1. The magnitutde of this project is betw een $100,000 and $250,000. This general statement of the magnitude of the project is furnished for hte convenience of the prospective offerors. The actual extent of w ork required w ill be governed by the contract specif ications and draw ings.
2. Funds are NOT presently available for this project. If FY 15 funds become available, purchase order aw ard w ill be made no later than 31 Jul
15. Quotations shall be valid until 31 Jul 15. The Government reserves the right to cancel this solicitation before or after the closing date w hen cancellation is clearly in the Government's best interest.
3. CONTRACTOR LICENSING REQUIREMENTS: Any contract resulting from this solicitation w ill be aw arded and performed in its entirety in the country of Japan. Contractors must be duly authorized to operate and conduct business in Japan, and must fully comply w ith all law s, decrees, labor standards, and regulations of Japan during the performance of the contract. Prior to aw ard, offerors must be registered to do business in Japan and possess a construction license (Kensetsu-Gyo Kyoka) issued by the Ministry of Land, Infrastructure and Transport, or prefectural government.

In general, the contractor shall install a new radon mitigation system. The work being conducted shall include, but is not limited to the following: Conduct a pre-installation inspection, gather relevant site data, and review existing drawings and radon sampling data. Determine size, location, and configuration of mitigation system to achieve radon reduction goals <4.0 picocuries per liter (pCi/L) for each housing unit. Install new mitigation system that is needed based on pre-testing results, where results yielded 4.0 pCi/L or greater. Where possible, mitigation systems shall be completed using active soil depressurization (ASD) techniques and employ sub-slab depressurization (SSD) methods. All installed exterior hardware and components shall be corrosion resistant suitable for coastal marine environments. Provide electrical wire from new suction fan to existing circuit breaker panelboad. New interior conduit shall run above ceiling as much as possible. Upon completion of the installation work, the Contractor shall provide a final report summarizing all field activities, and conclusions/recommendations. In addition, the Contractor shall provide (either as an appendix to the final report or a separate report) operations, maintenance and warranty information on installed systems. Include detailed photos of pre-existing conditions, work performed, and final conditions. Photos shall include unit numbers and written descriptions as necessary. This line item includes all work associated with ACM/LBP testing and abatement.

1.0 General Scope: In general, the contractor shall install a new radon mitigation system. The work being conducted shall include, but is not limited to the following:

1.2 Installation: Install new mitigation system that is needed based on pre-testing results, where results yielded 4.0 pCi/L or greater. Where possible, mitigation systems shall be completed using active soil depressurization (ASD) techniques and employ sub-slab depressurization (SSD) methods. All installed exterior hardware and components shall be corrosion resistant suitable for coastal marine environments.

1.3 Preparation of Final Report: Upon completion of the installation work, the Contractor shall provide a final report summarizing all field activities, and conclusions/recommendations. In addition, the Contractor shall provide (either as an appendix to the final report or a separate report) operations, maintenance and warranty information on installed systems. Include detailed photos of pre-existing conditions, work performed, and final conditions. Photos shall include unit numbers and written descriptions as necessary.
2.0 Project Schedule And Time Constraints:
2.1 Completion of Work: The Contractor shall be required to complete the entire work, ready for use, not later than 189 calendar days after the date of Notice to Proceed (NTP). This includes 65 calendar days for design and 124 calendar days for performance time. The time for completion shall include final clean-up the premises.
2.2 Design Schedule: The pre-design conference will be scheduled by the Contracting Officer. This project shall thereafter be completed in accordance with the following schedule. Contractor shall attend review meetings as scheduled. Floor plans for this unit will be provided before the start of design. Existing radon test results for this unit will also be provided if available. The 95% design submission shall include asbestos and lead test results, and radon mitigation recommendations and plans.

2.3 Construction Schedule: The building to be worked on will be occupied during the construction period. The Contractor shall determine the most efficient work plan to minimize inconvenience to the user. The Contractor can begin on-site work with approval from the Government upon satisfactory response actions at the revised 95% design. The Contractor shall be allowed to work 0730 to 1630 hours, Monday through Friday, excluding US legal holidays. Construction schedule will be flexible. Some occupants may not be available on requested dates due to TDY, leave, PCS, etc,.
2.4 Performance Time: The contractor shall be completed within 124 calendar days. This includes material submittals, government material submittal review, material procurement and construction time.

3.0 Design and Construction Requirements:
3.1 The design for this project shall provide for the completion of the work identified on this Statement of Work within the contract price. The design shall fully accommodate usage requirements of the Government facility and minimize interference with existing work. All design work shall be in accordance with applicable criteria. The Government shall not be responsible for any additional costs which could have been avoided through an appropriate contractor’s design.
3.2 Applicable Documents: Comply with applicable (1) federal, state, and local environmental statutes, instructions, manuals, handbooks, regulations, guidance, policy letters, and rules (including all changes and amendments), and (2) Presidential Executive Orders, in effect on the date of issuance of this project.
All design and construction must conform to UFC Standards, current IBC, UFGS (www.wbdg.org), JEGS, OSHA, NFPA 101 and all other applicable publications, codes, and regulations. The Government shall not be responsible for any additional costs which could have been avoided through an appropriate contractor’s design and construction.

3.3 As-built drawings: Site/Building floor plan and details will be provided to the Contractor for reference only in their present form. The Government bears no responsibility for the accuracy of these drawings. Actual field investigation and survey of the facility must be conducted by the Contractor to ascertain existing conditions before start of construction. The Contractor shall visit each construction site before attempting to design.
3.4 The material submittals must be coordinated and approved with 718CES/CENMB. Required submittals shall be identified on an AF Form 66 Material Submittal Log by the government based upon the contractor’s design proposal. Submittals shall be provided using an AF Form 3000.
3.5 Designer certification requirement for design engineers: When the Government indicates minimum requirement of Design Engineers, Design Engineer shall be as indicated in the Qualification Matrix below. All design professionals shall have current registration to practice in the United States or Japan. Approval shall be indicated on all documents by having the professional signature or stamp/seal of the architect or engineer with personal signature over same appearing on all sheets as applicable to their specialties.

3.5.1 Certification: The work must be accomplished by a contractor with advanced experience in mitigating high levels of radon in Single and Multi family homes with slab on grade and crawl space construction. The work is to be performed under the technical direction of an individual certified under the NEHA NRPP (National Environmental Health Association National Radon Proficiency Program) and the American Association of Radon Scientists and Technologists. Contractor shall provide proof of certification at the time of bidding.
3.5.2 Conference Records: The contractor shall within seven (7) working days after each conference or discussion, either telephonic or in person, prepare a written record of the meeting and/or discussions and furnish a copy (in electronic format and hard copy) to the COR or 718 CES Engineer identified in the individual task order via the Contracting Officer. The written report shall include the project name, contract number, subject, the name of the participants, an outline of discussions, the recommendations, and conclusions. Number each written record for the particular project under design in consecutive order
3.6 Protection of properties: The Contractor shall protect all equipment, furniture and other items not removed in the work area with approved protective materials. The items that will be moved by the Contractor shall be properly stored and protected and shall be returned to the original place by the Contractor.
3.7 Asbestos Containing Materials (ACM) and Lead Based Paint (LBP) survey:
(1) Existing building materials may or may not contain lead base paint and/or asbestos, and can be surveyed if suspected.
(2) Asbestos sampling at 0.1% and LBP sampling, Asbestos and Lead Paint Sampling areas impacted by this project will be in accordance with Japanese Environmental Government Standards (JEGS).
(3) Sampling Plan must be submitted to Mr. Kinjo/18AMDS/SGPB (Bioenvironmental Engineering) and 718 CES/CEIN (Environmental) for review and approval. Floor plan (small scale) of the building shall be submitted at the completion of the survey and testing, and shall indicate specific locations where samples were taken; i.e., ceiling, wall, floor, etc.
(4) Sampling schedule must be submitted to 718 CES/CEH for approval. The contractor shall provide written notice to the Military Family Housing Occupants indicated on this statement of work (SOW) within a minimum of fourteen (14) calendar days prior to starting on-site work.
(5) Actual material sampling and testing shall be done by a certified local Industrial Hygienist acceptable to Kadena Environmental & Bioenvironmental Engineer. All special assessments will be conducted to meet 718 CES/ Environmental Flight standards in addition to the bioenvironmental standards.
(6) Destructive Sampling shall be taken from areas that are not easily visible and shall be repaired with a matching finish.
(7) Where LBP/ACM is found after testing, work shall be done with High Efficiency Particulate Air Filter (HEPA) vacuum cleaner and dust generated shall be disposed of in accordance with all applicable Code of Federal Regulation (CFR), Environmental Protection Agency (EPA), Government of Japan (GOJ), Japan Environmental Governing Standards (JEGS), Prefectural and local regulations. The work area shall be protected by temporary vinyl sheets to collect possible debris/dust. Prepare portable HEPA vacuum to clean-up visible debris and dust.
(8) The contractor shall apply the HEPA vacuum cleaner for construction work on walls, floors and ceilings to avoid escape of the waste media into the surrounding environment which may contain asbestos material or lead base paint in accordance with GEGS and Federal, Prefectural and local regulations.
3.8 Project design must be coordinated with the Using Agency, 718 CES/CENM, 718 CES/CEH, 18 AMDS/SGPB (Bio-Environmental), and other agencies involved in the projects.
3.9 Submittals: Project documents to be submitted for each design stage are as follows.

*Submit one CD (all drawings, specifications and design calculations) to 18 CONS/LGCC and the rest to 718 CES/CENMB directly at each design stage.
3.10 START WORK NOTIFICATION: Before starting on-site work the Contractor shall notify the CO and Project Manager in writing, thirty (30) calendar days prior to on-site-work starting. After approval of on-site work, the contractor shall provide written notice to the Housing Manager and Military Family Housing Occupants indicated on the Task Order within a minimum of Fourteen (14) calendar days prior to starting on-site work.
3.11 WORK CLEARANCE REQUEST: It is the Contractor’s responsibility to prepare the Air Force Form 103 at their own expense. The work clearance request is used for any work and is submitted to the Contracting Officer Representative, reviewed and approved by the listed organizations, Security Police, Safety, Communications, Commercial Utility Company and Chief of Engineering Division or Engineering Flight Chief. Only after the permit is approved and signed by organizations and other agencies listed on the Air Force Form 103, the contractor may commence with work. Without a permit, the excavation work will be stopped, until an approved permit is obtained.
3.12 WORK PROGRESS SCHEDULE: In addition to the requirement of the construction schedule set forth in the contract, the contractor shall, when required by the Government, provide and submit to Contracting Officer Representative (COR) photographs and a detailed work progress schedule in the form of a Bar Chart Work Schedule showing the step-by-step phases of work to be accomplished prior to performing actual work. The contractor shall require coordination with COR to make a work progress schedule. The work shall, so far as practicable, be done in definite sections or divisions and confined to limited area which shall be completed before work in other sections or divisions are begun.
3.13 AVILABILITY OF LATRINE: Latrine is not available in the work area. The Contractor shall be responsible for providing latrines for his personnel. Location of the latrines shall be designated by the COR. Any and all Contractor latrines shall be removed from US Government property prior to issuance of final payment.
3.14 AVAILABILITY OF UTILITY SERVICE: The Government will furnish electricity and water to the Contactor at no cost in accordance with the contract clause entitled “Availability and Use of Utility Services” if utilities are available in the vicinity of the project site.
3.15 BARRICADES: The Contractor shall provide bilingual barriers/signs to keep unauthorized persons from entering the airfield work or restricted work areas. Battery-powered lights will be used at night as warning signs when directed by the CO. The construction areas shall be continuously illuminated while on site work is performed during hours of darkness.
3.16 CLEAN UP OF AREA: All Contractor generated debris and rubbish shall be collected at the end of each day and deposited at locations designated by CO.
3.17 DEBRIS AND RUBBISH CONTROL: For removal and transport of debris and rubbish, use only roadway and street designated by the COR. Remove and transport debris and rubbish in a manner so as to prevent spillage on streets or adjacent area. Contractor shall be responsible for clean-up for any spills on roadways, streets, and other areas. Burning of debris and rubbish on U.S. installations is prohibited. Concrete, masonry, and other materials not permitted to remain in place, shall be disposed of outside the limits of Government-controlled land by the Contractor.
3.18 PROTECTION OF PROPERTIES: The contractor shall protect existing plants and appliances during construction. The items that will be moved by the Contractor shall be properly stored and protected and shall be returned to the original place by the Contractor. Plumbing, wiring and other existing materials damaged during the contractor’s operations shall be repaired and replaced at no cost to the Government.
3.19 Daily Report: Contractor shall provide progress photos and reports if requested by the Government.
3.20 Joint job site inspection shall be conducted by the project engineer, user, contractor, and 18 CONS/LGCC representatives.
3.21 Photo Documentation: Prepare digital photo documentation in support of project submittals, as necessary.
Include photo documentation of sites and buildings under investigation, field activities, and sample locations.
Photography of any kind must be coordinated through the installation, customer, or facility Point of Contact.
3.22 As-built Documents: The Contractor shall provide the corrected contract drawings in white paper and 2 copies of CD containing all drawings in electronic CADD Standard Release 2.0 and PDF and all documents in WORD or EXCEL and PDF. (Reference: 718 CES As-Built Numbering Standard)
3.23 DOCUMENTS REQUIRED: All documents required under this Contract, including but not limited to safety hazard analysis plans, all correspondence of any nature whatsoever and all other written communications, documents, drawings, reports, and etc. of every description shall all be written in English or translated in English before being provided to the Government by the Contractor. Delays encountered due to a translated copy where not submitted shall be at the Contractor’s expense.
3.24 DD Form 1354: Contractor shall complete the DD Form 1354 for government approval.
4.0 DESCRIPTION OF WORK:
4.1 Civil:
1. Excavation, removal and disposal of soil

4. Drilling the core at exterior concrete wall under floor slab to provide 4” Ø PVC radon exhaust pipe.
4.2 Architectural:
1. Repair and patchwork to match existing adjacent finishes.
2. Remove and replace floor tile and/or ceiling as necessary.
4.3 Electrical:
1. Provide electrical power for exhaust fan. The interior conduit shall run above ceiling as much as possible

CONST-E-7140B PRE-FINAL AND FINAL INSPECTION
In accordance with FAR 52.211-10 the contractor is required to complete this project, ready for use, within the number of calendar days specified in the Task Order. To ensure adequate time to correct any deficiencies noted by the government during the final inspection, it is highly recommended that the contractor schedules such final inspection through the contract administrator at least seven (7) calendar days prior to the scheduled project completion date. The contractor is solely responsible for scheduling this inspection and is reminded that failure to complete the project ready for use in accordance with the contract terms and conditions may result in the assessment of liquidated damages as described in FAR 52.211-12, Liquidated Damages - Construction. Occasionally, minor discrepancies may be detected during the final inspection that may be considered "punchlist" items. If only minor "punchlist" items are found, another inspection may not be required. However, all punchlist items must be corrected by the contractor, and verified by the project inspector, prior to expiration of the contract period unless otherwise authorized in writing by the contracting officer. The determination as to what constitutes a minor discrepancy rests solely with the contracting officer and nothing in this paragraph shall be construed as a waiver of the government's rights under any clause in this contract, including the right to assess liquidated damages as may be appropriate for failure to complete the project in accordance with the terms and conditions contained therein.

This clause requires invoices for supplies and services purchased by the DoD be submitted via electronic means. The Wide Area Workflow (WAWF) system satisfies this mandatory requirement. WAWF can be accessed on the internet at https://wawf.eb.mil/.
Before using WAWF, ensure your company’s Electronic Business POCs listed in your System for Award Management (SAM) are the same individuals that will invoice through WAWF. If they aren't, changes can be made to your company’s SAM profile at https://www.sam.gov/portal/public/SAM/. Click on "Update” or “Renew” using “TPIN". Section H - Special Contract Requirements
CONST-H-7200 U.S. DOLLAR VERSUS YEN EQUIVALENT
For the duration of this solicitation the contractor shall use the following exchange rate in determining the Yen equivalent to referenced US dollar amounts in this solicitation provisions/contract clauses $1.00 = JPY103.9439 evaluation of the award. All referenced US dollar amounts in this solicitation provisions/contract clauses are modified to include "or Yen equivalent". Yen rate will be established per delivery order or task order.
CONST-H-7201 REQUIRED INSURANCE (In addition to that required by Japanese Law)
a. The contractor shall procure and maintain during the entire period of his performance under this contract the following minimum insurance on each vehicle used by the contractor at the work site.
Type: Automobile Property Damage and Bodily Injuries Amount: JPY3,000,000 or Dollar equivalent for automobile property damage JPY30,000,000 or Dollar equivalent for bodily injuries
b. Prior to the commencement of work hereunder, the contractor shall furnish to the contracting officer a certificate or written statement of the above required insurance. The policies evidencing required insurance shall contain an endorsement to the effect that cancellation or any material change in the policies adversely affecting the interests of the US Government in such insurance shall not be effective for such period as may be prescribed by the laws of the country of Japan in which this contract is to be performed and in no event less than thirty (30) days after written notice thereof to the contracting officer.
c. The contractor agrees to insert the substance of this clause, including this paragraph (c) in all subcontracts hereunder.
CONST-H-7202 CONCILIATION
Except as otherwise provided in this contract, any disagreement arising under this contract which is not resolved by the parties to this contract may be submitted to the United States - Japan Joint Committee for Conciliation in accordance with Paragraph 10, Article XVIII, of the Status of Forces Agreement under Article VI of this Treaty of Mutual Cooperation and Security between Japan and the United States of America. Requests by the contractor for conciliation shall be made in accordance with the following procedures:
a. In the event the contractor desires conciliation after the decision of the contracting officer has been served upon him, he will first file his appeal from such findings of facts with the appropriate authority in accordance with the clause of this contract entitled “DISPUTES”, and within the time limits described therein before filing request for conciliation with the Joint Committee and then request the appellate authority under the clause of this contract entitled “DISPUTES”, to suspend its action on his appeal until such time as the Joint Committee has had an opportunity for effecting conciliation.
b. The request for conciliation will be submitted by the contractor through the nearest Defense Facilities Administration Bureau, to the Contract Conciliation Panel of the Joint Committee. Upon the filing of the request with the Joint Committee, the contractor will immediately notify the contracting officer in writing that the request for conciliation has been filed.
c. In the event the disagreement submitted to the Joint Committee under subparagraph (b) above has been resolved through conciliation, it will be the responsibility of the contractor to notify the appellate authority designated in the clause of this contract entitled “DISPUTES”, of the settlement of the dispute and to withdraw his appeal.
d. In the event the contractor who has submitted a request for conciliation to the Joint Committee under subparagraph (b) above desires; notwithstanding tendency of his request for conciliation, that action by the appropriate authority under the clause of this contract entitled “DISPUTES” be resumed on his appeal, it is his responsibility to so request the said authority in writing. The Joint Committee shall be immediately informed by the contractor of his action taken hereunder.
e. No request for conciliation can be submitted to the Joint Committee in the case of a dispute upon which the final decision of the appropriate authority under the clause of this contract entitled “DISPUTES” has been rendered.
Pending the hearing of Conciliation Panel the contractor shall proceed diligently with the performance of the contract in accordance with the contracting officer’s decision. The provisions of this clause shall not prejudice any right which the parties to the contract may have to file in a civil suit.
CONST-H-7207 BASE DEFENSE ACT/WORKMAN'S COMPENSATION/WORK ON AN
OVERSEAS INSTALLATION ( JUN 2004 )
This contract includes FAR 52.228-3, Workers’ Compensation Insurance (Defense Base Act) (Apr 1984), and FAR 52.228-4, Worker’s Compensation and War-Hazard Insurance Overseas (Apr 1984). FAR 52.228-3 applies only if the contractor has United States’ citizen or national employees, regardless of whether they have SOFA status or a status of residence in Japan. FAR 52.228-4 applies if the contractor has non-United States’ citizen or national employees. One or both clauses may apply to the contractor.
GEN-H-7951 TAX EXEMPTION CERTIFICATE (AUG 2001)
The Contractor will submit requests for gasoline or diesel oil tax exemption certificates to the Contracting Officer.
This request will show the actual amount of gasoline or diesel oil used exclusively for the performance of this contract, with documentary evidence and detailed item breakdown to certify the accurate consumption of gasoline or diesel oil, or other necessary information and data as may be required. The Contracting Officer will issue to the Contractor a Tax Exemption Certificate for the actual amount of gasoline or diesel oil used exclusively for the performance of this contract. The Contractor shall claim all tax and customs exemptions as listed in clause CONST- H-7250, entitled, “EXEMPTION FROM JAPANESE CUSTOMS DUTIES AND TAXES PURSUANT TO

GEN-H-7952 JAPANESE ROAD LAWS (AUG 2001)

The agency managing a road may, when there is reasonable cause for him to feel that items or cargo being carried in or on a motor vehicle may fall out and damage, pollute, or otherwise deface the road in such manner as to hinder or obstruct the traffic, order the operator of the vehicle in question to take steps or measures necessary to prevent occurrence of such hindrance by stopping operation of the vehicle, correcting the method of loading or traveling, or operation of the vehicle.
GEN-H-7953 REGISTRATION OF MOTOR VEHICLES (AUG 2001)
All company-owned and privately-owned motor vehicles required to be on U.S. Government facilities during the performance of this contract must be registered with Vehicle Registration, Camp Foster. Necessary decals and/or permits will be issued and displayed on the vehicles in the manner as directed by the Vehicle Registration Officer.
Vehicle decals shall be removed from vehicles and turned in to Vehicles Registration, Camp Foster, at the end of the contract period or sooner when vehicles are no longer used in the performance of the contract or as directed by the Contracting Officer.

52.223-3 HAZARDOUS MATERIAL IDENTIFICATION AND MATERIAL SAFETY DATA (JAN 1997)
(a) "Hazardous material", as used in this clause, includes any material defined as hazardous under the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract).
(b) The offeror must list any hazardous material, as defined in paragraph (a) of this clause, to be delivered under this contract. The hazardous material shall be properly identified and include any applicable identification number, such as National Stock Number or Special Item Number. This information shall also be included on the Material Safety Data Sheet submitted under this contract.

(c) This list must be updated during performance of the contract whenever the Contractor determines that any other material to be delivered under this contract is hazardous.
(d) The apparently successful offeror agrees to submit, for each item as required prior to award, a Material Safety Data Sheet, meeting the requirements of 29 CFR 1910.1200(g) and the latest version of Federal Standard No. 313, for all hazardous material identified in paragraph (b) of this clause. Data shall be submitted in accordance with Federal Standard No. 313, whether or not the apparently successful offeror is the actual manufacturer of these items.
Failure to submit the Material Safety Data Sheet prior to award may result in the apparently successful offeror being considered nonresponsible and ineligible for award.
(e) If, after award, there is a change in the composition of the item(s) or a revision to Federal Standard No. 313, which renders incomplete or inaccurate the data submitted under paragraph (d) of this clause, the Contractor shall promptly notify…
