Attachment 1 - Statement of Work Audiology Sound Suite

1. Introduction: Audiology Flight of the Wright-Patterson Medical Treatment Facility requires a new Audiology Sound Suite.

2. Background: The current testing suite is over 30 years old and is no longer conducive to pediatric testing. The current booth needs major technological updates to ensure a standard of care is met when compared to standards within the region.

3. Delivery Date: Delivery of all items shall be made no later than 120 days after contract award.

5. Building Address: Wright Patterson Medical Treatment Facility, 4881 Sugar Maple, Bldg. 830, WPAFB, OH 45433, Area A, Room 2C10.

6. Scope: The scope addresses the removal of the existing booth and installation of a new booth with accurate measurements, described below in item 7 (Specific Requirements). To ensure the booth fits the available space, vendor will need to take measurements and adjust specs below as needed.

The booth is to be a prefabricated audiometric examination booth suitable for the use in testing, calibration, and recording of aural acuity at the Wright-Patterson Medical Treatment Facility. A booth is defined as a free-standing double walled examination room. Henceforth, the term "audiometric booth" will be used generically when referring to booth, room, or suite. An audiometric booth covered by this specification includes all enclosure panels, components, wiring, lighting fixtures (including dimmers), ventilation silencers, and installation to make the booth completely operable.

7. Specific Requirements:

a. Current booth removal must include:

i. Removal process to be performed, by the vendor, on a Friday afternoon, Saturday and/or Sunday only

ii. Space to be completely cleaned

iii. Old booth to be disposed of by the vendor

b. Minimum requirement: Audiology Sound Suite (Quantity: 1)

i. Booth Size: Audiometric Sound Suite must have the following characteristics

1. Double wall construction – floors, walls, and ceiling shall be constructed with materials of the same sound attenuation properties.

2. Exam Room: no smaller than 7’0” (W) x 9’4” (L) x 6’10” (H)

3. Control Room: no smaller than 8’4” (W) x 5’0” (L) x 7’5” (H)

4. Outside dimensions no greater than 9’0” (W) x16’8” (L) x 8’2” (H)

ii. Booth access shall comply with the Americans with Disabilities Act (ADA).

iii. Design:

1. Due to the vast age ranges tested at the Wright-Patterson Medical Treatment Facility, the patient side should be large enough to incorporate a play area to accommodate the child, parent and Audiologist to sit and interact.

2. The Sound Suite must have outlets for other audiometric equipment, fire panels, and have ease of access for handicapped individuals, including wheelchair bound patients.

3. The Sound Suite must come ready for Pediatric Testing, i.e. equipment so testing can be accomplished inside the patient test side.

4. Two quadplex outlets for additional equipment beneath observation window, control side.

5. An internal cable raceway system.

6. Three acoustic doors (1 door for the control room and 2 doors for the patient/exam room)

a. Window in the control room door no smaller than 8” (W) x 20” (H)

b. Windows in the patient/exam doors with size between 16” (W) x 42” (H) and 16”

(W) x 60” (H)

7. Window for ease of viewing the patients between control room and exam room.

Observation window between the rooms will be no smaller than 36” (W) x 32” (H).

8. Acoustic ventilation silencer system and installation

9. Thermostat re-install and programing

10. Trim: extended roof apron panels, vertical closure panels to close gaps to booth’s walls.

11. Recessed lighting.

iv. Performance Requirements:

1. Direct feed/PC Controlled clinical audiometer, compatible with DoD mandated software (AudBase).

2. AudBase Software as required by DoD, justification supplied on request.

3. System must be Windows 7 AND Windows 10 compatible.

8. Turn-key installation at the Wright Patterson Medical Treatment Facility

9. Minimum sound specs as listed below:

Sound absorption in accordance with ASTM C423-84a standards:

125 Hz - Absorption Coefficient 0.79 250 Hz - Absorption Coefficient 1.08 500 Hz - Absorption Coefficient 1.10 1000Hz - Absorption Coefficient 1.02 2000Hz - Absorption Coefficient 1.04 4000Hz - Absorption Coefficient 1.00 NRC - Absorption Coefficient 1.05

Noise Reduction in accordance with ASTM E-596:

Center Frequency 125 Hz – Noise Reduction 27 Center Frequency 250 Hz – Noise Reduction 37 Center Frequency 500 Hz – Noise Reduction 44 Center Frequency 1000Hz – Noise Reduction 53 Center Frequency 2000Hz – Noise Reduction 58 Center Frequency 4000Hz – Noise Reduction 60 Center Frequency 8000Hz – Noise Reduction 62

10. Audiometric Equipment Standards

a. Shall adhere to Air Force Instruction (AFI) 48-127 – Occupational and Hearing Conservation program

b. Audiometers and earphones shall meet standards of ANSI S3.6-2010 (or current ANSI standard) as stated in AFI 48-127 Attachment 3, paragraph A3.2. - Audiometric Equipment Standards

c. Audiometric testing room shall meet standards in ANSI S3.1-1999 (R2008), Maximum Permissible Ambient Noise Levels for Audiometric Test Rooms as stated in AFI 48-127 Attachment 3, paragraph A3.2. - Audiometric Equipment Standards

d. Audiometric testing environment shall meet the requirements in AFI 48-127 paragraph 2.19.11 to establish background octave band SPLs are not greater than the following:

i. For 500 Hertz (Hz), 27 dB

ii. For 1,000 Hz, 29 dB

iii. For 2,000 Hz, 34 dB

iv. For 4,000 Hz, 39 dB

v. For 8,000 Hz, 41 dB

11. Packaging, Packing and Shipping Instructions:

Demolition, removal and cleaning of the current sound booth. Once the site is ready for installation any additional room modifications should not be required. The material should not be delivered until time of installation is set to commence due to the lack of storage space in this location.

12. Performance Monitoring:

The vendor, in conjunction with the Chief of Audiology and Facility Management at the Wright-Patterson Medical Treatment Facility, will determine if the installed sound booth meets the size, sound attenuation and other requirements stated in the SOW. The booth will need to attenuate sound as stated in Section 9.0 above.

11. Security Requirements:

The Contractor will not have any access to Wright-Patterson Medical Treatment Facility computers or any patient information. This contract will not require contract personnel to have access to Privacy Information. If privacy information is inadvertently revealed, Contract personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations.

12. Other Pertinent Information or Special Considerations:

Calibrated sound level meter measurements will be taken by a government Biomedical Equipment Technician (BMET) and used to measure various frequencies to make sure specification are met.

13. Period of Performance:

Contractor will remove old booth and install new booth in conjunction with each other within 120 calendar days of receipt of a purchase order. To lessen the impact of patient care in the ENT and Audiology clinics, the removal of the old booth and installation of the new booth should be completed on a Friday, Saturday and Sunday if possible. If unable to complete in said timeframe, the vendor shall lessen the impact and complete the work in the evenings after patient care.

1 year manufacturer’s warranty
