STATEMENT OF WORK
Project No.: LXEZ 164313
Project Title: Install Radon Mitigation, NON-GOH (91 UN)

Telephone No: 632-7070 Location: Various Military Family Housing, Okinawa, Japan
1.0 General Scope: In general, the contractor shall install a new radon mitigation system. The work being conducted shall include, but is not limited to the following:
1.1 Conduct a pre-installation inspection, gather relevant site data, and review existing drawings and radon sampling data. Determine size, location, and configuration of mitigation system to achieve radon reduction goals <4.0 picocuries per liter (pCi/L) for each housing unit.
1.2 Installation: Install new mitigation system that is needed based on pre-testing results, where results yielded 4.0 pCi/L or greater. Where possible, mitigation systems shall be completed using active soil depressurization (ASD) techniques and employ sub-slab depressurization (SSD) methods. All installed exterior hardware and components shall be corrosion resistant suitable for coastal marine environments. Provide electrical wire from new suction fan to new enclosed circuit breaker panelboard. New interior conduit shall run above ceiling as much as possible.
1.3 Preparation of Final Report: Upon completion of the installation work, the Contractor shall provide a final report summarizing all field activities, and conclusions/recommendations. In addition, the Contractor shall provide (either as an appendix to the final report or a separate report) operations, maintenance and warranty information on installed systems. Include detailed photos of pre-existing conditions, work performed, and final conditions. Photos shall include unit numbers and written descriptions as necessary.
2.0 Project Schedule And Time Constraints:
2.1 Completion of Work: The Contractor shall be required to complete the entire work, ready for use, not later than 276 calendar days after the date of Notice to Proceed (NTP). This includes 81 calendar days for design and 195 calendar days for performance time. The time for completion shall include final clean-up of the premises.
2.2 Design Schedule: The pre-design conference will be scheduled by the Contracting Officer. This project shall thereafter be completed in accordance with the following schedule. Contractor shall attend review meetings as scheduled. Floor plans for this unit will be provided before the start of design. Existing radon test results for this unit will also be provided if available. The 95% design submission shall include asbestos and lead test results, and radon mitigation recommendations and plans.

2.3 Construction Schedule: The building to be worked on will be occupied during the construction period. The Contractor shall determine the most efficient work plan to minimize inconvenience to the user. The Contractor can begin on-site work with approval from the Government upon satisfactory response actions at the revised 95% design. The Contractor shall be allowed to work 0730 to 1730 hours, Monday through Friday, excluding US legal holidays. Construction schedule will be flexible. Some occupants may not be available on requested dates due to TDY, leave, PCS, etc,.
2.4 Performance Time: The contractor shall be completed within 195 calendar days. This includes material submittals, government material submittal review, material procurement and construction time.

3.0 Design and Construction Requirements:
3.1 The design for this project shall provide for the completion of the work identified on this Statement of Work within the contract price. The design shall fully accommodate usage requirements of the Government facility and minimize interference with existing work. All design work shall be in accordance with applicable criteria. The Government shall not be responsible for any additional costs which could have been avoided through an appropriate contractor’s design.
3.2 Applicable Documents: Comply with applicable (1) federal, state, and local environmental statutes, instructions, manuals, handbooks, regulations, guidance, policy letters, and rules (including all changes and amendments), and (2) Presidential Executive Orders, in effect on the date of issuance of this project.
All design and construction must conform to UFC Standards, current IBC, UFGS (www.wbdg.org), JEGS, OSHA, NFPA 101 and all other applicable publications, codes, and regulations. The Government shall not be responsible for any additional costs which could have been avoided through an appropriate contractor’s design and construction.

3.3 As-built drawings: Site/Building floor plan and details will be provided to the Contractor for reference only in their present form. The Government bears no responsibility for the accuracy of these drawings. Actual field investigation and survey of the facility must be conducted by the Contractor to ascertain existing conditions before start of construction. The Contractor shall visit each construction site before attempting to design.
3.4 The material submittals must be coordinated and approved with 718CES/CENMB. Required submittals shall be identified on an AF Form 66 Material Submittal Log by the government based upon the contractor’s design proposal. Submittals shall be provided using an AF Form 3000.
3.5 Designer certification requirement for design engineers: When the Government indicates minimum requirement of Design Engineers, Design Engineer shall be as indicated in the Qualification Matrix below. All design professionals shall have current registration to practice in the United States or Japan. Approval shall be indicated on all documents by having the professional signature or stamp/seal of the architect or engineer with personal signature over same appearing on all sheets as applicable to their specialties.

3.5.1 Certification: The work must be accomplished by a contractor with advanced experience in mitigating high levels of radon in Single and Multifamily homes with slab on grade and crawl space construction. The work is to be performed under the technical direction of an individual certified under the NEHA NRPP (National Environmental Health Association National Radon Proficiency Program) and the American Association of Radon Scientists and Technologists. Contractor shall provide proof of certification at the time of bidding.
3.5.2 Conference Records: The contractor shall within seven (7) working days after each conference or discussion, either telephonic or in person, prepare a written record of the meeting and/or discussions and furnish a copy (in electronic format and hard copy) to the COR or 718 CES Engineer identified in the resultant contract via the Contracting Officer. The written report shall include the project name, contract number, subject, the name of the participants, an outline of discussions, the recommendations, and conclusions. Number each written record for the particular project under design in consecutive order 3.6 Protection of properties: The Contractor shall protect all equipment, furniture and other items not removed in the work area with approved protective materials. The items that will be moved by the Contractor shall be properly stored and protected and shall be returned to the original place by the Contractor.
3.7 Asbestos Containing Materials (ACM) and Lead Based Paint (LBP) survey:
(1) Existing building materials may or may not contain lead base paint and/or asbestos, and can be surveyed if suspected. If such materials exist, the contractor may include sampling and testing, as an optional cost in their proposal.
(2) Asbestos sampling at 0.1% and LBP sampling, Asbestos and Lead Paint Sampling areas impacted by this project will be in accordance with Japanese Environmental Government Standards (JEGS).
(3) Sampling Plan must be submitted to Mr. Kinjo/18AMDS/SGPB (Bioenvironmental Engineering) and 718 CES/CEIE (Environmental) through 18 CONS for review and approval. Floor plan (small scale) of the building shall be submitted at the completion of the survey and testing, and shall indicate specific locations where samples were taken; i.e., ceiling, wall, floor, etc.
(4) Sampling schedule must be submitted to 718 CES/CEH through 18 CONS for approval. The contractor shall provide written notice to the Military Family Housing Occupants indicated on this statement of work (SOW) within a minimum of Fourteen (14) calendar days prior to starting on-site work.
(5) Actual material sampling and testing shall be done by a certified local Industrial Hygienist acceptable to Kadena Environmental & Bioenvironmental Engineer. All special assessments will be conducted to meet 718 CES/ Environmental Flight standards in addition to the bioenvironmental standards. If testing is positive for asbestos or lead-based paint, a modification for abatement will be considered. If asbestos or lead based paint is discovered during the course of construction as an unforeseen site condition, a contract modification may have to be written.
(6) Destructive Sampling shall be taken from areas that are not easily visible and shall be repaired with a matching finish.
(7) Where LBP/ACM is found after testing, work shall be done with High Efficiency Particulate Air Filter (HEPA) vacuum cleaner and dust generated shall be disposed of in accordance with all applicable Code of Federal Regulation (CFR), Environmental Protection Agency (EPA), Government of Japan (GOJ), Japan Environmental Governing Standards (JEGS), Prefectural and local regulations. The work area shall be protected by temporary vinyl sheets to collect possible debris/dust. Prepare portable HEPA vacuum to clean-up visible debris and dust.
(8) The contractor shall apply the HEPA vacuum cleaner for construction work on walls, floors and ceilings to avoid escape of the waste media into the surrounding environment which may contain asbestos material or lead base paint in accordance with GEGS and Federal, Prefectural and local regulations.
3.8 Project design must be coordinated with the Using Agency, 718 CES/CENM, 718 CES/CEH, 18 AMDS/SGPB (Bio-Environmental), and other agencies involved in the projects.
3.9 Submittals: Project documents to be submitted for each design stage are as follows.

*Submit one CD (all drawings, specifications and design calculations) to 18 CONS/LGCC and the rest to 718 CES/CENMB directly at each design stage.
3.10 START WORK NOTIFICATION: Before starting on-site work the Contractor shall notify the CO and Project Manager in writing, Thirty (30) calendar days prior to on-site-work starting. After approval of on-site work, the contractor shall provide written notice to the Housing Manager and Military Family Housing Occupants within Fourteen (14) calendar days prior to starting on-site work.
3.11 WORK CLEARANCE REQUEST: It is the Contractor’s responsibility to prepare the Air Force Form 103 at their own expense. The work clearance request is used for any work and is submitted to the Contracting Officer Representative, reviewed and approved by the listed organizations, Security Police, Safety, Communications, Commercial Utility Company and Chief of Engineering Division or Engineering Flight Chief. Only after the permit is approved and signed by organizations and other agencies listed on the Air Force Form 103, the contractor may commence with work. Without a permit, the excavation work will be stopped, until an approved permit is obtained.
3.12 WORK PROGRESS SCHEDULE: In addition to the requirement of the construction schedule set forth in the contract, the contractor shall, when required by the Government, provide and submit to Contracting Officer Representative (COR) photographs and a detailed work progress schedule in the form of a Bar Chart Work Schedule showing the step-by-step phases of work to be accomplished prior to performing actual work. The contractor shall require coordination with COR to make a work progress schedule. The work shall, so far as practicable, be done in definite sections or divisions and confined to limited area which shall be completed before work in other sections or divisions are begun.
3.13 AVAILABILITY OF LATRINE: Latrine is not available in the work area. The Contractor shall be responsible for providing latrines for his personnel. Location of the latrines shall be designated by the COR. Any and all Contractor latrines shall be removed from US Government property prior to issuance of final payment.
3.14 AVAILABILITY OF UTILITY SERVICE: The Government will furnish electricity and water to the Contactor at no cost in accordance with the contract clause entitled “Availability and Use of Utility Services” if utilities are available in the vicinity of the project site.
3.15 BARRICADES: The Contractor shall provide bilingual barriers/signs to keep unauthorized persons from entering the airfield work or restricted work areas. Battery-powered lights will be used at night as warning signs when directed by the CO. The construction areas shall be continuously illuminated while on site work is performed during hours of darkness.
3.16 CLEAN UP OF AREA: All Contractor generated debris and rubbish shall be collected at the end of each day and deposited at locations designated by CO.
3.17 DEBRIS AND RUBBISH CONTROL: For removal and transport of debris and rubbish, use only roadway and street designated by the COR. Remove and transport debris and rubbish in a manner so as to prevent spillage on streets or adjacent area. Contractor shall be responsible for clean-up for any spills on roadways, streets, and other areas. Burning of debris and rubbish on U.S. installations is prohibited. Concrete, masonry, and other materials not permitted to remain in place, shall be disposed of outside the limits of Government-controlled land by the Contractor.
3.18 PROTECTION OF PROPERTIES: The contractor shall protect existing plants and appliances during construction. The items that will be moved by the Contractor shall be properly stored and protected and shall be returned to the original place by the Contractor. Plumbing, wiring and other existing materials damaged during the contractor’s operations shall be repaired and replaced at no cost to the Government.
3.19 Daily Report: Contractor shall provide progress photos and reports if requested by the Government.
3.20 Joint job site inspection shall be conducted by the project engineer, user, contractor, and 18 CONS/LGCC representatives.
3.21 Photo Documentation: Prepare digital photo documentation in support of project submittals, as necessary. Include photo documentation of sites and buildings under investigation, field activities, and sample locations. Photography of any kind must be coordinated through the installation, customer, or facility Point of Contact.
3.22 As-built Documents: The Contractor shall provide the corrected contract drawings in white paper and 2 copies of CD containing all drawings in electronic CADD and PDF and all documents in WORD or EXCEL and PDF. (Reference: 718 CES As-Built Numbering Standard)
3.23 DOCUMENTS REQUIRED: All documents required under this Contract, including but not limited to safety hazard analysis plans, all correspondence of any nature whatsoever and all other written communications, documents, drawings, reports, and etc. of every description shall all be written in English or translated in English before being provided to the Government by the Contractor. Delays encountered due to a translated copy not submitted, shall be at the Contractor’s expense.
3.24 DD Form 1354: Contractor shall complete the DD Form 1354 for government approval. A draft DD Form 1354 shall be provided for review with the 95% design submittal. The contractor must then update the DD Form 1354 and provide it to the Government at or before the final inspection/acceptance. Any contract modifications not captured on the DD Form 1354 at the time of final acceptance must be provided within 60 days.
4.0
DESCRIPTION OF WORK:
4.1 General:

4.2 Civil:
1. Excavation, removal, disposal and back fill of soil.
2. Provide aggregate.
3. Remove and replace sodding including topsoil for damaged surface.
4. Drilling the core at exterior concrete wall under floor slab to provide 4” Ø PVC radon exhaust pipe.
4.3 Architectural:
1. Repair and patchwork to match existing adjacent finishes.
2. Remove and replace floor tile and/or ceiling as necessary.
4.4 Electrical:
1. Provide electrical power for exhaust fan. The interior conduit shall run above ceiling as much as possible
2. Interior exposed conduit shall be metal wiremold and exterior exposed conduit shall be rigid steel galvanize (thick wall).
3. Provide a circuit breaker 1P, 125V, 20A with enclosure by the existing circuit breaker panel for power supply of new exhaust fan.
4. Assume approximate distance from new exhaust fan to existing circuit breaker panel is as follows:
a. 1 Story unit: 19m (Assume 16m of conduits run above ceiling.)
b. 2 Story unit: 32m (Assume 29m of conduits run above ceiling.)

Conduct insulation resistance test for all new electrical wires and submit test result.
Minimum resistance shall be 250,000 ohms.
4.5 Mechanical:
1. Provide 4” Ø PVC radon exhaust pipe, fittings, and pipe supports from the radon suction pit under the floor slab to above the roof.
2. Provide weather proof type radon exhaust fan with radon system monitor and all associated accessories. Provide corrosion-proof protective cover for fan with paint to match existing exterior wall.
3. Provide rain cap with bird screen for radon exhaust outlet pipe.
4.6 Asbestos / LBP Test:
1. Asbestos Test: One sampling and test with 24 different housing styles. (Total 24)
2. LBP Test: Two samplings and test with 24 different housing styles. (Total 48)
