PERFORMANCE WORK STATEMENT (PWS)
FOR
PEST CONTROL AND MECHANICAL CLEANING

1. DESCRIPTION OF SERVICES. The Contractor shall provide all personnel, supervision, equipment, tools, materials and services required for pest control. This includes animal carcass removal, and mechanical rodent control cleaning methods such as sweeping and vacuuming spider cobwebs in shelters, test cells, production/storage facilities, tunnels, and administrative areas. These areas will be identified at the Task Order level. The areas that the Contractor will service are areas that are not part of the Government Civil Engineering (CE) Office’s Charter to service/maintain. All work will be accomplished at the Ogden Air Logistics Complex (OO-
ALC) at Hill Air Force Base (HAFB), Utah (UT) and at the Utah Test and Training Range
(UTTR) located approximately 100 miles West of HAFB.
1.1. BASIC SERVICES. The Contractor shall remove dead rodents, birds, animal carcasses and related biological waste or debris using method outlined in Appendix F. The Contractor shall clean the production/storage facilities, shelters, test cells, covered breezeways and tunnels listed in Appendix D. The Contractor shall set traps and lay poison to exterminate live rodents. The Contractor shall manually remove poisonous insects to include, but not limited to, Black Widow, Hobo, Brown Recluse, Wolf and Yellow Sack Spiders in areas identified in Appendix D to include the use of High Efficiency Portable Air (HEPA) vacuum cleaning measures. However, when manual efforts are not sufficient the
Contractor, when needed, may use PT 565 insecticide aerosole or any pesticide listed in the
Approved Standardized DoD Chemical Listing after gaining approval from the OO-ALC
ESOH Office and the Pest Management Office for products that are not listed in Appendix
E. The Contractor is liable for the health and welfare of their personnel. The Contractor shall ensure work is in accordance with (IAW) Center for Disease Control, HAFB
Department of Public Health and Department of Entomology guidelines. The Contractor shall perform to the standards in this contract and have a minimum of 5 years’ experience.
1.1.1. Mechanical Cleaning Tasks. The facilities shall be mechanically cleaned every 6 months, (on the first (1 st ) and third (3 rd ) quarter). The Contractor shall perform mechanical cleaning by removing dirt and debris from inside the facilities to include walls (up to 8 feet, or as high as the Contractor can reach standing flat on the ground using a broom), floors, maintenance pits, lifting grates and cleaning the drains mechanically. If the Contracting Officer Representative (COR) determines that the drains need to be flushed, then the Contractor shall flush the drains with water using the
Government’s water source/faucets. The Contractor shall mechanically clean foundations, shelves, ledges and around and under missile rails. The Contractor shall mechanically clean and disinfect inside facilities, under elevated flooring, and underground tunnel (below ground level) maintenance pits to meet the safety needs of the
OO-ALC as identified in this PWS. Each time the facilities are cleaned, the Contractor shall remove rodent droppings, carcasses, nests, litter, dust, soil and any other biological debris related to rodents outside of equipment (such as compressors, Heating, Ventilation and Air Conditioning (HVAC) systems, etc.) and the entire winch assembly. The
Contractor shall mechanically clean the brushes above the doors by hand to drag nests and branches from underneath and behind them to remove anything that power washing does not clean out.
1.1.2. Rodent Control. The Contractor shall provide rodent control measures, which include placing traps with at least one one-ounce bar of poison per trap in each defined area. The Contractor shall not use poison in administrative areas. As a general rule, the number of traps and poison for each facility shall be as follows: There must be one trap placed with the appropriate bait on the trap for every 2,000 square feet of facility as a minimum. Every facility with less than 2,000 square feet total shall have a minimum of two traps. However, if it deems more or less traps are necessary, the COR will coordinate with the Contractor to lay the proper amount. Each specific Task Order will identify the COR and the appropriate method of approval. For every trap placed there must be at least one one-ounce bar of poison placed in each non-administrative facility.
The Contractor shall place the traps and poison in the areas most appropriate for effective rodent/pest control in coordination with the COR. The Contractor shall replace the poison, reset traps, and remove carcasses every three months (once a quarter) and perform mechanical cleaning every 6 months, (on the first (1 st ) and third (3 rd ) quarter).
The Contractor shall comply with Air Force Instruction (AFI) 32-1053, Pest Management
Program. The Contractor shall provide data on all pesticides (rodenticides and insecticides) used on a monthly basis IAW AFI 32-1053 (Contractor Data Requirements
List (CDRL A001). The Contractor shall also provide a monthly report identifying the number of carcasses found in each building during the cleaning process (CDRL A001).
The Contractor shall identify by building number and specific location any areas needing repair (such as door sweeps, holes, cracks, drainage, etc.) to deter or prevent rodents or spiders monthly from entering these facilities (CDRL A001). This report shall include date performed, application site, applicator name/certification number, name and amount of pesticides (rodenticides and insecticides) used and be provided to the OO-ALC
Contracting Officer’s Representative (COR), and the 75 Civil Engineering Section (CES)
Entomology Program Manager on the 5 th day of each monthly basis (CDRL A001).
Should the 5 th fall on a weekend or holiday, the report shall be provided the workday before the 5 th
. The Contractor shall provide the Bioenvironmental Office with Material
Safety Data Sheet (MSDS) or Safety Data Sheet (SDS) information prior to initial usage of product (CDRL A002). MSDS and SDS shall be received by Bioenvironmental Office within 2 weeks after award of contract or 2 weeks prior to change of pesticide
(rodenticide and/or insecticide).
1.1.2.1. The Contractor shall ensure all applications of pesticides (rodenticides and insecticides) are made by personnel certified by the State of Utah in the appropriate pest control categories.
1.1.2.2. The Contractor shall ensure all pesticides (rodenticides and insecticides) are registered by the Environmental Protection Agency (EPA) and used according to label instructions. A list of all approved pesticides (rodenticides and insecticides) is listed in Appendix E. This list is not all inclusive and is subject to change. The Contractor shall use only pesticides (rodenticides and insecticides) listed in the Approved Standardized Department of Defense (DoD) Chemical
Listing. The Contractor shall get approval from the OO-ALC Environmental, Safety and Occupational Health (ESOH) Office and the Pest Management Office prior to use of products that are not listed in Appendix E. The Contractor shall contact the Bioenvironmental and 75 CES Entomology Program Managers prior to using any pesticides not listed in Appendix E. MSDS and SDS shall be received by OO-ALC Environmental, Safety and Occupational Health (ESOH)
Office and the Pest Management Office and approved for use 2 weeks after award of contract or 2 weeks prior to change of pesticides.
1.1.2.3. The Contractor shall furnish copies of pesticides (rodenticide and insecticide) labels and MSDSs or SDSs to the ESOH, HazCell Project Manager, Hazardous Material Compliance Office prior to initial usage of product (CDRL
A003).
1.1.3. The Contractor shall not use cleaning measures that will adversely affect the weapon systems in the facilities and their related explosives. The Contractor shall follow safety measure listed in Appendix F.
1.1.4. The Contractor shall take precautions during cleaning procedures to prevent hardware, tools, liquids or personnel from bumping, hitting, scraping or even touching any part of the test equipment (e.g., test stands) and weapon systems (e.g., motors, boosters, nozzles, etc.) at all times. It is imperative that special care is taken to ensure that no grounding wires at any time be disconnected, and that special care must be taken to insure that no part of the weapon system is sprayed with any chemicals.
1.1.5. The Contractor shall clean all Real Property Installed Equipment (RPIE) such as winch housing controls, mounting rails, etc. During cleaning, the Contractor shall not open any electrical panel boxes, air handling units (AHUs), compressors, air conditioners
(A/C), and heating units within any facility. The Contractor shall not use any devices
IAW Safety briefing given by the Missile training representative prior to being escorted around the facilities. The Contractor shall not have any radio frequency devices (e.g., cell phone, radios, key fobs, tablets, computers, iPads, etc.) or any camera or picture-taking devices in their possession at any of the buildings or areas being serviced. The
COR accompanying the contractor will stop cleaning operation if any cleaning measure adversely affects weapon system or RPIE or if any radio frequency devices is observed on their possession.
1.1.6. The Contractor shall carry two 10 pound (lb) Bicarbonate (BC) type fire extinguishers in their vehicle(s) at all times in case a spark from their vehicle or a spark from other equipment should occur, especially at UTTR where a fire could get out of control very quickly. The Contractor shall provide fire extinguisher training to their employees.
1.1.7. The Contractor shall remove all bird excrement, feathers and nesting material and any other bird debris from buildings identified at the Task Order level. This debris is found mainly on the outside of and above the large sliding cargo doors (over 10 feet) and in the trenches beneath these doors. The Contractor shall power wash the outside of the following buildings on an as needed bases utilizing CLIN 0002: 1320, 1321, 1403, 1404, 1405, 2329, and 2330. The electrical power must be turned off prior to any pressure cleaning. The Contractor shall not spray directly on electrical equipment and door seals.
A manual drive shaft and manual disconnect lever (for manual operation of these doors) are located at the top of each of the doors. Ensure the drive shaft, disconnect lever and areas around them are clean of bird debris. The Contractor shall notify the COR when these buildings have been power washed within 2 business days after completion.
1.1.8. The Contractor shall comply with all procedures established by Federal, State and local Government Offices that detail how to minimize risk and deal with Hanta Virus and other potential animal borne disease identification and cleanup. For further information on the Hanta Virus disease and other potential animal borne disease, to include cleanup and disposal procedures, contact the HAFB Public Health Flight at (801) 586-9768. The
Contractor can also refer to Appendix F for training on Hanta Virus.
1.1.9. When required, the Contractor shall respond to emergency situations within 2 business days after notification by the COR and/or the Contracting Officer (CO). The
COR and/or CO will verbally advise the Contractor of the effort required. Each Task
Order will identify the appropriate details of who notifies the Contractor.
1.1.10. On occasion, the Contractor’s services shall be required to support a contingency plan utilizing the Over and Above (O&A) Contract Line Item Number (CLIN 0002). The
Program Manager (PM) will notify the Contractor if this should occur. Special or emergency situations (e.g., weather-related situations, base closures, reduced activity around Christmas/New Year’s Day) may necessitate the Contractor operating on a reduced basis. Again, the CO will notify the Contractor of any reduced hours. If the activation or exercise requires effort outside normal duty hours or workload outside the parameters of, or any variation in, workload provisions of this contract (such as increase in Contractor effort) an upward equitable adjustment will be negotiated. Likewise, should any activation, exercise or reduced hours result in decreased Contractor effort; a downward equitable adjustment will be negotiated. If PM determines service is an emergency, contractor shall respond within 2 business days. All non-emergencies shall be complete within 1 work week.
1.2. SPECIFIC TASKS
1.2.1. The Contractor shall clean all buildings/areas identified at the Task Order level.
Proposal on such tasks will be based on square footage (see Appendix D for square footage per building).
1.2.2. The Contractor shall follow all Occupational Safety and Health Act (OSHA) standards when cleaning inside/outside the buildings for spiders, webs, and other insects.
The Contractor shall clean above cargo doors inside/outside the buildings, which could be higher than 10 feet.
1.2.3. If the Contractor identifies the need to move an item or equipment, the Contractor shall notify the COR. If the COR identifies the need to move an item or equipment, the
Government will provide personnel and equipment to move the object.
1.2.4. The Contractor shall clean in non-igloo type buildings between missile storage rails, to five feet from the ends of the missile motors currently being stored. The
Contractor shall be required to clean all walkways in between all sets of missile rails.
1.2.5. During the pre-inspection, or while cleaning any building, if the Contractor finds any evidence of potentially damaged, suspect, asbestos containing materials, they shall be identified to the COR in the monthly report (CDRL A001). The COR will inform the CE
Asbestos Shop regarding sampling and cleanup of these suspect materials. The
Contractor shall not clean areas where there is evidence of damaged, suspect, asbestos containing material.
1.2.6. The Contractor shall be responsible for disposal of bird, rodent and spider carcasses and excrement. The Government will not provide an area to dispose of bird, rodent, and spider remains and excretions on or off base. The Contractor shall dispose of carcasses and waste in accordance with local, State, and Government regulations.
1.2.7. The Contractor shall maintain the temperature and humidity in each of the buildings they clean per the following directions: Doors shall be kept open for no longer than three hours and then closed for four hours before they can be re-opened. No more than one cargo door shall be open at one time. This will allow the temperature to be maintained within the desired limits.
1.2.8. The Contractor shall use a HEPA Vacuum and ensure that it has electrical grounding capability (three wire power cord) prior to use. Guidelines for HEPA Vacuum use are as follows:
1.2.8.1. The missile shall be in a storage condition with all Electro-Explosive
Devices (EEDs) in a Safe condition.
1.2.8.2. The storage is a Class 2 explosive area (e.g., explosive dust, vapors, gasses, etc.).
1.2.8.3 Upon request contractor will provide COR with vacuum cleaner manuals indicating and validating that the radiated emission is not higher than f140 dBuV/M at any frequency
1.2.9. Because the storage bunker can have an explosive dust atmosphere, the vacuum cleaner must be intrinsically safe with regards to Electro-Static Discharge (ESD). This includes, but is not limited to the following: the vacuum must have a three wire power cord (i.e. power, neutral, and ground), and all metal parts on the vacuum and hose must be electrically bonded to the ground wire.
1.2.10. Since the EEDs and the missiles are in a storage condition, the vacuum cleaner must not have radiated emissions in excess of 140 dBuV/m at any frequency. This provides a safety margin of 26 dBuV for worst-case conditions. The Contractor shall refer to the HEPA Vacuum technical specifications for radiated emissions safety margin prior to use.
1.3. SPECIFIC TASKS FOR BUILDING TYPES
1.3.1. The Contractor shall clean the outside the buildings identified at Task Order level for bird excrement, feathers and nesting material. The Contractor shall be permitted to use high pressure cleaning to remove debris. The Contractor shall not spray directly on electrical equipment and door seals. The electrical power must be turned off prior to any high pressure cleaning. The Contractor shall contact the Facility Manager to have the power turned off. A list of Facility Managers for each building shall be provided at the
Task Order level.
1.3.2. In buildings where applicable, the Contractor shall be required to remove the winch clutch covers to clean and reinstall after cleaning. The Contractor shall not spray directly on the clutch, cable and cable spool. The Contractor shall mechanically clean the rail structure and under the missile/motors in applicable buildings.
1.3.3. The Contractor shall clean under the raised floor in buildings 30025, 60005, 60020 at the UTTR. The Contractor shall not step or walk on any area under the raised floor.
The Contractor shall use caution in spraying and vacuuming this area. The tunnels are considered to be confined areas, but a permit is not required.
1.3.4 The COR will perform a 10% random sampling of cleaned buildings per month.
Buildings will be selected from the plan/cleaning schedule for that month. The COR will complete a visual inspection of the buildings the contractor has cleaned within 24 hours of completion. Observation of dead rodent, bug, bird, droppings, carcasses, nests, debris, and unclean winch housings constitute a defect. More than 3 cleaning defects per month indicate the contract is not meeting the standard of this PWS.
1.4. OVER AND ABOVE SERVICES
1.4.1. The Contractor shall provide additional services on an hourly, as-needed basis that are undefinable in extent, duration, size, and amount in accordance with FAR 16.601(c).
These types of services include, but are not limited to, out of cycle infestations; cleaning and/or power washing soiled areas to include breezeways and building exteriors; and out of cycle pest and rodent control and cleaning that is in excess of the typical cycle requirements. Other examples of out of cycle cleaning may include unexpected events such as inspections, Distinguished Visitor (DV) visits and unexpected services within the scope of this PWS as needs arise.
1.4.1.1 The COR will notify the Contractor of over and above services required.
The Contractor shall provide the COR the number of labor hours needed to perform the service. The COR will validate that the labor hours quoted are fair and reasonable and forward the quote to the CO for approval. The CO will notify the COR of the final determination who in turn will inform the Contractor.

3. GOVERNMENT FURNISHED PROPERTY, EQUIPMENT & SERVICES. Under this contract the Government will not provide any Government furnished property. The
Government will provide services listed in para 3.1 (below).
3.1. GOVERNMENT FURNISHED SERVICES.
3.1.1. Telephone. The Government will provide telephone service limited to matters related to the performance of this contract (Official Business/Local Calls Only).
3.1.2. Base Civil Engineering. The Government will provide fire prevention and protection; inspection and maintenance of Government furnished fire extinguishers and systems, pest control, and ground maintenance. At HAFB the Fire Department telephone extension is 911 for emergencies and (801) 777-3021 for routine calls. At UTTR and
Oasis, the Fire Department telephone extension is 911 for emergencies (using a land line only) and for emergencies and routine calls using a cell phone (801) 777-1555 (if you dial 911 on a cell phone at UTTR/Oasis it will direct you to Wendover or Toole).
3.1.3. Emergency Medical Service. The Government is not responsible for, but will provide, emergency medical treatment and emergency patient transportation service if
Contractor personnel are injured or become critically ill during the performance of this contract. The Contractor shall reimburse the Government for the cost of medical treatment and patient transportation service at the current inpatient treatment rate as appropriate.
3.1.4. Employee Training. The Government (OO-ALC) will provide training for explosive safety, emergency preparedness, and mishap preservation and security awareness only if unavailable elsewhere.
3.2. FOREIGN OBJECT DEBRIS AND TOOL CONTROL. The Contractor shall at a minimum comply with tool and FOD control and accountability procedures as listed herein when Contractor owned tools or equipment are brought into any maintenance area within the complex:
3.2.1. The Contractor shall mark Contractor owned tools with a Contractor unique identifier (i.e., company name) enabling traceability back to the Contractor.
3.2.2. The Contractor shall maintain a tool listing to inventory tools and equipment, at the beginning and end of each job task, change in job location and at the end of the day when work is completed. Inventories must be documented and are subject to inspection by the
COR upon request. Use of the AFMC Form 309 (AFMC Tool Control Inventory Record) is optional.
3.2.3. The Contractor shall immediately report any lost tools, equipment, and Foreign
Object Debris (FOD), and comply with lost tool procedures outlined in the most current version of Chapter 10 of: AFI 21-101, AFI 21-101 AFMC SUP, and AFI 21-101 AFMC
SUP OO-ALC Guidance Memorandum. The Contractor shall notify the COR of the lost tool and provide the OO-ALC Tool Manager a copy of AFMC Form 310 Lost/Found
Item Report.

3.2.4. The Contractor shall identify Contractor-owned rags to enable traceability back to the Contractor. The Contractor shall inventory all rags and work residue, at the beginning and end of each job task, change in job location and at the end of the day when work is completed. Inventories must be documented and are subject to inspection by the COR upon request.
3.2.5. The Contractor shall ensure contract employees are trained on tool and FOD control and accountability procedures. A monthly tool, equipment and rag inspection will be conducted by the COR to ensure all items are marked and accounted for.
4. CONTRACTOR EMPLOYEES.
4.1. The Contractor shall not employ persons for work on this contract if such employee is identified to the Contractor by the acting officer as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.
4.2. Contractor personnel shall present a neat and clean appearance and be easily recognized as Contractor employees. The Contractor shall furnish an identification badge, which will include as a minimum, the person’s name, the name of the Contractor. Each employee will wear the badge on the outer clothing on the front of the body between the neck and waist so that the badge is visible at all times.
4.3. The Contractor shall ensure that employees have the appropriate current and valid professional certifications IAW AFI 32-1053, paragraph 4.7.3 and 4.7.4, before starting under this contract. The Contractor shall have all state and local business licenses, certification, certificates required to perform this effort.
4.4. CONTROL OF CONTRACTOR EMPLOYEES.
4.4.1. The selection, assignment, reassignment, transfer, supervision, management, and control of Contractor employees in performance of this PWS will be the responsibility and prerogative of the Contractor. However; the Contractor shall comply with the requirements set forth in the PWS, and in HAFB regulations, concerning conduct of employees as referenced herein.
4.5. SECURITY REQUIREMENTS.
4.5.1. All security requirements will be IAW the DD Form 254; Industrial Security
Classification Specification attached to this contract.
4.5.2. The Contractor shall complete a "Request for Identification Credential” (Air Force
Materiel Command (AFMC) Form 496)) for each employee of the Contractor requiring access to HAFB. The requests will be submitted to Pass and Registration (Building
1296).
4.5.3. The Contractor shall complete an “Unescorted Entry Authorization Certificate”
(AF Form 2586) for each employee requiring entry into controlled areas. The request will be submitted to Pass and Registration. The Government will provide “Air Force Entry
Control Card” (AF Form 1199).
4.5.4. The Contractor shall possess or obtain a facility clearance at the classification level of Secret. Personnel involved in the actual on site work effort of this contract shall possess a clearance of Secret.
4.5.5. Prior to the start of the first operational performance period, the Contractor shall ensure that sufficient personnel on duty have appropriate security clearances to meet and maintain requirements specified in the PWS. This requirement must be completed by the end of the orientation period.
4.5.6. The Contractor is required to comply with all security regulations and directives as identified herein and other security requirements as are shown elsewhere in this contract.
The Contractor shall comply with DD Form 254, DoD Contract Security Classification
Specification.
4.5.7. Physical Security: The Contractor shall be responsible for safeguarding all
Government property they have access to. At the end of each work period, all
Government facilities, equipment and materials shall be secured.
4.5.8. Lock Combinations. (NOTE: OPTIONAL DEPENDING ON LOCAL
REQUIREMENTS). The Contractor shall establish and implement methods of ensuring that all lock combinations are not revealed to unauthorized persons. The Contractor shall ensure that lock combinations are changed when personnel having access to the combinations no longer have a need to know such combinations. These procedures will be included in the Contractor’s Quality Control Plan.
4.6. EMPLOYEE TRAINING
4.6.1. The Contractor shall be required to contact the 309 MMXG Training Manager within seven business days after contract award to obtain a registered account for all
Contractor employees to be entered into the Training Scheduling System (TSS) and the
Production Acceptance Certification System (PACS). The 309 MMXG Training
Manager will schedule the Contractor and his employees for all mandatory training classes. The Contractor shall complete all mandatory training and certifications and ensure they are loaded in TSS and PACS. The Contractor shall provide their own
Hazardous Communication training and Forklift training to their employees if they will be using a Forklift, JLG high lift or any type of aerial lift. The Contractor is also responsible for renting their own forklift, JLG or aerial lift equipment if they are needed.
The Contractor shall also provide fire extinguisher training to their employees. This training shall be documented in TSS and/or PACS.
4.6.2. Contractor personnel must receive Explosive Safety Awareness Training if their duties require entry into the Explosive Clear Zone. This training will be provided by the
OO-ALC Weapons Safety Manager for all area located at HAFB. For UTTR and Oasis, Contractor personnel must receive Explosive Safety Awareness Training that will be provided by the Additional Duty Weapons Safety Manager (ADWSM), 75 CEG/CEUE,
(801) 775-6209. Also, all other safety issues will be directed to the CO. All training will be completed during the orientation period of this contract. This training must be accomplished before an Air Force (AF) Entry Control Card (AF Form 1199) will be issued.
4.7. HOURS OF OPERATION
4.7.1. Normal Hours of Operation: The Contractor shall perform the services required under this contract on the following days and hours (except on Federal Holidays):
MMXG buildings at HAFB and UTTR: Monday through Thursday, 7:00 a.m. to 3:00 p.m.
MMXG Buildings at UTTR Oasis: Monday, 12:00 p.m. to 5:00 p.m., Tuesday and Wednesday, 7:00 a.m. to 5:00 p.m. and Thursday, 7:00 a.m.
to 12:00 p.m.
CMXG Buildings at HAFB: Monday through Thursday, 5:00 a.m. to 3:30 p.m.
The Contractor shall not perform the services required between 1100 a.m. and 1200 p.m.
(unless otherwise specified by CO or COR). Schedule adjustments will be approved by the COR. Shifts are susceptible to change during Task Order period of performance
(PoP) due to Government worksite availability, i.e., Government furloughs. The
Contractor shall work, with prior approval of the CO, extended hours to ensure timely completion of work at no additional cost to the Government. If we are in lock down for the Strategic Arms Reduction Treaty (START) with Russians inspectors, the cleaning must cease until the inspections are finished. This is usually only a 24 hour delay where the Contractor would have to stand down.
4.7.2. Recognized Holidays: The Contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial
Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.
4.7.3. Emergency/Urgent Services: On occasion, services may be required to support a contingency plan outside the normal duty hours described above.
4.8. CONSERVATION OF UTILITIES
4.8.1. The Contractor shall instruct employees in utilities conservation practices. The
Contractor shall be responsible for conserving the use of utilities, which include but not limited to the following:
4.8.1.1. Lights shall be used only in areas where and when work is actually being performed.
4.8.1.2. Water faucets or valves shall be turned off after the required use has been accomplished.
4.8.1.3. Government telephones shall be used only for official Government business.
4.8.1.4. Doors on environmentally controlled buildings shall be kept closed during inclement weather.
4.9. ENVIRONMENTAL CONTROLS
4.9.1. Compliance with Laws and Regulations. The Contractor shall be knowledgeable of, and comply with, applicable Interstate, Federal, State, and local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the Contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the Contractor shall inform the CO pursuant to notice requirements
(see Federal Acquisition Regulation (FAR) 52.243-7, 43.104)) and negotiate a modification to the contract.
4.9.2. Notification of Environmental Spills. If the Contractor spills or releases any substance contained in 40 Code of Federal Regulations (CFR) 302 into the environment, the Contractor or its agent will immediately report the incident to the HAFB
Environmental Spill Line at (801) 430-3860 and report the incident. The liability for the spill or release of such substances rests solely with the Contractor and its agent.
4.9.3. Material Storage and Use. The Contractor shall follow manufacturer’s guidelines and professional recommendations for control of humidity, temperature, cleanliness, and materials handling. This includes hazardous materials.
4.9.4. Targeted Chemicals. The Contractor is not required to purchase or use any Class I
Ozone Depleting Compounds (ODCs). The Contractor shall not use any Class I ODCs or
EPA 17 chemicals without notifying the CO in advance of the anticipated usage
(quantity, rate, and time period) of each such substance.
4.9.5. Government Observations. Government personnel, other than COs and CORs, may from time to time, with CO coordination, observe Contractor operations. However, these personnel may not interfere with Contractor performance.
4.10. SAFETY REQUIREMENTS
4.10.1. In performing work under this contract, the Contractor shall:
4.10.2. Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.
4.10.3. Take such additional immediate precautions as the CO may reasonably require for safety and mishap prevention purposes.
4.10.4. Develop and provide at the start of the orientation period or the start of the first operational performance period a safety plan for the protection of Government facilities and property and to provide a safe work environment for Contractor personnel.
4.10.5. Provide protection to Government property to prevent damage during the period of time the property is under the control or in possession of the Contractor.
4.10.6. Include a clause in all subcontracts to require sub-Contractors to comply with the safety provisions of this contract as applicable.
4.10.7. Record and report promptly (within one hour) to the CO or designated COR, all available facts relating to each instance of damage to Government property or injury to either Contractor or Government personnel.
4.10.8. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO.
4.10.9. If the Government elects to conduct an investigation of the accident/mishap, the
Contractor shall cooperate fully and assist Government personnel in the conduct of investigation until the investigation is completed.
4.10.10. Include a clause in each applicable subcontract requiring the sub-Contractor’s cooperation and assistance in accident reporting and investigation.
4.10.11. Comply with OO-ALC safety requirements listed in the technical publication
Appendix C within this PWS.
4.11. LABOR HOURS REPORTING. The Contractor shall report all Contractor labor hours (including sub-Contractor labor hours) required for performance of services provided under this contract for the maintenance services via a secure data collection site. The
Contractor is required to completely fill in all required data fields in the Enterprise-wide
Contractor Manpower Reporting Application (eCMRA) at http://www.ecmra.mil.
4.11.1. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September.
While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the eCMRA

4.11.2. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and Contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the Contractor name and contract number associated with the data.
4.11.3. User Manuals: Data for Air Force service requirements must be input at the Air
Force eCMRA link. However, user manuals for Government personnel and Contractors are available at the Army eCMRA link at http://www.ecmra.mil.
5. PROJECTED WORKLOAD DATA. The Contractor shall provide a cleaning plan/cleaning schedule to the PM and COR as stated in CDRL A004

SECTION I - GENERAL REQUIREMENTS
A. Safety Program Requirements.
The Contractor will implement a Safety Program plan that ensures protection of
Government personnel and property. The program will consist of, as a minimum:
1. Mishap reporting, as defined in paragraph B1 below.
2. A Safety Plan that addresses, as a minimum, the subjects listed in Section II –Specific
Requirements, and will be used during the performance of the work described in the contract.
The Safety Plan will be accepted by OO-ALC/SE (Safety Office) prior to commencement of any work described in this contract.
3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.
4. Competent personnel to provide surveillance of the Safety Plan.
5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.
6. All Contractor personnel shall be trained and qualified to perform their duties safely.
7. The Contractor shall include a clause in all subcontracts requiring the sub-Contractors to comply with the safety provisions of this contract, as applicable.
B. Mishap Notification
1. The Contractor shall notify OO-ALC/SE (801-586-6038) or the Hill AFB Command Post
(801-777-3007) after normal duty hours, and the designated Government Representative (GR), i.e., the ACO, PCO, or COR within one hour of all mishaps or incidents at or exceeding $2,000
(material + labor) in damage to DoD property entrusted by this contract, even if the Government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to OO-ALC/SE. For information not available at the time of initial notification, the Contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.
Mishap notifications shall contain, as a minimum, the following information:
(a) Contract, Contract Number, Name and Title of Person(s) Reporting
(b) Date, Time and exact location of accident/incident
(c) Brief Narrative of accident/incident (Events leading to accident/incident)
(d) Cause of accident/incident, if known
(e) Estimated cost of accident/incident (material and labor to repair/replace)
(f) Nomenclature of equipment and personnel involved in accident/incident
(g) Corrective actions (taken or proposed)
(h) Other pertinent information
2. The Contractor shall cooperate with any and all Government mishap investigations.
Additionally if requested by Government personnel or designated Government representative
(GR), i.e., the ACO, PCO, or COR, the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.
3. The Contractor shall provide copies of Contractor data related to mishaps, such as Contractor analyses, test reports, summaries of investigations, etc. as necessary to support the Government investigation.
4. The Contractor shall support and comply with the safety investigation and reporting requirements of AFI 91-204, Chapters 1 through 5.
C. General Safety Requirements:
If the safety plan is modified, the Contractor shall submit the proposed modification, in writing, to the Contract Administration Office Safety Representative.
The Contractor is solely responsible for compliance with all federal, state and local laws, the Occupational, Safety and Health Act (OSHA) (Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable, Air Force Occupational
Safety and Health Standards (AFOSH STD), to include AFI 91-203, Air Force Consolidated
Occupational Safety Instruction, AFI 91-204, Safety investigations Reports, AFPAM 91-210, Contract Safety, and the protection of their employees. Additionally, the Contractor is responsible for the safety and health of all sub-Contractor employees. All documents required as a result of OSHA 29 CFR 1910 and 1926 regulations, (i.e. certifications, training, respiratory protection program, workplace hazard assessments), shall be made available to the COR upon request.
The Contractor shall ensure assigned personnel are adequately trained and qualified for the task being performed. Brief all personnel on the hazards involved with operations and applicable precautions to be taken.

Hazard Assessment. In accordance with 1910.132(d)(2), the Contractor shall conduct a certified Hazard Assessment on all work areas. A follow-on survey shall be conducted by the
Contractor when the workload or environment conditions change. If respirators are necessary to protect the health of the employee or whenever respirators are required by the Contractor, the
Contractor employer shall establish and implement a written respiratory protection program according to 29 CFR 1910.134.
Contractor personnel must receive Explosive Safety Awareness Training if their duties require entry into the Explosive Clear Zone. This training will be provided by the OO-ALC Safety

A Risk Assessment will be required prior to any work being accomplished, if munitions will remain in the facility.
Compliance with OSHA and other applicable laws and regulations for the protection of
Contractor employees is exclusively the obligation of the Contractor. Note: AFOSH STDs are annotated because many of the Air Force Standards exceed the OSHA standard criteria. If a conflict is noted, the most stringent requirement takes precedence. The Government shall assume no liability or responsibility for the Contractor's compliance or non-compliance with such requirements. The Contractor shall furnish to each of his/her employees a place of employment, which is free from recognized hazards. The Contractor shall brief his/her employees on the safety requirements of this contract and on hazards associated with prescribed tasks. The Contractor is responsible for compliance with OSHA Public Law and the resultant standards identified within. In addition, the Contractor is required to flow down the safety requirements/specification to all sub-Contractors. This applies to Federal Acquisition Regulation
(FAR) 12 commercial acquisitions as well. This contract shall in no way require persons to work in surroundings or under working conditions which are unsafe or dangerous to their health. The
