Instructions, Conditions, & Notices to Offerors or Quoters
Miscellaneous text in this section has been modified to:
ADDENDUM TO 52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (OCT 2018)
1. GENERAL INSTRUCTIONS
Failure to meet a requirement may result in a quotation rated non-responsive or unacceptable and therefore ineligible for award.
Complete blocks 12, 17a, 17b and 30a, b, and c of the Standard Format SF1449. In doing so, the Offeror accedes to the contract terms and conditions as written in the solicitation, with attachments.
The solicitation constitutes the model contract.
Acknowledged Solicitation Amendments. Provide all executed Standard Forms 30, or other appropriate amendment acknowledgements (if applicable).
Quotations must demonstrate a comprehensive understanding of the nature and scope of work required. The Government will not search for data to cure problems or address inconsistencies in quotations. The Government may consider any failure to comply with these instructions to be indicative of what could be expected from an Offeror during performance and may consider it a deficiency of the quotation.
Statements such as "will comply," "noted and understood," "IAW industry best practices/standards," etc. without supporting narrative are not acceptable. Cursory responses or responses that merely reiterate or paraphrase the Performance Work Statement (PWS) language will not be considered to satisfy the requirements of the Request for Quotations (RFQ). Each Offeror shall submit a complete quotation in accordance with (IAW) these instructions.
The Government's terms, conditions, and respective clauses contained within this solicitation are prescribed in accordance with United States Federal Acquisition Regulations (FAR), and are not subject to conditionally proposed revisions or changes requested by offerors. Offerors' specific terms and conditions that may be contingently proposed for subsequent inclusion into the contract if awarded, will not be considered by the Government in its evaluation of quotations. Offerors that are not able to comply with the aforementioned conditions, will be determined as non-responsive to the solicitation requirements, and therefore will be removed from consideration for award.
Submit signed and dated quotations to this office prior to the date and time specified in this RFQ.
Quotations shall be submitted on the Standard Form SF1449.

2. SPECIFIC QUOTATIONS PREPARATION INSTRUCTIONS
2.1 To assure timely and equitable evaluation of quotations, offerors must follow the instructions contained herein. This section specifies the format and content that offerors shall use in this Request for Quotation (RFQ). The intent is not to restrict the offerors in the manner in which they will perform their work but rather to ensure a certain degree of uniformity in the format of the responses for evaluation purposes. Offerors must submit a quotation that is legible and comprehensive enough to provide the basis for a sound evaluation by the Government. Information provided should be precise, factual and complete. Legibility, clarity, completeness, and responsiveness are of the utmost importance.
Quotations shall contain a response to each of the areas identified below. Any offer which does not provide as a minimum, which is required in this solicitation may be determined to be substantially incomplete/non-compliant and not warrant any further consideration at the Government's discretion.
Quotations must be completed and self-sufficient. In order to determine responsive an offeror must submit all the below documentation:
- Signed copy of the Standard Form 1449. This includes: Acknowledged Solicitation Amendments.
Provide all executed Standard Forms 30. Complete the necessary fills-in and certifications in provisions
- The proposed t documentation as required in the PWS including:
i. Relevant employment history detailing the individual's experience IAW the PWS, including history of performing manual therapy and Graston Technique soft tissue mobilization.
ii. A copy of the individual's Bachelor's Degree or higher in Athletic Training or equivalent from a CAATE accredited college or university.
iii. Board Certification CAATE BOC
2.2 In addition the requirements outlined above required to determine responsiveness of prospective offerors, the quotation shall consist of two separate parts.

Factor II - Price Proposal
By submission of its quotation the offeror agrees to hold the prices firm for 120 calendar days from the closing date.
2.3 The Contracting Officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial quotations, the Contracting Officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional price information will be requested. However, if at any time during this competition the Contracting Officer determines that adequate price competition no longer exists; offerors may be required to submit information to the extent necessary for the Contracting Officer to determine the reasonableness and affordability of the price.
2.4 Complete the necessary fill-ins and certifications in provisions. Return the provision FAR 52.212-3, Alt 1, Offeror Representations and Certifications - Commercial Items along with the quotation. For other provisions and clauses in the solicitation, the offeror is required to submit the pages that require an offeror fill-in.
2.5 Formal questions shall be submitted with your company information NO LATER THAN 04:00 PM Local time 30 May 2019.
2.6 Quotations must be received NO LATER THAN 10 June 2019 04:00 PM Local Time.

2.7 Gaining Access to Base Facilities: 31 CONS is located on an U.S. Air Force installation, and all access to the installation is controlled by the U.S. Air Force. An escort may be required as access will not be granted based solely on the need to submit an offer. If necessary, as pre-coordinated with the points of contact listed above, the Government will provide a representative to meet the offeror at the gate to the installation to accept the quotation submission. Furthermore, the U.S. Forces conduct random exercises which may require the closure of the main entrance gate and the utilization of an alternate base entry. Any delay based on access will not be accepted as a reason for late receipt of the offer by the Government.
2.8 In accordance with FAR Subpart 4.8., Government Contract Files, the Government will retain one copy of all unsuccessful offers.
2.9 The contractor MUST obtain DUNS NUMBER and CAGE CODE. In order to obtain the CAGE CODE, the contractor shall submit the request through the following website https:// www.siac.difesa.it.
In order to obtain the DUNS NUMBER, please follow the on line registration: http://dbitaly.dnb.com.

The following information is provided for obtaining Cage Code in Italy. Note that NATO Code is synonymous to Cage Code.
In order to obtain the code Offerors shall submit the required form (Questionario per l'Assegnazione del Codice NATO del Costruttore o del Fornitore) to the Contracting Officer. The form can be obtained from the Contracting Officer upon request.
3. QUOTATION ORGANIZATION
3.1 The offeror shall prepare the offer as set forth in the Quotation Organization Instructions below.
The titles and contents of the volumes shall be as defined in this table, all of which shall be within the required page limitation and with the number of copies as specified. Pages exceeding the page limitations set forth in this Section will be removed from the quotation and will not be read or evaluated and may render the offer unacceptable.
Pages shall be, letter or A4 size, Font - Times New Roman or Arial 12 pitch. Elaborate formats, bindings or color presentations are not desired or required.

4. VOLUME I - TECHNICAL OFFER
Technical offers must demonstrate the offeror's ability to meet the needs of the Government. The Offeror shall describe, in detail, its technical approach and overall ability to perform Athletic Trainer

services at Aviano AB, Italy. This includes listing in detail the offeror's proposed personnel for the required specialty and their respective experience and qualifications.
The offer shall include:
i. Relevant employment history detailing the individual's experience IAW the PWS, including history of performing manual therapy and Graston Technique soft tissue mobilization.
ii. A copy of the individual's Bachelor's Degree or higher in Athletic Training or equivalent from a CAATE accredited college or university.
iii. Board Certification CAATE BOC
5. VOLUME II - PRICE OFFER
5.1 Insert prices for CLIN 0001 all option periods for that CLIN (1001, 2001, 3001, 4001) as well as total prices in the Price Schedule for: Base Year and four (4) option periods (price for 6 additional months will be considered by 31 CONS evaluation team utilizing the last option year monthly price).
Insert proposed unit and extended prices. CLIN 0002 and all option CLINS associated with Travel (1002, 2002, 3002, 4002) will be evaluated at $5,000.00 for each CLIN for all received Offers. CLIN 0003 will evaluated at $25,000.00 for all received offers. Please do not insert separate prices into these CLINS as they will they will be evaluated at $5,000.00 for each travel CLIN and $25,000.00 for the relocation CLIN, regardless of what offeror inputs (PRE-PRICED).
NOTE: All proposed prices shall be submitted in U.S. Dollars. Unit prices no more than 2 decimals.
5.2 While DODDS support is authorized under logistical support, the cost associated with tuition is not covered separately. It is recommended that the contractor account for any additional cost it may encounter while supporting its proposed individuals and their family needs with DODDS tuition.
a) DODDS tuition rates are as follows:
i. Elementary Student (K-6) $23,360.00/ year
ii. Middle school student (7-8) $24,592.00/ year
iii. High School (9-12) $25.820.00/ year
b) If the contractor will cover these costs for their employees, they must include it into their Total Price. A separate invoice for tuition reimbursement will not be accepted.
