PERFORMANCE WORK STATEMENT(PWS)
FOR
Nikon Laser Radar Scanner Factory Maintenance
Prepared By
OC-ALC/OBCA
22 December 2015
Section One
1.1 Scope: The Contractor shall provide all labor, tools, equipment, test equipment, material, parts, quality control, transportation and inspection manuals necessary to perform the Calibration of the Nikon Laser Radar Scanners and controllers for the Oklahoma City–Air Logistics Complex (OC-ALC) at the Contractor’s site. The equipment is listed in Appendix B (Off-Base).
1.1.1 The Contractor shall be responsible for insuring all services, parts and Original Equipment Manufacturer (OEM) components provided under this contract are in accordance with (IAW) the terms and conditions outlined herein, conform to the manufacturer’s equipment specifications and this PWS. Contractor shall furnish a copy of equipment specifications to the Contracting Officer Representative (COR) after service is rendered.
1.2 Known Problems: The Nikon Laser Radar Scanners (S/N MV330-2333 & MV330-2334) and Controllers (RCD333 & RCD334) must undergo factory recertification annually
1.3 Requested Services: The Contractor shall perform the required calibration on the Nikon Laser Radar Scanners (S/N MV330-2333 & MV330-2334) and Controllers (RCD333 & RCD334) IAW Original Equipment Manufacturer’s specifications.
1.4 Calibration: The Contractor shall provide commercial calibration and certification on Two
(2) each Nikon Laser Radar Scanners (S/N MV330-2333 & MV330-2334) and Controllers
(RCD333 & RCD334).
1.4.1 The calibration of the equipment and the associated calibration certificate must meet International Organization for Standardization (ISO) 17025 requirements. The Contractor shall place a calibration sticker on two (2) Nikon Laser Radar Scanners and controllers after the calibration is completed.
1.5 Certificate of Traceability: The Contractor shall provide to the COR, a Calibration Certificate of Traceability for two (2) each Nikon Laser Radar Scanners and controllers. The Calibration Certificate shall meet ISO 17025. The certificate shall include the following information as a minimum:
• Title, e.g. "Calibration Report" or "Calibration Certificate".
• Name and address of laboratory, and the location where the calibrations were carried out, if different from the address of the laboratory.
• Unique identification of the calibration report or certificate (e.g., the serial number), and on each page an identification in order to ensure that the page is recognized as a part of the calibration report or certificate, and a clear identification of the end of the calibration report or certificate.
• Name and address of customer.
• NOTE: A requirement for the name and address of customer may be waived with customer
• Coordination/concurrence.
• Identification of the method used.
NOTE: The calibration report or certificate may identify the method used in a manner similar to the following examples: a calibration procedure number; an ASTM, IEC, or other standard specification; or a narrative description of the method used (i.e. compared with laboratory standard X at 10 points between 10% and 100% of full scale).
• A description of, the condition of, and unambiguous identification of the item(s) calibrated.
• Date of receipt of the calibration item(s) where this is critical to the validity and application of the results, and the date(s) of performance of the calibration.
• Reference to the sampling plan and procedures used by the laboratory or other bodies where these are relevant to the validity or application of the results.
NOTE: The calibration report or certificate might not reference a sampling plan as sampling plans rarely apply to calibration; sampling plans normally are used in testing laboratories.
• The calibration results with, where appropriate, the units of measurement.
NOTE: Units of measurement would be appropriate if needed by the User (calibration factors, correction factors, etc) to operate the TMDE.
• The name(s), function(s) and signature(s) or equivalent identification of person(s) authorizing the calibration report or certificate.
• Where relevant, a statement to the effect that the results relate only to the items calibrated.
• The conditions (e.g., environmental) under which the calibrations were made that have an influence on the measurement results.
• The uncertainty of measurement or a statement of compliance with an identified metrological specification or clauses thereof.
NOTE: A calibration report or certificate that contains a statement of compliance to original manufacturer's specifications (with any exceptions or limitations noted) may satisfy this requirement.
• Evidence that the measurements are traceable.
CMXG Engineering will be responsible for packaging and shipping of all items.
1.6 Completion of Calibration Services: The Contractor, upon completion of the calibration services, shall provide proof to the COR and Government Subject Matter Expert (SME) that all aspects of the system are calibrated and are properly functioning IAW the manufacturer’s specifications, industry standards and this PWS.
1.7. Shipping Instructions.
Shipment of Equipment listed in Appendix B (Off-Base).
1.7.1 Shipment to Contractor’s Facility.
CMXG Engineering Point Of Contact (POC) will be responsible for packaging and shipping the equipment to the Contractor. CMXG Engineering POC will provide to the COR all documents showing shipment to the Contractor.
1.7.2. Shipment from Contractor’s Facility:
The Contractor shall be responsible for packaging all equipment for shipment to Tinker AFB.
The Contractor shall provide to the CMXG Engineering POC and COR the date the equipment is ready to be shipped to Tinker AFB. The CMXG Engineering POC will be responsible for all costs associated with shipping the equipment from the Contractor’s facility to the address listed below.

1.8 Quality Assurance: According to the contract’s inspection clause, the Government will evaluate the Contractor’s performance under this contract. The Government Surveilance COR will participate in the administration of this contract. Any matter concerning a change to the scope, prices, terms, or conditions of this contract shall be referred to the CO. All services to be performed by the Contractor during the period of this contract will be subject to review by the Contracting Officer (CO) or COR.
1.9 Environmental Certification: The Government will ensure the part or equipment is cleaned, drained and purged before it is transferred to the Contractor for repair. The Government will provide written certification of such to the Contractor prior to shipment or pick up of the part or equipment. The written certification will be printed on organization letterhead and include a statement which specifies the part or equipment is certified to be cleaned, drained and purged. In addition, as a minimum, said letter will contain the description of the part or equipment, make, model, serial number, and OC number, if applicable. This certification will be provided to the COR three (3) business days prior to the pick-up or shipment of the part or equipment. The COR will, in turn, provide it to the Contractor no later than (NLT) two (2) business days prior to the pick-up or shipment of the part or equipment.
1.9.1 The Contractor shall ensure and provide written certification that the repaired (equipment name or part) is cleaned to remove all hazardous contaminants, dirt, shavings, oil, or grease, and drained and purged to remove all fluids, oils, and liquids before returning it to Tinker AFB. In addition, as a minimum, said letter shall contain the contract number, description of the part or equipment, make, model, serial number, and OC number, if applicable. This certification shall be provided to the COR NLT two (2) business days prior to the return of the part or equipment.
1.10 Safety and Health off Government Installations: While performing work under this contract the Contractor shall comply with all applicable federal, state and local regulations regarding occupational safety and health. During the execution of the contract, the Contractor shall notify the CO, COR, and Government Contract Administrator (CA) within one (1) business day of any damage to Government property in which the dollar value exceeds $500,000.00 and within two (2) work days, for any damage to Government property less than $500,000.00.
1.10.1 Mishap notifications shall contain, as a minimum, the following information:
• Contract, Contract Number, Name and Title of Person(s) Reporting
• Date, Time and exact location of accident/incident
• Brief Narrative of accident/incident (Events leading to accident/incident)
• Cause of accident/incident, if known
• Estimated cost of accident/incident (material and labor to repair/replace)
• Nomenclature of equipment and personnel involved in accident/incident
• Corrective actions (taken or proposed)
• Other pertinent information
If requested by the designated CO, the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by the Procuring Safety Office. Contractors will not dispose of contract data related to a mishap until notified to do so by the CO.

The Contractor shall also provide Personal Protective Equipment (PPE) and clothing, to include reflective clothing (belt, vest, etc.); hearing and eye protection, to include prescription safety glasses; safety harnesses; fall protection equipment; lifting equipment and any incidentals necessary to perform the requirements of this Performance
