General Information

1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

2. This solicitation is being issued as a(n) Request For Proposal (RFP).

3. This solicitation document and incorporated provisions and clauses are those in effect through:

Instructions to Offerors

FAR provision 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. As prescribed in FAR 12.301(b)(1), the following addendum is provided for this solicitation and hereby amends any language therein:

A. To assure timely and equitable evaluation of the proposal the offeror must follow the instructions contained herein. Offerors are

501st Combat Support Wing (USAFE) required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Failure to meet a requirement may result in an offer being ineligible for award. The response shall consist of three (3) parts; Part I - Price, Part II - Technical, and Part III -Past Performance.

B. The Government's terms, conditions, and respective provisions and clauses contained within this solicitation are prescribed in accordance with United States Federal Acquisition Regulation (FAR), and are not subject to conditionally proposed revisions or changes requested by offerors. Offeror's specific terms and conditions that may be contingently proposed for subsequent inclusion into the contract if awarded will not be considered by the Government in its evaluation of proposals. Offerors that are not able to comply with the aforementioned conditions, will be determined as non-compliant with the solicitation requirements, and therefore may be removed from consideration for award.

C. PROPOSAL CONTENTS:

PART I - PRICE PROPOSAL

Pricing is required to be submitted on Attachment 1, Pricing.

Please Note: Pricing is to be in United States Dollars.

PART II - TECHNICAL PROPOSAL

Offeror shall provide a detailed and comprehensive proposal to include:

(a) TRAINING AND QUALIFICATIONS - The contractor shall submit their training plan/procedures for all contractor personnel that demonstrate method(s) that reduces the risk of performance defaults. The contractor shall submit the resume of individual to fill the position. All required valid documentation that addresses qualifications, training, and the ability to perform any/all portions of the PWS such as, but not limited to; professional certifications, licenses, permits, degrees shall be provided for validation as proof of any qualifications and training. All submitted training and qualification must be valid for at least 6 months of the base period of performance. If any training/qualification expires during the first year of the contract performance; the contractor must demonstrate in the training plan the methods that will be used to renew any qualification in order to continue performance of this requirement without interruption of service.

The resume shall include, but not limited to, the following:

1. The applicant is a U.S. Citizen

2. The past 5 years, from date of solicitation, work history to include references.

3. Professional knowledge of planning, implementation, and assessment techniques and principles as they apply to the strength and conditioning/human performance programming and education sufficient to serve as training resource advisor/consultant.

4. Knowledge of operational concepts and missions sufficient to understand, plan, deliver and evaluate/analyze physical requirements and how strength and conditioning impact mission accomplishment.

5. Knowledge and skill in using of state-of-the-art computer-based software and other instructional/evaluative material, current and highly specialized strength and conditioning equipment, facility recommendations and practices, training aids and devices applicable to program objectives, training effectiveness, and efficiency.

6. Knowledge of medical conditions and terminology as well as of human behavior change theories, motivational strategies, and counseling techniques to resolve the most difficult problems associated with uncooperative or unmotivated airmen necessary for rehabilitation.

7. Skill in data analysis, fact finding, problem analysis, problem resolutions, and development of concrete action plans to solve problems.

8. Skill in written and oral communication to advise, counsel, influence and make recommendations to management officials

501st Combat Support Wing (USAFE) concerning medical, health, and administrative aspects of the Human Performance and Integrated Operational Support Programs.

The following documents shall be provided with the resume:

1. A prescribed certification approved by a nationally recognized professional organization; e.g., National Strength and Conditioning Association: Certified Strength and Conditioning Specialist (CSCS); American College of Sports Medicine, Certified Exercise Physiologist (C-EP).

2. Master's Degree transcripts from an accredited university program in physical therapy, exercise science, exercise physiology, biomechanics, strength and conditioning, or related field is highly desirable.

3. Basic Life Support (BLS) certification to include Automated External Defibrillator (AED) training.

4. Proof of current Secret Security Clearance.

5. A current passing score on a USAF physical fitness test (AFI 36-2905) at the male/female 30-39 years of age standards within the last year.

(b) Quality Control Plan (QCP) - State the approach or approaches you will use to perform to the standards of the PWS and to correct deficient performance. NOTE: If selected for award, the offerors Quality Control Plan will be incorporated into the contract.

(c) SAM Registration - The contractor shall provide a copy of their active SAM.gov registration.

PART III - PAST PERFORMANCE

(a) Offerors shall complete Section A of Attachment 3, the Past Performance Questionnaires (PPQs) with their company information and send the PPQs to their current or former customers for completion of Sections B and C. After completion, the PPQs are to be e-mailed to christina.wolf.4@us.af.mil, james.downs.5.gb@us.af.mil and 501csw.rco@us.af.mil. The number of PPQs for each offeror is limited to a maximum of five (5).

The government will use all questionnaires received from the offeror's referenced customers and data independently obtained from other government and commercial sources to determine if the Past Performance information is recent and relevant and then assess it for acceptability.

- Recent means performance within five (5) years of the date of this solicitation.

- Relevant means performance of similar requirements to those stated in the PWS either in whole or in aggregate if performed simultaneously under multiple contracts.

(b) In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or is so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance. Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, "unknown" shall be considered "unacceptable".

Evaluation Criteria

FAR Provision 52.212-2, Evaluation -- Commercial Items, applies to this acquisition. As prescribed in FAR 12.301(c), the following addendum is provided for this solicitation and hereby amends any language therein:

(A) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(1) Price

- Quality Control Plan

(3) Past Performance

(B) A Lowest Price Technically Acceptable (LPTA) process in accordance with Federal Acquisition Regulation (FAR) Part 12 and Subpart 13.5 will be utilized for the purpose of the Government's evaluation of proposals and resultant award determination. Under the LPTA evaluation process, Technical Capability will be evaluated and rated on an acceptable or unacceptable basis whereby unacceptability of any of the respective sub-factors will result in an unacceptable technical capability-rating. In addition, ratings for past performance will be assigned utilizing an evaluation basis of acceptable or unacceptable.

(C) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced.

Evaluation of options shall not obligate the Government to exercise the option(s).

(D) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

The Government intends to award a contract without discussions with respective offerors. However, the Government reserves the right to conduct discussions if deemed in its best interest. Exchanges conducted with regard to adverse past performance or to resolve minor or clerical errors will not constitute discussions, and the contracting officer reserves the right to award a contract without the opportunity for proposal revision.

BASIS FOR CONTRACT AWARD

By submission of its offer, the offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications. The evaluation process shall proceed as follows:

A. Price Evaluation

An offeror's proposed prices will be evaluated based on the Total Evaluated Price (TEP). The TEP = Sum of the total price of all CLINS + the Evaluated Price of Option to Extend Services IAW FAR 52.217-8. The Evaluated Price of Option to Extend Services shall be calculated by multiplying half the quantity by the unit price of CLIN 4001. The price for the Option to Extend Services shall not be included in the contract value, and should the Government choose to exercise this option, a CLIN will be added to the contract at the time the option is exercised. The offeror's proposed price will be determined by multiplying the quantities identified by the proposed unit price for each Contract Line Item Number (CLIN) to confirm the extended amount for each. If there is a discrepancy, the Government will multiply the Unit Price by the Quantity for the extended amount for each.

Offers will be ordered by Price, with only the lowest priced offer being evaluated for technical acceptability. If the lowest priced offer is deemed unacceptable then the next lowest offer will be evaluated for technical acceptability. This process will continue until an offer is deemed technically acceptable.

Only the technical acceptable proposal will be evaluated for past performance. If the technical acceptable offer is deemed unacceptable for past performance the next acceptable technical proposal will be evaluated for acceptable past performance (per the process above). This process will continue until an offer is deemed to have acceptable past performance, and then the evaluation process will be complete.

B. Technical Acceptability

The government shall evaluate the Technical proposal submitted by offerors and assign a rating of Acceptable or Unacceptable to each subfactor based on whether or not they meet the requirements of the solicitation. A rating of Unacceptable on any subfactor will result in the entire Technical Plan being rated as Unacceptable.

The Technical Quotes shall be evaluated against the following subfactors:

(a) Training and Qualifications: The contractor submitted their training plan/procedures for all contractor personnel that demonstrate method(s) that reduces the risk of performance defaults. The contractor submitted the resume of individual to fill the position with all required information and documents. All required valid documentation that addresses qualifications, training, and the ability to perform any/all portions of the PWS such as, but not limited to; professional certifications, licenses, permits, degrees were provided as proof of any qualifications and training. For instances of any training/qualification that was due to expire during the first year of the contract performance; the contractor demonstrated in the training plan the methods that will be used to renew any qualification in order to continue performance of this requirement without interruption of service.

(b) Quality Control Plan: Contractor stated the approach or approaches that will be used to perform to the standards of the PWS and to correct deficient performance.

(c) SAM registration: Contractor provided Active SAM.gov registration.

C. Past Performance

(2) The government will evaluate past performance on a pass/fail basis and assign an Acceptable or Unacceptable rating as described below:

Acceptable = Past performance demonstrates a reasonable expectation that the offeror will successfully perform the requirements of the PWS.

Unacceptable = Past performance does not demonstrate a reasonable expectation that the offeror will successfully perform the requirements of the PWS.

In the evaluation of Past Performance, the Government will consider the information provided through the questionnaires, as well as information obtained from other sources, such as, but not limited to, the Past Performance Information Retrieval System (PPIRS).

When a completed PPQ does not match the Contractor Performance Assessment Report (CPAR) rating, CPAR will take precedence.

The Offeror's past performance efforts will be assess for: Recency, Relevancy and Performance Quality. The Government will only evaluate performance quality on past efforts that are determined to be both recent and relevant.

(i) Recency: Services must have been performed within the last five (5) years from the issuance date of solicitation.

(ii) Relevancy: Past performance has to be similar in scope. Scope is defined as Strength and Conditioning Specialist personnel or similar to the work requirements outlined in this solicitation and associated Performance Work Statement.

In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the past performance rating will be considered "unknown". For the purpose of evaluation, an "unknown" past performance rating will be considered an unacceptable rating and will not be considered for award.

(iii) Performance Quality: The quality assessment consists of an evaluation of recent and relevant past performance information available, regardless of its source. The quality assessment may reveal positive or adverse past performance information. Adverse past performance is defined as past performance information that supports an unsatisfactory rating on any evaluation element or any unfavorable comment received from sources without a formal rating system. Multiple ratings of less than "Satisfactory" in Contractor Performance Assessment Reporting System (CPARS) and/or Past Performance Questionnaires (PPQs) may result in a past performance rating of "unacceptable". Offerors may be given an opportunity to respond to adverse past performance information for which they have not previously had an opportunity to address.

B) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

52.217-9. Option to Extend the Term of the Contract

(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5years and 6 months.

52.232-18, Availability of Funds

Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer.

5352.201-9101 Ombudsman

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for

501st Combat Support Wing (USAFE) this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.

(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).
