STATEMENT OF WORK

FOR

AMXG Conference Room A/V Equipment and Installation

AFMC/OC-ALC/OBWK

22 Oct 2015 Revised 17 Dec 2015

Description:

The Contractor shall provide all labor, tools, equipment, test equipment, material, parts, quality control, transportation and inspection manuals necessary to perform the AMXG Conference Room A/V (Audio Video) Equipment and Installation upgrade for the Oklahoma City–Air Logistics Complex (OC-ALC). The Contractor shall also provide Personal Protective Equipment (PPE) and clothing, to include reflective clothing (belt, vest, etc.); hearing and eye protection, to include prescription safety glasses; safety harnesses; fall protection equipment; lifting equipment and any incidentals necessary to perform the requirements of this Statement of Work (SOW). The Contractor shall be responsible for insuring all services, parts and Original Equipment Manufacturer (OEM) components provided under this contract are in accordance with (IAW) the terms and conditions outlined herein, conform to the manufacturer’s equipment specifications and this SOW. The equipment is listed in Appendix A.

Specifications/ Installation Requested Upgrade: The Contractor shall install the listed A/V Equipment to the designated location at Tinker AFB The contractor shall upgrade and install all items listed on the Item Description attached to this SOW. The Contractor shall demonstrate to the contracting Officer Representative (COR) identified Government Technical Point of Contact, that the equipment is fully operational before leaving Tinker AFB.

Parts and Materials: The Contractor shall provide and use only new parts and material in performance of this SOW unless otherwise authorized by the CO.

Upgrade Report: Immediately upon completion of installation and upgrade and before leaving the OC-ALC, the Contractor shall provide a written Service Report to the Government Technical POC or COR. The Government Technical POC will forward the service report to the COR within two (2) business days of receipt. The Contractor shall provide, by fax or email, the final service report within five (5) business days to the COR. The Government will not pay an invoice submitted until an acceptable service report has been received from the Contractor. As a minimum, the final service report shall contain the following information:

· Name of the company

· Requested service date

· Contract number

· Equipment ID#

· Description of services provided

· Additional services recommended (if any)

· Printed name and signature of technician who performed the services

· Printed name and signature of on-site Government identified Technical POC or COR

· Start and completion dates

· Complete breakdown of the labor cost (hours and rate)

· Materials and parts used

· Details of any other charges

· Printed name and signature stating only new parts and materials were used

· Written verification statement that a performance check to verify operational condition was performed and that the equipment is in working order.

Contractor Personnel:

The Contractor shall provide the name of the Contract Manager (CM) and any alternates who shall act for the Contractor when the CM is absent. They shall be designated to the CO and COR, in writing, within five (5) business days after the contract award date. The Contractor shall provide telephone numbers and email addresses where the CM and alternates may be contacted at any time. The CM or alternate shall have full authority to act for the Contractor on all matters relating to operation of this contract at the OC-ALC on Tinker Air Force Base (AFB). The CM or alternate shall be available to meet on the installation with Government personnel designated by the CO to discuss problem areas. The Contractor shall provide a CM who can read, write, speak, and understand English fluently.

The Contractor and Contractor personnel shall be required to identify themselves as Contractor personnel by introducing themselves or being introduced as Contractor personnel. The Contractor personnel shall also display distinguishing badges or other visible identification for meetings with Government personnel or while performing work for the OC-ALC on Tinker AFB. The Contractor and Contractor personnel shall identify themselves as Contractor employees in telephone conversations and in formal and informal written correspondence.

Listing of Contractor Employees: The Contractor shall provide an initial listing of Contractor employees’ names and position titles to the COR within five (5) business days after contract award date. Within five (5) business days, the Contractor shall also provide an updated listing of personnel to the COR when employees affecting the work of this contract change status or position.

Environmental Protection & Safety: The Contractor shall be knowledgeable of and comply with all applicable federal, state, and local laws, regulations, instructions, and requirements regarding environmental protection and occupational safety and health. The Contractor shall comply with any laws, instructions, or regulation changes during the term of this contract.

Contractors shall be compliant with the Tinker AFB Supplement to Air Force Instruction (AFI) 32-7086, “Hazardous Materials Management”, Tinker AFB procedures per Tinker AFB Instruction (TAFBI) 32-7004, “Hazardous Waste Management”, and Complex procedures per OC-ALC Sup to AFI 32-7086, “Hazardous Material Management” and OC-ALC Instruction (OC-ALCI) 32-101 “Air Quality Record Keeping Procedure”. Publications are available digitally on the e-Publishing website at www.e-publishing.af.mil for downloading or ordering.

Housekeeping: The Contractor shall keep the work areas clean and neat IAW Industry Standards, OSHA, Fire and Safety Standards and this PWS.

Hours of Operation:

7:00AM to 4:00PM Monday thru Friday (Excluding Weekends and Federal Holidays). Unless prior approval from the Governement.

3001 Staff Drive Tinker AFB, OK 73145 AMXG Conference Room, Post 2Y55, B3001

Delivery:

Units and upgrade must be delivered within thirty (30) days after award of contract.

SOW/PWS Safety and Health Statement

Safety and Health:

While performing work under this contract the contractor shall comply with all applicable federal, state and local regulations regarding occupational safety and health. The contractor shall notify the Contracting Officer (CO), within eight (8) hours of any damage to government property where the dollar value exceeds $500,000.00 and within two workdays, for any damage to government property less than $500,000.00 during the execution of the contract.

Mishap notifications shall contain, as a minimum, the following information:

a. Contract, Contract Number, Name and Title of Person(s) Reporting

b. Date, Time and exact location of accident/incident

c. Brief Narrative of accident/incident (Events leading to accident/incident)

d. Cause of accident/incident, if known

e. Estimated cost of accident/incident (material and labor to repair/replace)

f. Nomenclature of equipment and personnel involved in accident/incident

g. Corrective actions (taken or proposed)

h. Other pertinent information If requested by the designated CO, the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by the Procuring Safety Office. Contractors will not dispose of contract data related to a mishap until notified to do so by the CO.

Emergency Procedures: The Contractor personnel shall follow the direction of the Government personnel with regard to emergency procedures, such as fire, tornado, active shooter, bomb threats, or other emergency. It is the responsibility of the Contractor to provide guidance and to establish procedures for its employees to respond to an emergency. It is the Contractor’s responsibility to account for their employees following an emergency evacuation. The Contractor personnel shall participate with the Government personnel in all fire and tornado drills at no additional cost to the Government.

No hazmat shall be used on Tinker AFB for this SOW.

Conference Room Equipment Upgrade Description of Requirements:

(Gov’t to retain all existing computer equipment removed) Display:

· Remove existing rear projection system.

· Remove screen and replace with 2X2 video wall using 4 – 60” LED Professional grade displays (shall include all required structure and mounting hardware).

· Podium display shall mimic on wall display (Monitor provided by the Gov’t).

Electronic Rack:

· Relocate existing rack from conference room to room behind projector screen. Install access wall plate for DVI video connection to the display via the rack.

· Add new HDMI matrix switch/scaler and video wall processors to rack.

· Remove existing control system and replace with new processor.

· Relocate existing two (2) computers from podium to rack location (computers may be VGA or DVI outputs).

· Integrate existing gear for control.

Table Reconfiguration:

· Rewire half (one of each pedestal) of existing connectors (4 VGA / 3.5 audio plates) to HDMI/USB and 3.5 audio and install plates. (Result=4 – VGA existing and 4 – HDMI new) Lighting:

· Remove old mechanical switches and dimmers and replace with controllable dimmers and switches (controlled by Crestron unit).

User interface:

· Remove existing wall mount touch panel and replace (outside of first door) with a new 7” Crestron (SIMPL Windows) touch panel with room scheduling capabilities.

· Remove existing podium touch panel and replace with a new 10” Crestron (SIMPL Windows) touch panel.

Speakers:

· Replace existing 5” speakers with 6ea. - 70 volt in ceiling 6.5” speakers.

Capabilities:

· Video Tele Conference (equipment provided by Gov’t), dual monitor (any combination of inputs), Two computer inputs (CPUs provided by Gov’t), DVD, Digital Projector
