waste.
1.1.7 Domestic Waste Service: Any waste accumulated inside the CONUS is considered domestic waste and shall be disposed of properly using any serviceable trash receptacles.
Contractor shall complete performance within three (3) clock hours after receipt of tasking.
1.1.8 Sanitization of Equipment: Contractor shall sanitize food and beverage racks and containers in the Fleet Service Sanitizing Facility in Building 4044, Area A. Contractor shall remove containers and all small galley items (oven racks and trays) from the aircraft and transport to the Fleet Service Building (4044). Contractor shall sanitize and maintain all items that may come in contact with food in accordance with the Food and Drug Administration (FDA) Food Code, Chapter 4. After cleaning and sanitizing, hot cups and oven racks shall be packaged in plastic to prevent contamination prior to use. Store large liquid containers and ovens upside down or with doors closed to prevent contamination prior to use. Sanitized equipment shall be separated from non-sanitized equipment.
1.1.9 Prioritization of Service: The Government shall prioritize the order of flight line fleet services for 445th aircraft. When a conflict exists for servicing 445th aircraft or Transient Alert aircraft, the prioritization decision shall be coordinated between Base Operations and the 445th Air Wing Command Post and Base Operations shall notify the COR of all prioritized decisions.
1.1.10 Aircraft Equipment Discrepancies: Contractor shall notify the Government of all fleet service equipment discrepancies prior to the Contractor leaving the aircraft. Provide written documentation of the type aircraft, tail number, and the discrepancy to the flight line expediter or the assigned aircraft crew chief. Contractor shall log and maintain all information in the fleet service daily log to include daily activities and all problems in servicing the assigned aircraft or equipment.
1.1.11 Transient Alert Aircraft. Transient Alert aircraft are aircraft which transit through but are not assigned to Wright-Patterson AFB, Ohio. Contractor shall provide fleet service to transient aircraft on an as-needed basis. Transient Alert aircraft may be any type aircraft; therefore, Contractor shall service all aircraft types. Notification for transient aircraft service shall be made by Base Operations (Building 206) to the Contractor. Contractor shall perform services within two
(2) clock hour of notification during normal duty hours (0800-1800, Monday through Friday) and within three (3) clock hours of notification after normal duty hours on weekdays, weekends, and holidays.
1.1.11.1 The Contractor shall maintain an updated and current list of phone numbers to be used as point of contacts (POC’s) for transient alert aircraft. This list shall be provided to Base Operations at the start of the contract and as changes occur.
1.1.11.2 The Contractor shall develop a list of services performed along with cost for each service prior to award of contract.
1.1.11.3 Each transient aircraft shall provide payment for the fleet services provided. The payment method shall be one of the following methods: DoD Fleet Card, AIR Card, SEA Card, or any major credit card. Foreign aircraft may use United States Dollars to pay for services, but receipts shall reflect as to the reason why credit cards were not used. Contractor shall submit a monthly report to the COR and Contracting Officer (CO) containing the following information: all payment transactions processed for each transient aircraft service, the number of transient aircraft serviced, the service(s) provided and dollar amount of each, and a description of the aircraft including its tail number. The monthly report shall be submitted no later than 5 (five) business days after the last day of the month.
1.2 FLEET SERVICES MANAGEMENT
1.2.1 The Contractor shall maintain a current Fleet Services Daily Log which shall include all services provided in support of the contract. Where aircraft are involved, the log shall include the type of aircraft and aircraft tail number. The Contractor shall provide a monthly summary of services log to the 445th COR.
1.2.2 The Contractor shall provide training to Contractor employees for all assigned tasks and all training in accordance with this PWS.
1.2.3 The Contractor shall provide during normal duty hours available access to all assigned facilities, areas, records and equipment to the Government authorized personnel for inspections, surveys, fire protection services, and maintenance and repair.
1.2.4 Contractor personnel shall participate in meetings, and training courses as requested by the Government.
1.2.5 The Contractor shall have the capabilities of receiving aircraft flying schedules and changes to the schedule via fax machine for planning purposes and dispatches to perform services...
1.3 FLEET SERVICES OPERATIONS
1.3.1 The Government's request for fleet services made by the 445th Maintenance Control Center (flight line expediter), 445th Command Post, or 88th OSS (Base Operations), shall be official notification of service required. The Contractor shall respond to the priorities as requested.
During normal duty hours, 0800-1800, the response time starts upon official notification by the flight line expediter or Maintenance Operations Control (MOC). The Contractor shall contact flight line expediter or production supervisor upon arrival for each duty day. Changes to the daily missions, cross country returns and any other information regarding the daily flying schedule activities shall be briefed by Contractor to fleet service. Transient Alert response times will be predicated upon the stated expected arrival time of the transient aircraft as relayed to the Contractor by the 88th OSS (Base Operations). Service shall be provided to 445th AW aircraft scheduled to arrive/depart on weekends and holidays with prior coordination. De-Fleet shall be required on a returning aircraft over the weekend if mission dictates. Contractor shall contact the 445th MOC, (937 257-0400) or Wing Planning (937 257-0399), for the requirements prior to close of business each duty day. The Contractor shall provide personnel on a stand-by basis for unscheduled aircraft.
1.4 CONTINUATION OF ESSENTIAL CONTRACTOR SERVICES
1.4.1 Continuation of Essential Contractor Services. All of the Contractor services performed under this contract are required to support mission-essential functions.
1.4.2 The Contractor shall provide fleet service 24-hours per day, 7 days per week, as required.
The Contractor shall provide fleet service in support of contingency plan implementation, activations, exercises, deployments or other emergencies during and after normal duty hours. Fleet service personnel have been identified as mission essential; therefore, the Contractor shall ensure uninterrupted performance to ensure fleet service is provided for all operations specified in this
PWS.
1.4.3 The Contractor shall use the priorities established by the Government for all fleet services. If the activation or exercise of multiple contingency plans generates conflicting support requirements and the Contractor cannot accomplish a routine workload due to the activation or exercise of any contingency plan, the Contractor shall document when higher priorities cause non-support of lower priorities and describe higher priorities. This documentation shall be immediately available to authorized Government personnel (COR) as requested.
1.4.4 Surge Requirement Capabilities: Upon notification by the Contracting Officer, the Contractor shall provide fleet services to support national commitments up to and including a declared war that would generate surge requirements. These services shall be provided 24 hours a day, seven days a week, as required, for the duration of the emergency.
1.4.5 Reserve Training: The Contractor shall train members of the 445th Airlift Wing (to include any future Reserve component) required to augment the Contractor in the event of a national emergency or contingency. These personnel shall be under the direction of the Fleet Services during and for the purpose of such training. The Contractor shall provide lavatory servicing equipment for On-the-Job Training (OJT) of Air Force Reserve personnel. An OJT plan for such events shall be completed by the Contractor.
1.4.6 Base Closures and Delays: From time to time, the Center or Base Commander or other authority may decide to close or delay access to all or part of the base in response to emergencies including but not limited to adverse weather, earthquake, security issues, terrorist attack, acts of war, disasters, etc. After an official decision to close or delay access to the base has been made, the 445th Command Post (CP) shall call 88th Base Operations to determine if Fleet Services will be needed on that day.
1.5 CONTRACTOR PERSONNEL
1.5.1 The Contractor shall provide an on-site person who shall be responsible for the performance of this contract. The name of this person and an alternate who shall act for the Contractor shall be designated in writing to the Contracting Officer (CO) at the start of the contract and as changes occur. The alternate shall have full authority to make decisions.
1.5.2 The Contractor representative shall have full authority to act for the Contractor on all contract matters relating to the daily operation of this contract. The Contractor shall provide telephone numbers for these personnel.
1.5.3 The Contractor representative shall be available within 30 minutes during normal duty hours to meet on the installation with Government personnel designated by the Contracting Officer to discuss problem areas. The Contractor representative shall be on-site during normal duty hours and contingencies requiring work beyond normal duty hours. After normal duty hours, the Contractor representative shall be available within one hour to meet on the installation with Government personnel designated by the Contracting Officer.
1.5.4 All Contractor personnel shall be designated as mission essential and shall report for work during any periods requiring mission essential personnel.
1.5.5 The Contractor shall not employ personnel for work on this contract if such personnel are identified to the Contractor by the Contracting Officer as a potential threat to the health, safety, security, and general well-being or operational mission of the installation and its population.
1.5.6 Contractor personnel shall present a neat appearance and be easily recognized. This shall be accomplished by the wearing of distinctive clothing bearing the name of the company and the employee's name. The Contractor shall be responsible for acquiring appropriate clothing for the needs of personnel engaged in actual fleet services operations. The Contractor shall furnish administrative personnel with an identifying badge, which shall include as a minimum, the person's Last name, the name of the Contractor, and the words "Fleet Services." Each employee shall wear the badge on the outer clothing on the front of the body between the neck and waist so that the badge is visible at all times. Sweat suits, halter-tops, shorts, shall not be worn during normal or contingency duty hours. Note: Blue jeans are permitted; however, they shall present a neat appearance and not be ragged, torn, or faded in color.
1.5.7 The Contractor shall ensure that employee drivers receive Airfield Driving training and comply with flight line driving approval from Base Operation’s prior to start of the basic contract period. Contractor shall obtain AF Form 2293, US Air Force Motor Vehicles Operators Identification Card and AF IMT 483, Certificate of Competency for airfield driving. The Contractor shall ensure each driver maintains a valid driver's license and keeps the license with them during normal and contingency duty hours and during entering and exiting the base installation. The Contractor shall train and certify personnel in fleet services operations prior to each employee performing the fleet services at the installation. Fleet service personnel shall have "flight line access" annotated on the AF Form 1199, Entry Control Card.
1.5.8 The Contractor is cautioned that off-duty military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours or deployment. Military Reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the Contractor's ability to perform and the absence of these personnel at any time shall not constitute an excuse for nonperformance under this contract.
1.5.9 The Contractor is prohibited from employing Contracting Officer Representatives (CORs) employed by the United States Government.
1.5.10 Control of Contractor Employees. The selection, assignment, reassignment, transfer, supervision, management, and control of Contractor employees in performance of this work statement shall be the responsibility and prerogative of the Contractor. The Contractor shall comply with all the requirements set forth in this PWS and Wright-Patterson AFB regulations concerning conduct of employees as referenced herein.
1.5.11 Contractor personnel shall comply with directives pertaining to operation of privately owned vehicles on Wright-Patterson Air Force Base (Per AFI 31-204. Air Force Motor Vehicle
Traffic Supervision as supplemented).
1.6 ORIENTATION PERIOD
1.6.1 Transition. To ensure a smooth transition in the change of work effort from the current contract, the Contractor shall begin orientation 10 workdays prior to the start of the contract. The purpose of this orientation is to:
(1) Observe work accomplished by current employees.
(2) Become thoroughly familiar with work requirements and work procedures.
(3) Complete personnel requirements (work force) including the hiring of personnel to assure satisfactory performance beginning on the contract start date. Soliciting personnel for employment during their duty hours is prohibited, unless interview arrangements are made through the contracting and personnel offices at this installation.
(4) Obtain security clearances, as required.
(5) Complete training requirements and accomplish training of Contractor employees.
(6) Complete the development of work plans/procedures.
(7) Complete the development of quality control plans and procedures.
(8) Become thoroughly familiar with the computation method for withholding payments resulting from deficiencies exceeding the number allowed by the
PWS.
1.6.2 Facility Access. The Contractor shall be allowed access to the facilities to familiarize supervisors, key personnel with equipment, work scheduling and procedures. However, such access shall not interfere with the production efforts of current Contract personnel. To preclude such interference, arrangements for access to the Government facilities shall be made with the Contracting Officer. Access shall be limited to the following categories of personnel:
(1) Contractor supervisory and clerical personnel.
(2) Contractor equivalent of a Government stock clerk.
(3) Contractor foreman level personnel shall be permitted access to observe operations, work flow, priorities, scheduling, equipment handling, storage, parts, safety, security, etc.
1.7 HOURS OF OPERATION
1.7.1 Normal Hours: The Contractor shall perform work during the following hours:
1.7.1.1 Fleet Service Operations: Shall be operational 24 hours per day, seven days per week, including all Federal Holidays.
NOTE: Fleet Services personnel are subject for recall after normal duty hours due to mission requirements at no additional cost to the Government. The Contractor shall be on-site during normal duty hours and contingencies requiring work beyond normal duty hours. After normal duty hours, the Contractor shall be available within two hours on-site, if recalled.
1.7.1.2 Normal hours of operations for Fleet Services, shall be from 0800 to 1800 hours Monday through Friday and as missions dictate. After normal hours of operation, the Contractor shall be on call at all times, including weekends and holidays. Contractor shall provide service to 445th AW aircraft “scheduled” to arrive/depart on weekends and holidays with prior coordination. Contractor shall contact the 445th MOC (937-257-0400), for the scheduled requirements prior to 1700 at the close of business each duty day. Contractor shall provide accurate personnel contact information maintained in the aircraft forms binder.
1.8 RECORDS.
1.8.1 Record Maintenance: If requested by the Government, the Contractor shall provide the original record, or a reproducible copy of any such record required within 24 hours of receipt of the request.
1.8.2 Electronic Files: At the conclusion of this contract, or in case of Contractor default, all electronic files, forms and output products shall be provided to the Government in electronic format by the last date of the contract.
1.8.3 Monthly Report: No later than five (5) business days after the last day of each month, Contractor shall provide a report to the COR and to the CO IAW paragraph 1.1.11.3.
1.9 ENVIRONMENTAL CONTROLS.
1.9.1 Compliance with Laws and Regulations. The Contractor shall abide by the laws and regulations for disposal of trash both domestic and international. They shall also abide by local laws and regulations concerning the disposal of waste from aircraft latrines. In the event environmental laws/regulations change during the term of this contract, the Contractor is required to comply with such laws as they come into effect.
1.9.2 Notification of Environmental Spills. If the Contractor spills or releases any substance contained in 40 CFR Part 302 into the environment, the Contractor or Contractor’s agent shall immediately (within 1 hour) report the incident to the 445th flight expediter or production supervisor. The liability for the spill or release of such substances rests solely with the Contractor and its agent, as determined by the Contracting Officer. These costs shall include, but not be limited to, the following:
a. Base Fire Department costs for standing by or First Response Actions.
b. Base Spill Teams Costs for containment and clean-up.
c. Office of Environmental Management cost to contract spill containment and clean-up.
d. Damage repair costs to Government and private property.
1.9.3 Contractor Containment and Cleanup of Spills. The Contractor shall place absorbent material on all spills that occur during latrine servicing process.
Note: Sufficient material shall be used to entirely absorb the entire spilled product. The Contractor shall containerize the spill debris in appropriate container meeting the necessary requirements IAW 40 CFR Part 260-268. The Contractor shall affix the appropriate labels on the container and notify the 88th ABW/EM at (937) 257-7152 for disposal instructions.
1.9.4 Spill Notification: The Contractor shall immediately (within 1 hour) notify the 445th MOC upon discovery of any spill. The following information shall be provided:
a. Name of employee discovering the spill.
b. Location.
c. Type of Emergency.
d. Type of Material Spilled.
e. Estimated Quantity Spilled.
f. Information on whether the spill is continuing or has been stopped.
1.9.5 Extent of Spill Clean-Up and Damage Repair: The extent of all spill containment and clean-up shall be determined by the Environmental Response Team. The extent of the damage repair shall be as requested by the custodian of the property affected. If the Contractor feels any request is excessive, he or she shall notify the Contracting Officer in writing for a determination as soon as possible. At no time may the Contractor attempt to modify the work being performed during a remediation activity being undertaken by the Office of Environmental Management.
1.10 MATERIAL STORAGE AND USE.
1.10.1 Guidelines. The Contractor shall follow manufacturers guideline and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This includes hazardous materials.
1.11 GOVERNMENT OBSERVATIONS.
1.11.1 Observation. Government personnel, other than Contracting Officers and COR(s), may from time-to-time, with Contracting Officer coordination, observe Contractor operations.
However, these personnel may not interfere with Contractor performance unless it involves the imminent safety of personnel or Government property.
1.12 SAFETY REQUIREMENTS.
1.12.1 Performance Requirements: In performing work under this contract, the Contractor shall:
1.12.1.1 Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.
1.12.1.2 Take precautions as the Contracting Officer requires for safety and mishap prevention purposes.
1.12.1.3 Develop and provide at the start of the orientation period a Safety Plan for the protection of Government facilities and property and to provide a safe work environment for Contractor personnel. The Contractor shall submit a detailed written Safety Plan to the Contracting Officer at the pre-performance conference. The Contractor agrees not to deviate or make changes to the plan without prior review and acceptance by the Contracting Officer. The Safety Plan shall include, but not be limited to:
a. Safety Policies, Objectives and Standards.
b. Job and Task Hazard Analysis.
c. Safety Training.
d. Inspection Requirements.
e. Hazard Reporting.
f. Accident Investigation and Reporting Procedures.
1.12.1.4 The Contractor shall require subcontractors to comply with the safety provisions of this contract.
1.12.1.5 Record and report (within one hour) to the Contracting Officer or designated Government representative (GR) all available facts relating to each instance of damage to Government property or injury to either Contractor or Government personnel.
1.12.1.6 In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the Contracting Officer.
1.12.1.7 If the Government elects to conduct an investigation of the accident/mishap, the Contractor shall cooperate fully and assist Government personnel in the conduct of investigation until the investigation is completed.
1.12.1.8 Accident/Incident Reporting and Investigation. The Contractor shall notify the 445th Safety Officer within one (1) hour of any incidents/accidents involving Government owned property, or an injury to contract personnel. A detailed report to the Contracting Officer and the Fleet Service COR shall be provided within twenty-four (24) hours of such incident/accident. All Contractor employees are considered to be employees in a sensitive position. Therefore, the Contractor is required and shall maintain a program for achieving a drug-free work force. All personnel involved in or suspected of drug usage, shall be relieved of duty immediately pending completion of investigation. .
1.12.1.9 Known hazardous conditions applicable to this project are identified in the Technical Orders with appropriate "caution" or "warning" statements. The Contractor shall assure that review is made of the Technical Orders to identify completely and in sufficient detail the hazardous conditions recognized. Prior to beginning work, all concerned personnel shall be apprised of the dangers involved, and the proper safeguards required.
1.12.1.10 Should hazardous conditions arise during the course of this work, which have not been recognized in the applicable Technical Orders, or by this section, Contractor shall be directed by the Contract Manager or Contractor's designated representative, to cease work immediately, and notify the Contracting Officer. Work recognized as being hazardous shall not be resumed until the Contracting Officer provides direction to the Contractor concerning the hazardous situation.
1.12.2 Industrial Safety Requirements. Requirements and provisions are as follows
1.12.2.1 Fire extinguishers are provided for the protection of Government property.
1.12.2.2 Personnel engaged in any type of fleet service operation shall be prohibited from wearing finger rings, ear rings, or any loose jewelry. REF: AFOSH Standard 91-38 and AFOSH Standard 91-501.
1.12.2.3 The Contractor shall develop and implement a tool control program within 10 days of contract implementation to prevent lost tools/equipment on the aircraft. A sign in and sign out process shall be established and enforced by the Contractor at all times.
1.12.2.4 Weather advisories shall be communicated through 445th Command Post and 88th Base Operations Automated Notification System (ANS). The 445th AW Maintenance Operations Control shall take appropriate action to notify fleet services management of the weather advisory.
1.12.2.5 The Contractor shall designate in writing, during the orientation period, to the Contracting Officer and Fleet Services COR office, a Safety person to act as point of contact for safety matters and to implement the Contractors Safety Plan.
1.12.2.6 The Contractor, the Base Bioenvironmental Engineer, and Wing Safety Office, shall designate tasks and working areas requiring personal protective equipment. This will be accomplished so that the COR can ensure compliance in Safety standards.
1.12.2.7 Material Safety Data Sheets (MSDS) for deodorants, anti-freeze solutions, general purpose cleaners and disinfectants shall be submitted to Environmental Management (EM) at 257-7355 for approval prior to use and disposal. Ethylene glycol shall be added to the latrine disinfectants prior to an aircraft flying into cold weather. Vehicles transporting latrine waste shall meet all Federal, State and local Safety Requirements, and have all appropriate Federal, State and Local Environmental permits and insurances.
1.12.2.8 Contractor personnel shall leave ignition keys in fleet service support vehicles when units are parked on the flight line or inside of Government facilities.
1.12.2.9 Corrective actions shall be taken immediately (within 1 hour) when leaks or spills are detected during lavatory service operations. If the above is detected, operations shall be suspended immediately.
1.13 QUALITY CONTROL.
1.13.1 Quality Control Program. The Contractor shall establish and maintain a complete Quality Control Plan to ensure the requirements of this contract are provided as specified. The Contracting Officer shall notify the Contractor of acceptance or required modifications to the plan before the contract start date. The Contractor shall make appropriate modifications (at no additional costs to the Government) and obtain acceptance of the plan by the Contracting Officer before the start of the first operational performance period. One copy of the Contractor’s Quality Control Plan shall be submitted with the Contractor’s technical proposal. An updated copy shall be provided to the Contracting Officer on the first day of the orientation period and as changes occur. The Government shall monitor Contractor compliance with the quality control plan. The plan shall include, but not limited to the following:
1.13.1.1 Inspection System. A description of the inspection system covering all services in the PWS. It shall specify the frequency of inspections and titles of the individuals who shall perform the inspection and their organizational placement.
1.13.1.2 Deficiency Prevention. A description of the methods for identifying and preventing defects in the quality of service performed before the level becomes unacceptable.
1.13.1.3 Inspection Files. A description and sample of the inspection records to be kept to document all inspections conducted by the Contractor and the necessary corrective or preventative actions taken. This documentation shall be made available to the Government during the term of the contract and for the period after contract completion until final settlement of any claims under this contract. The inspection and corrective actions records shall be maintained at a location or locations designated in the Contractor’s quality control plan.
1.13.1.4 Complaint. A customer complaint feedback system for identification and correction of complaints. All customer complaints shall be acknowledged and reported to the COR within one workday after receipt of complaint. Each customer complaint reported to the COR shall have a scheduled completion date subject to COR approval. The Contractor shall make available to each customer the Customer Complaint Record. The name, location, address, and phone number of the COR shall be posted in readable format for easy customer identification in a publicly accessible location in the Contractor area. The Contractor shall forward customer complaints to the COR not later than close of business the next workday. (See Attachment 1)
1.13.1.5 The Quality Control Plan shall be updated, at no additional cost to the Government, as required to preclude a recurrence of the complaint
1.14 QUALITY ASSURANCE.
According to FAR 52.212-4, the Government shall evaluate the Contractor's performance under this contract. The COR shall document all surveillance observations. When an observation indicates unacceptable performance, the COR shall require the Contractor to initial the observation indicating acknowledgment of deficiency. The initialing of the observation does not necessarily constitute Contractor concurrence with the observation, only acknowledgment that the Contractor has been made aware of the defective performance.
1.14.1 Discrepancies. Major findings are contract non-conformances which are considered critical or major. IAW FAR 46.101 a major nonconformance means a nonconformance, other than critical, that is likely to result in failure of the supplies, or to materially reduce the usability of the supplies or services for their intended purpose. A critical nonconformance means a nonconformance that is likely to result in a hazardous or unsafe condition for individuals using, maintaining, or depending upon the supplies or services; or is likely to prevent performance of a vital agency mission. If at any time COR identifies a condition as having a significant adverse effect on the quality of the activity, such as those stated below, COR shall document their findings and notify the Contracting Officer immediately in writing. Email is acceptable.
A. Contractor failure to meet a Performance Threshold.
B. Failure to provide adequate corrective action to preclude reoccurrence of Government identified findings.
C. Failure to provide corrective action to deficiencies identified by the Contractor within a prescribed suspense period.
D. Any failure to adhere to security and/or safety regulations that results in a security or safety incident.
Minor Finding. IAW FAR 46.101 a minor nonconformance means a nonconformance, that is not likely to materially reduce the usability of the supplies or services for their intended purpose, or is a departure from established standards having little bearing on the effective use or operation of the supplies or services. When COR identifies a minor finding, the COR shall document the findings, but is not required to notify the Contracting Officer. However, if the same minor finding is repeatedly identified, it may be an indication that a major finding is occurring or has occurred because the Contractor has not taken proper steps to prevent recurrence. In this case, the COR shall notify the Contracting Officer in writing (email is acceptable).]
1.14.1 Performance Evaluation Meetings. The Contractor shall attend all meetings with the COR and the Contracting Officer as requested.
1.15 KEY CONTROL.
The Contractor shall establish and implement methods of ensuring accountability of all keys/key cards issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons.
NOTE: No keys issued to the Contractor by the Government shall be duplicated. Key control procedures shall be included in the Quality Control Plan and shall be referenced in the property control plan. Procedures shall include turn-in of any issued keys by personnel who no longer require access to locked areas.
1.15.1 Lost/Duplicated Keys. The Contractor shall immediately (within 1 hour) report the incident of lost or the need for duplicate keys to the COR.
1.15.2 Locks. In the event keys, other than master keys, are lost or duplicated the Contractor shall, upon request of the CO, re-key or replace the affected lock or locks; however, the Government, at its option, may replace the affected lock or locks or perform re-keying. When the replacement of locks or re-keying is performed by the Government, the total cost of re- keying or the replacement of the lock or locks shall be deducted from the monthly payment due the Contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the Government and the total cost deducted from the monthly payment due the Contractor.
1.15.3 Key Responsibility. The Contractor shall prohibit the use of keys issued by the Government to any persons other than the Contractor's employees. The Contractor shall prohibit the opening of locked areas by the Contractor’s employees to permit entrance of persons other than authorized personnel engaged in performance of work requirements in those areas.
1.16 SECURITY REQUIREMENTS.
1.16.1 The Contractor shall ensure employees comply with all security requirements imposed by the Base Commander at all times while their employees are on the installation.
1.16.2 The Contractor shall complete AF Form 2586, Unescorted Entry Authorization Certificate, for each employee requiring entry into controlled areas. This form shall be signed by all contacts listed on the AF Form 2586. The request shall be submitted to Pass and Registration (Bldg. 286). The Government shall provide AF Form 1199, AF Entry Control Card. The credentials shall be displayed and surrendered as directed in AFI 31-101 and AFI 31-501.
1.16.3 The Contractor shall provide sufficient information within 60 days to obtain favorable National Agency Check investigations for their employees’ entrance to restricted areas. Restricted area badges shall be requested for employees according to AFI 31-101, The Air Force Installation Security Program (FOUO) and AFI 31-501, Personnel Security Program Management.
1.16.4 Within six months of contract award, the Contractor shall ensure that appropriate security clearances have been initiated to meet and maintain requirements. Interim clearances may be made available to employees who have submitted required clearance documents when no derogatory information is apparent.
1.17 Contractor Full-time Equivalent Reporting. The Contractor shall report ALL Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Financial Management Systems Helpdesk via a secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil. Reporting inputs shall be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September.
While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.

2. 3 CONTRACTOR PAYMENT:
2.3.1 For performance of a service that does not exceed the PT, the Contractor shall be paid the percentage of the monthly contract line item price indicated in Remedy Column of the attached SS charts for that service.
2.3.2 If performance defects for a service exceed the PT for a service inspected, the Government shall calculate payment as follows:
2.3.2.1 The maximum contract payment per month for all services is multiplied by the maximum payment percentage for a specific service to determine the maximum payment for acceptable service. This payment is multiplied by the percentage of the sample found acceptable to determine the percentage of the contract price that the Contractor shall be paid for the specific service. The total numbers of discrepancies found, not just those in excess of the reject level, are used to determine the percentage of the sample found unacceptable. The percentage of the sample found unacceptable subtracted from 100 percent determines the percentage of the contract found acceptable.
2.3.2.2 When surveillance is accomplished using methods other than random sampling (100% surveillance, periodic surveillance and customer complaint), all service outputs which are not inspected are presumed to be acceptable. For those services that are performed less frequently than monthly, the payment computation will be determined for the entire surveillance period and will be based upon the total maximum payment available for the entire surveillance period.
2.3.2.3 Any deductions from payment shall be taken from the payment for the month in which the CO makes the determination that deduction is appropriate regardless of the period in which the performance occurred.
SECTION III
GOVERNMENT FURNISHED PROPERTY AND SERVICES
3.1 GENERAL.
3.1.1 The Government shall provide, without cost, the facilities, equipment, material, and services identified in this section and in associated appendices. The Government reserves the right to change, alter, and/or modify the facilities without incurring additional cost from the Contractor.
3.2 GOVERNMENT-FURNISHED FACILITIES AND FACILITY EQUIPMENT.
3.2.1 Government-Furnished Facilities. The Government shall furnish or make available the facilities described in Section 3.7. Facilities have been inspected for compliance with the Occupational Safety and Health Act (OSHA). No hazards have been identified for which workarounds have been established.
