INSTRUCTIONS TO OFFERORS—COMMERCIAL ITEMS (Jun 2008)

This section is for solicitation purposes only. This section will be physically removed from any resultant award, but will be deemed incorporated by reference.

LOWEST PRICE TECHNICALLY ACCEPTABLE ACQUISITION

(IAW FEDERAL ACQUISITION REGULATION FAR PART 12 AND 15.101-2)

ADDENDUM - 52.212-1, Instructions to Offerors – Commercial Items

Each Offeror is required to submit their proposal as specified in Request for Proposal (RFP) provision 52.212-1, Instructions to Offerors-Commercial Items, along with the following addendum. Failure to provide any required information may result in an offer being determined non-responsive.

A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all commercial solicitation requirements, including terms and conditions, representations and certifications, and technical requirements.

Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of two (2) separate parts, Part I – Technical

Capability and Management, and Part II – Pricing.

B. General Instructions: This is a Lowest Price Technically Acceptable (LPTA) acquisition.

Technical Capability and Management will be evaluated and given ratings of acceptable or unacceptable. Unacceptable offers are not eligible for award. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Offerors shall describe in detail their proposed approach. Offerors shall use the Government-provided

Performance Work Statement (PWS) to develop their technical approach. Offerors shall specifically explain how their approach will meet the key requirements of the PWS. Offerors shall explain how their approach allows for flexibility to incorporate schedule and personnel changes, without a major schedule slip of the requirement. Offerors shall explain how their approach and choice of personnel will accomplish the tasks outlined in the PWS. Offerors shall not merely reiterate the objectives or reformulate the requirements specified in the solicitation or will be deemed technically unacceptable. Award will be made based on an integrated assessment of the evaluation factors. It is our intent to award a contract to the contractor who will provide the best overall value to the Air Force as determined by an integrated assessment. The proposal instructions in this section are designed to assist offerors in preparing a complete response that reflects a full understanding of the approach proposed to accomplish all work required herein.

C. SPECIFIC INSTRUCTIONS.

1. PART I – TECHNICAL CAPABILITY AND MANAGEMENT

Technical Capability Proposals will be given a rating of Acceptable or Unacceptable.

Determination Definition

Acceptable Proposal clearly meets the minimum requirements of the solicitation.

Unacceptable Proposal does not clearly meet the minimum requirements of the solicitation.

A technically acceptable proposal shall have a sound technical approach that meets all (a)

Performance Work Statement (PWS) requirements; (b) demonstrates technologies used in the approach are viable, procurable and producible; (c) proposal has an approach that allows the flexibility to incorporate new technologies without a major revamp of the requirement; (d) has a sound risk mitigation approach for all identified technical risks; and provides an effective approach to manage and minimize schedule and personnel disruptions.

Factor 1. Technical Capability

Sub-Factor 1: Address how the Lean For Healthcare course of instruction will fit the needs of the AFMS.

The proposal shall:

a. Address the degree to which the contractor’s proposed training will instruct participants in how to identify high-leverage areas for improvement in accordance with tasks outlined in the PWS. (PWS paragraph 1.3, 2.1.1)

Sub-Factor 2: Demonstrate how the contractor will ensure CME/CEU credits are awarded for participants.

The proposal shall:

a. Address the contractor’s ability to meet the requirements of the essential areas and policies of the Accreditation Council for Continuing Medical Education.

(PWS paragraph 2.1.7)

b. Address the contractor’s plan for overseeing and awarding CME/CEU credits for the training in accordance with tasks as described in the PWS. (PWS paragraphs 2.1.6, 2.1.8)

Sub-Factor 3: Address instructor’s level of experience in providing classroom instruction and hands-on simulation/learner interaction.

The proposal shall:

a. Clearly demonstrate knowledge of tools and concepts such as value stream mapping, standardized work, A-

3 reporting, Lean leadership, Six Sigma/Lean Healthcare integration, Lean facility design and maintenance.

(PWS paragraph 2.1.4)

b. Clearly demonstrate a sound knowledge and application of Lean philosophies and tools in a simulated environment. (PWS paragraphs 2.1.4).

c. Clearly demonstrate how the hands-on interaction part of the instruction will be conducted as described in the PWS. (PWS paragraph 2.1.1, 2.1.4).

2. PART II – PRICE PROPOSAL

a. Complete blocks 12, 17a (including cage code), 26, 30a, 30b, and 30c of the Standard Form

(SF) 1449. In doing so, the offeror accedes to the contract terms and conditions as written in the RFP.

The RFP constitutes the model contract. Provide signed SF 1449 and signed SF 30/Amendments, if applicable. All solicitation amendments must be acknowledged in accordance with FAR 52.212-1, Instruction to Offerors-Commercial Items and submitted with the proposal. Include name, title, and signature of individual authorized to sign on behalf of the company.

b. Insert proposed unit and extended prices in the Schedule of Supplies/Services for each Contract Line

c. Complete the necessary fill-ins and certifications and Representations and Certifications in solicitation and return in their entirety.

3. The Government reserves the right to make award without discussions. Therefore, the offeror’s initial proposal should contain the offeror’s best terms from a price and technical standpoint. The Government reserves the right to conduct discussions if the contracting officer later determines them to be necessary. If the Government awards without discussions, the offeror’s may be given the opportunity to clarify certain aspects of proposals or to resolve minor clerical errors.

4. The offeror’s proposal shall include all data and information requested and shall be submitted in accordance with these instructions. The offer shall be compliant with the requirements as stated in the Performance Work

Statement. Non-conformance with the instructions provided may result in an unfavorable proposal evaluation, rendering it ineligible for award.

5. The proposal shall not simply rephrase or restate the Government’s requirements, but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of their experience and will base its evaluation on the information presented in the offeror’s proposal. The offeror’s proposal must convey to the Government that the offeror is capable; possesses sufficient technical expertise and experience; possesses sufficient resources; and is able to plan, organize, and use those resources in a coordinated and timely fashion such that technical requirements will be achieved and costs will be controlled.

6. Offerors should address questions, concerns, or requests for clarification in writing via email only. Questions received via telephone, fax, or letter will not be answered. 3. Competitive Range: The Contracting Officer may determine that the number of proposals that would otherwise be in the competitive range exceeds the number at which an efficient competition can be conducted. In accordance with FAR 15.306(c)(2) the Contracting

Officer may limit the number of proposals in the competitive range to the greatest number that will permit an efficient competition among the most highly rated proposals (10 U.S.C.

52.212-2 ADDENDUM

FAR 52.212-2

EVALUATION – COMMERCIAL ITEMS (Jan 1999)

This section is for solicitation purposes only. This section will be physically removed from any resultant award, but will be deemed incorporated by reference.

LOWEST PRICE TECHNICALLY ACCEPTABLE ACQUISITION

(IAW FEDERAL ACQUISITION REGULATION FAR PART 12 AND 15.101-2)

ADDENDUM - 52.212-2, Evaluation – Commercial Items

This is a competitive Lowest Price Technically Acceptable (LPTA) acquisition in which award will be made on the basis of the lowest evaluated price of proposals meeting the acceptability standards. By submission of its offer, the offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors and sub factors. Failure to meet a requirement may result in an offer being determined technically unacceptable. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.

Evaluate Technical, Rank by Price

Step 1 - Evaluate all proposals for technical acceptability.

Step 2 - Evaluate price reasonableness of all technically ―acceptable‖ proposals, then rank by total evaluated price.

A. Technical Acceptability. Initially, the government technical evaluation team shall evaluate the technical proposals on a pass/fail basis, assigning ratings of acceptable or unacceptable. The proposals shall be evaluated against the following factors:

The proposal shall:

a. Clearly demonstrate knowledge of tools and concepts such as value stream mapping, standardized work, A-3 reporting, Lean leadership, Six Sigma/Lean Healthcare integration, Lean facility design and maintenance.

B. Factor 2 - Price Evaluation. Next, the Government shall rank all technically acceptable offers by price, including any option prices if applicable. An offeror’s proposed prices will be determined by multiplying the quantities identified in Section B by the proposed unit price for each Contract Line Item Number (CLIN) or Subcontract Line Item Number to confirm the extended amount for each. Prices will be compared to the Independent Government Cost

Estimate and other prices received. The price evaluation will document the reasonableness, realism, and balance of the proposed total evaluated price.

(1) Reasonableness of offeror's proposal is evaluated through price analysis techniques as described in FAR Subpart 15.305 (a) (1) and (4). For the cost to be reasonable, it must represent a price that provides best value to the Government when consideration is given to prices in the market, technical and functional capabilities of the offeror and risk.

(2) Realism is evaluated by assessing the compatibility of proposed costs with proposal scope and effort. For the price to be realistic, it must reflect what it would cost the offeror to perform the effort if the offeror operates with reasonable economy and efficiency.

Proposals unrealistically high or low in price, when compared to the Government’s independent cost estimate and the market price, may be indicative of an inherent lack of understanding of the solicitation requirements and may result in proposal rejection without discussion.

(3) Balance – A proposal is determined to be unbalanced when despite the total evaluated price, the price of one or more CLINs is significantly overstated or understated as indicated by the application of analysis techniques. The Government shall analyze each offer to determine whether they are unbalanced with respect to separately priced line items. Prices proposed will be compared and evaluated to assure that a logical progression exists as related to price and quantity changes.

Offerors are cautioned to submit sufficient information and in the format specified in Addendum to FAR 52.212-1, Instructions to Offerors – Commercial Items. Offerors may be asked to clarify certain aspects of their proposal. Communication conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision.

The Government intends to award a contract without discussions with respective offerors. The

Government, however, reserves the right to conduct discussions if deemed in its best interest.
