To ensure timely and equitable evaluation of proposals, offerors must follow the instructions herein. Offerors are required to meet all solicitation requirements, including terms and conditions, and representations and certifications, in addition to those identified as evaluation factors. Failure to meet a requirement may result in an offeror being ineligible for award. The Government intends to award without discussions, but reserves the right to conduct discussions if necessary. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Proposals must be complete, self-sufficient, and respond directly to the requirements of this solicitation; alternate proposals will not be accepted. The response shall consist of three (3) separate volumes: Volume I – Price/Contract Documentation Proposal, Volume II- Technical Proposal, and
Volume III- Past Performance. The offer acceptance period shall be a minimum of 180 calendar days.
A. POINT OF CONTACT/RFP QUESTIONS

Government to verify the number of e-mails that are sent by a single offeror (e.g. 1 of 2, 2 of 2). It is the offeror’s responsibility to verify that the Government timely received the e-mail; the offer is complete; and, the Government can view the offer. The Government will be responsible neither for offers not received nor for any failure of the offeror to verify the receipt of the e-mailed offer.

VOLUME PARAGRAPH # TITLE
I L-1.D Price/Contract Documentation
II L-1.E Technical
III L-1.F Past Performance Information
1. Page Limitations. Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal. The CO may place page limitations on responses to
Evaluation Notices (ENs). The specified page limits for EN responses will be identified in the letters forwarding the ENs to the offerors. Unless otherwise indicated in the Volume details, each page shall be counted except cover pages; table of contents and indexes; list of figures, tables, or drawings; tabs; blank pages;
glossaries; cross reference matrices and document revision history tracking records.
2. Page Size and Format. Page size shall be 8.5 x 11 inches, not including foldouts. For all volumes, a page is defined as each face of a sheet of paper containing information. Pages shall be typed and single spaced at minimum. Tracking, kerning, leading values, and footnote font size shall not be changed from the default values of the word processing or page layout software. Use at least 1 inch margins on the top and bottom and ¾-inch side margins. Pages shall be numbered sequentially by volume, with each volume starting at page one. Pages that do not count towards the page limitation shall be numbered in a manner that clearly identifies the page as a page that does not count toward the page limitation.
3. Cross-Referencing. To the greatest extent possible, each volume shall be written on a stand-alone basis so that its contents may be evaluated with a minimum of cross referencing to other volumes of the proposal.
Information required for proposal evaluation which is not found in its designated volume will be assumed to have been omitted from the proposal. Cross-referencing within a proposal volume is permitted where its use would conserve space without impairing clarity.
C. ADDITIONAL INFORMATION

2. High Probability of Adequate Price Competition: The Procuring Contracting Officer (PCO) has determined there is a high probability of adequate price competition for this acquisition. Upon examination of the initial offerors, the PCO will review this determination and if, in the PCO’s opinion, adequate price competition exists, no additional price information will be requested. However, if at any time during this competition the
PCO determines that adequate price competition no longer exists, offerors may be required to submit additional information such as Other than Certified Cost or Pricing Data to the PCO for determination of fair and reasonable pricing.

Examples include past performance surveys, copies of performance records, CPARs, etc. Copies of CPARs are not required, but offerors must note whether a CPARs rating is available for all government contracts cited. The past performance evaluation considers each offeror's demonstrated record of performance to include quality in supplying products and services that meet the solicitation requirements.
2. Confidentiality of Sources. Past performance information, including questionnaires and past performance sources, are confidential and are not to be released to anyone outside the Government source selection process.
Offerors are cautioned that contacting or seeking out source(s) of past performance information in an attempt to influence past performance ratings is strictly prohibited and could negatively impact the offeror’s performance rating. Any questions regarding source selection information shall be addressed directly to the CO and not to any other individuals. Offerors are responsible for ensuring that all their teaming partners, joint venture partners, and/or major subcontractors are aware of this information and are in compliance.
3. Volume Organization. The past performance volume shall be organized according to the outline below:
a. Volume Table of Contents
b. Glossary (if applicable)
c. Citation Past Performance Information, including:
a. Past Performance Information Documents/Forms

4. Recency and Relevancy. Recent is defined as efforts that are ongoing or have been performed for any period of time during the previous three (3) years from the date of issuance of this solicitation. Relevant is defined as flying training support services similar in scope to the attached USAFE JTAC CAS requirement, at a magnitude in excess of 100 flight hours per calendar year. Past performance information that does not meet these criteria will not be evaluated.
5. Past Performance Information Document (PPID). Offerors shall submit past performance information only on the citations detailed in the Technical volume. Each citation is limited to a one (1) single-side page, and should include the following information:
a) Contracting Agency/Customer
b) Non-technical Point of Contact, Telephone Number and e-mail address

6. Past Performance Questionnaires. The offeror is responsible for the distribution of the Past Performance
Questionnaire contained in Section J, Attachment L-1. For each questionnaire distributed, include the cover letter contained in the attachment. Provide with the proposal a letter from all subcontractors/partners, consenting to the release of their past performance information to the prime contractor.
8. Organization Structure Change History. Many companies have acquired, been acquired by, or otherwise merged with other companies, and/or reorganized their divisions, business groups, subsidiary companies, etc.
In many cases, these changes have taken place during the time of performance of relevant past efforts or between conclusion of recent past efforts and this source selection. As a result, it is sometimes difficult for the
CO to determine what past performance is relevant to this acquisition. All offerors shall include in this proposal volume a "roadmap" describing all such changes in the organization of the offeror’s company for at least the 3
(three) years prior to the date of the issuance of the solicitation. As part of this explanation, show how these changes impact the relevance of any efforts the offeror identifies for past performance evaluation/performance confidence assessment. Since the Government intends to consider past performance information provided by other sources as well as that provided by the offeror(s), the "roadmap" should be both specifically applicable to the efforts identified and general enough to apply to efforts on which the Government receives information from other sources. This information is required on the offeror, teaming partners, joint venture partners, and/or major subcontractors, as necessary.
File details come from the government source that posted it. Updated .
