PERFORMANCE WORK STATEMENT (PWS) FOR
Removal and Installation High Efficiency Condenser Boilers B1083 Tinker Air Force Base (TAFB), OK Work Order #58147

1. DESCRIPTION OF SERVICES
1.1 The work shall consist of furnishing all plant, labor, equipment, instruments, materials, transportation, and incidentals required to replace two (2) each boilers in B1083.
1.2 The "work" shall mean to provide and install a fully operating and functioning system, assembly, framework, fixture or apparatus that is complete, finished and ready for use by the Government. "Retrofit" shall mean to also provide the additional work required to modify or integrate an existing system, assembly, framework, fixture or apparatus as necessary to accomplish the "work" and ready for use by the Government.
1.3 The Contractor shall be responsible for identifying the necessary logistical, architectural, structural, mechanical and electrical work elements required to install the new systems and assemblies as described herein. Assemblies, framework, fixtures or apparatus not readily identified on plans or the SOW shall be provided by the Contractor as required for a complete, fully operating and functioning system or systems, ready for use by the Government.
1.4 All information such as floor plan layouts; dimensions; structural, mechanical and electrical data; and location of underground utilities provided by the Project Coordinator.
1.5 Code Requirements: Contractor shall be required to follow current NEC, NFPA, UFGS and UFC requirements for the work to be performed.
2. BASIC SERVICES
2.1 The Contractor shall provide labor and materials necessary to provide removal/disposal of existing non-operational 1250 MBH input + 800 MBH input = 2050 MBH total Input units and installation of two (2) each High Efficiency Condensing Boilers in B1083 at TAFB AFB, OK.
2.1.1 Dispose of the boiler at Building 808 on Tinker AFB. Before removing and disposing of the unit coordinate the delivery with 72 ABW/CEY, Contract Surveillance Division, at 1-405-734-3301.
2.2 The Contractor shall install two (2) each high efficiency condensing boilers with 1681 MBH output each, the assembly, framework, fixture, or apparatus that is complete, finished and ready for use by the Government.
2.2.1 The Contractor shall be responsible for identifying the necessary logistical, architectural, structural, mechanical, and electrical work elements required to remove the old system and install the new system and assembly as described herein. Assemblies, framework, fixtures or apparatus not readily identified on plans or the PWS shall be provided by the Contractor as required for a complete, fully operating, and functioning system or systems, ready for use by the Government.
2.2 The Contractor will conduct an operational demonstration of all system components and/or functions to test and verify the system is fully operational, as well as, safe to operate.
2.3 New piping shall be tested and pass all American Society of Mechanical Engineers (ASME) testing requirements before insulating and placing the new lines into service.
2.4 The contractor will replace flue piping and reconnect new boilers to existing piping.
3. SPECIAL REQUIREMENTS
3.1 Code Requirements: Contractor shall be required to follow current National Fire Protection Agency (NFPA) 70 National Electric Code (NEC), Unified Facilities Guide Specifications (UFGS) and Unified Facilities Criteria (UFC) requirements for the work to be performed. All natural gas piping shall be in accordance with the requirements of American National Standards Institute (ANSI) and the American Society of Mechanical Engineers (ASME) B31.1 “Power Piping” (which applies to steam piping systems), and B31.1 “Gas Transmission and Distribution Piping Systems.”
3.2 Remaining portions of the facility shall remain occupied throughout construction. Contractor will be required to consult with the Government Project Manager (PM)/Construction Inspector and the Using Agency POC to develop a timeline that accommodates the continued performance of user’s mission function during the renovation. Project is located in a secured area. Upon award of the contract, the contractor shall be provided with a point of contact that shall direct them to the work areas and provide all applicable escorts. We will need an EAL and VAL for the project
3.3 In multiple boiler installations, the lead boiler should operate up to full capacity prior to starting the next boiler. During heating season, multiple boilers should be kept warm and ready should the lead boiler fail to operate. On multiple boiler installations with the largest boiler off line, the remaining boiler(s) must be capable of carrying not less than 65 percent of the maximum winter design load. Where the smallest boiler installed has a capacity of more than twice the minimum summer load, provide an additional boiler or hot water heater sizes for the anticipated summer load.
3.4 Warranty: The Contractor will warranty the operational status of the unit for a minimum of one (1) year.
4. SERVICE SUMMARY (SS)

5. GOVERNMENT FURNISHED PROPERTY (GFP) AND SERVICES
5.1 The Government will not provide any GFP equipment, parts, or materials to the Contractor.
5.2 The Contractor shall be responsible for safeguarding all Government property provided for Contractor use.
5.3 Plan information such as floor plan layouts; basic dimensions; design requirements for structural, mechanical, and electrical data; and location of utilities provided by the Government for this installation are based on available as-built documentation, field observation, and equipment selected as design basis by the Government. This information shall be considered approximate and shall be field verified by the Contractor. This information will be provided prior to the beginning of the work.
6. GENERAL INFORMATION
6.1 SPECIAL QUALIFICATIONS:
6.1.1 The Contractor shall ensure site supervisory personnel are responsive to emergencies, problems (personnel, workmanship, etc.), or to answer questions, between the hours of 0600 and 1530 MST. A primary on-site supervisor and an alternate shall be provided to the Contracting Officer (CO) and Government PM NLT 5 days after contract award to include cell phone numbers. A 24-hour response is required on all other shifts, weekends, and holidays, unless otherwise specified.
6.1.2. The Contractor shall require all motorized vehicle operators to possess and maintain current certifications and state driver’s licenses. Vehicle operators shall comply with posted base speed limits, the base policy of no texting while driving, and mandatory seat belt usage by all vehicle occupants).
7. SECURITY REQUIREMENTS:
7.1 The Contractor shall be responsible to gain access to TAFB through the CO.
7.2 REAL ID ACT EXTENSION STATEMENT and Qs &As as of 15 September 2016:
STATEMENT: The Department of Homeland Security extended the October 10, 2016, deadline to January 10, 2017 (90‐days), for some state/territory IDs that do not meet the REAL ID Act requirements.
Q1: Which state/territory IDs are extended until Jan. 10, 2017?
A1: The following states received the 90‐day extension: Alaska, Arkansas, California, Guam, Idaho, Illinois, Kentucky, Louisiana, Maine, Massachusetts, Michigan, Montana, Northern Marianas, New Hampshire, New Jersey, New Mexico, New York, North Carolina, North Dakota, Oklahoma, Oregon, Pennsylvania, Puerto Rico, Rhode Island, South Carolina, Texas, Virginia, and the U.S. Virgin Islands.
Q2: Are there any state/territory IDs that are not compliant now?
A2: Yes, American Samoa, Missouri, Minnesota and Washington did not receive an extension and are no longer compliant as of September 15, 2016. The exceptions to this restriction are Washington and Minnesota Enhanced Driver’s Licenses (EDLs) which display a red, white and blue American flag on the front of the ID card. EDLs exceed REAL ID Act guidelines and are considered acceptable forms of identification. Standard issue driver’s licenses from the four above states, however, are not compliant.
Q3: Are all other state/territory IDs compliant?
A3: The following state/territory IDs are REAL ID Act compliant: Alabama, Arizona, Colorado, Connecticut, Delaware, District of Columbia, Florida, Georgia, Hawaii, Indiana, Iowa, Kansas, Maryland, Mississippi, Nebraska, Nevada, Ohio, South Dakota, Tennessee, Utah, Vermont, West Virginia, Wisconsin, and Wyoming.
Q4: What is the process now for civilians to get on base, and how will that change on Jan. 10, 2017?
A4: All individuals who work or live on base already possess military issued credentials for base access. Identity proofing was done using approved identification credentials with a background check conducted through law enforcement authoritative databases. That will not change. What will change is which ID’s for proof of identity will be accepted. Acceptable alternative forms of ID are the following:
U.S. Passport U.S. Passport Card Permanent Resident Card/Alien Registration Receipt Card (Form I‐551).
A foreign passport with a temporary (I‐551) stamp or temporary (I‐551) printed notation on a machine readable immigrant visa.
An employment authorization document that contains a photograph (Form I‐ 766).
A current/valid driver’s license or identification card issued by a state or outlying possession of the United States provided it contains a photograph and biographic information such as name, date of birth, gender, height, weight, eye color, and address;
must be in compliance with the REAL ID Act of 2005.
U.S. Coast Guard Merchant Mariner Cards/Credentials.
PIV or Federally‐Issued PIV‐1 Cards (Personal Identification Verification) issued by the Federal Government.
PIV‐I card (Personal identification verification‐Interoperable Issued by Non‐Federal Government entities).
DHS “Trusted Traveler Cards” (Global entry, NEXUS, SENTRI, FAST).
Merchant Mariner card issued by DHS/ United States Coast Guard (USCG).
Border Crossing Card (Form DSP‐150).
U.S. Certificate of Naturalization or Certificate of Citizenship (Form N‐550) and U.S.
Permanent Resident Card (Form I‐551).
U.S. Refugee travel document or other travel document or evidence of immigration status issued by DHS containing a photograph.
A Foreign Government Issued Passport.
Q5: How will the REAL ID Act affect visitors?
A5: All visitors are required to have the proper identification to access an Air Force installation.
A REAL ID Act compliant credential will be required to establish individual identity. Those who do not have the accepted documentation for installation access will not be permitted to enter the base without an approved escort.
Q6: What about people who make deliveries and have no special ID now?
A6: Delivery drivers will be required to receive an access pass and background check. It should only add a few minutes to the delivery time, provided the delivery driver meets the criteria for receiving an access pass. In order to conduct the background check, a REAL ID Act compliant credential will be required to establish individual identity. Those who do not have the accepted documentation for installation access will not be permitted to enter the base without an approved escort.
Q7: What about civilian employees at Tinker?
A7: There is no change for base access to individuals who already possess military issued credentials. Those incoming employees who do not currently have military issued credentials will need identity proofing using approved identification credentials with a background check conducted through law enforcement authoritative databases.
1. QUALITY CONTROL
1.1 The Contractor shall have established a complete Quality Control Program (QCP) to ensure the requirements of this contract are provided as specified. The Contractor's QCP shall comply with the minimum quality management systems elements specified in the American National Standards Institute (ANSI) American Standards Organization (ASO), and International Standard Organization (ISO) standards.
1.1.1 The Contract Manager may be required to meet with the CO and the Government QAP during the Period of Performance (PoP) of this contract. Meetings will be as often and as necessary as determined by the CO. The written minutes of all performance evaluation meetings may be prepared by the Government and signed by the Contract Manager, CO, and QAP. Should the Contractor non-concur with the minutes; the Contractor shall so state any areas of non-concurrence in writing to the CO NLT ten (10) calendar days of receipt of the signed minutes. The minutes will be included in the contract file.
1.2 The Government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints.
1.3 Inspection and acceptance will be made by 72nd Air Base Wing (ABW) Civil Engineering (CE) assigned personnel.
1.4 Unsatisfactory performance shall result in re-performance by the Contractor at no additional cost to the Government.
2. HOURS OF OPERATION
2.1 The Contractor shall have access to perform services during normal working hours up to Force Protection Condition (FPCON) Charlie (C). Normal working hours at TAFB are typically 7:00AM – 4:00 PM Central Standard Time (CST) Monday through Friday. The COR will contact Contractor, if work is required on the weekend or holidays.
2.2 Unless otherwise required by the Government, the Contractor shall not have access to Government facilities on the following federal holidays: New Year’s Day, Martin Luther King’s Birthday, President’s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans’ Day, Thanksgiving Day, and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. If the holiday falls on Saturday, it is observed on Friday.
2.3 All visits shall be scheduled by the CO. Schedules may have to be modified in the case of incidents or emergencies, to include natural hazards, furloughs, or similar circumstances.
10. GOVERNMENT OBSERVATION
10.1 Government representatives, other than CO and COR(s), may from time to time with CO coordination observe Contractor operations. However, these shall not interfere with Contractor performance nor are these representatives authorized to make any changes to the contract.
11. ENVIRONMENTAL
11.1 Compliance with Laws and Regulations. The Contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection as it relates to the work required in this PWS. In the event environmental laws/regulations change during the term of this contract, the Contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the Contractor shall inform the CO pursuant to notice requirements and negotiate a modification to the contract.
11.2 The Contractor shall not use any Class I Ozone Depleting Substances (ODS) as identified in AF policies in the performance of this contract, nor shall the Contractor deliver any Class I ODS in any item or as a part of any service. If the Contractor determines that Class I ODS’s will be required under this contract, advance approval from the CO must be obtained before use.
11.3 Notification of Environmental Spills. If the Contractor spills or releases any substance contained in 40 CFR 302 “Designation, Reportable Quantities, and Notification”, including hazardous materials, into the environment, the Contractor or its agent shall immediately call 911. If the call is made from an on base phone it will go directly to TAFB dispatch. If using a cell phone it will go off base, ask the 911 dispatcher to be transferred to TAFB. The liability for the spill or release of such substances rests solely with the Contractor and its agent.
11.4 Material Storage and Use. The Contractor shall follow manufacturer’s guidelines and industry standards for control of humidity, temperature, cleanliness, and materials handling to include the handling of hazardous materials.
12. SAFETY PROGRAM
12.1 The Contractor shall implement a safety program that ensures protection of Government personnel and property. (See Appendix B). As part of the Safety Program the Contractor shall establish a Safety Plan IAW Appendix B of this PWS. The Contractor shall include and submit the Safety Plan with the contract proposal.
12.2 In the event of an accident/mishap, the Contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence. A written or email copy of the mishap/incident notification shall be sent NLT three calendar days to the Government Representative, who will forward it to the TAFB safety office.
12.3 Occupational Noise Exposure: It may be necessary to control the occupational noise exposure to personnel and to comply with Federal regulations (Title 29, Code of Federal Regulations, Parts 1910.95 and 1926.52). Sound levels at the worksite where personnel may be located shall not exceed 120 decibels (adjusted) (dBA) when measured on the A-scale of a standard sound level meter at slow response.
13. MANPOWER REPORTING
13.1 The Contractor shall report ALL Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract.
13.1.2 Reporting inputs will be for the labor executed during the PoP for each Government Fiscal Year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported NLT 31 October of each calendar year.
13.2 Uses and Safeguarding of Information: Information is considered to be proprietary in nature when the contract number and Contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the Contractor name and contract number associated with the data.

APPENDIX B
UNITED STATES AIR FORCE TINKER AIR FORCE BASE, OK 73145
SECTION I - GENERAL REQUIREMENTS
A. Safety Program Requirements.
1. NOTE: The Contractor will implement a safety program plan that ensures protection of Government personnel and property. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.
2. Competent personnel to provide surveillance of the Safety Plan.
B. Mishap Notification
(1) The Contractor shall notify AFSC/72ABW (405-734-3301) or the Tinker AFB Security Forces (405-734-3737) after normal duty hours, and the designated Government Representative (GR), i.e., the ACO, PCO, or DCMA COR (Contracting Officer's Representative) NLT one hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract, even if the Government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to OC-ALC/SE. For information not available at the time of initial notification, the Contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.
Mishap notifications shall contain, as a minimum, the following information:
(a) ) Contract, Contract Number, Name and Title of Person(s) Reporting
(b) Date, Time and exact location of accident/incident
(c) Brief Narrative of accident/incident (Events leading to accident/incident)
(d) Cause of accident/incident, if known
(e) Estimated cost of accident/incident (material and labor to repair/replace)
(f) Nomenclature of equipment and personnel involved in accident/incident
(g) Corrective actions (taken or proposed)
(h) Other pertinent information
1. The Contractor shall cooperate with any and all Government mishap investigations. Additionally if requested by Government personnel or designated Government representative (GR), i.e., the ACO, PCO, or DCMA COR, the Contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.
2. The Contractor shall provide copies of Contractor data related to mishaps, such as Contractor analyses, test reports, summaries of investigations, etc. as necessary to support the Government investigation.
3. The Contractor shall support and comply with the safety investigation and reporting requirements of AFI 91-204, Chapters 1 – 5.
SECTION II – SPECIFIC REQUIREMENTS
PEDESTRIAN CROSSWALKS: All Contractor personnel are required to use the closest crosswalk, or traffic controlled intersection when crossing the road.
Pedestrians must look both ways to ensure the coast is clear before stepping out into the crosswalk. Pedestrians DO NOT have the right of way unless they are already in the crosswalk. Contractor vehicle operators have the same responsibilities as pedestrians, to share the road and mutually observe and yield to pedestrians.
MOTOR VEHICLES: Contractor shall comply with the standards in: DoD Directive 5525.4, Enforcement of State Traffic Laws on DoD Installations", Nov 2, 1981, Para 3-4; DODI 6055.4, DoD Traffic Safety Program, 20 Jul 99; AFI 91-207, USAF Traffic Safety Program, 22 May 07; and AFI 91-207 AFMC SUP1, The US Air Force Traffic Safety Program, 1 Oct 07. Each applies to all persons at any time on an Air Force Installation and includes all leased, owned, or privatized property including housing areas. In addition: AFI 13-213, Airfield Management, 29 Jan 08, Paragraphs 1.3.6 and 4.4.2.1 applies to all Contractors, sub-Contractors, vendors, commercial delivery companies, and all other private business vehicles who operate anywhere on Tinker Air Force Base, including the airfield (to include the industrial areas and any buildings or hangars located upon the airfield) in support of their mission.
HOUSEKEEPING: Housekeeping shall be conducted according to the requirements in OSHA Standard 29 CFR 1910.141. CLEAN AS YOU GO will be enforced.
The authorized GR (OC-ALC Safety Office) shall approve the Contractor’s procedures within the Health and Safety Plan as meeting the intent of the below referenced standards, with the exception of Mishap Notification / Reporting Procedures, which are outlined in Section I, paragraph B of this Appendix C.
