PERFORMANCE WORK STATEMENT
TO INSPECT AND PROVIDE CERTIFIED
CALIBRATION CHARTS FOR FUEL TANKS

1. DESCRIPTION OF SERVICE. The Contractor shall verify site conditions, conduct field surveys and obtain all measurements as necessary to complete this contract at Yokota Air Base and Tama Service Annex. All performance under this project shall meet or exceed U.S. Government and Japanese Commercial Standards. The Contractor shall provide all labor, materials, equipment and services to this contract to ensure the overall objective of inspecting, providing certified drawings, and calibration charts of the specified fuel tanks in Appendix A.
1.1. Overall Objective. This contract will allow for tank inspections, document tank dimensions, create certified calibration charts and produce an inspection certificate in accordance with AFI 23-204, paragraph 7.2. The charts will include sizing increments no larger than 1/16-inch (2mm) and be provided with both U.S. gallons and kiloliters.
1.2. DESCRIPTION OF WORK. The Contractor shall provide a projected work schedule to the Contracting Officer (CO) and Government Representative (GR) 5 days before work begins.
1.2.1. The Contractor will replace existing fuel from the existing tank to a Contractor’s fuel truck which can be used as a temporary tank with tank user approval. Fine solids and debris in the existing fuel shall be removed.
1.2.2. The calibration work will start with Underground Storage Tanks (USTs) 705 A and B at facilities
714. Concerning these USTs, the Contractor will move the existing fuel from the first tank to tanks owned by 374 CES/Boiler Shop before the installation of its hatches begins. Once the calibration work is completed on one of the tanks, fuel will be transferred from the next tank to fill it, then, the Contractor can start the calibration work on the next UST. Once the calibration work is completed on both tanks, 374 LRS/POL section will fill the empty tank. If any emergency occurs where the tank must be refueled, it can be accomplished.
1.2.3. Regarding UST 2435 at facility 570, the Contractor will remove pipelines connecting to the access hatch. After the calibration work completion, the Contractor will re-install them.
1.2.4. The Contractor must maintain fuel supply to existing system during performance. Fuel supply can be from existing fuel in the Contractor’s fuel truck or the Contractor may provide another temporary tank that can supply fuel to the existing system during the service.
1.2.5. The Contractor shall clean the inside of existing tanks prior to the calibration work. Pictures will be taken before and after cleaning operations and shall be provided to the GR and CO. The Contractor will also clean the inside fuel supply and return piping. The Contractor will comply with all U.S. Air Force and Japan regulations regarding confined spaces. This work will also include the installation of access hatches as directed by the GR and CO.
1.2.6. The Contractor will measure the tank dimensions and capacity in accordance with the Nihon Kaiji Kentei Kyokai regulations and certification. Storage tanks at and Yokota facilities 714 and will require hatches to be installed. Underground storage tanks will require concrete removal in order to install the hatch.
1.2.7. The Contractor will ensure tanks are returned back to normal operations to include reinstalling piping, regulators, access hatches, and other infrastructure. The Contractors will test for leaks on all fuel connections, tank, and access hatches. The contractor will test all equipment for one hour to ensure the tanks provide the required fuel to the generators and ensure proper operation.
1.2.8. The contractor shall provide the certified calibration charts and dimension drawings in accordance with the inspection certification as stated in 1.1. All documents, drawings, and calibration charts created for this contract will be submitted on one CD which will provided to the GR .
1.2.9. The Contractor will be responsible for all safety practices while performing work to include curing, oil barricading, safety measuring, and installing scaffolding.
1.2.10. The Contractor shall submit an Air Force Form 3000 if chemicals will be used during the project to the GR and CO within 5 days after contract award.
2.0. GENERAL INFORMATION.
2.1. Performance Period. The performance period of this project is 200 days after day of contract award.
2.2. Safety. The Contractor shall comply with U.S. Federal regulations, applicable industry standards, and commercial practices regarding safety while performing this contract. The Contractor’s personnel shall take all reasonable steps and precautions to prevent accidents during the performance.
2.3. Notification. The contractor will be responsible to provide all notifications, coordination, and work clearance (AF FM 103) regarding all users, facility managers, base organizations, (Safety, Fire, Bio, LRS) and Civil Engineer flights prior to starting work.
2.4. Material Disposal. The Contractor shall remove and dispose all project related debris and excess fuel off-base on a daily basis in accordance with Japanese laws. The debris will need to be properly packed or containerized in compliance with debris regulations and any of the debris shall not be scattered.
Qualified personnel with license or certification shall be at the job site, either directly supervising or performing architectural, electrical, and mechanical work. A minimum of two (2) certified personnel are required to submit license or certification. However, all electricians and fire protection personnel shall submit their license or certification. During the work, the GR and CO will verify disposal actions.
2.5. Hours of Operation.
The Contractor shall perform the service from 0800 to 1600, Monday through Friday including Japanese Legal Holidays, but excluding the following U.S. Legal Holidays. Holiday falling on a Saturday may be observed on the preceding Friday. Holidays falling on Sunday may be observed on the following Monday.
2.6. U.S. HOLIDAYS.

2.7. Security Requirement. The Contractor and his employees must comply with all security requirements imposed by the local installation commander at all times while performing the requirements of this contract on this military installation. The prime contractor is responsible for understanding and insuring subcontractor compliance with AFI 31-204, Motor Vehicle Traffic supervision.
2.8. Base Passes. The U.S. Government will issue all required Installation/Base Passes and Controlled Area Badges to Contractor personnel at no cost to the Contractor. Vehicle insurance certificates are required to obtain Base Passes and must cover the automobile liability insurance in the types and minimum amounts of (a) Bodily Injury - ¥30,000,000 per person per occurrence, in addition to the Japanese Compulsory Insurance (JCI), and (b) Property Damage - ¥3,000,000 per occurrence. The prime Contractor shall be responsible for all passes issued to his/her own or sub-Contractor’s employees under this contract. The Contractor shall return all applicable passes upon termination of employment or completion of this contract to the Japanese Pass Section, and a certificate of receipt shall be obtained.

2.9. Spill Prevention and Response.
2.9.1. The Contractor shall conduct all operations in a manner that prevents spills of POL and Hazardous Substances. The Contractor is required to familiarize personnel with spill prevention and response procedures, fire suppression systems, and MSDSs for all materials used and/or stored on the project site.
The Contractor shall provide and maintain spill equipment, sufficient in both type and quantity, at all sites involving the storage, use, or handling of POL and/or hazardous substances.
2.9.2. Site-Specific Spill Contingency Plan (SSCP). An SSCP must be developed for any project at which POL or Hazardous Substances are used and/or stored at the project site. The SSCP shall include spill response procedures, an inventory of POL and/or hazardous substances, probable spill routes, a project site layout diagram, and a spill response equipment inventory. A template SSCP can be obtained from 374CES/CEIE. The Contractor shall train employees on the contents of the SSCP and the use of spill response equipment and shall document such training.
2.9.3. Spill Reporting. The Contractor shall immediately report all POL or Hazardous Substances spills to the Fire Department (374 CES/CEF) at 911 or 042-507-6560.
2.9.4. Spill Clean-up. The Contractor is responsible for clean-up of POL and/or hazardous substance spills and disposal of clean-up material. The Contractor shall determine, as quickly as possible, the nature of the spilled substance and implement necessary safety precautions to protect both human health and the environment at no additional cost to the US Government, and shall only be performed by personnel adequately trained in spill response and cleanup techniques for the severity of the spill incident.
2.9.5. Costs incurred from any Contractor spills are the responsibility of the Contractor. If the US Government has to perform emergency spill response and cleanup, due to the non-availability of designated personnel or if the spill is beyond the capability of designated personnel, the Contractor shall be held liable for all costs associated with performing said work.
3.0. CONTRACTOR PERSONNEL.
3.1. Contract Manager. The Contractor shall provide a contract manager (CM) who shall be responsible for the performance of the work. The name of this person and an alternate(s) who shall act for the Contractor when the manager is absent shall be designated in writing to the CO.
3.2. The Contract manager or alternate shall have full authority to act for the Contractor on all matters relating to daily operation of this contract. The contract manager or alternate, upon notification, shall be available during normal duty hours within 8 hours to meet on the installation with government personnel designated by the CO to discuss problem areas.
4.0. PERFORMANCE REQUIREMENT.
4.1. Work Clearance Permit. The Contractor shall be responsible for obtaining approved work clearance request (AF Form 103) to the GR and CO within 5 days in advance of any site work. Areas containing underground utilities will be designated caution areas. Carefully excavate designated caution areas. Hand excavate within 3 feet of all identified existing utility lines.
4.2. Traffic Access. The contractor will be responsible for all traffic control if road restriction is required. The contractor will maintain at least one open lane throughout all roadwork. The contractor will provide at least one flag person when a two-way traffic is reduced to a single lane.
5.0. GOVERNMENT FURNISHED PROPERTY AND SERVICES:
5.1. Utilities. Water and electricity, where available at the existing work sites, will be provided at no cost to the Contractor. However, The Contractor shall make sure employees practice utility conservation.
The Contractor shall be responsible for operating under conditions that prevent the waste of utilities.
5.2. Security Force and Fire Protection. Upon request for assistance, the following units will provide services on base as necessary:

5.3. Any time The Contractor uses one of the above services, the CM shall immediately report to the GR with all of the details. The CM will be required to submit, in writing, within two (2) days, the details surrounding the use of this service to the CO.
