PERFORMANCE WORK STATEMENT
TO
REMOVE,
REPAIR/OVERHAUL,
SET, TEST,
AND REINSTALL SAFETY VALVES
For Boilers
Located in
Area A at BUILDINGS 31240 & 34019
And
Area B at BUILDINGS 20770 & 20080
WRIGHT-PATTERSON AFB, OHIO
DESCRIPTION OF SERVICES
1.1 SCOPE OF WORK
The contractor shall furnish all equipment, labor, and supervision required to perform annual inspections of all safety valves and relief valves for boilers at the designated locations on Wright-Patterson AFB as listed in Appendix A & B.
1.2 During removal, contractor shall seal pressure vessel openings during the period that the safety valves are removed. The contractor shall have 2 work weeks to arrive at each plant, after the plants shut down, to remove the safety valves. After removal of the safety valves, the contractor has four work weeks, from date of removal, to return the safety valves & install them.
1.3 All safety valves shall meet pressure-setting applications for the facility and shall be in accordance with ASME code. All safety valves shall be tested to make sure they lift at their proper pressure, & reset.
1.4 The contractor shall provide certification that each safety valve has been repaired, pressure tested, & function tested, by a certified safety valve facility. Each valve shall have tags having stamped capacity verification (code symbol stamp), attached to each safety valve for the facility.
1.5 During reinstallation of valves, all applicable connections shall be in place to include drains. All drain lines and drain line fittings shall be repaired or replaced, by the contractor. The contractor shall return the safety valves, which they removed for maintenance, to the place the contractor removed them from. The contractor shall install valves within 4 work weeks after removal, during normal outage periods.
1.6 The work also includes removal, repair/overhaul, set, test, and reinstallation of valves and when requested by the Contracting Officer or the Contracting Officer Representative
(COR).
1.7 REPORT
The contractor shall submit a complete report to the Contract Officer Representative within 2 work weeks after returning and installing the valves. The report shall include each valve worked on, a listing of all replacement parts and part numbers for the valve, along with description of repairs/inspections on each valve. The contractor shall provide a copy of all maintenance, tests, repairs, parts, certifications, to include months, days, and year, and what was done on each valve. The contractor shall perform a pressure & functional test, as part of the maintenance & certification and submit a copy to the
Contracting Officer’s Representatives.

III. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
No Government Furnished Property will be included as part of this contract.
3.1 SECURITY FORCE AND FORCE PROTECTION

IV. GENERAL INFORMATION.
4.1 The contractor performing valve repair shall have a Certificate of Authorization in the
National Board
Valve Repair (“VR” Stamp) Program. All requirements pertaining to the use of the National Board “VR” symbol are presented in the National Board Inspection
Code (NBIC) Part RA-2200.
4.2 Contractor personnel shall comply with all requirements of the National Fire Codes and
Air Force Regulations
4.3 The contractor shall not perform any welding or use any flame apparatus without obtaining clearance prior to work from the Fire Inspector. The Fire Inspector 88
ABW/CEFP shall be contacted via phone at (937) 257-4075 or (937) 904-3157. The contractor shall notify the Fire Inspector at least 24 hours in advance of the intended use of any flame apparatus. After receiving approval, when flame apparatus is in use, the contractor shall provide fire protection devices, such as non-combustible coverings and fire extinguishers as directed by the Fire Inspector. The contractor shall use a certified welder for all welding requirements.
4.4 The contractor shall conduct work so that Government property, personnel, and work areas shall be protected at all times from inconvenience, damage of any nature, or injury caused by this work.
4.5 Project Safety: The work shall be accomplished in accordance with the provisions of all
Federal, State (Ohio Industrial Commission) and American Standard Safety Codes for parts of base safety and fire prevention, Air Force Regulations, and all other safety measures applicable as determined by the base ground safety office.
4.6 The contractor shall maintain a clean and orderly work area at all times.
4.7 Combustible wastes shall be disposed of according to Section 01 02 20 General
Environmental Requirements (WPAFB) 2/09 at the end of each work day and shall not be allowed to accumulate.
4.8 The contractor shall provide transportation vehicles that comply with Federal, State and local regulations for size and weight limitations.
4.9 The Contractor shall not discharge fuels, oils, pesticides, or any other toxic or hazardous substance into the storm or sanitary sewers on base.
4.10 INSTRUCTIONS TO THE CONTRACTOR:
Prior to performance of work, the contractor shall contact the 88CEG/CEOES
Contracting Officer Representative in the office of Building 30022 by phone at
(937) 656-3548, at least 72 hours prior to starting any work on the safety valves.
4.11 The contractor shall visit the work area and become thoroughly familiar with all details of the work and working conditions, verify necessary dimensions at the facilities, and advise the Contracting Officer or the Contracting Officer Representative of any discrepancies within 1 work week before performing any work.
4.12 COORDINATION AND SCHEDULING: No written schedule required.
4.13 All work shall be coordinated with the Contracting Officer or
Contracting Officer Representatives.
4.14 The accomplishment of all work set forth in this Performance Work Statement, shall be coordinated by the Contract Officer Representative based on the status of plant operations.
4.15 OUTAGE PERIODS
4.16 Area A starts outage period April 15th, weather dependent.
4.17 Area B, Bldg. 20770 starts large gas boilers outage period June 1st. Bldg. 20080 also starts June 1st. Gas boilers at bldg. 20770 are normally accomplished during winter months (small gas boilers are to be done before 1 May)
4.18 WORK HOURS
Work shall be performed during normal working hours of 0715 to 1600 Monday through
Friday, except Government Holidays and emergency situations as designated by the government. Government Holidays are the following:

4.19 Continuation of Contractor Services during Base Exercises, Local Emergencies, or
Contingencies.
“Continuation of Contractor Services during Base Exercises, Local Emergencies or Contingencies, and Implementation of Local Contingency Plans. It has been determined that none of the services specified in this PWS are required to be performed during (and/or in support of, whichever is appropriate) base exercises, local emergencies or contingencies or implementation of local contingency plans.

Appendix C
Contractor Manpower Reporting Requirements
The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
Reporting inputs will be for the labor executed during the period of performance for each
Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the CMRA help desk.
*Reporting Period: Contractors are required to input data by 31 October of each year.
However, due to the delayed implementation, input of FY12 data is required by 31 Mar 13.
Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.
User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the Army
CMRA link at http://www.ecmra.mil.
Appendix D
Voluntary Protection Program
APPLICABILITY: This section of the PWS applies to “Applicable Contractors” as defined below.
DEFINITIONS:
Applicable Contractor. A contractor whose employees worked at least 1,000 hours at the site in any calendar quarter within the last 12 months and is NOT directly supervised by the applicant
(installation).
Days Away, Restricted, and or Transfer Case Incident Rate (DART). The number of recordable injuries and illness cases per 100 full-time employees resulting in days away from work, restricted work activity, and/or job transfer that a site has experienced in a given time frame.

Total Case Incidence Rate (TCIR) - Total number of recordable injuries and illness cases per
100 full-time employees that a site has experienced in a given time frame.
Voluntary Protection Program - The Voluntary Protection Program (VPP) promotes effective worksite-based safety and health. In the VPP, management, labor, and OSHA establish cooperative relationships at workplaces that have implemented a comprehensive safety and health management system. Approval into VPP is OSHA’s official recognition of the outstanding efforts of employers and employees who have achieved exemplary occupational safety and health.
REQUIREMENTS:
Wright-Patterson AFB is in the process of pursuing VPP recognition or has already been recognized under the OSHA Voluntary Protection Program (VPP). VPP impacts all “applicable contractors” operating on Air Force Installations. The contractor shall be responsible for ensuring their employees and managers have a comprehensive understanding of VPP as well as full compliance with OSHA requirements. The contractor shall follow the safety and health rules of the installation or VPP site. Detailed information on VPP is available on the OSHA website at http://www/osha.gov/dcsp/vpp/index.html.
The contractor shall submit a Safety and Health Plan and corresponding site safety checklist to the Contracting Officer not later than 10 calendar days after contract award. The contractor’s plan shall include appropriate measures to ensure the contractor reacts promptly to investigate, correct, and track alleged safety and health violations and/or uncontrolled hazards in contractor work areas. The plan shall:
Demonstrate a management commitment to employee safety and health;
Identify the application of the safety and health plan to subcontractors;
Identify the roles and responsibilities of the following individuals with respect to safety and VPP: Management, Supervisors, Employees, and Safety Coordinator;
Identify applicable safety rules and regulations;
Include a worksite hazard analysis to include base-line hazard identification and required control measures;
Include a job site analysis to include hazards of tasks required to control measures;
Identify employee safety and health training requirements and the documentation process;
Include a workplace inspection frequency, to include identifying the individual conducting the inspections;

Include employee hazard reporting procedures;
Identify individual(s) responsible for corrective action of hazards;
Identify first aid/injury procedures;
Identify procedures for accident investigation and reporting;
Identify emergency response procedures; and
Identify the process for tracking controlled hazards in contractor work areas.
The contractor shall submit their Total Case Incidence Rate (TCIR) and Days Away, Restricted, and or Transfer Case Incident Rate (DART) rates and OSHA Form 300A annually to the contracting office for consolidation and submission as part of the installation’s annual VPP
Safety and Health Management report. TCIR and DART rates are due by the 15th of January of each year.
The contractor’s Quality Control Plan shall identify the processes and procedures the contractor will use to track compliance with the Safety and Health Plan, and the process and procedures that will be used to correct violation
The contractor shall establish these VPP requirements for all subcontractors who qualify as applicable contractors under the resulting contract.
