STATEMENT OF WORK (SOW)

SUPPLY AND INSTALL NEW OUTDOOR

SCOREBOARD

TYNDALL AFB, FL

17 SEP 2018

1.0 OBJECTIVE

The existing outdoor scoreboard at Tyndall AFB’s track and field location needs to be replaced with a new outdoor scoreboard in accordance with this Statement of Work. Removal and disposal of the old scoreboard and its existing support structure (I-Beams) is required. The new outdoor scoreboard and new support structure shall be installed at the same location as the existing scoreboard.

2.0 DESCRIPTION OF SERVICES

The contractor shall provide all equipment, tools, labor, and materials required to remove and dispose of one each existing outdoor scoreboard and support system (I-Beams) and install one each new outdoor scoreboard and its supporting structure in accordance with this statement of work.

2.1 Summary of Work

1. Remove and dispose of existing scoreboard currently located at field

2. Remove and dispose of existing support structure (I-Beams/2 ea.) currently located at field

a. Existing I-Beams may either be cut a minimum of 6 inches below the ground and grinded to remove any sharp edges or completely excavated

3. Install new support structure as required to safely secure the new outdoor scoreboard

a. New support structure may either be I-Beams or metal tubing as long as the solution is engineered to safely secure the the new outdoor scoreboard in accordance with all applicable codes and regulations

4. Install new outdoor scoreboard on new support structure (scoreboard shall meet specification identified in SOW para 2.2)

a. The bottom of the new outdoor scoreboard shall be mounted 10 feet from the ground

5. Minor electrical relocation services will be performed by the 325th Civil Engineering Squadron

a. Contractor shall coordinate with the 325 CES to ensure electical relocation requirements are conducted in tandem with contractors performance to fulfill the requirement IAW this SOW.

6. Provide site clean-up after all work is complete

7. Perform final systems testing and commissioning to ensure proper function of scoreboard

2.2 Outdoor Scoreboard Minimum Specifications

1. Shall be a minimum of 18’ x 6’6’’

2. Shall be compatible for scoring football and soccer

3. Shall be capable of providing the following game statistics:

a. Minutes and Seconds remaining in period

b. Period

c. Home Score

d. Guest Score

4. Color LED lights

5. Shall have wireless control capability of 2.4GHhz (Shall not exceed 2.4GHz)

6. Shall include one each wireless handheld controller compatible with supplied scoreboard

7. Contractor shall provide the required scoreboard receiver kit (base station) for scoreboard operation

8. Shall be able to withstand wind loads of 150 miles per hour

9. Shall be able to withstand heat, cold, humidity, and rain in accordance with industry standards for

Florida

2.3 Warranty

The contractor shall provide a minimum three-year warranty on the scoreboard parts and components in accordance with standard commercial terms and conditions.

2.4 Location

2.5 Period of Performance. The contractor shall complete all work in accordance with this period of performance within 90 days of contract award.

2.5 Work Hours. The contractor shall perfrom all work between the hours of 0730 and 1630 CST Monday through Friday. The contractor may work, with prior approval of the Contracting Officer, extended hours and/or weekends to ensure timely completion of work at no additional cost to the government.

3. GENERAL INFORMATION

3.1. SECURITY REQUIREMENTS. The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles:

3.1.1. SUITABILITY INVESTIGATIONS. In accordance with AFI 31-101, Personnel working in a government facility 6 months or less or working anywhere on Tyndall AFB property are required to have a Local Files Check. The requiring activities Unit Security Manager will coordinate the Background Affidavit (with Pass & ID) and the DD 1172 Base Access Request form – allow 3-5 day process time.

3.1.2. PASS AND IDENTIFICATION ITEMS. The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles:

a. DBIDS, Defense Biometrics Identification System - Contractor Identification Card.

b. AF Form 1199, USAF Restricted Area Badge, or a locally developed badge (if applicable).

c. AF Form 75, Visitor/Vehicle Pass (AFI 31-204): The Service Provider shall adhere to local security policy when appling for short/long term visitors’ passes. Any contractor requesting a pass for more than 3 days will complete a Local Files Check with 325 SFS/Pass & ID.

3.1.3. RETRIEVING IDENTIFICATION MEDIA. The contractor shall retrieve all identification media from employees who depart for any reason before the contract expires; e.g., terminated for cause, retirement and submit them to the COR.

3.1.4. LISTING OF EMPLOYEES. The contractor shall maintain a current listing of employees. The list shall include employee's name, social security number and level of security clearance or base access (to include if escort privileges are authorized), on company letterhead.

The list shall be validated and signed by the company facility security officer (FSO), provided to the COR (and Unit Security Manager) prior to the contract start date, and to the Contracting Officer (upon request). Updated listings shall be provided when an employee's status or information changes.

3.2. FREEDOM OF INFORMATION ACT (FOIA). The contractor shall comply with DoD Regulation 5400.7Air Force Supplement, DoD Freedom of Information Act Program requirements. The regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

3.3. PRIVACY ACT (PA) PROGRAM. The contractor shall comply with DoD 5400.11-R, The Privacy Act Program requirements and Air Force Instruction 33-332. The regulation sets mandatory guidelines for collecting, safeguarding, maintaining, using, accessing, amending and disseminating personal information kept in systems of records to comply with the Privacy Act, Title 5, United States Code (U.S.C.), Section 552a.

3.4. KEY CONTROL. If applicable, the contractor shall not duplicate keys issued by the government

3.5. TRAFFIC LAWS. The contractor and its employees shall comply with base and local traffic regulations.

3.6. UTILITIES. The Government will furnish electricity, water, and sewage as necessary for accomplishment of work in accordance with this contract. All facilities do not receive the same utility services. The Contractor shall not change or modify any utility system or component; or connect any Contractor property, equipment or system without prior CO review and approval.

The contractor shall practice energy conservation in the Government provided facilities.The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions which prevent the waste of utilities which include the following:

3.6.1. Lights shall be used only in areas where and when work is actually being performed.

36.2. Mechanical equipment controls for heating, ventilation, and air conditioning systems shall not be adjusted by the contractor or by contractor employees unless authorized.

3.6.3. Water faucets or valves shall be turned off after the required use has been accomplished.

3.10. ENVIRONMENTAL LAWS & REGULATIONS. The contractor shall be knowledgeable of and comply with all applicable federal, state, and local laws and permits, and DoD, Air Force, Air Combat Command (ACC), and Tyndall AFB environmental regulations, policies, and guidance. The contractor shall establish policies and procedures to protect the environment and minimize or eliminate the risks of negative environmental impacts, and minimize or eliminate risks of environmental pollution. Contractor operations are subject to environmental compliance inspections without notice.

3.10.1. SPILL CONTROLS. The contractor shall maintain spill control material on hand at all times sufficient to contain a worse case spill, both volume and hazard level.

3.10.1 HAZARDOUS MATERIALS INVENTORY. The contractor shall maintain an up-to-date hazardous material inventory with copies of MSDS(s) for all materials used on the job site.

The contractor shall maintain a complete copy of all approved MSDS and AF Form 3952s.

3.10.4 HAZARDOUS MATERIALS MANAGEMENT. The contractor shall remove all hazardous materials by the end of each workday. Contractor shall not conduct process treatments (e.g. paint thinner solvent recycling) on base.

3.10.5 HAZARDOUS WASTE MANAGEMENT. Any HAZWASTE generated under this contract shall be managed IAW TAFB procedures which include 40 CFR Parts 260-265. TAFB HW procedures are found in TAFB HW Management Plan. All HWs will be sent to a permitted treatment, storage, and disposal facility (TSDF), and all HWs will be transported by licensed HW transporters as approved by Tyndall AFB hazardous waste manager. The hazardous waste manifest must be signed by the Tyndall AFB hazardous waste manager before any waste is removed from the installation. Copies of final disposal certificates shall be provided by the contractor to Tyndall AFB hazardous waste manager within 35 days of transport from TAFB.

3.10.6. HAZARDOUS MATERIAL APPROVAL. All chemicals used on the installation must be tracked by the Hazardous Materials Management Office (HAZMO). The HAZMO shall approve all hazardous materials brought on-site by the contractor prior to use. The contractor shall obtain approval by submitting the TAFB Forms 81 & 82 along with the manufacturer specific safety data sheets of the chemicals that will be brought onto TAFB to the HAZMO ten days prior to starting work. A TAFB Form 83 must be submitted to the HAZMO monthly until completion of the contract.

The contractor shall furnish TAFB Forms 81, 82, and 83 to the HAZMO, for anticipated quantities, application of use, and method of disposal for all materials to be used under this SOW. The listing shall include, but not be limited to, lubricants, spray cleaning solvents, and any known hazardous materials. The contractor must find a suitable substitute for any chemical not allowed for use on the base.

3.11. SAFETY REQUIREMENTS. In performing work under this contract, the contractor shall:

3.11.1. Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.

3.11.2. Take such additional immediate precautions as the contracting officer may reasonably require for safety and mishap prevention purposes

3.11.3. Record and report promptly (within 24 hours) to the contracting officer or designated government representative (GR), all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

3.11.4. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the contracting officer.

3.11.5. If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.

3.11.5 PERMITS, LICENSES, CERTIFICATIONS AND RECORDS. The contractor shall obtain all necessary permits, licenses, and certifications (i.e. refrigeration, electrical, etc.); give all required notices; and comply with applicable Federal, State, County, and local laws, codes, and regulations in performance of the requirements of this contract. Maintain records of such requirements and make the documentation available to the Government for review.

3.12. Publications. Publications can be accessed on-line at http://www.e-publishing.af.mil/.

Supplements or amendments to listed publication from any organizational level may be issued during the life of the contract.

3.12.1. Compliance with all publications, regulations and operating instructions provided by the

Government is required when:

3.12.2. They pertain to the procedures for materials expediting herein and where the contractor is authorized by the statement of work to accomplish the work specified in the publication, regulation or operating instructions.

3.12.3. The publications prescribe USAF policies, use of materials, procedures and processes applicable to the work requirements. The contractor is required to acquire and work on the latest version of the publication.
