1. DESCRIPTION OF SERVICES/GENERAL INFORMATION: The awarded contractor shall furnish all personnel, supervision, tools, materials, equipment, transportation and other replenishment supplies and services necessary for performance of this requirement. Quality of services shall be consistent with the best commercial practices to ensure that medical treatment facilities commercial standard custodial services are performed at 31st Medical Group Aviano AB, Italy, to include the Pordenone Liaison Office, in accordance with this performance work statement (PWS).

1.1. PERFORMANCE OBJECTIVE: The contractor shall accomplish all custodial tasks to meet the completeness, requirements and quality set forth in this documents’ Service Summary (SS) and the minimum cleaning frequencies outlined in Appendix IV, and the Price schedule in Appendix IX. Any surface not bearing carpets shall be swept, dust mopped, damp mopped, wet mopped, dry buffed, and/or spray buffed, to ensure they have a uniform, glossy appearance, free from dirt, debris, dust, scuff marks, heel marks, other stains, discoloration, and other foreign matter. A clean, neat and uniform appearance shall be displayed upon completion of services.

1.2. BASIC CLEANING SERVICES.

1.2.1. Remove Trash and Recycling. All trash and recyclable receptacles for dry, wet/food, plastic/can, glass and paper located inside and outside of the vicinity of the facilities shall be emptied. The interior of all trash receptacles must be clean and free of any kind of debris. Soiled or torn plastic trash/recyclable receptacle liners shall be replaced with serviceable proper fitting liner. The trash/recyclables shall be deposited in the nearest outside trash/recyclable collection container that is assigned to the facility. Cleaned trash receptacles shall be returned to their designated location.

1.2.2. Reserved.

1.2.3. Removal, Handling and Storage of Regulated Medical Waste Containers. All regulated medical waste containers that have been closed and sealed must be removed. The contractor must not handle or remove regulated waste containers that are unsealed. Custodial personnel shall replace sealed containers with a new standard marked medical waste container and bag. The contractor personnel shall store the sealed regulated medical waste containers in a designated location. Healthcare waste shall always be handled, treated, destroyed, packaged and labeled, and disposed of as specified in this PWS, USAFE 23-104. USAFE Command Dangerous Goods Program, Linee guida regionali per la gestione dei rifiuti sanitari and/or the ADR, and applicable italian legislation. If there is a conflict between standards, the most stringent will be followed. Contractor personnel must wear appropriate PPE during waste handling.

1.2.4. Clean Glass/Mirrors/Doors. Clean all interior glass including doors, partitions, walls, display cases (exterior only), directory boards, and exterior of entrance doors (under 2m). After glass/mirror cleaning, there shall not be traces of film/streaks, dirt, smudges, water, or other foreign matter (this paragraph shall not include computer monitors, pictures, posters, personal affects, etc.).

1.2.5. Dusting of Low Surfaces. The contractor shall clean with the appropriate cloth, all low surfaces to include; all furniture, chairs, televisions, oscillating fans, table lamps, computer monitors, windowsills, ledges, handrails, wall-mounted items etc. No dusting of medical staff personal effects is required. Upon completion of task, services shall be free of dust, lint, smudges and any foreign matter.

1.2.6. Maintain Floors. Any surface not bearing carpets shall be swept, dust mopped, damp mopped, wet mopped, dry buffed, and/or spray buffed as applicable to ensure they have a uniform, glossy appearance and free from dirt, debris, dust, scuff marks, heel marks, other stains and discoloration, and other foreign matter. Baseboards, corners, and wall/floor edges shall also be clean. All floor maintenance solutions shall be removed from baseboards, furniture, trash receptacles, etc. Chairs, trash receptacles, and other moveable items shall be moved to maintain floors underneath these items. All moved items shall be returned to their original and proper position.

1.2.6.1 Sweeping of Floors. The contractor shall sweep floors to remove debris, which include corners, grouts and baseboards, in such a way as to minimize the dust particles in the air. The contractor must tilt or move chairs, trash receptacles, and easily moveable items and sweep underneath. All moved items must be returned to their proper position upon completion of sweeping services. Upon completion of task, floor and baseboards shall be free of dirt, trash, and any foreign matter.

1.2.6.2 Mopping of Floors. The contractor shall mop all accessible floor areas to include corners, grouts and baseboards and rubber entrance mats ensuring they are clean and disinfected. Disinfecting-solution shall be changed after every three rooms or more often in heavily soiled areas. Once mopping is complete no detergent residue, evidence of soil or standing water shall remain. Mop head shall be replaced with a new mop/mop head when the detergent solution is changed, after cleaning a blood spill or at least once daily. The mop buckets shall not be placed back into storage without mop water being properly disposed of and bucket rinsed. The contractor must tilt or move chairs, trash receptacles, and easily moveable items and mop underneath. All moved items must be returned to their proper position upon completion of mopping services. The contractor shall remove splash marks or mop streaks on furniture, wall baseboards, etc., or mop strands remaining in the area. Cleaning of floors shall be performed in a way that prevents medical staff and patients from crossing wet floors. The contractor shall ensure that wet floor signs are posted and visible at all times floors are wet. When a wash/vacuum machine is used in corridors, waiting rooms and entrances special precautions shall be taken to prevent any hazards to patients or medical staff. Upon completion of task, the floor and baseboards shall be free of dirt, mop strands, residues, and any foreign matter.

1.2.6.3. Vacuuming of Floors. The contractor shall vacuum all carpeted floors to include carpet-type or polyester entrance mats. All tears, burns, and raveling shall be brought to the attention of government CORs on the day they are noticed prior to departure of the contractor employees from the work site. The contractor must tilt or move chairs, trash receptacles, and easily moveable items and vacuum underneath. All moved items must be returned to their proper position upon completion of vacuuming services. Upon completion of task, the area shall be free of dirt and any foreign matter.

1.2.6.4. Vacuum and Clean Interior/Exterior Floor Mats. The contractor shall vacuum and clean interior and exterior floor mats (regardless of material type). After vacuuming or cleaning, mats shall be free of all visible lint, litter, soil and other foreign matter. Soil and moisture underneath mats shall be removed and mats returned to their designated location.

1.2.7. Clean Stairways. All stairway surfaces shall be cleaned in accordance with paragraph series 1.2.6., as appropriate for floor covering. Grease, gunk, and grime shall be removed from stair guards, handrails and baseboards. Contractor shall remove all marks, dirt, smudges, scuffs, and other foreign matter from adjoining stairwell walls to provide or maintain a clean, uniform appearance.

1.2.8. Linen Collection and Re-Supply. The contractor shall collect linen from all areas at the end of medical staff duty day and deposit it at the nearest collection point. At the collection point, the government personnel shall weigh and log all dirty linen bags delivered to and all clean linen bags received from the linen-cleaning contractor. Contractor shall bring to the attention of government personnel when linen level is low or when specific linen type is needed (ref. identification on linen carts for linen requirements).

1.2.9. General Dusting and Cleaning. All surfaces below 2 meters must be dusted and cleaned to present and maintain a neat, clean, professional medical facility appearance. Surfaces shall be free of all dirt, dust, or other foreign debris. Areas to be dusted include non-personal horizontal surfaces, tables, conference tables, chairs, windowsills, cable runs, and baseboards. The contractor is expected to dust around items if possible. The contractor is responsible for dusting electronic equipment and desks.

1.2.10. General Spot Cleaning. Spot cleaning shall be performed on a continual basis, and immediately upon identification of a discrepancy to the identified standards. Spot cleaning includes but is not limited to removing or cleaning smudges of fingerprints, marks, streaks, spills, etc. from washable surfaces including, but not limited to walls, partitions, vents, grillwork, doors, door guards, door handles, push-bars, kick-plates, light switches, climate controls, fixtures, carpets, and hard floors. After spot cleaning is performed, the item shall have a clean uniform appearance, free of streaks, spots and other evidence of soil. Spot-cleaned carpet shall be cleaned in accordance with manufacture’s recommendations or standard commercial practices and be free of recent removable spills. Any spot or group of spots that are larger than one square meter or that requires the use of carpet extraction equipment is not considered spot cleaning. This classification occurs when the contractor attempts three consecutive attempts to remove the stain using other methods. When the stain will not lift under these circumstances, then it is considered in need of carpet extraction. If the contract personnel find gum, or any other sticky substance on the floor, the contract personnel will remove the gum or sticky substance on site, with as little harm to the flooring, or chairs as possible.

1.3. BASIC RESTROOMS/LOCKER ROOMS CLEANING SERVICES.

1.3.1. Clean and Disinfect. Completely clean and disinfect all surfaces of sinks, toilet bowls, urinals, lavatories, showers, shower mats, dispensers, plumbing fixtures, saunas, partitions, doors, walls, toilet brushes, toilet brush holders, and other such surfaces, in accordance with approved commercial standards. Clean all types of furniture to include chairs, benches, tables and/or any other furniture like items placed in restroom. After cleaning, receptacles will be free of deposits, dirt, streaks, mold, mildew and unpleasant odors. Disinfect all surfaces of partitions surrounding urinals, toilets, sinks, stalls, stall walls, entry doors, including handles, kick plates, ventilation grates, metal guards, tile walls, and wall areas adjacent to wall mounted lavatories, urinals, and toilets.

1.3.2. De-scale Shower. Toilet Bowls and Urinals. De-scaling shall be performed to keep all areas free of scale, soap films, mold, mildew and other deposits. After de-scaling, surfaces shall be free from streaks, stains, scale, scum, mold, mildew, bodily waste deposits, rust stains, etc.

1.3.3. Cleaning/Disinfecting Showers. The contractor shall scour and disinfect shower to include walls, shower curtains/ rod, door, pipes, fixtures, benches and floor mats using the appropriate color coded cloth or scouring pad. Any benches within the showering area shall be cleaned. Upon completion of the task, the surfaces shall be free of scale, mildew, calcium deposits, water spots, streaks, smudges, soap residue and any foreign matter.

1.3.4. Cleaning of Sinks and Drinking/Non Drinking Water Fountains, and Ice Machines. Clean and disinfect all porcelain and polished metal surfaces, including the orifices and drain, as well as exterior surfaces of fountain. Sinks, ice machines, and drinking/non drinking water fountains shall be free of residue, deposits, watermarks, streaks, stains, spots, smudges, scale, and other soil and foreign matter.

1.3.5. Re-Supply Restroom Supplies/Cleaning of Dispensers. Contractor shall ensure restrooms are stocked sufficiently with supplies such as soap, toilet paper, paper towels, air deodorizers, baby changing stations with appropriate items. Supplies shall be stored in designated areas. All restrooms shall contain soap, toilet paper, paper towels, and air deodorizer even where/when dispensers are not provided or are broken. Contractor shall use wall-mounted soap dispenser-not bottles, unless Contractor has written authorization from the COR. Re-supplying of supplies shall only be done to Government-Furnished dispenser, such as toilet paper holder, hand paper towel dispenser, and soap dispenser. All surfaces upon completion of restock of supplies will be free of soap, streaks and dust build up.

1.3.6. Sweep, Mop and Disinfect Restroom Floor. After sweeping and mopping, the entire floor surface including grout, shall be free from litter, dirt, dust and debris. Grout on wall and floor tiles shall be free of dirt, scum, mold, mildew, residue, or any other foreign matter. Floors shall have a uniform appearance without streaks, swirl marks, detergent residue, sticky residue, or any evidence of soil, stain, film or standing water. Readily moveable items shall be moved to sweep and wet mopped underneath, and placed back in their original locations.

1.3.7. Remove Trash and Recycling. All trash, recyclable, and feminine hygiene receptacles shall be emptied. All trash containers shall be cleaned and disinfected. Soiled or torn plastic trash/recyclable receptacle liners shall be replaced with a serviceable proper fitting liner. The trash/recyclables shall be deposited in the nearest outside trash/recyclable collection container that is assigned to the facility. Trash/recyclable receptacles shall be left clean, free of foreign matter and unpleasant odors. When trash receptacles are finished being cleaned, they shall be returned to their initial location.

1.4. BASIC MONTHLY CLEANING SERVICES. See Appendix IV for frequency, and selected facilities to receive prescribed cleaning services. All basic monthly periodic cleaning must be performed after duty hours.

1.4.1. Maintenance of Floors. Prior to commencement of floor maintenance services, the contractor shall familiarize themselves with the types of floors and method of floor installation in all facilities to determine the best cleaning and maintenance method. NOTE: The types of floors may change during the life of the contract. All moveable furniture and items such as trash receptacles, chairs etc. shall be removed from area to access as much of the floor as possible. Once services are completed, items removed shall be placed in their proper positions in a neat, clean, and orderly manner.

1.4.2. Ceramic Floors. All ceramic floors and grouts are to be cleaned in a manner that presents a surface free of dirt, scum, mold, mildew, residue, or any other foreign matter. Floors shall have a uniform appearance without streaks, swirl marks, detergent residue, sticky residue, or any evidence of soil, stain, film or standing water.

1.4.3. High Dusting. Dusting above two (2) meters shall be performed as outlined in Appendix IV so that all surfaces, floor to ceiling, horizontal and vertical, are dusted to present a clean appearance. Areas listed, but not limited to, air conditioning grills, suspended heaters, wall mounted cameras, ledges, window blinds, and diffusers shall be free of all dirt, dust, cobwebs, or other foreign debris. The contractor shall provide all necessary equipment (i.e. ladders, man-lifts, etc.) to reach areas above two (2) meters in height.

1.4.4. Cleaning Of Window Coverings. The contractor shall vacuum entire surface of curtains and fabric Venetian blinds thoroughly. Upon completion of task, the surfaces shall be free of dust, lint and any foreign matter.

1.5. BASIC QUARTERLY CLEANING SERVICES: See Appendix IV for frequency, Appendix III for Facilitiy List and Appendix to receive prescribed cleaning services. All basic quarterly periodic cleaning must be performed after duty hours.

1.5.1. Removal of Privacy Curtains in a Rotation. The contractor shall remove privacy curtains and replace them with new ones, that are provided by the government, at a minimum of six times per quarter or when directed.

1.5.2. Cleaning of Ceiling. The contractor shall ensure all light fixture covers and ceiling mounted light fixtures are clean. In case the light fixture needs to be removed so that cleaning can be performed then the contractor must ensure that it is reinstalled in the original location once the task is completed. The contractor shall also clean all tiles/or surface that is present around the air vents. Upon completion, the area should be free from dust, cobwebs/spider webs and other debris.

1.5.3. Clean/Shampoo Carpets. All carpets shall be cleaned in accordance with the manufacturer’s recommendation or common commercial practices. After shampooing, the carpeted area will be uniform in appearance and free of stains and discoloration. All cleaning solutions shall be removed from baseboards, furniture, trash receptacles, chairs and other similar items. Chairs, trash receptacles, rugs, and other small non-personal items shall be moved by the contractor in order to clean the carpets underneath.

1.5.4. Strip, Scrub, Seal, Wax and Spray Buffing Of Floors. Strip, scrub, seal, buff, wax, and spray-buff floors to maintain a uniform lustrous high gloss appearance on all floors. A non-skid wax is required. A uniform lustrous high gloss appearance is free of scuffmarks, heel marks, wax build-up, and other stains and discoloration. The contractor must maintain a uniform lustrous high gloss appearance on all high traffic floor areas and ensure that all furnishings are properly positioned once task has been completed. The contractor shall clear the area from any moveable items where possible. All moved items must be returned to their proper position upon completion of the service.

1.5.5 Stripping of Floors. The contractor shall strip floors of their finish. Upon completion of the task, the floor and baseboards shall be free of stripping residue, and present a flat unfinished appearance.

1.5.6 Sealing of Floors. The contractor shall apply a high stain resistant polymer sealer after floors have been stripped. Upon completion of task floor and baseboards shall be free of boot polish, scuffs, sealant residues, and present a high gloss finish.

1.5.7. Waxing of Floors. This service shall apply to the entire facility excluding zones A6-A7. The contractor shall apply a high lustrous gloss wax finish after floors have been stripped. It may be necessary for the contractor to apply several coats of lustrous wax finish to achieve a high gloss. Upon completion of task, the floor and baseboards shall be free of wax residues, boot polish, scuff marks and present a high lustrous gloss finish.

1.6. BASIC SEMI ANNUAL CLEANING SERVICES (These must be performed after duty hours)

1.6.1. Cleaning of Windows. The contractor shall clean window interior/exterior surfaces. All curtain rod, rings, rings, brackets, window ledges, frame, etc. Upon completion of task, the surfaces shall be free of dust, lint, streaks and any foreign matter.

1.6.2. Window Screen Cleaning. The contractor shall clean window screens without causing any damage. Upon completion of task, the surfaces shall be free of dirt, dust, streaks, any bodily fluids, or foreign matter.

1.6.3. Air Vent Cleaning. The contractor shall clean all air vent slats using the proper color-coded cleaning cloths. Upon completion of task, the vent surface shall be free of dust, dirt, streaks, lint, cobwebs, spider webs, and any other foreign matter.

1.7. SPECIAL CLEANING REQUIREMENTS

1.7.1. Zones A3 to A5 (Patient Care Area and Ambulatory Care Area). The contractor shall perform all basic cleaning services as outlined in paragraph 1.2 through 1.5. In addition, the contractor must use disinfecting cleaning solution.

1.7.2. Zones A3 to A5 (Labor/Delivery/Operating Room and Center Core). The contractor shall perform all basic cleaning services outlined in paragraphs 1.2 to 1.5 in the corridor area of the operating room to include the hallway that is located behind the operating room.

1.7.2.1. Daily Cleaning Requirements. The contractor shall perform all basic cleaning services as outlined in paragraphs 1.2 through 1.5, using a disinfecting solution to complete services. The contractor shall clean and disinfect all wall surfaces daily.

1.7.2.2. Operating Rooms Only. The contractor will be required to remove all furniture from the room. The contractor shall fully clean and disinfect the entire surface of the floor, cabinets, furniture, equipment, ceilings, overhead lights, and light covers. The contractor shall clean all exterior surfaces of the Autoclaves according to manufacturer’s specifications, which will be provided by government personnel.

1.7.2.2.1. Zones A3 to A5 (Patient Care/Ambulatory Care Area/Labor/Delivery/Operating Room and Center Core). The contractor will be required to remove from surfaces patients body fluids (i.e. urine, blood,, saliva, spills etcetera) using proper Infection Control precautions and PPE.

1.7.2.2.1.1 Annual Cleaning

Bldg 103/108 Gazebo and Playground cleaned by power Washing, clean Bldg 14300 Canopy/Fences and B1415 WRM Warehouse (spider webs, cobwebs, lint dirt etc).

1.7.2.3. Weekly Cleaning Requirements

1.7.2.3.1. Center Core Only. The interior and exterior of all cabinets shall be cleaned. Upon completion of the task, the surfaces shall be free of, dust, dirt, streaks, water spots, smudges, soap residue and any foreign matter.

1.7.2.3.2. Room Turnover (Between surgical cases and as required). The contractor’s staff shall enter surgical areas after removal of patient. Clean and disinfect overhead light, all furniture and operating room table or labor/ delivery bed. Mop the entire floor, operating table or labor/delivery bed (this includes moving the table or bed). The contractor will be required to change the mop after every surgical case turnover. The contractor will be required to wet vacuum the floors using above-mentioned guidelines. The contractor will be required to commence all cleaning/disinfecting tasks within 5 minutes of notification and shall complete cleaning/disinfecting within 15 minutes.

1.7.2.3.3. Room Terminal Cleaning (M-F at the end of duty day). The contractor shall clean and disinfect all overhead lights, floors as well as underneath the furniture, ensure that the floor surfaces are fully disinfected and wet vacuum floor. The contractor shall clean and disinfect all surfaces and equipment and ensure that they are free of dust, dirt, bodily fluids, stains, streaks, smudges, and any foreign objects or marks. The contractor shall ensure that furniture and equipment that was moved is returned to its original position upon completion.

1.7.2.3.4. Clean the Ct Scanner Room on Tuesday and Thursday. The contractor shall perform all basic cleaning services, as outlined in paragraph 1.2 through 1.6 to include disinfect surface during the cleaning process.

1.7.3. ZONE A-5 (In-Patient Area). The contractor shall perform all basic cleaning services, as outlined in paragraph 1.2 through 1.6 to include disinfecting surfaces during the cleaning process.

1.7.3.1. Daily Cleaning Requirements

1.7.3.1.1. Clean and disinfect all wall surfaces below the height of 2 meters. This step should also include all light switches, doorknobs, rails, and other obvious areas.

1.7.3.2. Terminal Discharge Cleaning (Between patients and as required)

1.7.3.2.1. Whenever a patient is discharged or relocated within the hospital, the following requirements must be met prior to a new patient being admitted to the room. In private rooms, the basic cleaning services outlined in paragraphs 1.2 through 1.6. using disinfectant solution must be completed. In multi-bed rooms, the linen changing and cleaning of the bed, closets and drawers shall apply to those used by the discharged patient only.

1.7.3.2.2. Changing of bed linen. Contractor shall carefully remove soiled linen from the bed and place in laundry bag. Contractor shall also remove any trash left on the bed by patient before placing linen in laundry bag. For each pillow, two pillowcases are to be used so that both ends of pillow are enclosed by a case (unless pillow is already in a hygienic cover). Then, the bed must be raised to the highest position with the over-the-bed table placed over the bed.

1.7.3.2.3. Cleaning of bed and surrounding area. All mattress surfaces must be cleaned. Clean and disinfect entire bed including the four sides of mattress, the wheels, bed control unit, rails, headboard and footboard. Damp wipe of pillow can only be done if pillow is enclosed in plastic cover. Report any tears in protective coverings and in mattress protective outer covering to the nurse manager. The contractor shall clean and disinfect telephones/nurse call devices and bedside stand inside and out. Upon completion of this task, all interior and exterior surfaces shall be cleaned and disinfected, in addition to the over-the-bed table. The contractor shall notify the charge nurse upon cleaning completion that the unit is suitable for use.

1.7.3.2.4. Discharge unit shall not be cleaned if patient’s belongings are still in the room. The nursing staff shall be notified in such a case.

1.7.3.3. Time requirements. During day and evening shifts, the contractor shall begin terminal cleaning within 60 minutes of notification by the medical staff.

1.7.4. Zone A-6 (Isolation Room)

1.7.4.1. Clean only when directed by COR

1.7.4.1.1. The contractor shall perform all basic cleaning services as outlined in paragraphs 1.2 through 1.6. using a disinfecting solution.

1.7.4.1.2. Isolation rooms shall be cleaned after all other patient rooms.

1.7.4.1.3. Hands shall be washed before entering and upon leaving each room even when gloves have been used.

1.7.4.1.4. Cleaning cart stocked with all necessary equipment and supplies to perform required tasks shall not enter rooms.

1.7.4.1.5. Any paper products taken into the room shall be disposed of in the room and must not be used again.

1.7.4.1.6. Mops and rags used in the cleaning of each isolation room shall be appropriately discarded (either as infectious linen to launder or as regulated waste to be destroyed).

1.7.4.1.7. Protective clothing (scrubs must be donned prior to entrance of all sterile environments in the facility).

1.7.4.1.8. Knock and announce yourself before entering room.

1.7.4.1.9. Clean all equipment used in the room and return it to the cart.

1.7.4.1.10. Place disposable soiled mop head and cloths in a new regulated medical waste plastic bag (government furnished) and place by the door. The contractor shall not use a room’s existing or in use biohazard container/bag system to dispose of soiled mop heads and or clothes.

1.7.4.1.11. Remove protective clothing while in room and dispose of it in the new regulated medical waste bag placed by door.

1.7.4.1.12. Seal Biohazard bag containing protective clothing etc. prior to removing it from room. If the outside of the regulated medical waste plastic bag is soiled, place it inside another clean regulated medical waste bag.

1.7.4.1.13. Dispose of regulated medical waste bags at designated collection point immediately.

1.7.4.1.14. Inform the nurse manager of any problems or needed repairs.

1.7.4.2. Terminal ( Discharge Cleaning)

1.7.4.2.1. The contractor shall perform all tasks as outlined in paragraphs 1.2 through 1.6. using a disinfecting solution.

1.7.4.2.2. Remove all linen from bed and deposit in water-soluble, impervious infectious linen bag .

1.7.4.2.3. Infectious linen bag shall be sealed once all reusable soiled linen has been placed inside, then deposited at appropriate collection point immediately.

1.7.5. Zone A-7 – Regulated Medical Waste & Linen Storage

1.7.5.1. The contractor shall clean and disinfect all surfaces to include walls and racks.

1.7.5.2. The contractor shall remove all floor plastic anti-slip pallets, clean, disinfect and place them in the original position.

1.7.5.3. The floor needs to be swept and mopped as outlined in 1.2.6.

1.7.6. Zone A-8 - Exterior Cleaning & Grounds Maintenance

1.7.6.1. Daily Cleaning Exterior of Buildings.

1.7.6.1.1. The contractor shall provide the applicable services outlined in paragraphs 1.2 through 1.5.

1.7.6.1.2. Exterior mats and foot scrapers shall be washed and cleaned a minimum of monthly, or when it is deemed visibly dirty, or as requested by the COR. All soil and moisture shall be removed from underneath mats and runners before being returned to their normal location. After cleaning is complete all exterior and interior mats and foot scrapers shall be free from dirt, dust, mud, streaks, stains, scale, scum, mold, mildew, and foreign matter.

1.7.6.1.3. Remove cobwebs, mud, dirt, and litter from the outside walls, canopies, entrance lights and the immediate vicinity of the entrances.

1.7.6.1.4. The contractor shall wash down the walkways, ramps, loading docks within 5 meters of building, except when the ambient temperature drops below 1 degree Celsius (33 degrees Fahrenheit). In the event the temperature drops below 1 degree Celsius, the contractor shall be required to salt and clean all ice/snow from all sidewalks entrances and patient entrance walkways surrounding the facility.

1.7.6.2. Semi annual. Exterior light fixture covers will be removed and the fixture and cover will be thoroughly cleaned. After cleaning is completed cover will be put back in place.

1.7.6.3. Grounds Maintenance

1.7.6.3.1. The contractor shall pull weeds from sidewalk areas and planters as needed, except during cold winter seasons when weeds do not grow.

1.7.6.3.2. Water plants around the buildings and in planters as needed.

1.7.6.3.3. The contractor shall remove all leaves and debris from grass or walkways. Remove leaves, tree limbs and debris from all A-8 areas. After a storm, all leaves, twigs, dirt, dust, and weeds must be removed from area A-8 as well.

1.7.6.3.4. The contractor shall mow grass and trim weeds in parking lot and edge around the grass including the islands as required, except during cold winter seasons when weeds don’t grow. Remove all grass and clipping after mowing/trimming and edging. Upon completion of the tasks, the grass, weeds, bushes, and edged surfaces shall present a maintained, clean uniform cut throughout the surface.

1.7.6.3.5. Prune, trim and purge bushes of dead limbs and leaves as required.

1.7.6.3.6. Plant and flower containers shall be replenished with new flowers and plants as required to maintain visually appealing grounds. Flower and plant selections shall be pre-approved by the COR

1.7.6.3.7. Loose bulk items (grass, dirt, leaves, rocks, trimmings, etc.) will be disposed of in designated dumpsters indicated during site-visit and pre-performance meeting. These items must be placed in large garbage bags prior to disposal.

1.7.6.3.8. Removal of snow and ice within 5 meters of buildings shall be completed 30 minutes before the beginning of the day shift and as necessary to prevent accumulation of more than 2 cm (1 inch) of snow. The contractor shall clean entire walkway from entrance/exit to end of building. Walkways will be clean and safe for pedestrian traffic at all times. At a minimum, ice control shall consist of removal of ice and/or application of salt during both day and evening shifts to maintain safe conditions for pedestrian traffic.

2. SERVICE SUMMARY (SS)

2.1. Overview. This Services Summary (SS) is a summary of the minimum performance objectives and performance thresholds required by the government in contractor performance. The Government reserves the right to inspect all services specified in the contract, and determine whether the performance objectives and goals are met.

2.1.1. The absence of any contract requirement from the SS shall not detract from its enforceability nor limit the rights or remedies of the Government under any other provision or clause of the contract including FAR clause 52.212-4, Contract Terms and Conditions- Commercial Items, paragraphs (a) and (m).

2.1.2. SS will be used as the baseline to develop a QASP. The QASP prescribes the objective in having the services, the goals of the multi-functional team, team members and their roles and responsibilities (to include the contractor performing the service), and how the multi-functional team will assess contractor performance and manage the contract to obtain efficiencies, improved performance, and cost savings throughout its life cycle.

2.1.3. Surveillance methods may include customer complaints, periodic inspection, and random sampling based inspections by the COR.

2.1.4. Methods of surveillance can change after contract award based on, but not limited to the following:

a. In accordance with the Medical Treatment Facility Environmental Tour Form and any other form/s of QC that the government may deem appropriate.

b. Contractor performance – based on results of previous inspections.

2.2. PERFORMANCE EVALUATION. Performance of the service will be evaluated to determine whether it meets the performance threshold. Re-performance is the preferred method of correcting any unacceptable performance.

The floors should be washed, scrubbed, sealed, scaled and waxed presenting a lustrous appear hence.
