Statement of Work

(SOW)

For Fabrication and Purchase of

One Portable Blast Fence System

Prepared By:

1. INTRODUCTION

The Aircraft Maintenance Group (AMXG) requires design, fabrication, delivery and Assembly of one Portable Blast Fence System for use in support of the Air Force Programmed Depot Maintenance (PDM) on the East and West sides of the ALC ramp Area, at Tinker Air Force Base, Oklahoma. The Contractor shall provide a complete Turn-key design, fabrication, inspection, testing, documentation, training, delivery, tools, Equipment installation, Portable Blast Fence System, labor and other incidentals Necessary to provide such a Portable Blast Fence System in accordance with this Statement of Work (SOW), attachments and supporting data as provided by the Government.

The Portable Blast Fence System will be used on the East and West ALC ramp areas To support personnel while conducting engine run-ups on the following Weapons System: C/KC-135, B-52 and E-3. The Portable Blast Fence System will have two Trailers with blast fence assemblies permanently affixed to the trailer. The Portable Blast Fence System will provide jet blast deflection during engine run-up tests.

After award of contract, the Contractor must be able to expedite the delivery of the Portable Blast Fence System to support the PDM program.

1.1. Problem Statement. Currently there is a need for an additional Portable Blast Fence System to be purchased and fabricated for use while performing PDM Maintenance and support of the War-Fighter.

1.2. Scope. The Portable Blast Fence System will be used on the East and West ALC ramp areas and be capable of being moved and used in multiple areas around the ramp. The Contractor shall provide complete design, fabrication, inspection, testing, documentation, training, delivery, tools, assembly, labor and other incidentals necessary to make the Portable Blast Fence System operational, usable, and repairable. The Government owns the design for any future procurement. Contractor shall provide components, equipment, and parts that are commercial-off- the- shelf (COTS). Contractor shall identify all components, equipment, and parts that are not COTS. The contractor shall provide the Government with unlimited data rights for this purchase. These rights shall include but not limited to the drawings, any related stress tests, and any other rights associated with the design, development, delivery, and build of the system.

2. DESIGN REQUIREMENTS

2.1. Design Objectives

Reference Drawing: Portable Blast Fence Configurations

2.1.1. Portable Blast Fence System Configurations are as follows:

System: One (1) Trailer and One (1) Blast Fence – Left Side Configuration

One (1) Trailer and One (1) Blast Fence – Right Side Configuration

2.1.2. Portable Blast Fence System shall consist of 2 each new AEB6, 10,000 gallon, High Gloss, Aluminum, Round, Baffled, D.O.T. 406 Trailers or equivalent with Blast Fence assembly permanently affixed to the trailer.

2.1.3. Portable Blast Fence System shall be equipped with adjustable air operated landing gear with pads mounted near the front of the trailer for height adjustment when connecting and disconnecting from the fifth wheel or tractor. This is in addition to any factory equipped landing gear.

2.1.4. Portable Blast Fence portion shall be constructed of the following materials: 3" X 3" X %" steel square tubing, 2" X 2" X %" steel square tubing, 3" X 3" X %" angle iron, 2.5" X 2.5" X 1/8" angle iron, 2" X 2" solid steel square stock, 4' X 10' X %"steel plate, 4' X 8' X 1" steel plate, 4' X 8' X %" steel plate.

2.1.5. Portable Blast Fence portion shall be equipped with outriggers that have landing pads on the load or back side of the Blast Deflector.

2.1.6. Portable Blast Fence System shall be weatherproof. System shall be designed for outdoor storage and use.

2.1.7. Portable Blast Fence System Surfaces (except for tank portion) shall be primed and painted with 2 coats using MIL-PRF-85285 Type 1 Color Chip 26173.

2.1.8. Portable Blast Fence Assembly shall be rated for no less than 30,000 lbs. of thrust.

2.1.9. Portable Blast Fence Assembly shall be able to withstand blast velocities in excess of 200 mph.

2.1.10. Portable Blast Fence Assembly shall be able to withstand temperatures in excess of 400 degrees Fahrenheit.

2.2. Safety

2.2.1. Portable Blast Fence System shall have reflective markings complying with AFOSH 91 -501 and shall be placed as appropriate.

2.2.2. Portable Blast Fence System shall have warning signs and placed as appropriate throughout the system and shall be a minimum size of 5-inches high by 10-inches long.

Each sign must have a white faceplate with %-inch high red lettering

2.2.3. Static Discharge. All Portable Blast Fence System shall have a static ground wire to discharge accumulated static electricity to the ground.

2.2.4. Surface Edges. Exposed surfaces shall be free from sharp edges, burrs or other hazards to personnel. Problem areas shall be smooth and rounded off.

2.2.5. Chemical Resistance. The Portable Blast Fence System and components shall maintain full function and durability for operations using the following chemicals listed but not limited to: Hydrochloric Acid, Acetic Acid, Phenol, Ammonia Water, Ethyl Alcohol, Acetone, Toluene, Ethyl Acetate, Methyl Ethyl Ketone, Jet Fuel (JP 4-5-8), Skydrol 500 B-4, PR148 Adhesion Promoter, Hydraulic Fluids (PRF5606H, MIL-H- 5606, MIL-PRF- 87257A), Alodine

2.2.6. Nameplate Labeling. Portable Blast Fence System shall be labeled on the base frame for identification. Nameplate labels shall be as permanent as the useful life expectancy of the Portable Blast Fence and shall be corrosion resistant. Legibility shall be as required for ready readability. Information contained on the nameplate shall be of a color that is in contrast to the color of the surface of the plate. Letters shall be capitals without serifs (sans-serifs) such as Arial, Gothic, or other sans-serifs font. Numerals shall be Arabic. Characters generated by automation processes shall be permitted. All Words engraved on plates shall be in the English language. Words and numerals shall be engraved, etched, embossed, or stamped in bold-faced letters on a contrasting background. The nameplate label shall contain information as follows:

2.2.7. Operating instructions shall be labeled on all landing gear and outriggers to allow ease of use.

2.2.8. All hydraulic and air will be labeled with psi.

2.3. Durability.

2.3.1. Design, construction, methods, and materials used in the manufacture of the Portable Blast Fence System and all system components shall incorporate ease of maintenance. It will also provide a durable and sustainable useful life of at least 20 years. All components must be off-the-shelf items to the fullest extent possible.

2.4. Or-Equal Clause Where reference is made in the specification or on the Drawings to specific manufacturers and/or model numbers of equipment or materials, the information is provided to indicate design intent and to establish the minimum quality standards, physical attributes, and performance characteristics which are required for the referenced item. The use of specific names and/or model number is not intended to limit selection of other manufacturers; equipment or materials meeting these criteria may be submitted to the PCO for consideration and shall be clearly identified, in writing, as a substitution. The burden of proving equality for proposed substitutions shall be the responsibility of the Contractor and submittals for said substitutions shall include detailed manufacturer's specifications, dimensioned drawings, performance data and any other information necessary to clearly indicate that the proposed item meets or exceeds the quality standards, physical characteristics, and performance criteria established by the contract documents. Unless otherwise noted to be an exact match.

3. DOCUMENTATION

3.1. References. All work and equipment provided under this solicitation shall follow good engineering practices as documented in accepted engineering literature, and shall satisfy as a minimum the following publications of the issue in effect on the date of request for proposal that form a part of this statement of work.

3.1.1. American Welding Society (AWS)

3.2. Order of Precedence. In the event of a conflict between the text of this Statement of work and the references cited herein, the text of the statement of work shall take precedence. Nothing in this document, however, supersedes applicable laws and regulations unless a specific exemption has been obtained.

4. REQUIREMENTS

The Contractor shall provide all necessary qualified personnel, management, and non-personal Administrative and technical services, materials and contractor facilities in Support of the requirements of this SOW.

4.1. Task Description. The Contractor shall provide engineering services and Deliverables to support the following tasks:

4.1.1. Baseline Requirements. The Contractor shall provide complete design, fabrication, installation, inspection, testing, documentation, training, delivery, tools, labor, and other incidentals necessary in accordance with this SOW.

All reports or submittals by the Contractor shall be submitted to the Procurement Contracting Officer (PCO) and Contracting Officer's Technical Representative (COTR), 76AMXG/QPS.

4.1 .1.1. Pre-Fabrication Review (PFR). The Contractor shall hold a PFR at Tinker AFB before platform fabrication within 30 business days after the award of contract. A fabrication data package and integrated master schedule shall be provided by the Contractor within three (3) working days prior to the PFR. The Contractor shall prepare the agenda and minutes for the PFR. The Contractor shall address as a minimum the following:

A. Integrated Master Schedule

b. Design Drawings

c. Personnel Training

d. Equipment Technical Data

e. Warranty Description

The fabrication shall be reviewed by the Government and the PCO shall provide the Contractor written comments. If any change requested by the Contractor and approved by the PCO, the PCO may require an additional submittal of the updated design drawings to be submitted within two (2) calendar days after review.

4.1.2. Final Verification I Limited Production. Upon completion and acceptance of the task requirement 4.1.1 of this contract, the Contractor shall begin the fabrication and delivery of the Portable Blast Fence System. The Contractor shall manufacture the Portable Blast Fence System that will be delivered and assembled on-site at Tinker AFB. The Contractor shall also support Government personnel with a full functional check-out demonstration of the Portable Blast Fence System with a KC-135 aircraft.

This functionality-check shall be performed by Government personnel, with the Contractor present for consultation and documentation of this function-check. If any discrepancies are noted, the Contractor shall document the deficiencies and corrective actions for submission to the COTR and PCO for approval prior to making corrections on all applicable drawings, stand components, and documentation. Any changes necessary (technical or non-technical) shall be requested by the COTR to the PCO and then issued by the PCO.

4.1.2.1. Fabrication. The Contractor shall procure the equipment and hardware necessary to support the fabrication of the Portable Blast Fence System. All equipment and hardware shall be off the shelf items to the fullest extent possible.

The Contractor shall perform all fabrication activities at the Contractor's location and final assembly as required at Tinker AFB.

4.1.2.2. Shipment. The Contractor shall be responsible for Blast Fence System preservation, packaging, loading, shipping, unloading, on-site placement and storage at Tinker AFB IAW the manufacturer's commercial practices unless otherwise specified herein. The Contractor shall provide a fully authorized representative on-site at all times during the receipt, assembly, installation, testing, and acceptance. The Government shall provide storage for intermittently shipped equipment.

4.1.2.3. Assembly.

4.1.2.3.1. General Assembly. Safety equipment shall be

used during Portable Blast Fence System assembly. Safety equipment shall contain up-to-date certifications and shall be in good working condition. Remediation of any component that cannot be assembled properly as a result of errors in manufacturing or deformation due to handling or transportation shall be the responsibility of the Contractor. The Contractor shall immediately report any discoveries to the PCO. Repair methods shall have prior Steel base frames, tubing, angles, connecting devices, or other components shall be done by methods approved by the original component manufacturer and shall follow industry standards. The Contractor shall replace all items damaged beyond acceptable repair.

4.1.2.3.2. Assembly Site Limitations. The Contractor shall

take the following precautions on-site:

a. All assembly work shall be completed in such a manner as to not disrupt Government activities. The Contractor shall perform any onsite work that shall disrupt Government activities during non-production hours and shall be coordinated and approved by the PCO and the COTR.

b. The Contractor's work area shall be maintained in a clean, neat, and orderly manner. Debris shall be removed and disposed of daily.

c. Material cutting shall be conducted in a contained area such that it will not introduce shavings or chips in the ramp area, onto or around the aircraft, aircraft access panels, or aircraft engines.

d. All phases of work shall be accomplished safely to comply with OSHA 29 CFR 1910.29, 1926.450, 1926.451, 1926.452, and AFOSH 91-501.

e. The Contractor shall be responsible for handling equipment and supplies during progress of the contract.

f. Utility requirements shall be the responsibility of the Contractor Sources of electrical power and compressed air shall be provided on an "as available" basis. The Contractor shall be responsible for Ensuring adequate utility supplies and hookups are available on-site prior to the time of need.

g. All welding shall be approved and a burn permit shall be provided by the Fire Department prior to any welding work.

No welding is permitted within fifty (50) feet of an .aircraft.

h. Storage of flammable materials such as paints and solvents shall be in authorized areas, stored in approved manner, and approved through the PCO, Safety, Bio and Environmental.

i. The Contractor shall comply with base requirements on handling hazardous materials. Disposal of hazardous materials shall be coordinated with the PCO and performed by the Contractor.

j. The Contractor employees shall not wear watches, rings, or jewelry within aircraft maintenance areas. Proper personal protective equipment (safety glasses, hearing protection, hard hats, safety shoes and protective coveralls) shall be worn in and around all production areas.

4.1.2.4. Installation Requirements.

4.1.2.4.1. General. The Contractor shall provide all

equipment necessary to support the delivery; assembly and installation of all items identified in this SOW. The Contractor shall supply all necessary manpower for the on-site installations. The Contractor shall coordinate the design and assembly details with COTR, Aircraft Facilities and Engineering (76 AMXG/QPS) prior to start. Failure to obtain appropriate coordination shall be a cause to stop all activities until proper coordination is obtained.

4.1.2.4.1.1. Supervision. The Contractor shall

provide technical supervision and shall be available on-site at all times during the assembly, testing, and acceptance by the Government.

4.1.2.4.1.2. Storage. The Contractor shall provide

lockable storage for security of tools and equipment.

The Government shall not be responsible for the safety and security of Contractor property and shall not assume any risk on behalf of the Contractor or any sub-Contractors.

4.1.2.4.2. Installation. The Contractor shall be

responsible for any and all equipment and personnel requirements supporting transportation of equipment during receipt, to and from any storage area, and on the installation site. The storage area may not provide weather protection and are subject to change depending on storage space available. Installation will begin no later than five (5) working days after receipt of fabrication equipment with completion being accomplished within 60 working days after start. All work shall be performed during normal 8-hour day shifts (07:30AM to 16:30 PM) Monday through Friday.

4.1.2.4.3. Driving. Each contractor needing to drive a

vehicle, having a company logo/sign, to surface areas surrounding the south end of Building 3001; all ramp and controlled access areas surrounding buildings 3102, 3225, 3228, 2280, 2121, 2122, 2136, and 2101;

all ramp areas surrounding Building 240, 262, 260, 278 and 935 Control Tower (as designated during the pre-bid walk through) is required to take a training course and pass an exam to receive a "Ramp Driver’s License". Gate pass to the ramp area can be obtained from the PCO and must be kept to a minimum.

Privately owned vehicles are not allowed on the ramp.

4.1.2.4.4. Function Tests. The Contractor shall

perform a complete function test around the KC-135 aircraft. This test shall be conducted in the presence of Government personnel under the direction of an authorized Contractor representative. Aircraft personnel, familiar with placing Portable Blast Fences around the aircraft, shall be provided to perform the function test of the Portable Blast Fences behind the aircraft under the supervision of the Contractor representative. The Government is responsible for moving the Portable Blast Fences into the correct vicinity of the aircraft as designated by the aircraft production foreman. The contractor shall contact 76 AMXG/QPS to determine aircraft availability for the Function Testing. Function Testing shall be scheduled upon aircraft schedule/availability. The Contractor has ten (10) working days to correct deficiencies found during the test.

4.1.2.4.4.1. The Portable Blast Fence System will be

fully functional upon completion of assembly. The Portable Blast Fence System shall be capable of a total four (1) hour Set-up time by four (4) aircraft personnel and one (1) towing vehicle.

4.1.2.5. Scheduling. Upon notification to the PCO, the Government will establish a storage and preassembly location. Within two (2) working days, the Contractor will be notified when and where he/she may off-load materials on-site. All work shall be performed during normal 8-hour day shift hours (07:30 AM to 16:30 PM) Monday through Friday, and must minimize disruptions to Government activities. The Contractor shall perform any on-site work that will disrupt Government activities during nonproduction hours and shall be coordinated and approved by the PCO and the

COTR. No work will be allowed on weekends or Government holidays unless coordinated and Approved by the PCO and the COTR.

4.1.3. Technical Data

4.1.3.1. As-Built Drawings. The Government will furnish generic sketches. Any modification to these drawings must be approved by Government engineer(s) and the contract officer. Reference Appendix A for sketch. It is the responsibility of the Contractor to furnish as-built drawings.

4.1.3.2. Commercial Off The Shelf (COTS) Manuals. The

Contractor shall furnish three (3) hard copies and two (2) CDs each at completion, of literature and/or catalogs for the following COTS Manuals:

a. Operation Manuals.

b. Maintenance Manuals. The Contractor shall supply his recommendations for an on-going maintenance program. Program recommendations shall include recommended inspection criteria, intervals and periodic and long-term Maintenance operations. Recommendations shall also include quantities and frequency of repair parts to be kept on hand for frequent repairs.

c. Inspection Manuals

d. Training Manuals

e. Spare Parts List - Contractor's recommended spare parts list

f. AutoCAD 2010 or previous version drawings of manufacturing drawings

g. AutoCAD 2010 or previous version drawings of Assembly drawings

4.1.4. Spare Parts and Proprietary Tools. The Contractor shall provide a spare parts kit based on the Contractor's recommended spare parts list.

Proprietary or uncommon tools required for maintenance or assembly shall be provided with the Portable Blast Fence System in the quantity of two each per System set.

4.1.5. Warranty. The Contractor shall provide a warranty period equal to the Standard industry practice for the entire Portable Blast Fence, components, tools, parts, and labor. The warranty period must exceed at least one year from the date of acceptance.

4.2. TASK REQUIREMENT SUMMARY

5. GOVERNMENT FURNISHED EQUIPMENT I MATERIAL

5.1. Facilities. Government shall prepare area for final assembly and furnish Adequate, as determined by the Government, work space at installation site.

5.2. Personnel. The Government shall provide aircraft personnel to assist the Contractor in moving the Portable Blast Fence System into place around the aircraft for performance of function testing, or for Contractor requested exceptions when approved by the COTR. The Contractor representative shall coordinate and supervise the movement and placement of the Portable Blast Fence System.

5.3. Technical

5.3.1. The Government shall make available, to the selected Contractor, at least one (1) copy of all referenced Government regulations and technical orders. The Contractor shall not reproduce any Government furnished technical data without written approval from the PCO.

5.3.2. Existing Drawings. The Government shall make available, to the selected Contractor, one (1) generic drawing of requested Portable Blast Fence System. However, all dimensions will need to be field verified. These drawings shall not be modified or reproduced without prior approval by the Government engineer(s) and contracting officer.

5.3.2.1. All plan information such as dimensions; structural, mechanical data provided by the Government are based on available as-built documentation, and equipment selected as design basis by the Government. This information is to be considered approximate and shall be field verified by the Contractor.

6. SITE VISITS AND CORRESPONDENCE

6.1. Site Visits. The PCO is the point of contact scheduling all site visits. Visitor Control is located at the Tinker Gate.

6.2. Contractor Representative: The Contractor shall appoint and identify to the Government a project manager who shall act as the Contractor's representative in all Communications with the Government throughout the overall project. The project Manager shall be qualified to interpret designs and discuss solutions in all areas of the project with Government representatives, particularly the using agency (76 AMXG/QPS). The Contractor's representative shall be on-site to supervise all Fabrication, assembly and testing of the equipment.

7. GENERAL INFORMATION

7.1. Quality. The Contractor shall comply with all standard industry practices and Procedures and manufacturer's specifications.

7.2. Hours of Operation. The installation site shall be available to the Contractor From 07:30 to 16:30 hours, Monday through Friday, excluding federal holidays.

7.3. Government Acceptance. The Government representatives shall evaluate the Contractor's performance through inspections and acceptance of services performed Based on requirements of the statement of work, referenced drawings and specifications.

7.4. Parts and Materials. The Contractor shall use only new equipment, hardware, parts, and materials in performance of this statement of work, unless authorized by the Contracting Officer who shall seek technical guidance from the assigned COTR. All equipment, hardware, parts and materials shall be off-the-shelf items to the fullest Extent possible.

7.5. Tool Control. The Contractor shall comply with the requirements of Tinker AF B's Tool Control Program while performing contract services in or around the flight line and maintenance production shop areas at Tinker AFB, OK.

7.5.1. The Contractor shall maintain a tool inventory checklist of all tools taken to the work site on Tinker AFB, OK.

7.5.2. The Contractor shall inventory every tool on their tool inventory checklist at the beginning and end of each day they are working on-site at Tinker AFB, OK and at the end of each job.

7.5.3. The Contractor shall secure all tools located at the work site when the Contractor is not in the immediate area. No tools shall be left unattended/unsecured at any time.

7.5.4. The Contractor shall make the tool inventory checklist and tools available for random inspection by assigned Government representatives.

7.6. FOO Prevention. The Contractor shall comply with the requirements of Tinker AFB's Foreign Object Damage and Prevention Program.

7.6.1. The Contractor shall practice good housekeeping at all times, in all areas and ensure all tools, equipment, and hardware is accounted for at the end of each task. The Contractor personnel shall remove watches, rings, chains, and other jewelry that could be dropped or become loose.

7.6.2. The PCO shall assign Government personnel to brief FOO awareness and prevention to the Contractor prior to the Contractor starting work in FOO critical areas. The Contractor's on-site project manager shall be responsible for briefing their work force, on Tinker AFB FOO prevention requirements, before each work shift.

7.7. Environmental Protection and Safety.

7.7.1. Environmental Protection. The Contractor shall ensure none of their activities on Tinker AFB emit material, fumes or compounds that are hazardous to the ecological System of Tinker AFB and its surrounding communities. If during this project the Base Environmental Management office determines ecological hazards are being emitted then the Contractor shall immediately cease operations until approved mitigation measure are employed. Approval of all mitigation measure shall be at the discretion of The Base Environmental Management and Bioenvironmental offices.

7. 7 .1.1. Environmental Restrictions. The following materials, substances and compounds are strictly forbidden from use on Tinker AFB and shall not be brought on The installation by the Contractor, their direct workers and their subordinate Contractors and workers.

7.7.1 .1.1. Lead: Lead and lead-based compounds including lead-based paints are strictly forbidden.

7.7.1.1.2. Mercury. Mercury, mercury compounds

and free mercury are strictly forbidden.

7. 7 .1.1.3. Asbestos. Asbestos or asbestos containing materials are strictly forbidden.

7. 7 .1.1.4. Ozone Depleting Compounds. All class 1 ozone depleting compounds (ODCs) or any use during manufacture is strictly forbidden.

7.7.2. Bioenvironmental Protection. The Contractor shall ensure all activates do not subject worker to bioenvironmental hazards including excessive noise, production of dust particles, toxic fumes, inadequate lighting, inadequate ventilation and excessive heat.

7.7.3. Safety. The Contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations and requirements regarding environmental protection. In the event environmental laws or regulations change during the term of this contract, the Contractor shall comply with such laws when enacted. The Contractor shall comply with all safety practices posted in the assigned work area.

8. SPECIAL REQUIREMENTS

· 8.1. Security. The Contractor shall comply with existing Government procedures for obtaining Contractor credentials to work on-site, and Government procedures for Contractor access to Government legacy data System. While inside DoD facilities, the Contractor shall wear Government-approved identification badges on their outer Garment in the chest area. DOD facilities are defined as any space open or covered that is directly owned by or leased to the Department of Defense for the United States of America.

8.2. Base Location. The Contractor and Contractor representatives shall contact the Government representative, assigned by the PCO, for directions to the base.

After arriving at the designated entry gate, the Contractor shall telephone the assigned Government representative for further instructions or to be escorted on to the base.

8.3. Foreign Nationals. A foreign national is defined as any person who does not Possess a legal certificate of citizenship in the United States of America. The Contractor shall notify the PCO of foreign nationals prior to allowing them entry to any DOD facility. Special rules shall apply to foreign nationals and shall be explained to the Contractor once such individuals have been identified to the

PCO.

8.4. Safety Requirements. The Contractor shall comply with Industrial Safety Requirements provided in Appendix C of this SOW.

8.5. Travel. The Contractor shall travel, as required, to complete this effort.

8.6. Quality Assurance. The Contractor shall establish, document, and maintain Quality system certified to the International Organization for Standardization (ISO) 9001 regulation. This certification is required as a means of ensuring quality control for services under this contract. The Contractor shall submit a copy of their ISO 9001 Certification to the Government prior to beginning work.

8. 7. Other Information. The Contractor may be required to directly interface with Competitive system development and/or upgrade source selection sensitive information. The Contractor shall accomplish non-disclosure agreements prior to any access to competition-sensitive or proprietary data, information, or documents.

8.8. Periods of Performance. Period of performance is 6 months through Fabrication, assembly and function test of the Portable Blast Fence System.

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements. The Contractor shall implement a safety program that ensures protection of Government personnel and property. The program shall Consist of, as a minimum:

1. Mishap notification/reporting as defined in Section l.B. below.

2. A Safety Program that addresses, as a minimum, the subjects listed in Safety Program Elements, Section II and to be used during performance of the work described in THE CONTRACT.

B. Mishap Notification.

1. The Contractor shall notify the. Procuring Safety Office or a designated Government Representative (GR) within one (1) hour of all mishaps or incidents. A Written report of the mishap/incident shall be sent within three calendar days to the GR, who shall forward it to the Procuring Safety Office. For information not available at the time of initial written report, the Contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the Safety Office.

2. Mishap notifications shall contain, as a minimum, the following information:

a. Contract, Contract Number, Name and Title of Person(s)

Reporting

b. Date, Time and exact location of accident/incident

c. Brief Narrative of accident/incident (Events leading to accident/incident)

d. Cause of accident/incident, if known

e. Estimated cost of accident/incident (material and labor to repair/replace)

f. Nomenclature of equipment and personnel involved in accident/incident

g. Corrective actions (taken or proposed)

h. Other pertinent information

3. If requested by the designated Government representative, the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent Maintenance and training records, until released by the Procuring Safety Office.

B. The safety provisions of this contract, shall apply to any subcontracts/subcontractors. Safety requirements listed herein that do not concern Contractor's operations or services shall be considered self-deleting as mutually agreed by the Contractor and the procuring office.

SECTION II - SAFETY PROGRAM ELEMENTS

The Contractor's Safety Program shall clearly define procedures, personnel Qualifications, facilities and required equipment necessary to fulfill the following Elements:
