1. DESCRIPTION OF SERVICES

1.1 SCOPE.

The Contractor shall perform Advisory and Assistance Services (A&AS) for a variety of program management, research and development, acquisition life cycle, and management tasks.

The Contractor shall assist with monitoring and tracking of capability gaps in support of the Surgeon General Research and Acquisition Program Management Office Directorate (SG5P).

The Contractor shall participate in the scientific and technical reviews of medical modernization research initiatives. The Contractor shall establish and maintain lines of communication with Major Commands (MAJCOM), SG5, Research and Acquisitions Operations Directorate (SG5O), Joint Program Committees (JPCs) and other stakeholders that are involved with or influence AFMS medical research. The Contractor shall provide administrative and clerical support.

2. SUMMARY OF REQUIREMENTS

The Contractor shall provide 21 Full Time Equivalents (FTEs) to provide technical expertise and experience that is defined in the PWS.

2.1 TASK I – ADMINISTRATIVE ASSISTANT SUPPORT

2.1.1 Responsibilities. The Contractor shall provide three (3) Administrative Assistants, Level III FTEs for executive administrative support to leadership. The Contractor shall complete the following subtasks. The Contractor shall provide three (3) FTE to support the program located at DHHQ (7700 Arlington Blvd, Falls Church, VA).

2.1.1.1 Use office automation software, such as the current version of Microsoft Office, to create and edit documents, presentations, graphics, tables, electronic drafts, electronic mail. Update existing databases or spreadsheets. Review, edit and update reports from SG5 staff to include checking for correct format, spelling, grammar, capitalization, and punctuation. Prepare/submit documents in appropriate formats in accordance with (IAW) Air Force Handbook (AFH) 33-337, The Tongue and Quill.

2.1.1.2 Receive telephone calls, greet visitors and determine the nature of the calls or visits, screen for requests that can be handled without assistance. Update and maintain office procedures to ensure effective and efficient operations. Assist other AFMSA divisions in locating and selecting the appropriate guidelines, references, and procedures for application to specific taskings and inquires. Ensure correspondence and communication with returned calls, emails or faxes are promptly addressed within one (1) business day.

2.1.1.3 Plan and coordinate travel arrangements within twenty four (24) hours of request in the Defense Travel System. Travel arrangements shall include scheduling transportation, making reservations, securing protocol/area clearances, preparing travel itineraries and preparing travel orders/vouchers based on general travel intentions or known preferences of the traveler accordance with established travel regulations. All military and Government civilian travel must be in accordance with the Joint Travel Regulations Uniformed Service members and DOD Civilian Employees at:

2.1.1.4 Maintain and collect requests for office supplies as requested and approved by the

SG5P Division Chief for SG5. All requests will be coordinated with the Government Purchase Card Holder for the Directorate for ordering and receipt of supplies and equipment.

2.1.1.5 Attend meetings and record meeting minutes and action items as requested by the

Government leads to obtain clear, concise, and accurate minutes so that decisions, recommendations or courses of action can be quickly made. Establish draft suspense list of all actions that came out of the meetings. Track/monitor all unit/department administrative suspense dates so that the Government PMs are able to ensure all suspense actions are completed in a timely and efficient manner.

Provide meeting minutes to the Government within five (5) business days of the conclusion of the meeting.

2.1.1.6 Review, process and route all incoming and outgoing correspondence, reports and other written materials submitted for review and, coordination for signature.

2.1.1.7 Coordinate and schedule all meetings and conferences.

2.1.1.8 Assist the Government with records management, to ensure that files are filed according to policy; archive expiring files into archive folder; delete duplicate files according to the Records Custodian; and support the Division Chief of Records with the management of Division files IAW AFI 33-322, Records Management Program

2.1.2 Administrative Assistant Support Level III Qualifications, located at the DHHQ:

- High school diploma and a minimum of four (4) years of work related experience in the listed areas below:

- Experience in preparing and editing executive documents

- Experience working with Department of Defense or senior level civilian or commercial agencies

- Experience with Defense Travel System (DTS)

2.2 TASK II – PROJECT MANAGEMENT SUPPORT

2.2.1 Responsibilities. The Contractor shall provide five (5) Project Managers, Level I FTEs to support multiple research programs that are part of the research portfolios at Ft Detrick and DHHQ. Project Managers are essential for the technical and scientific review of medical research programs, customer outreach, and active communication with portfolio stakeholders.

The Contractor shall provide one (1) FTE to support the program located at DHHQ (7700 Arlington Blvd, Falls Church, VA) and four (4) FTEs will be located at Ft Detrick (1270

Montevue Lane, Ft Detrick, MD). The Contractor shall complete the following subtasks on projects.

2.2.1.1 Assist in identifying gaps in AFMS capability based assessment (CBA) to include new or revised technologies. Assist in researching technology, functionality, impact, performance, suitability to tasks, and the need of the clients to assess gaps. Actively engage with the Research Area Managers (RAMs) and Deputy RAMs (dRAMs) to reinforce the alignment of the 59th Medical Wing and 711th Human Performance Wing research programs. Establish and maintain timelines for research and provide feedback to leadership.

2.2.1.2 Collect, analyze and organize information from a variety of sources. Gather input from military, Government, and Contractor subject matter experts (SME) and use statistical analysis and simulations to show information.

2.2.1.3 Examine information to determine what is relevant to the project and what methods should be used to analyze the project. Provide SMEs with recommended solutions to address a capability gap.

2.2.1.4 Integrate the use of formats and management processes into periodic reviews for decision making.

2.2.1.5 Maintain a repository of portfolio related documentation.

2.2.1.6 Prepare status reports, program reviews and presentations in accordance with the defined format that is established in the Air Force Handbook 33-337, Tongue and Quill writing guide. Update and maintain portfolios to accommodate development and revision of strategic business plans, execution plans, and requirements analysis to ensure that they align with Division research strategies and guidance.

2.2.1.7 Attend JPC steering committees, working groups and Integrated Product Teams

(IPT)s, High Performance Teams (HPT), related JPC, In-Process Reviews (IPR), and Risk and Analysis (R&A) meetings. Remain apprised of JPC activities and progress, and assist the Government representative to ensure AFMS gaps and requirements are represented in JPC priorities using the Integrated AFMS Capability Gaps (Material/Knowledge products).

2.2.2 Project Management Support Level I Qualifications:

- Bachelor’s degree and a minimum two (2) years of related work experience in the listed areas below:

- Medical research and development projects

- Acquisition Life Cycle Management

- Database Management

2.3 TASK III – CLINICAL INVESTIGATIONS PROJECT MANAGER

2.3.1 Responsibilities. The Contractor shall provide one (1) Project Manager Level III, FTE to support the Clinical Investigations Program (CIP) and align execution plans with business strategies for the streamlining of execution funds. The Contractor shall act as point of contact (POC) for the AF Clinical Investigation Facilities (CIF) Research Program. The Contractor shall provide full cradle-to-grave analysis of the modernization project, coordination with principle investigators, SG leadership, and the contracting office. The Contractor shall provide one (1) FTE to support the program located at DHHQ (7700 Arlington Blvd, Falls Church, VA).

2.3.1.1 Analyze proposals for technical, scientific, and fiscal content as it relates to current trends in the clinical investigations program. Provide technical analysis of research to achieve results within the clinical investigation program in order to specifically align priorities based on current AF requirements and needs.

2.3.1.2 Maintain and update the portfolio of the financial management of programmed

Operations and Maintenance (O&M) dollars, and Research, Development, Test and Evaluation (RDT&E) dollars.

2.3.1.3 Gather and prepare statistics for the Annual Data Call and briefing to the Under

Secretary of Defense for Health Affairs for the status of the CIP.

2.3.1.4 Develop justifications and strategic avenues of approach to obtain further funds to add to the CIP financial plan. Coordinate the Intramural Call for Proposals including maintenance of procedures on the CIP website, focal point for all field submissions, coordination of technical reviews, and award announcement and rejections for all intramural proposals.

2.3.1.5 Maintain direct contact with all principal investigators for acceptance of complete intramural packages as well as all quarterly and final reports.

2.3.1.6 Develop and maintain project schedules, milestones, and develop decision briefs in support of approximately 100 projects as required by SG5 division leadership.

2.3.1.7 Assist the Government with maintaining and updating the centralized infrastructure budget for the (7) seven CIFs which includes Clinical Investigations Programs at Lackland AFB, TX, Travis AFB, CA, Keesler AFB, CA, Nellis AFB, NV, Wright Patterson AFB, OH, the United States Air Force Academy, CO, and the School of Aerospace Medicine, OH

2.3.2 Clinical Investigations Project Manager Level III Qualifications:

- Bachelor’s degree

- Minimum of five (5) years of related work experience in the listed areas below:

- Medical research and development projects for the DoD

- Experience with Human and animal research compliance

- Experience with Database Management

2.4 TASK IV – SMALL BUSINESS INNOVATION RESEARCH (SBIR) SUPPORT

2.4.1 Responsibilities. The Contractor shall provide one (1) Project Manager, Level III FTE to support the SBIR program. The Contractor shall provide one (1) FTE to support the program in Falls Church, VA for AFMSA/SG5P and will be located at the DHHQ, 7700 Arlington Blvd, Falls Church, VA. The Contractor shall complete the following subtasks:

2.4.1.1 Provide SG5 management with relevant statistics or simulations for making decisions on the administrative and programmatic aspects of AFMS SBIR program to ensure core AFMS capability gaps are tracked and developed into topics that result in successful projects as required. The Contractor shall develop quantitative analysis (using existing project metrics within AFMSA) for all AFMS sponsored SBIR projects by utilizing government applications.

2.4.1.2 Gather information from SMEs in all aspects of research and examine information for the short and long term SBIR program. Assess current SBIR business approaches and identify possible areas of improvement as required by the Government Program Manager (PM).

2.4.1.3 Maintain a centralized AFMS SBIR Program Management process for SBIR related activities by utilizing existing mature SBIR processes and infrastructure at AF Research Laboratory and Medical Research Materiel Command (MRMC). Create a data repository on the AFMS SBIR Knowledge Exchange portal to be updated, as required by the PM.

2.4.1.4 Assist in identifying alternative funding options for SBIR Projects approaching Phase

III and beyond by networking with other AF organizations to seek alternative funding pathways. Brief timeline, management, and possible risks to PM and Government leadership on projects for funding as required.

2.4.1.5 Assist in identifying MAJCOM champions and sponsors for approved SBIR Projects.

Ensure the MAJCOM gaps are filled and projects reach maturity.

2.4.1.6 Identify and enhance existing business management practices. Improve program awareness, training, and outreach activities. Provide improvement program recommendations to SG5 management.

2.4.1.7 Collaborate with Small Business Concerns (SBCs) participating in the SBIR Program by establishing review gates and roadmaps to monitor project progress.

2.4.1.8 Attend SBIR related conferences, as required, for data collection to build Defense

Health Programs (DHP) funded SBIR data repositories.

2.4.2 Small Business Innovations Research (SBIR) Support Qualifications:

- Bachelor's degree

- A minimum of five (5) years of combined related work experience in the listed areas below:

- Medical research and development projects

- Acquisition Life Cycle Management and CBA

- Experience with Database Management

2.5 TASK V – MODERNIZATION THRUST AREA (MTA) PROGRAM MANAGEMENT

SUPPORT

2.5.1 Responsibilities. The Contractor shall provide one (1) Medical Research Scientist SME for the Directorate and MTAs. The SME shall provide expert level support in the development and alignment of strategic business plans, projects, execution plans, and requirement analysis. In collaboration with the MTA Manager, the contractor shall assess and prioritize the financial requirements for achieving research goals and objectives. The Contractor shall provide one (1) FTE to support the program located at DHHQ, 7700 Arlington Blvd, Falls Church, VA. The Contractor shall complete the following subtasks:

2.5.1.1 Complete scientific and technical analysis of modernization research proposals submitted for project review and funding. Provide written analyses, explanations and recommendations to leadership. Analyze program reviews to ensure alignment with the following: Department of Defense (DoD), Air Force Medical Service (AFMS), Headquarters Air Force (HAF)/Surgeon General (SG) Staff.

2.5.1.2 Develop mathematical and statistical models to analyze, identify and define problems on programs based on analytical information in order to assist with management plans for each MTA and the Directorate programs.

2.5.1.3 Interview, conduct surveys and simulations to maintain and improve lines of communication within MAJCOM Surgeon General Representative (SGR) and other MTA stakeholders.

2.5.1.4 Coordinate and assist membership participation in JPC steering committees, working groups and IPTs. Remain apprised of JPC activities and progress. Ensure AFMS gaps and requirements are represented in JPC priorities using the Integrated AFMS Capability Gaps (Materiel/Knowledge products).

2.5.2 Modernization Thrust Area (MTA) Program Management SME Qualifications:

- A minimum of a Master’s degree and a minimum of ten (10) years of related work experience to include the following area below.

- Medical Research and Development projects

2.6 TASK VI – MEDICAL COUNTER CHEMICAL, BIOLOGICAL RADIOLOGICAL

AND NUCLEAR (MC-CBRN) SUPPORT (PROJECT MANAGER LEVEL III)

2.6.1 Responsibilities. The Contractor shall provide one (1) Project Manager Level III, FTE to provide analytical services in support of the AF organizational medical readiness, emergency management, MC-CBRN and home station medical response goals and objectives. The FTE (1) will be located at Ft Detrick (1270 Montevue Lane, Ft Detrick, MD). The Contractor shall complete the following subtasks:

2.6.1.1 Use statistical analysis or simulations to analyze information and develop practical solutions to ensure force survivability and mission continuation of the AF.

Contractor shall develop, sustain, and modernize the MC-CBRN capabilities across the spectrum of AF operations.

2.6.1.2 Gather input from Government and AF SMEs to assist, analyze, and evaluate policies, procedures and operational effectiveness. Recommend changes that will enhance the efficiency of AF MC-CBRN capabilities.

2.6.1.3 Provide MC-CBRN program recommendations for internal and external customers and command policy. Recommendations shall be in accordance with the following:

AFI 41-106 Medical Readiness Health Program Management.

2.6.1.4 Assist in the implementation of the Air Force Incident Management System (AFIMS) programs and objectives. Participate in working groups/teams that address MC- CBRN program requirements. Assist subordinate installations to baseline and assess MC-CBRN capabilities. Identify capability shortfalls and prepare recommendations to resolve shortfalls.

2.6.1.5 Support the Chief of AF MC-CBRN Defense Program in the lifecycle process.

Develop, field and maintain joint CBRN capabilities and practical medical solutions.

2.6.1.6 Identify CBRN capability gaps as they relate to the AF concept of operations

(CONOPS), employment, and deployment requirements/locations.

2.6.1.7 Identify, validate and elevate medical CBRN requirements for consideration to the

AF priority CBRN Joint Priority List.

2.6.1.8 Actively work with leadership in various stakeholder communities to de-conflict issues, presents unresolved issues/potential Courses of Action (COA) to the leadership for ultimate resolution.

2.6.1.9 Provide subject matter expertise to MAJCOM SGRs and program managers on the

JCIDS process including all required documentation, staffing procedures and other relevant policies and guidelines.

2.6.1.10 Coordinate medical JCIDS documentation, doctrine and policy positions across the

AF.

2.6.2 MC-CBRN Project Manager Level III Qualifications:

- Bachelor's degree and a minimum of five (5) years of experience in the listed areas below:

- Experience in supporting C-BRN Joint Program Executive Office medical programs

- Experience with JCIDS

- Acquisition Life Cycle Management

- Skill to communicate effectively with senior Government and non-government personnel

- Secret security clearance required.

2.7 TASK VII – REQUIREMENTS MANAGEMENT SUPPORT

2.7.1 Responsibilities. The Contractor shall provide five (5) Project Managers level III, FTEs to support the Operations Division (SG5O) in developing, facilitating and aligning Strategic Planning, Programming and Operations Support. The Contractor shall provide assistance with business and project planning to support cradle-to-grave deliberate planning process at an enterprise level. The Contractor shall assist the Government with organizing, facilitating, and managing IPTs and HPTs. The Contractor shall provide five (5) FTE to support the program located at DHHQ (7700 Arlington Blvd, Falls Church, VA). The Contractor shall complete the following subtasks.

2.7.1.1 Lead development of CBA projects through coordinating and facilitating IPTs and

HPTs. Organize and coordinate all aspects of an IPT and HPT and/or a CBA Team.

The IPT is a precursor to the HPT. The HPT captures, articulates, and documents the operator’s requirements in minimum time, while achieving stakeholder needs.

The HPT shall consist of 7-11 core participants, which includes a lead, a facilitator, AF leaders (i.e., operators, systems engineers, acquirers, testers, logisticians, intelligence support managers, etc.), government agencies and other Services (as required).

2.7.1.2 Assist in developing the overall vision to meet a project; detailed planning that includes adjustments for real-world limitations of Doctrine, Organization, Training, Materiel, Leadership and Education, Personnel, and Facilities (DOTMLPF-P).

2.7.1.3 Set and maintain a schedule by performing feasibility estimation on projects.

2.7.1.4 Attend approved conferences, seminars that routinely include MAJCOM and installation participation to formulate and develop AFMS capability-based planning that supports AF and DoD requirements. Travel requirements are stated in Para 7, of the PWS.

2.7.1.5 Review funding profiles, review procurement schedules, analyze programs for enhancement and problem solving. Assist all AF/SG divisions and branches in trend and requirements analysis. Prepare results and deliver detailed responses, briefings and demonstrations to the PM as required.

2.7.1.6 Perform CBAs analysis, execution plans, policy reviews, and development that aligns with all existing and proposed AFMS plans and initiatives. Provide an integrated

CBA analysis within sixty (60) calendar days after signed CBAs are received from MAJCOMs.

2.7.1.7 Review AFMS requirements and provide suggestions on alignment as appropriate with existing Military Health System (MHS) programs and requirements to ensure regulatory compliance review.

2.7.1.8 Gather, review and provide Surgeon General Requirements for Operational

Capabilities Council (SGROCC) pre-briefings to AF/SG5 and AF/AFMSA leadership.

2.7.1.9 Utilize the JCIDS process and other multimedia databases to drive medical modernization, Joint Staff led initiatives, CBAs and futures strategic planning to meet the national defense objectives.

2.7.1.10 Work with decision support systems such as the Knowledge Management and

Decision Support (KMDS) and Integrated Risk Information System (IRIS) to aid in the decision making process.

2.7.1.11 Prepare and brief status updates as required at Senior Military Medical Advisory

Committee (SMMAC), SGROCC, Air Force Requirements Oversight Counsel (AFROC), and Joint Requirements Oversight Counsel (JROCC). Attend meetings as required and provide meeting minutes no later than (NLT) five (5) business days of the conclusion of the meeting.

2.7.1.12 Collect monthly metrics, analyze aggregate data and develop initial findings and recommendations for senior leaders to review 20 business days before briefing requestor.

2.7.1.13 Assist in preparing, coordinating, and providing complete requirements documentation for approval/signature. Documentation includes the following:

- Initial Capabilities Document (ICD) Due to the PM NLT 30 business days from the date of assignment by the SG5O Division Chief or delegated official

- Capabilities Development Document (CDD) Due to the PM NLT 30 business days from the date of assignment

- Capabilities Production Document (CPD) Due to the PM NLT 30 business days from the date of assignment

- Requirements Matrix due to the PM NLT ten (10) business days after Team lead approval

2.7.1.14 Act as a point of contact between the AF/ SG and other Air Staff, MAJCOM offices, and SG Field Operating Agencies (FOAs) for modernization issues and processes.

2.7.1.15 Complete the following on-line Defense Acquisition University training courses no later than 90 calendar days after contract award.

- CLR 101 – Introduction to JCIDS

- RQM 110 – Core Concepts for Requirements Management

- CLR 151 – Analysis of Alternatives

- CLR 250 – Capabilities Based Assessments

- CLR 252 – Developing Requirements

- CLB 007 – Cost Analysis

2.7.2 Requirements Management Support Project Manager Level III Qualifications:

2.8 TASK VIII – OPERATIONS AND TECHNICAL PLANNING SUPPORT LEAD

(PROJECT ADMINISTRATOR LEVEL I)

2.8.1 Responsibilities. The Contractor shall provide one (1) Project Administrator Level I, FTE to coordinate, monitor and plan projects using Task Manager Tool (TMT). The Contractor shall provide the support necessary to facilitate the AF/SGROCC. The Contractor shall provide one (1) FTE to support the program located at DHHQ, 7700 Arlington Blvd, Falls Church, VA.

The Contractor shall complete the following subtasks:

2.8.1.1 Coordinate the SGROCC meetings with the AFMS leadership. The Contractor shall be responsible for all administrative and logistical actions in the coordination of the SGROCC, which meets every other month, to include preparation of read ahead, slides, documentation of proceedings, video and teleconferencing, and room set up.

2.8.1.2 Attend conferences, seminars, and working groups that routinely include MAJCOM and/or installation participation to formulate and develop AFMS capability-based planning that supports AF and DoD requirements. Provide meeting minutes to the PM NLT five (5) business days of the conclusion of the meeting.

2.8.1.3 Ensure that prior to coordinating a meeting or a conference that the PM or

Contracting Officer Representative (COR) has followed the guidance in the Deputy Secretary of Defense Memorandum dated September 29, 2012, Implementation of Conference Oversight Requirement and Delegation of Conference Approval Authority.

2.8.1.4 Prepare and brief status updates as required by SG5O Director at SMMAC, SGROCC, the AFROC, and the JROCC as required by SG5O Director.

2.8.1.5 Collect monthly metrics, analyze aggregate data and develop initial findings and recommendations for senior leaders to review as required by SG5 leadership.

2.8.1.6 Collect and organize information from a variety of sources. Gather input from military, Government, and Contractor SMEs.

2.8.2 Operations and Technical Planning Support Project Administrator Level I Qualifications:

- Bachelor of Science in a business, information technology, logistics or healthcare

- Minimum of two (2) years of experience in the following areas: program or project management, logistics, information technology, requirements management, administrative support or healthcare.

- Ability to communicate effectively with senior Government and non-government personnel

- Secret security clearance required

2.9 TASK IX - TRAINING AND EQUIPMENT SUPPORT

2.9.1 Responsibilities. The Contractor shall provide one (1) Training Specialist Level III FTE to complete subtasks 2.9.1.3 and 2.9.1.6. The Contractor shall provide two (2) Training Administrators, FTEs to complete subtasks 2.9.1.1, 2.9.1.2, 2.9.1.3, 2.9.1.4 and 2.9.1.5. The Contractor shall provide training and technical assistance with staff development. The Contractor shall provide three (3) FTE to support the program located at DHHQ, 7700 Arlington Blvd, Falls Church, VA. The Contractor shall complete the following subtasks.

2.9.1.1 Assist the training manager with tracking military performance reports, awards, decorations, that are tracked and monitored by the AFMSA’s Commanders Action Group.

2.9.1.2 Provide assistance to the unit training manager and commander support staff regarding the following activities: in-processing, ancillary training, medical readiness training, and consultation with national certification organizations, such as the Project Management Professional Certifications.

2.9.1.3 Monitor and document compliance via the appropriate tracking system (i.e., Air Force

Training Record (AFTR), Armed Forces Research Institute of Medical Sciences (AFRIMS), Advanced Distributed Learning Service (ADLS), etc.,) or methodology which includes using various automated software, accessing different training systems and entering appropriate data.

2.9.1.4 Report compliance to the Government oversight/unit training manager on a weekly basis covering all aspects and types of training, to include Officer Performance Report/Enlisted Performance Report /decoration timeliness, fitness currency, and Self Aid Buddy Care/Suicide Prevention/Performance Assessment Feedback completion on military, civilian and Contractor personnel as required by the COR.

2.9.1.5 Provide planning and logistical support to AFMSA/ Research and Acquisition

Meetings. Contractor shall provide assistance with audio-visual, speaker preparation, displays, collecting/distributing briefing slides to speaker/leadership and provide attendance rosters to attendees.

2.9.1.6 Assists the PM in the management of the AFMS electronic training records of approximately 38,000 AF enlisted service members holding 17 medical specialty ratings. Create ad-hoc reports to assist the Government with determining the status of medical readiness training. Provide assistance to programmers/analysts in program development, testing and implementation of application features and support system release roll-outs ensuring functionalities are operationally effective.

2.9.2 Training Specialist Level III Qualifications:

- Bachelor’s degree

2.10 PERSONNEL REQUIREMENTS

2.10.1 Specific Personnel Qualifications Requirements. Personnel assigned to or utilized by the Contractor in the performance of this contract shall, as a minimum, be a United States citizen, have experience with Microsoft Professional Suite and shall be fully capable of performing in an efficient, reliable, and professional manner.

2.10.2 Language Requirement. On-site employees shall read, understand, speak and write English.

2.11 GENERAL REQUIREMENTS

2.11.1 During the course of this contract, Contractor personnel shall not release any personal or medical/patient information to include patient content with personal health information.

Information shall be treated FOR OFFICIAL USE ONLY in accordance with Privacy Act, Organizational Conflict of Interest, and Non-Disclosure Agreements guidance as follows:

2.11.2 Privacy Act. The Contractor shall comply with the Privacy Act of 1974 (5 U.S.C. § 552a), which includes Public Law 100-503, Department of Defense Directive (DoDD) 5400.11, and DoD 5400.11-R and must be treated as FOR OFFICIAL USE ONLY.

2.11.3 Organizational Conflict of Interest (OCI). The Contractor shall provide a signed OCI Avoidance or Mitigation Plan, addressing the activities of all contract employees, to the COR within ten (10) business days after contract award. (Appendix B)

2.11.4 Non-Disclosure Agreements (NDA). The Contractor shall provide a signed NDA to the COR, for each contract employee, five (5) business days prior to start date. Contractor shall provide patient information only to Government employees and employees of Contractors having a need-to-know. Only information required for the performance of duties for this project shall be provided. (Appendix C)

2.11.5 Task Leader. The Contractor shall provide a point of contact for tasking who shall be responsible for the performance of the work. The POC shall have full authority to act for the Contractor on all matters relating to the daily operation of this contract. The Contractor shall designate this individual, in writing, to the Contracting Officer (CO) and the COR before the contract start date. The Contractor shall identify those times when the alternate shall be the primary point of contact (POC). The Government has no preference to on-site or off-site PM

POC.

2.11.6 Conduct of Contractor Personnel. The Contractor personnel are required to observe all base and facility parking, safety and traffic regulations that apply to all facility employees.

Alcoholic beverages on the job will not be tolerated. There shall be no loud, profane or abusive language used on the job.

2.11.7 Appearance. Contract personnel shall present a neat well-groomed appearance. Neat, clean, casual business attire shall be worn.

2.11.8 Resumes and Vacancies. All resumes submitted by the Contractor shall be reviewed by the Contracting Officer (CO) and COR. The Contractor shall submit redacted resumes of candidates to the CO with the proposal. The Contractor shall staff all positions no later than (NLT) five (5) business days after resume has been approved by the COR and CO. The Contractor shall provide a redacted resume to the CO within five (5) business days after an individual has left the position. The Contractor shall fill the position NLT five (5) business days after the CO and COR have reviewed the resume and verified that the qualifications of the replacement candidate meet the requirement. Contractor shall not invoice and Government will not pay for positions vacant longer than five (5) business days after the Government has approved the resume.

2.11.9 Notification of Vacancy. The Contractor shall provide the Government a two-week notification prior to a vacancy.

2.11.10 Retrieving Identification Media. The Contractor shall retrieve all identification media including vehicle passes from employees who depart before the contract expires.

4.1.1 Criteria for Acceptance. All deliverables shall be submitted in a draft format mutually agreed upon by the contractor and the COR.

4.1.2 Delivery Instructions. All deliverables shall be delivered to the COR NLT the date specified in the deliverables chart above.

4.1.3 Deliverables Schedule. The Contractor shall ensure that all deliverables developed under this contract shall meet or exceed the quality standards for documentation. The Contractor shall ensure that all documents, data and deliverables described within this PWS are submitted on time and in the required format. At the completion or termination of this contract, the Government shall own all products developed while performing this task.

4.2 MONTHLY STATUS REPORT (MSR).

The Contractor shall prepare and submit a MSR identifying all tasks performed, current status, issues and anticipated actions consistent with the PWS NLT the 10th business day of each month to the COR.

4.2.1 Specific content of the MSR shall include the following information below and delivered to the COR in electronic format.

- Program status, to include objectives met, work completed and work outstanding

- Issues or obstacles impeding progress and recommended solutions

- Status of deliverables/milestones

- Issues and resolutions

- Resource planning/status if applicable

- Topics or issues identified by the Government PM, COR or on-site Government representative

- Description of work completed and plans for next month

- Summary of the efforts of each primary task in the Government PWS

4.3 QUALITY CONTROL PLAN

The Contractor shall be responsible for quality control of work accomplished during the performance of this contract. The Contractor shall provide a Quality Control Plan (QCP) to the Contracting Officer within 5 business days after contract award and any updates as they occur with COR approval.

4.4 POST AWARD MEETING.

A post award meeting will be held NLT five (5) business days after contract award. The intent is to initiate the communication process between the Government and Contractor by introducing key task participants and explaining their roles, reviewing communication ground rules, and assuring a common understanding of subtask requirements and objectives. The post award meeting shall be held at the DHHQ building located at 7700 Arlington Blvd, Falls Church, VA 22042. The meeting may also be conducted via a teleconference which would be coordinated by the CO. The completion of this meeting will result in the following:

- An introduction of contractor personnel performing work under this contract

- A demonstration by the Contractor to confirm their understanding of the work to be accomplished under this PWS.

5. GOVERNMENT FURNISHED INFORMATION/SERVICES

5.1 INTERNET.

The Government will provide access to Local Area Network (LAN). Access to classified military networks will be provided to Contractors who require access to classified information and maintain active clearances.

5.2 EQUIPMENT.

The Government will provide the Contractor with the following equipment determined to be incidental to the place of performance: office work space, desk, chair, filing cabinet, government laptop and docking station, monitor, software, individual identification badges, LAN, Common Access Card (CAC), desktop telephone, printer and fax access, and office supplies on site.

5.3 DATABASES AND SOFTWARE.

The Government will provide access to the applicable databases, software and hardware required within the scope of this contract.

5.4 INFORMATION.

The Government will provide the Contractor all required GFI to perform their tasks and duties.

The Government will provide existing information regarding the current organization structure, mission, goal, objectives, as well as other information requested by the contractor and deemed appropriate by the AF on-site representative.

5.5 DOCUMENTS.

Manuals, texts, briefs and other materials associated with the execution of contract requirements shall be provided. Documentation shall remain the sole property of the Government and shall be returned to the Government upon completion of this contract. The Contractor shall not disseminate information provided by the Government or developed by the contractor during the execution of this task without written approval by the COR.

5.6 POLICIES.

The Contractor shall comply with local, AF and DoD policies concerning information technology (IT) applications and connecting to authorized military systems.

5.7 ACCOUNTABILITY.

The Contractor shall ensure accurate control and accountability of all equipment in accordance with the terms and conditions of the contract.

5.8 CONTRACTOR FURNISHED ITEMS AND SERVICES.

Except for those items or services specifically stated as Government Furnished Information/Services, the Contractor shall furnish in its entirety, all items required to perform this contract.

6. SECURITY REQUIREMENTS

6.1 PERSONNEL SECURITY CLEARANCE REQUIREMENTS.

Project Manager Level III (Tasks MC-CBRN Support), Project Manager Level I (Requirement Management Support), and Project Administrator I (Operations/Technical Support), shall require personnel with a minimum Secret clearance. The Contractor shall be responsible for ensuring personnel security to include keeping the COR abreast of issues dealing with the security clearances and/or certifications. No foreign national candidates shall be utilized within the scope of this contract without prior approval of the Government. The Contractor shall comply with Department of Defense Directive (DoDI 8570.1M) Information Assurance Workforce Improvement Program-May 15, 2008 Change 1, and Personnel Security Program Requirements (5200.2-R).

Project Manager Level III (Tasks MC-CBRN Support), Project Manager Level I (Requirement Management Support), and Project Administrator I (Operations/Technical Support), shall require personnel with a minimum Secret clearance. 6.2 INVESTIGATION.

Tasks MC-CBRN Support, Requirements Management Support, and Operations/Technical Support shall require personnel to have a favorable National Agency Check, Local Files Check (NACLC) report, verified through the JOINT PERSONNEL ADJUDICATION SYSTEM (JPAS), and is needed as a condition of employment under this contract for those specific tasks.

Tasks Administrative Assistant, Project Management Support, Clinical Investigation Project Manager Support, SBIR Support, MTA Program Manager Support and Training/ Equipment Support shall require individuals to have a favorable National Agency Check with Inquiries (NACI) investigation. The Contractor shall immediately report to the appropriate Government representative if any contractor employee filling a sensitive position receives an unfavorable NACI adjudication, or if information that would result in an unfavorable NACI becomes available. Notify the COR, CO and Security Office within five (5) business days if at any time a contractor employee has privileges revoked to a DoD data or system data because of security concerns.

6.3 PHYSICAL SECURITY.

The Contractor shall be accountable for control of Government property IAW Physical Security, Air Force Joint Instruction (AFJI) 31-102. The Contractor is also responsible for the protection of any contractor-owned equipment or supplies brought onto the installation IAW AFJI 31-102.

Further, the contractor shall not remove any Government property from the installation.

6.4 INSTALLATION ENTRY.

The Contractor shall comply with established security procedures for entering the installation and its facilities to include any special security procedures that may be established for entry to Restricted Areas or mission essential or…
