STATEMENT OF WORK

Project No.: NJYS 14-4212 Project Title: REPLACE BOILER PLANT DOORS AND DOOR FRAMES, CK 873, 857, 1073, 1409 (68 UN), CAMP KINSER OKINAWA, JP.

1. General Scope: : The Contractor shall furnish all tools, equipment, materials, testing, labor and personnel necessary to plan, design, manage and supervise the project which shall consist of, but not be limited to replacing 4 double doors, frames and hardware and 4 personnel doors, frames and hardware for boiler rooms. The contractor shall replace door and related components to include removal, disposal and clean-up of adjacent areas. The Contractor shall develop and provide the best solution and provide recommendations not specifically stated elsewhere in this statement of work. A complete and usable product shall be furnished to the Government upon completion of this delivery order.

2. Project Schedule and Time Constraints:

2.1 Completion of Work: The Contractor shall be required to complete the entire work, ready for use, not later than 80 calendar days after the date of Notice to Proceed (NTP).

3. Design and Construction Requirements:

3.1 Special Assessments: (Asbestos, Lead Base Paint)

3.1.1 Asbestos Sampling/Testing

3.1.1.2 Existing building materials may or may not contain lead base paint and/or asbestos, and can be surveyed if suspected. The contractor may include sampling, testing, and abatement fees as an optional cost in their proposal. Sampling shall be taken from areas that are not easily visible and shall be patched with a match finish.

3.1.1.3 Asbestos sampling at 0.1% and LBP sampling, Asbestos and Lead Paint Sampling areas impacted by this project will be in accordance with JEGS Nov 2010.

3.1.1.4 Actual material sampling and testing shall be done by a certified local Industrial Hygienist acceptable to Kadena Clinic Bioenvironmental Engineer. All special assessments will be conducted to meet 718 CES/ Environmental Flight standards in addition to the bioenvironmental standards. If testing is positive for asbestos or lead-based paint, the abatement option will be exercised. If the testing is negative for asbestos or lead-based paint, the sampling/testing option will not be exercised and the abatement option will be credited back to the government via modification of the contract.

3.1.1.5 Prior to field sampling, the following must be submitted to 718 CES through 18 CONS for review and final approval: Certificate of Training, Sampling Plan, and Protective Equipment Plan. A floor plan of the building shall also be submitted showing specific locations where samples were taken i.e. walls, ceiling, roof insulations, pipes, supports, flange gaskets, etc. Test results shall be submitted as soon as possible.

3.1.1.5 Destructive sampling shall be taken from areas that are not easily visible and shall be patched with a match finish.

3.1.2 Asbestos/Lead Based Paint Abatement

3.1.2.1 If asbestos or lead based paint is discovered during the course of construction as an unforeseen site condition, the abatement option will be exercised. Otherwise, the abatement option will be credited back to the government via modification to the contract.

3.1.2.3 Prior to abatement work, the following must be submitted to 718 CES through 18 CONS for review and final approval: certificates of training, sampling plan, protective equipment plan, and all other requirements IAW current Environmental/Bioenvironmental regulations/standards.

3.2 Shop drawings shall be developed and provided that addresses the scope identified within this Statement of Work and be included as part of the contract cost. Shop drawings shall be developed in English and must show all door hardware, frames and thresholds. During the construction period, the Contractor shall minimize interference with existing work. Design must conform to UFC Standards, current IBC, UFGS (www.wbdg.org), JEGS, OSHA, NFPA 101 and all other applicable publications, codes, and regulations. The Government shall not be responsible for any additional costs which could have been avoided through an appropriate contractor’s design and construction.

3.3 As-built drawings depicting Architectural floor plans and details will be provided to the Contractor for reference only in their present form. The Government bears no responsibility for the accuracy of these drawings. Actual field investigation and survey of the facility must be conducted by the Contractor to ascertain existing conditions before start of construction. The Contractor shall visit the construction site before attempting to design.

3.4 The materials submittal must be coordinated and approved with 718CES/CENMA. Required Submittals shall be identified on an AF Form 66, Material Submittal Log by the government based upon the contractor’s design proposal, submittals shall be transmitted provided an AF Form 3000.

3.5 Protection of properties: The Contractor shall protect all elevator, stairs, hallway, equipment, furniture and other items not removed in the work area with approved protective materials. The items that will be moved by the Contractor shall be properly stored and protected and shall be returned to the original place by the Contractor.

3.6 Any interruption of utility services or building access shall be coordinated with Camp Services, Maintenance Shops and Housing Office through the 718CES/CENMA.

3.7 Daily Report Contractor shall provide progress photos and reports if requested by the Government.

3.8 Availability of Latrine: Latrine is available in the work area. The Contractor shall be responsible for maintaining existing latrine conditions.

3.9 Contractor shall provide a weather delay request within 48 hours of inclement weather that caused the work delay.

3.10 Joint job site inspection shall be conducted by the project engineer, user, contractor, and 18 CONS/LGCC representatives.

3.11 DOCUMENTS REQUIRED: All documents required under this Contract, including all correspondence of any nature whatsoever and all other written communications, documents, drawings, reports, and etc. of every description shall all be written in English or translated in English before being provided to the Government by the Contractor. Delays encountered due to a translated copy where not submitted shall be at the Contractor’s expense.

3.12 PROJECT SIGNS: The Contractor shall provide and maintain construction signs on all Task Order project sites, as determined by the Contracting Officer. Signs shall be installed on Task Order sites by the contract start date and shall be removed by the contract completion date. Location of project signs will be coordinated with the Task Order Facility Manager, and Project Manager. Contractor may install company signs at temporary site office locations. Request for company sign will be submitted to Contractor Officer and the Project Manager in writing for approval.

3.13 START WORK NOTIFICATION: Before starting on-site work the Contractor shall notify the Contracting Officer and the Project Manager in writing, Thirty (30) calendar days prior to on-site work starting. After approval of on-site work, the Contractor shall provide written notice to all Building Managers and Military Family Housing Occupants indicated on the Task Order within a minimum of Fourteen (14) calendar days before starting on-site work.

3.14 The Contractor shall be allowed to work 0800 to 1700 hours, Monday through Friday with the exception of all US Federal Holidays.

4.0 DESCRIPTION OF WORK: The work consists of the following elements.

4.1 Architectural:

1. Remove and dispose existing double doors as well as single personnel doors in listed facilities to include, thresholds, frames, doors and hardware.

2. Provide and install new typhoon rated aluminum doors, frames and include hardware to replicate existing, and thresholds. Doors shall be rated to 180 MPH winds. Re-install signage on doors.

3. Perform concrete repair as necessary from door demolition to include touch up painting of concrete surfaces to match existing.

5.0 Government Furnished Equipment of Materials: N/A

6.0 Warranty:

The construction contract requires the Contractor to warrant all workmanship, materials, and equipment for a period of one year from the date of substantial completion or beneficial occupancy, whichever occurs first times.
