SUBJECT: Request for Quotation, FA4897-18-Q-0005, Catholic Services
1. The 366th Contracting Squadron at Mountain Home Air Force Base, Idaho intends to establish a Firm Fixed Price contract with a one year base period and the option for two (2) one-year periods for Catholic Services located at the 366th Fighter Wing Chapel as listed in paragraph 6.
2. General Information: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Additionally, the Government will utilize simplified procedures in accordance with FAR Part 13.

3. This combined synopsis/solicitation is issued as a Request for Quotation (RFQ) with solicitation number FA4897-18-Q-0005.
4. A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-96 effective 6 November 2017, Defense Acquisition Circular 20171208 effective 8 December 2017, and Air Force Acquisition Circular 2017-1003 effective 3 October 2017.
5. The North American Industry Classification System (NAICS) code for this solicitation is 813110 – Religious Organizations. Small Business Size Standard is $7.5 Million.

6. The contractor shall provide one (1) Catholic Religious Education Coordinator, one (1) Catholic Music Director, one (1) Catholic Musician in accordance with the attached Performance Work Statements, dated 20 December 2017. The following table contains the description of requirements for the Contract Line Item Numbers (CLIN) items, the location services are to be performed, and the period of performance.

7. The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items (Jan 2017) and the addendum, applies to this acquisition.
Addendum to FAR 52.212-1, Instructions to Offerors – Commercial Items (Jan 2017) Request for Quotation (RFQ) shall contain the following information: RFQ NUMBER; TIME
SPECIFIED FOR RECEIPT OF OFFERS; NAME; ADDRESS; CAGE CODE; TELEPHONE
NUMBER OF OFFEROR; ANY DISCOUNT TERMS; AND ACKNOWLEDGEMENT OF
ALL SOLICITATION AMENDMENTS (if applicable). Quotes must indicate quantity, unit price and total amount for each item. Offerors shall include a statement specifying the extent of agreement with all terms, conditions and provisions included in the solicitation. Quotes shall also contain all other documentation specified herein. Resumes for each position are required.
Addendum to FAR 52.212-1(c), Period for Acceptance of Offers. The paragraph is tailored as follows: “The Offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers.”
8. The provision at FAR 52.212-2, Evaluation - Commercial Items (Oct 2014), and the addendum, applies to this acquisition.
Addendum to FAR 52.212-2, Evaluation – Commercial Items (Oct 2014)
(a) The Government will award a contract(s) resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government intends to make award to the lowest priced technically acceptable. The Government intends to award without discussions with respective offerors; however, the Government reserves the right to conduct discussions if deemed in its best interest. Additionally, the Government reserves the right to make multiple awards if, after considering the additional administrative costs, it is in the
Government’s best interest to do so. The following listed are evaluation factors that shall be used in the evaluation process:
(1) Price: The price evaluation will be made on the basis of the total price of all line items including all options. An offeror must therefore propose on all line items including options.
The Government shall evaluate based on lowest price technical acceptability. If the lowest offeror meets the Government technical acceptability standard as defined below, the contract may be awarded to the lowest offeror. If the lowest offeror fails to meet the Government technical acceptability as defined below, it will be eliminated from consideration for award.
(2) Technical Acceptability: Technical acceptability is met when the offeror submits proof of the mandatory license, certification and experience (resume) to the level as required in each attached Performance Work Statement (PWS).
(i) Past Performance: Past performance will be evaluated as acceptable, unacceptable, or neutral. Past performance is considered acceptable when the work performed is recent and relevant. “Recent” is defined as past or current contracts/work experience (paid or volunteer) performed within the last three (3) years. “Relevant” is defined as “like services” as those described in the attached PWS and of the same complexity of the effort. Offeror shall submit the reference list of one recent and relevant contract or work experience (paid or volunteered) performed. The Government will also use information submitted by the offeror and other sources such as other Federal Government offices, agency performance databases, commercial sources, and personal knowledge to assess the performance. If there is not a sufficient amount of recent and relevant past performance to determine if acceptable or unacceptable, the past performance shall be determined as acceptable.
(b) Options. The Government shall evaluate offers for award purposes by adding the total price for all options to the total price for the base period to determine a total contract price. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
9. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (Jan 2017), Alternate I (Oct 2014), with their offer, or the offeror shall complete paragraph (b) of FAR 52.212-3, if the offeror has completed the annual representations and certifications electronically at http://www.sam.gov.

FAR 52.217-9 Option to Extend the Term of the Contract (Mar 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days before contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 36 months.

In compliance with the Service Contract Labor Standards statute and the regulations of the Secretary of Labor (29 CFR part 4), this clause identifies the classes of service employees expected to be employed under the contract and states the wages and fringe benefits payable to each if they were employed by the contracting agency subject to the provisions of 5 U.S.C. 5341 or 5332.
