B2280, B2122, & B3225 OVERHEAD FALL

PROTECTION

STATEMENT OF WORK

TINKER AIR FORCE BASE, OKLAHOMA

STATEMENT OF WORK NO: QPE-SOW-JH003

29 September 2015

1. INTRODUCTION

The Aircraft Maintenance Group (AMXG) requires the installation of a rigid rail system to provide overhead fall protection in buildings 2122S, 2280 and 3225. The fall protection shall cover multiple types of aircrafts, for use in two (2) aircraft docks in the south side of building 2122 (west and center docks), two (2) aircraft docks in building 2280 (north and south docks) and one (1) dock in building 3225 at Tinker Air Force Base. The fall protection system shall cover the B-1, B-52, and KC-135 in the west dock of B2122S. The fall protection system shall cover the KC-135 in the center dock of B2122S. The fall protection system shall cover the B-1, B-52, C-130, KC-135, E-3, and E-6 in the south dock of B2280. The fall protection system shall cover the B-1, B-52, C-130, KC-135, KC-10, E-3, and E-6 in the north dock of B2280. The fall protection system shall cover the B-1, C-130, KC-135, E-3 and E-6 aircrafts in the dock at B3225.

Contractor shall provide a complete turn-key on-site application, design, fabrication, assembly, inspection, testing, documentation, training, delivery, tools, equipment installation, labor and other incidentals necessary to provide the systems in accordance with this Statement of Work (SOW), QPE-SOW-JH003 and drawing 0108617A.

1.1. Problem Statement.

Currently there is a need to update and replace the existing overhead fall protection in buildings 2122S, 2280 and 3225 to ensure that the systems are certified to meet AFI, OSHA, and ANSI standards. The new fall protection system must be compatible and parts interchangeable with the other fall protection systems in the facilities. The systems shall provide for an ergonomic, OSHA compliant, ISO compliant system with ease and speed of set-up, easy connectivity, height adjustable, general flexibility to support the users processes or technology; or requirements specific to aircraft maintenance.

1.2. Scope.

The new fall protection systems can be replaced in the current fall protection location if the angle requirements, codes and regulations are met for all aircraft worked in that hangar as provided in this SOW; it shall be the contractor’s responsibility to make that determination. Adjustments shall be required for new fall protection systems when necessary to update the fall protection systems to meet current fall protection requirements, codes and regulations.

1.2.1. The contractor shall provide a complete turn-key on-site application, design, fabrication, assemble, inspection, testing, documentation, training, delivery, tools, equipment installation, labor and other incidentals necessary to provide overhead fall protection systems in accordance with this Statement of Work (SOW), drawing 0108617A, Section 05 50 11, Section 11 24 12, and Section 11 24 13. The fall protection systems shall be operational and usable to facilitate fall protection for mechanics while working on the aircrafts.

1.2.1.1. The new enclosed rigid-rail system shall replace the existing fall protection system over the upper working surfaces of the B-1, B-52, C-130, KC-135, a system for working over the B- 52 vertical when removed from aircraft and laid flat on a stand at the northeast side, and a system for working off of aircraft, over the KC-135 vertical when removed from aircraft and laid flat on a stand at the southeast side, in the west dock of B2122S. See attached SOW drawing 0108617A sheet S101 and S501 for details.

1.2.1.2. The new enclosed rigid-rail system shall replace the existing fall protection system over the upper working surfaces of the KC135, and a system for working over the KC-135 vertical when removed from aircraft and laid flat on a stand at the northwest side, in the center dock of B2122S. See attached SOW drawing 0108617A sheet S102 and S501 for details.

1.2.1.3. The new enclosed rigid-rail system shall replace the existing fall protection system over the upper working surfaces of the B-1, B-52, C-130, KC-135, E-3, and E-6 aircrafts in the south dock of B2280. The new system shall not interfere with the Aerial Work Platforms (AWP) in the south dock of B2280. See attached SOW drawing 0108617A sheet S103 and S501 for details.

1.2.1.4. The new enclosed rigid-rail system shall replace the existing fall protection system over the upper working surfaces of the B-1, B-52, C-130, KC-135, KC-10, E-3, and E-6 aircrafts in the north dock of B2280. The new system shall not interfere with the Aerial Work Platforms (AWP) in the north dock of B2280. See attached SOW drawing 0108617A sheet S104 and S501 for details.

1.2.1.5. The new enclosed rigid-rail system shall replace the existing fall protection system over the upper working surfaces of the B-1, C-130, KC-135, E-3 and E-6 aircrafts in the dock of B3225. See attached SOW drawing 0108617A sheet S105 and S501 for details.

1.2.2. The contractor shall demo all existing overhead fall protection over the west and center dock in B2122S, over the wings in the south dock of B2280 and over north dock of B2280, and over dock of B3225. Contractor shall dispose of existing overhead fall protection cables and associated equipment on site at Tinker AFB as directed by contract monitor.

1.2.3. The contractor shall perform a site visit to field verify and to obtain site specific measurements for the design and preparation of plans and drawings for the fall protection systems.

1.2.4. The contractor must identify any issues or obstacles that will interfere with the design and placement of the new fall protection cables before the date of the installation.

1.2.5. The contractor shall provide and install an enclosed rigid-rail overhead fall protection system that provides protection for workers while they work on the aircraft surfaces in the west & center docks in B2122S, over south and north docks in B2280, and over the dock area of B3225. The contractor shall ensure that the system is designed for the worst case scenario, maximum number of workers falling at the same time and the same place.

1.2.5.1. When complete the west dock in B2122S shall have fall protection for a total of forty-eight (48) users:

a. Six (6) users on each wing system for a total of twenty-four (24) users.

b. Eight (8) users on each fuselage for a total of sixteen (16) users.

d. Two (2) users on the tail for B-52 aircraft for a total of two (2) users.

e. Three (3) users over the B52 vertical (removed from aircraft).

f. Three (3) users over the KC-135 vertical (removed from aircraft).

1.2.5.2. When complete the center dock in B2122S shall have fall protection for a total of twenty-three (23) users.

a. Six (6) users on each wing system for a total of twelve (12) users.

b. Eight (8) users on each fuselage for a total of eight (8) users.

c. Three (3) users over the KC-135 vertical (removed from aircraft).

1.2.5.3. When complete the south dock in B2280 shall have fall protection for a total of twelve

(12) users.

a. Six (6) users on each wing system for a total of twelve (12) users.

1.2.5.4. When complete the north dock in B2280 shall have fall protection for a total of twenty-eight (28) users.

a. Six (6) users on each wing system for a total of twenty-four (24) users.

b. Four (4) users on each fuselage for a total of four (4) users.

1.2.5.5. When complete B3225 shall have fall protection for a total of sixteen (16) users.

a. Six (6) users on each wing system for a total of twelve (12) users.

b. Four (4) users on each fuselage for a total of four (4) users.

1.2.6. The overhead fuselage fall protection shall allow the users to walk the entire length of the aircraft while the trolley remains approximately overhead. The wing fall protection shall allow the users to walk from wing tip to fuselage while the trolley remains approximately overhead.

The fall arrest distance for the wing systems shall be less than 5’6” and for the fuselage system shall be less than 10’-0”, and be designed to prevent the user from coming in contact with the next level below the supporting structure (e.g. work stands). The system shall be designed to be attached to the existing structure using engineered structural steel members. These members shall be fabricated in accordance with the American Institute of Steel Construction (AISC) Steel Construction Manual and the American Welding Society (AWS) D1.1 Structural Welding Code by certified welders and inspectors. The required structural members shall have material test certificates for full material traceability.

1.2.7. The system shall consist of structural steel as specified on the attached drawing. The systems shall be totally hands-free for the user and shall allow the user to work with both hands as he/she is traveling along the walking surface. Anchorage connectors shall be designed to withstand the maximum (worst case scenario, maximum number of users falling at the same time and place) fall arrest forces with a minimum safety factor as specified in ANSI/ASSE Z359.6.

The system shall have load carrying trollies. The trolley shall allow for workers to pass between lifeline supports without disconnecting from the cable and allow for hands free operation of the system. The supports shall be designed to support a load on the system of at least two (2) times the maximum design load without failure.

1.2.8. The contractor shall provide and install: Sixty-two (62) load carrying trolleys for B2122S, forty (40) load carrying trolleys for B2280, and sixteen (16) load carrying trolleys for B3225, for a total of one-hundred eighteen (118) load carrying trolleys for all three (3) buildings. Existing SRLs will be reused for this installation. Contractor shall secure, store, and reinstall the SRLs.

The fall protection systems shall be identified with an ID Plate. All fall protection systems shall be labeled for maximum number of users and labeled for visibility from floor level.

1.3. Impact Statement. If a new and updated overhead fall protection system is not installed, the aircraft wash, strip and paint operations will continue to experience safety write ups and maintenance personnel will continue to be exposed to a potential and unnecessary fall hazard.

2. DOCUMENTATION

2.1. Statement of Work. Statement of Work QPE-SOW-JH003, drawing 0108617A, Section 05 50 11, Section 11 24 12, and Section 11 24 13 describes the requirements and capabilities that shall be incorporated in the new overhead fall protection systems for B2122S, B2280 and B3225.

2.2. Additional Supporting Data. The government will provide applicable drawings and any additional required supporting data, based on authorized and availability, not provided in this SOW documentation package, as requested by contractor within 30 calendar days after receipt of award. The government provided drawings are approximate. Any signatures of drawing/layout will be approval of orientation with the aircraft and not actual layout in reference to the facility.

The government shall not verify dimensions. The contractor shall be responsible for taking all measurements and ensuring that those measurements are accurate to facilitate a successful fit and function test after installation. If the fit and function test fails the contractor shall be responsible to reorient fall protection to match aircraft at no expense to the government.

2.3. References. All work and equipment provided under this solicitation shall follow good engineering practices as documented in accepted engineering literature, and shall satisfy as a minimum the following publications of the issue in effect on the date of request for proposal that form a part of this statement of work.

2.4. Order of Precedence. In the event of a conflict between the text of this statement of work and the references cited herein, the text of the statement of work shall take precedence.

Nothing in this document, however, supersedes applicable laws and regulations unless a specific variance or waiver has been obtained.

3. REQUIREMENTS

The contractor shall provide all necessary qualified personnel, management, non-personal administrative and technical services, materials and contractor facilities in support of the requirements of this SOW.

3.1. Task Description. The contractor shall provide engineering services and deliverables to support the following tasks.

3.1.1. Program Management. The contractor shall provide a program plan describing work task, schedule, cost and deliverables. A program kickoff meeting (teleconference is acceptable) shall be conducted after task award and prior to delivery of the program plan to ensure all parties fully understand the requirements and are ready to proceed. Additionally, Technical Interchange Meetings (TIMs) shall be convened at intervals as required by the Contract Monitor, 76AMXG/QPE. The contractor shall prepare the agenda and minutes of the TIMs as required.

The contractor shall prepare and deliver monthly status reports.

Informal communication through face-to-face, telephone, electronic mail, and facsimiles throughout the project is encouraged and shall be supported. All communication either formal or informal shall be documented in writing and shall become a permanent portion of the next scheduled TIM minutes. Any communications either formal or informal that are not introduced as official items at the next scheduled TIM shall not be interpreted as approved by or for the government.

3.1.2. Baseline Requirements. The contractor shall provide complete design, fabrication, inspection, testing, documentation, training, delivery, tools, labor, and other incidentals necessary in accordance with this SOW, the design drawings and specifications.

3.1.2.1. Requirements Review. The contractor shall hold a requirements review at Tinker AFB or teleconference within 15 calendar days after contract award. The primary purpose of the review will be the identification of key personnel, and for final clarification of the requirements.

The contractor shall prepare the agenda and technical report of this review. Findings and recommendations will be incorporated into the meeting minutes.

All reports or submittals by the contractor shall be submitted to the Contract Monitor and the Contracting Officer (CO).

3.1.2.2. Pre-Installation Review (PIR). The contractor shall hold a PIR at Tinker AFB thirty

(30) calendar days before fall protection system installation. An installation data package and master schedule shall be provided by the contractor five (5) working days prior to the PIR. The contractor shall prepare the agenda and minutes for the PIR.

The contractor shall address as a minimum the following:

a. Master Schedule

b. Personnel Training

c. Equipment Technical Data

d. Warranty Description

The installation shall be reviewed by the government and the contract monitor shall provide the contractor written comments. Any change that is requested by the contractor must be approved by the CO may require an additional submittal of the updated drawings to be submitted within 30 calendar days after review.

3.1.3. Final Verification. Upon completion and acceptance of the task requirement 3.1.2 of this contract, the manufacturer shall begin the delivery and installation of the fall protection system. The contractor shall also support government personnel with a full functional checkout/demonstration of the fall protection system with aircraft present. This “fit and function test” shall be performed by government personnel, with the Contractor present for consultation and documentation of this fit and function test. If any discrepancies are noted, the Contractor shall document the deficiencies and corrective actions for submission to the contract monitor and CO for approval prior to making corrections on all applicable drawings, fall protection components, and documentation. Any changes necessary (technical or non-technical) shall be requested by the contract monitor to the CO and then issued by the CO. The contractor is fully responsible for ensuring that the fit and function test is a success. Any discrepancies shall be corrected by the contractor at no cost to the government. The contactor shall be responsible for all costs associated with corrective actions.

3.1.3.1. Installation. The contractor shall procure the equipment (including lifts) and hardware necessary to support the installation of the fall protection system. All equipment and hardware shall be off the shelf items to the fullest extent possible. The Contractor shall perform all installation activities on-site at Tinker AFB.

3.1.3.2. Shipment. The contractor shall be responsible for fall protection preservation, packaging, loading, shipping, unloading, on-site placement and storage at Tinker AFB IAW the manufacturer’s commercial practices unless otherwise specified herein. The contractor shall provide a fully authorized representative on-site at all times during the receipt, assembly, installation, testing, and acceptance. The government shall not provide storage for intermittently shipped equipment.

3.1.3.3. Assembly.

3.1.3.3.1. General Assembly. Safety equipment shall be used during fall protection assembly.

Safety equipment shall contain up-to-date certifications and shall be in good working condition.

Remediation of any component that cannot be assembled properly as a result of errors in manufacturing or deformation due to handling or transportation shall be the responsibility of the contractor. The contractor shall immediately report any discoveries to the contract monitor and CO. Repair methods shall have prior approval by the contract monitor before any work is performed. The contractor shall replace all items damaged beyond acceptable repair at no cost to the government.

3.1.3.3.2. Assembly Site Limitations. The contractor shall take the following precautions on-site:

a. All assembly work shall be completed in such a manner as to minimize disruption of government activities. The contractor shall perform any on-site work that shall disrupt government activities during non-production hours and shall be coordinated and approved by the contract monitor.

b. The contractor’s work area shall be maintained clean and orderly. Debris shall be removed and disposed of daily by contractor.

c. The contractor shall dispose of existing fall protection system on site at TAFB as directed by Contract Representative

d. Material cutting shall not introduce shavings or chips in, onto or around the aircraft, aircraft access panels, or aircraft engines.

e. All phases of work shall be accomplished safely to comply with OSHA 29 CFR

1910.29, 1926.450, 1926.451, 1926.452, and AFI 91-203.

f. The contractor tooling shall not be stored on-site unless in a secured container. All tooling shall be removed at the end of each work shift by the contractor.

g. The contractor shall be responsible for handling equipment and supplies during progress of the contract.

h. Utility requirements shall be the responsibility of the contractor. Sources of electrical power and compressed air shall be provided on an “as available” basis. The contractor shall be responsible for ensuring adequate utility supplies and hookups are available on-site prior to the time of need.

i. No welding is permitted within fifty (50) feet of an aircraft.

j. Storage of flammable materials such as paints and solvents shall be in authorized areas and approved through the contract monitor.

k. The contractor shall comply with base requirements on handling hazardous materials.

Disposal of hazardous materials shall be coordinated with the contract monitor and performed by the contractor.

l. The contractor employees shall not wear watches, rings, or jewelry within aircraft maintenance areas. Proper personal protective equipment (safety glasses, hearing protection, hard hats, safety shoes and protective coveralls) shall be worn in and around all production areas.

3.1.3.4. Installation Requirements.

3.1.3.4.1. General. The contractor shall provide all necessary equipment necessary to support the delivery, assembly and installation of all items identified in this SOW. The contractor shall supply all necessary manpower for the on-site installations. The contractor shall coordinate the design and installation details with the Aircraft Facilities and schedule, and contract monitor, (Engineering 76AMXG/QPE) prior to start. Failure to obtain appropriate coordination shall be a cause to stop all activities until proper coordination is obtained.

3.1.3.4.2. Installation Site Limitations. The contractor shall take the following precautions on-site:

a. All installation work shall be completed in such a manner as to minimize disruption of government activities. The contractor shall perform any on-site work that shall disrupt government activities during non-production hours and shall be coordinated and approved by the contract monitor.

b. The contractor’s work area shall be maintained clean and orderly. Debris shall be removed and disposed of daily by contractor.

3.1.3.4.2.1. Supervision. The contractor shall provide technical supervision and shall be available on-site at all times during the installation, testing, and acceptance by the government.

3.1.3.4.2.2. Storage. The contractor shall provide lockable storage for security of tools and equipment. The government shall not be responsible for the safety and security of contractor property and shall not assume any risk on behalf of the contractor or any sub-contractors.

3.1.3.4.3. Installation. Safety equipment shall be used during fall protection installation.

Safety equipment shall contain up-to-date certifications and shall be in good working condition.

The contractor shall be responsible for any and all equipment and personnel requirements supporting transportation of equipment during receipt, to and from any storage area, and on the installation site. Installation shall begin no later than five (5) working days after receipt of all equipment with completion being accomplished within eight (8) months for all docks, and five

(5) days to complete each dock once work has started. Contractor’s work schedule will be subject to changes and may require multiple visits to accommodate aircraft schedule, which means that each dock could be a separate visit or multiple visits. All mobilizations to complete the fall protection systems shall be included in the contractor’s proposal. No additional funds will be provided for additional mobilizations.

3.1.3.4.4. Fit and Function Tests. The contractor shall perform a complete ‘fit and function’ test on and around the aircrafts. This test shall be conducted in the presence of government personnel under the direction of an authorized contractor representative and is subject to schedule changes due to aircraft workload. Contractor may be required to schedule these tests multiple times to accommodate the production schedule and aircraft movement.

3.1.3.4.4.1. The entire fall protection systems shall be fully functional and operational upon completion of installation.

3.1.3.5. Scheduling. All work shall be performed during normal 8-hour day shift hours (6:45 AM to 3:30 PM) Monday through Friday, and must minimize disruptions to government activities. Contractor shall perform any on-site work that will disrupt government activities during non-production hours and shall be coordinated and approved by the contract monitor. No work will be allowed on weekends or government holidays unless coordinated and approved by the CO. Contractor schedule must be flexible in order to work within a programmed depot maintenance (PDM) environment and understand that schedules are constantly changing.

3.1.3.6. Acceptance Standards. Any modifications to the system or the drawings need to be approved and signed off by the assigned government engineer before the contractor can proceed.

3.1.3.7. Training. Upon acceptance of the installation, the contractor shall provide training sessions for the government personnel to include the operation, maintenance, preventive maintenance for the units and all the system components.

3.1.4. Technical Data

3.1.4.1. Facility Drawings. Government will furnish as-built facility drawings as needed.

Any modification or reproduction of these drawings must be approved by government engineer(s) and contract officer.

3.1.4.2. Documentation. The contractor shall furnish four (4) copies each at completion, electronic and hard copy, of literature and/or catalogs for the following:

a. Operation Manuals shall include assembly procedures.

b. Maintenance Manuals. The contractor shall supply his recommendations for an on-going maintenance program. Program recommendations shall include recommended inspection criteria, intervals and periodic and long-term maintenance operations.

Recommendations shall also include quantities and frequency of repair parts to be kept on hand for frequent repairs.

c. Inspection Manuals

d. As-Built drawings for the fall protection systems

e. Certifications. Load testing and calculations certifying all systems as fall protection systems. Load testing and calculations for certification of fall protection system and certification for load on facility structural support of fall protection system for worst case scenario with maximum number of users falling at the same time and same place. The contractor shall submit a separate proposal along with their recommendations if they determine that the structure cannot support the requested load. Certification documents shall show that all fall protection systems and system components meet ANSI Z359.1 and 29 CFR 1910.

f. Training Manuals

g. Spare Parts List – Contractor’s recommended spare parts list.

3.1.5. Spare Parts and Proprietary Tools. The contractor shall provide a spare parts kit based on the contractor’s recommended spare parts list. Proprietary or uncommon tools required for maintenance or assembly shall be provided with the fall protection system in the quantity of one each per fall protection system.

3.1.6. Warranty. The contractor shall provide a full warranty on all fall protection system components, tools, and workmanship for no less than one year from the delivery date. All warranty issues and repairs shall be conducted on-site at Tinker AFB.

4. GOVERNMENT FURNISHED EQUIPMENT / MATERIAL

4.1. Facilities. Government shall furnish adequate work space at installation site.

4.2. Personnel. The government shall provide aircraft personnel to assist the contractor in demonstrating/training personnel in the use of the fall protection on or around the aircraft. The contractor representative shall coordinate and supervise the demonstration/training of the fall protection is all aspects of its operation.

4.3. Technical Data. The Government shall make available, to the selected contractor, at least one (1) copy of all referenced Government regulations and technical orders. The contractor shall not reproduce any government furnished technical data without written approval from the CO.

4.4. Existing Drawings. The Government shall make available, to the selected contractor, as-built facility drawings. However, all dimensions shall be field verified by the contractor.

These drawings shall not be modified or reproduced without prior approval by the government engineer(s) and contracting officer.

5. SITE VISITS AND CORRESPONDENCE

5.1. Site Visits. The CO is the point of contact for scheduling all site visits. Visitor control is located at the Tinker Gate.

5.2. Contractor Representative: The contractor shall appoint and identify to the government a project manager who shall act as the contractor’s representative in all communications with the government throughout the overall project. The project manager shall be qualified to interpret designs and discuss solutions in all areas of the project with government representatives, particularly the using agency (76AMXG/QPE). The contractor’s representative shall be on-site to supervise all fabrication, assembly and testing of the equipment.

6. GENERAL INFORMATION

6.1. Quality. The contractor shall comply with all standard industry practices and procedures and manufacturer’s specifications.

6.2. Hours of Operation. The installation site shall be available to the contractor from 0645 to 1630 hours, Monday through Friday, excluding federal holidays. During periods of Base Closure the contractor will follow Tinker Air Force Base Operating procedures for non-essential employees. Information for Tinker’s operating status can be found on Tinker’s Straight Talk Line @ (405) 734-1900 or on Tinker’s Facebook page. During periods of severe winter weather local television channels can also be tuned into for base operating status updates.

6.3. Government Procedures. Contractor employees shall follow the direction of the Government employees in regards to emergency procedures, i.e. fire, tornado, active shooter, bomb threats. It is the responsibility of the contractor to provide guidance and establish procedures for responding to emergencies for their employees. It is the contractor’s responsibility to account for their employees following an emergency evacuation. Contractor personnel shall participate with the government personnel in all fire and tornado drills at no additional cost to the government.

6.4. Government Acceptance. The government representatives shall evaluate the contractor’s performance through inspections and acceptance of services performed based on requirements of the statement of work, referenced drawings and specifications.

6.5. Parts and Materials. The contractor shall use only new equipment, hardware, parts, and materials in performance of this statement of work, unless authorized by the Contracting Officer who shall seek technical guidance from the assigned contract monitor, AMXG/QPE Engineering.

All equipment, hardware, parts and materials shall be off-the-shelf items to the fullest extent possible.

6.6. Tool Control. The contractor shall comply with the requirements of Tinker AFB’s Tool Control Program while performing contract services in or around the flight line and maintenance production shop areas at Tinker AFB, OK.

6.6.1. The contractor shall maintain a tool inventory checklist of all tools taken to the work site on Tinker AFB, OK.

6.6.2. The contractor shall inventory every tool on their tool inventory checklist at the beginning and end of each day they are working on-site at Tinker AFB, OK and at the end of each job.

6.6.3. The contractor shall secure all tools located at the work site when the contractor is not in the immediate area. No tools shall be left unattended/unsecured at any time.

6.6.4. The contractor shall make the tool inventory checklist and tools available for random inspection by assigned government representatives.

6.7. FOD Prevention. The contractor shall comply with the requirements of Tinker AFB’s Foreign Object Damage and Prevention Program.

6.7.1. The contractor shall practice good housekeeping at all times, in all areas and ensure all tools, equipment, and hardware is accounted for at the end of each task. The contractor personnel shall remove watches, rings, chains, and other jewelry that could be dropped or become loose.

6.7.2. The contract monitor shall assign government personnel to brief FOD awareness and prevention to the contractor prior to the contractor starting work in FOD critical areas. The contractor’s on-site project manager shall be responsible for briefing their work force, on Tinker AFB FOD prevention requirements, before each work shift.

6.8. Environmental Protection and Safety.

6.8.1. Environmental Protection. The contractor shall ensure none of their activities on Tinker AFB emit material, fumes or compounds that are hazardous to the ecological systems of Tinker AFB and its surrounding communities. If during this project the Base Environmental Management office determines ecological hazards are being emitted then the contractor shall immediately cease operations until approved mitigation measure are employed. Approval of all mitigation measure shall be at the discretion of the Base Environmental Management and Bioenvironmental offices.

6.8.1.1. Environmental Restrictions. The following materials, substances and compounds are strictly forbidden from use on Tinker AFB and shall not be brought on the installation by the contractor, their direct workers and their subordinate contractors and workers.

6.8.1.1.1. Lead. Lead and lead-based compounds including lead-based paints are strictly forbidden.

6.8.1.1.2. Mercury. Mercury, mercury compounds and free mercury are strictly forbidden.

6.8.1.1.3. Asbestos. Asbestos or asbestos containing materials are strictly forbidden.

6.8.1.1.4. Ozone Depleting Compounds. All class 1 ozone depleting compounds (ODCs) or any use during manufacture is strictly forbidden.

6.8.2. Bioenvironmental Protection. The contractor shall ensure all activates do not subject worker to bioenvironmental hazards including excessive noise, production of dust particles, toxic fumes, inadequate lighting, inadequate ventilation and excessive heat.

6.8.3. Safety. The contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations and requirements regarding environmental protection. In the event environmental laws or regulations change during the term of this contract, the contractor shall comply with such laws when enacted. The contractor shall comply with all safety practices posted in the assigned work area.

7. SPECIAL REQUIREMENTS

7.1. Security. The contractor shall comply with existing government procedures for obtaining contractor credentials to work on-site, and government procedures for contractor access to government legacy data systems. While inside DoD facilities, the contractor shall wear government-approved identification badges on their outer garment in the chest area. DoD facilities are defined as any space open or covered that is directly owned by or leased to the Department of Defense for the United States of America.

7.2. Base Location. The contractor and contractor representatives shall contact the government representative, assigned by the CO, for directions to the base. After arriving at the designated entry gate, the contractor shall telephone the assigned government representative for further instructions or to be escorted on to the base.

7.3. Foreign Nationals. A foreign national is defined as any person who is not a citizen or permanent resident alien of the United States of America. The contractor shall notify the CO of foreign nationals prior to allowing them entry to any DoD facility. Special rules shall apply to foreign nationals and shall be explained to the contractor once such individuals have been identified to the CO.

7.4. Safety Requirements. The contractor shall comply with Industrial Safety Requirements provided in Appendix C of this SOW.

7.5. Travel. The contractor shall travel, as required, to complete this effort.

7.6. Quality Assurance. The contractor shall establish, document, and maintain a quality system certified to the International Organization for Standardization (ISO) 9001 regulation.

This certification is required as a means of ensuring quality control for services under this contract. The contractor shall submit a copy of their ISO 9001 certification to the government prior to beginning work.

7.7. Other Information. The contractor may be required to directly interface with competitive system development and/or upgrade source selection sensitive information. The contractor shall accomplish non-disclosure agreements prior to any access to competition-sensitive or proprietary data, information, or documents.

7.8. Periods of Performance. Period of performance for the entire contract (fabrication through installation of a complete turn-key fall protection system) is twelve (12) months for all docks once the contract has been awarded. The installation will be completed in phases, each dock install lasting no more than 5 days to complete.

APPENDIX A

INDUSTRIAL SAFETY REQUIREMENTS

SECTION 1 - GENERAL REQUIREMENTS

A. Safety Program Requirements. The Contractor shall implement a safety program that ensures protection of Government personnel and property. The program shall consist of, as a minimum:

1. Mishap notification/reporting as defined in Section 1.B. below.

2. A Safety Program that addresses, as a minimum, the subjects listed in Safety Program

Elements, Section 2 and to be used during performance of the work described in the contract.

B. Mishap Notification.

1. The Contractor shall notify the procuring Safety Office or a designated Government Representative (GR) within one (1) hour of all mishaps or incidents. A written report of the mishap/incident shall be sent within three calendar days to the GR, who shall forward it to the Procuring Safety Office. For information not available at the time of initial written report, the Contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the Safety Office.

2. Mishap notifications shall contain, as a minimum, the following information:

a. Contract, Contract Number, Name and Title of Person(s) Reporting

b. Date, Time and exact location of accident/incident

c. Brief Narrative of accident/incident (Events leading to accident/incident)

d. Cause of accident/incident, if known

e. Estimated cost of accident/incident (material and labor to repair/replace)

f. Nomenclature of equipment and personnel involved in accident/incident

g. Corrective actions (taken or proposed)

h. Other pertinent information

3. If requested by the designated government representative, the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by the Procuring Safety Office.

C. The safety provisions of this contract, shall apply to any subcontracts/subcontractors.

Safety requirements listed herein that do not concern Contractor’s operations or services shall be considered self-deleting as mutually agreed by the Contractor and the procuring office.

SECTION 2 - SAFETY PROGRAM ELEMENTS

The Contractor’s Safety Program shall clearly define procedures, personnel qualifications, facilities and required equipment necessary to fulfill the following elements.
