Memorandum for Record 19 May 2016

| Repair Retaining Wall B128 |
1. A pre-proposal site visit was held for subject solicitation on 19 May 2016 at 0930 at building 128. See attached list for attendees.
2. Following is a summary of comments made by the Contract Specialist during the administrative portion of the site visit:
a. The contractor shall furnish all supervision, labor, materials, tools, equipment, apparatus, testing, and transportation required completing the construction in accordance with the specifications and drawings contained in Section J, List of Documents, Exhibits and Other Attachments. Construction to include but not limited to repair/replacement of existing retaining wall located at headquarters building 128.
b. This project is a Total Service Disabled Veteran Owned Small Business (SDVOSB) set aside. Offers are only being solicited from certified Service Disabled Veteran Owned Small Businesses. Contract Clause 52.219-27 Notice of Total Service Disabled Veteran Owned Small Business Set Aside is included in the solicitation at Section I page 9. This project is a 100% Service Disabled Veteran Owned Small Business Set Aside. Awarded contractor must be a formally certified SDVOSB by bid submission date. Failure to provide proper certification will result in loss of Award. Intent to misrepresent status may lead awarding agency motioning to have offending contractor debarred.
c. This project has one line item. This is a sealed bid solicitation whereby the bid opening will take place in B504 at 1300 (1 o’clock) in room 119 on 9 June 2016. Offers will be publicly opened at that time. Envelopes containing offers must be sealed and marked with the offer’s name, the solicitation number, and the date and time the offers are due. Hand carried bids shall be delivered to the Contracting Office at Bldg 504, Room 115, not later than that time. Proposal package is to include required bonds and the Financial Institution Reference Sheet (Atch 4 of the solicitation).
d. Contractors are reminded that in order to receive an award as a prime contractor, they must be registered and active in the System for Award Management(SAM) database at website https://www.sam.gov as well as have completed the representations and certifications within SAM. Contractor must also be registered in SAM for NAICS code 238110.0
e. The proposal is to be based on the specifications/drawings. GC’s are urged to closely review the drawing notes. No changes are to be made unless received in writing from the Contracting Officer in the form of an amendment. It is the contractors’ responsibility to check the FBO website for updates and amendments. This agenda as well as attendees will be posted to the web site.
f. Contractors are to direct all questions to the Contracting Office in writing. Questions can be e-mailed or faxed to the attention of Joseph Perry at 330-609-1042 or email at joseph.perry.14@us.af.mil, emails are preferred. It is imperative all communications are identified with the full solicitation number, not project number, in the subject line. Any/all questions received, along with the response, will be posted to the FBO website. Questions must be received by close of business 26 May 2016 in order to get them to the engineer to answer and posted by 3 June 2016.
g. Contractor is reminded that the Buy American Act pertains per the terms of the contract. The contractor shall use only domestic construction material in performing this contract. (Reference Section I - Contract Clause 52.225-9.) Consideration should also be given to bio based products. This clause can be found in Section K.
3. The contracting officer emphasized the deadlines were very critical in order to meet deadlines. Key Dates: Bid Opening 9 June 2016 @ 1300 (notify the undersigned if you plan to attend so an escort can be provided); 26 May 2016 questions must be received by close of business to the undersigned
4. Jack Geiss, CE, provided a briefing on project requirements and expectations
5. The attendees were encouraged to check the FBO site often for amendments, updates to wage determinations, and related changes. At the conclusion of the walkthrough, the site visit was adjourned.
