PRINCIPLES OF OFFICE MEDICAL DOCTORS (English/Turkish)

DEFINITIONS:

GENERAL DEFINTIONS:

As used throughout this work statement, the following terms shall have the meaning set forth below:

1. CONTRACTING OFFICER (CO):

A person appointed with the authority to enter into, administer and terminate contracts on behalf of the United States Government.

2. GOVERNMENT QUALITY ASSURANCE (GQA):

Those actions taken by the U.S. Government to ensure services meets the requirements of the contract.

3. PERFORMANCE WORK STATEMENT (PWS):

A document that accurately describes a service in terms of the output requirements and the required quality level of standard of acceptable performance of those outputs.

4. QUALITY ASSURANCE PERSONNEL (QAP):

Individuals designated to perform quality assessment functions, and manage performance in accordance with the Performance Plan. They serve as on-site technical managers assessing contractor performance against contract performance standards. Personnel in this area have many titles, such as Quality Assurance Evaluator.

5. LOCAL NATIONAL:

Turkish civilian employee working for 39 ABW (tenants included, contractors excluded).

6. BIOENVIRONMENTAL FLIGHT (39 MDG/SGBP):

Organization responsible for USAF industrial hygiene in accordance with USAF criteria.

7. SAFETY OFFICE (39 ABW/SE/SE):

Organization responsible for USAF safety in accordance with USAF criteria.

8. FIRE PREVENTION OFFICE (39 CES/CEF):

Organization responsible for USAF fire prevention in accordance with USAF criteria.

9. LABOR HEALTH AND SAFETY BOARD:

By Regulation #25426 dated 07 April 2004

39 ABW Civilian Personnel Office

GENERAL:

INTRODUCTION

The contractor shall qualify in occupational medicine as described in Article 81 of the Labor Law and Article 18 of regulation promulgated at the Turkish Official Gazette #25318 dated 16 December 2003 on “Worksite Health Unit and Duties, Working Procedures and Principles of the Physician,” herein after will be referred as Regulation #25318, as well as Article 30 of regulation promulgated at the Turkish Official Gazette #27320 dated 15 August 2009 on “Worksite Health and Safety Units and Joint Health Units,” herein after will be referred as Regulation #27320, and their updates; and collaborate with the 39 ABW in order to perform the health surveillance to the assessment of all health and safety hazards and for all other related duties reported in any applicable Turkish occupational health and safety legislation. The contractor is responsible for his/her service and accomplishment IAW Article 32 of the Regulation #27320, and any future or any other applicable legislation. This service is extended to all of the Local National (LN) employees working under the 39 ABW and AAFES.

The contractor is expected to use innovative approaches to efficiently and effectively accomplish the statement of work requirements in a timely manner at a reasonable cost.

The contractor shall provide all materials; tools and equipments to perform his/her job, excluding the following furniture that is property of the U.S. government of the USA: Writing board, projection, computer, chairs, small tables, an examination table and a file cabinet and the required property IAW annex 1 of Regulation #27320.

The contractor’s performance will be evaluated by QAP periodic inspections, through anonymous questionnaires filled out by employees at the end of the visit, customers’ complaints and civilian personnel office feedback.

DESCRIPTION OF SERVICES

Worksite Physician (WP) shall operate Incirlik Workplace Health and Safety Unit in accordance with requirements of this Performance Work Statement. The Unit located in Building 486 at Incirlik Airbase shall be manned by either a nurse or admin person Monday through Friday, 0730HRS to 1630HRS. Physician services shall be provided for approximately 85 hours for ABW employees and 15 hours for AAFES employees.

1. WP shall qualify in occupational medicine as described in Article 18 of Regulation #25318 and Article 30 of the Regulation #27320.

2. The WP shall provide Turkish Local National (LN) employees with the medical portion of the occupational safety and health program as described in Article 81 of the Turkish Labor Law, Article 32 of Regulation #27230 and Article 22 of Regulation #25318, and any future or other applicable Turkish occupational health legislation.

3. The WP shall provide medical & physical examinations to all LN employees as in accordance with Articles 31, 32, 33, 34 of Regulation #27320 and Articles 22, 23, 24 of Regulation #25318, and any future or any other applicable legislation. The WP shall accomplish all examinations and testing of employees at Incirlik Air Base facilities using his/her own optional equipment and assistant(s).

4. The WP shall collaborate with the 39 ABW and AAFES on the performance of services under Article 22 of Regulation #25318 and Article 32 of Regulation #27320 and any future or any other applicable legislation. The WP may request assistance of other specialists of different medical branches as necessary.

5. The WP shall participate in Labor Health and Safety Board meetings as required IAW Article 80 of Labor Law #4857 and Article 5.c of the regulation promulgated at the Turkish Official Gazette #25426 dated 07 July 2004 on the “Labor Health and Safety Board”, and Article 22.a of the Regulation #25318 and Article 22.ş of the Regulation #27320.

6. The WP will perform “pre-employment” examinations or equivalents IAW applicable Turkish legislation such as 32.e and 32.f of Regulation 27320 when requested by the U.S. government.

7. WP shall notify the 39 ABW whenever the outcome of medical surveys and periodical visits outlines that a previously unrecognized/uncontrolled health or safety hazard exists in the work place IAW Article 32 of Regulation #27320, and any future or any other applicable legislation.

8. WP shall utilize, among others but not limited to, results of workplace risk assessments provided by 39 MDG/SGBP, 39 ABW/SE, and 39 CES/CEF, workplace walkthroughs, reports by ESRs and LN workers, and results of occupational physical examinations to determine if a potential health threat exists.

9. Industrial hygiene data collected by 39 MDG/SGOB shall be made available to the WP through 39 MDG/SGBP.

The WP shall use it to determine the content of the occupational physical examinations so far performed on LN workers.

10. WP shall report occupational illness, along with hazardous exposure to the appropriate Turkish surveillance body as requested by Article 32 of Regulation #27320, and any future or any other applicable legislation.

11. WP shall make recommendations for personal protective equipment as appropriate and as needed.

12. WP shall assess ergonomic hazards for LN employees and make appropriate recommendations IAW Article 32 of Regulation #27320, and any future or any other applicable legislation.

13. WP shall express his/her recommendations pertaining to any actions/items of this contract in writing and in a detailed and timely manner. Deadlines will be coordinated with QA. QA may require that WP provides his/her recommendation in writing also for specific case/contingent situations. Correspondence shall be bilingual, (EN/TR).

14. All certificates produced by WP shall be bilingual, (EN/TR).

15. WP shall assist the 39 ABW/SE in the evaluation of all health and safety hazards, along with recommended preventive and protective actions.

16. WP shall request roster of LN employees related jobs descriptions and current list of exposed personnel and any other pertinent data upon request and coordination from QA and/or 39 FSS/FSMC.

17. WP shall advise on occupational health and other related matters when requested by management.

18. WP shall make appointments to visit employees and delivers pertinent correspondence to employees through the appointed QAP.

19. WP shall assist in maintaining Wing compliance with all Turkish safety legislations that are applicable.

20. The WP shall submit quarterly reports to QAP and CO listing all the names of personnel visited and all exams performed within 15 days of the new quarter.

21. In case of replacement, the WP is required to pass all information and documentation to the next WP in order to ensure service continuity.

22. The WP shall ensure continuity of the service in order to grant management capability of: hiring, clarifying/settle urgent/contingent labor related health issues.

23. The WP shall join in any site visits requested by the QAP; all site visits will be coordinated through CPO.

24. WP shall be required to provide service to all 39 ABW and AAFES LN employees not less than the periods stipulated in Article 21 of Regulation #25318 and Article 31 of the Regulation #27320.

25. WP shall perform any other requirements stipulated in Article 32 of the Regulation #27320 and Article 22 of the Regulation #25318.

26. WP shall utilize 39 ABW computer systems to issue and process prescriptions.

27. WP is required to comply with safety policies of USAF.

Facility Space (Building 486 at Incirlik Airbase) will be provided; however, the Turkish Air Force may have additional requirements to allow the contractor to use the facility. 10th Tanker Command should be consulted prior to submitting a response to the solicitation
