Zone A-8:

Answers to the above mentioned questions are the below, from Zone A-1 to A-8:

1. Med waste containers are covered in para 1.2.3. of PWS which is applicable to all areas.

Approx. total: 300 waste containers

2. Toilets are covered in section 1.3 of PWS.

Toilet cleaning does not change by areas. Disinfecting cleaning solution must be utilized for higher risk areas. (A3 to A5)

There are 93 toilets within the 31MDG and they all do have mirrors.

For B121 - Mirrors and sink are also installed in # 104 offices and # 34 exam rooms.

3. Floor mats are generally placed at each bldg.'s entrances (interior and exterior).

Tot of mats ~ #56

4. There are #5 stairwells in B121, #1 in B108 and # 1 in B103.

5. There are a tot of 93 toilets and 10 lockers within the 31 MDG.

6. Reference answer # 2.

7. There are a total of # 3 ice machines and #20 water fountains within the

31 MDG.

8. Ref PWS appendix VI.

Window Cleaning (Estimate Sqft 6039 /SqM 561)

9. Window screen ~ 200 SqM

Zone A-6:

57. The requested data vary based on the hospital/clinics workload. Average outpatient visits: 4000/month

Average inpatient/admission: 35/month

Zone A-8:

67. Ref question #3.

68. There are ~ 180 external light fixture within the 31MDG.

Question # 1). Is a cleaning schedule to be submitted with the proposal? If so should it be submitted as an attachment with a no page count?

Answer:----As stated in the updated Attachment 1, Appendix VIII, a draft Cleaning Schedule must be submitted with the proposal and the final Cleaning Schedule during the phase-in period. There is no page count.

Question # 2). The submittal Registry Appendix VIII also states that a quality control plan and a safety plan should be submitted with the proposal. If so, should these be submitted as an attachment with a no page count?

Answer:----As stated in the Attachment 1, Appendix VIII, the quality control plan and the safety plan must be submitted with the proposal. The Quality Control Plan is part of the page count. The Safety Plan is a no page count.

Question # 3). The submittal Registry Appendix VIII also states that a Material Safety Data Sheet should be submitted with the proposal. Does this mean all material safety data sheets for all proposed products? If so, should they be submitted as an attachment with no page count?

Answer:----As stated in the Attachment 1, Appendix VIII, the Material Safety Data Sheet must be submitted with the proposal. No page count is required.

Answer to Questions 1-2-3: ----As specified in FAR Clause 52.212-1, Para 4. Factor II – TECHNICAL INFORMATION, from page 15 of 55 to page 17 of 55, Technical Information: Quality Control Plan, Program Management Plan, Past Experience, should be submitted with maximum number of 50 pages total (see page 14 of 55, Table 2.2 Proposal Organization).

Question # 4) If we submit as a Joint Venture Firm, does the Joint Venture Firm need to submit the required documents (1.14, 1.15, 1.16, and 1.17), and/or can each member firm of the Joint Venture submit a complete set of the specified documents as referenced in the above sections?

Answer:----The Joint Venture Firm including each member firm/subcontractor must provide a complete set of the specified documents as referenced in 1.14, 1.15, 1.16, and 1.17.

Question # 5) If the Government provides the needed information to prepare a realistic cleaning schedule, will an extended due date be granted and/or would the Government consider requesting the cleaning schedule submitted during the transition period?

Answer:----Contractor is required to provide a draft cleaning schedule which will be elaborated and perhaps modified if needed during the phase-in period. There will be no extension to the due date.

Question # 6) Is it possible to know how many cleaning personnel work today in the contract ?

How many hours they work?

Answer:----The Government does not know the number of personnel and how many hours they work on the current contract. The contractor decides the number of personnel and hours needed to perform the service.

Question # 7) We required clarification of the company documents required on para 1.1.4, 1.1.5, 1.1.6, 1.1.7, 1.1.8.

Answer:----All documents are separate and all of them are required.

1.1.4 are the current license and insurance of the company; 1.1.5 regards the past history and economical consistency; 1.1.6 DURC is a document that proves that the company is regularly paying all social security dues; and 1.1.7 Certificazione antimafia proves that the company doesn't have allegations related to Mafia.
